Polypharmacy-induced serotonin syndrome revealing underlying neurocognitive impairment and cervical myelopathy.

Serotonin syndrome is a life-threatening condition characterized by neuromuscular hyperactivity, autonomic dysregulation and altered mental status. Polypharmacy in older adults with multiple comorbidities is associated with increased risk of serotonin syndrome. We report a woman in her 50s with diabetes, hypertension and chronic kidney disease recently misdiagnosed with Parkinsonism. She presented with confusion, hyperreflexia, myoclonus and autonomic instability. Her medications included supratherapeutic paroxetine (40mg two times per day), aripiprazole and benztropine. Symptoms improved with

Clinical and experimental aspects of tirbanibulin treatments.

Tirbanibulin is a topical medication approved in 2020 as a 5 day topical treatment for actinic keratosis, a precancerous skin lesion characterized by the hyperproliferation of atypical keratinocytes. Tirbanibulin exerts its effects through dual mechanisms: microtubule inhibition, which disrupts cell division, and Src kinase inhibition, which interferes with cancer-associated signaling pathways. Although its primary use is in AK, these mechanisms suggested potential therapeutic applications in a broader range of epithelial cancers. In this literature review, we explored tirbanibulin's applicati

Microvascular Decompression for Trigeminal Neuralgia Secondary to Vertebrobasilar Dolichoectasia: A Case Report and Review of the Literature.

Vertebrobasilar dolichoectasia (VBD) is a vasculopathy characterized by elongation and dilation of the affected artery. We present a case of trigeminal neuralgia (TN) secondary to VBD, successfully treated with microvascular decompression (MVD) using an autologous muscle graft (AMG). Additionally, we conducted a review of the literature and meta-analysis of 14 studies involving 303 patients to evaluate the efficacy of this surgical approach. A 63-year-old man experienced recurrent, lancinating pain in the V1, V2, and V3 branches of the right trigeminal nerve. Due to the intractable symptoms, h

A comprehensive review on schizophrenia: epidemiology, pathogenesis, diagnosis, conventional treatments, and proposed natural compounds used for management.

Schizophrenia (SCZ) is a complex, comprehensible mental condition that creates alienation from reality. SCZ is a mental disease, which is marked by progressive deficits in working memory, attention, and executive functioning. Because the disease's etiology is unknown, current psychotherapy and pharmacological treatments merely treat symptoms and do not provide a cure. SCZ symptoms include hallucinations, delusions, disorganized behavior, and lack of desire. Long-term use of antipsychotic (antagonists at multiple G-protein-coupled receptors) (mGPCR antagonists) medicines for therapy has negativ

Use of Point-of-care Testing Devices for Patients on Clozapine: A Literature Review.

Clozapine, an antipsychotic medication for treatment-resistant schizophrenia, remains underutilized due to its serious side effects and need for extensive hematologic monitoring. Recently, discussions have focused on the use of point-of-care testing (POCT) devices to monitor laboratory values, including the absolute neutrophil count (ANC) and clozapine concentration, in patients being treated with clozapine. This review assesses whether these devices could ease the monitoring burden on patients and providers without compromising the accuracy of results and subsequent patient safety. We found s

The Impact of Displacement on Skin-Related Neglected Tropical Diseases in Sudan.

Displacement due to conflict and environmental crises in Sudan has intensified the burden of neglected tropical diseases (NTDs), particularly those with dermatologic manifestations. This narrative review explores the relationship between displacement and skin-related NTDs in Sudan by synthesizing epidemiological data, health infrastructure challenges, social impacts, and public health responses. Data were sourced from peer-reviewed literature, WHO reports, and humanitarian briefs published between 2000 and 2024. Findings reveal that overcrowded living conditions, malnutrition, and disruption o

Raynaud's Phenomenon of the Nipple: Epidemiological, Clinical, Pathophysiological, and Therapeutic Characterization.

Raynaud's phenomenon of the nipple is a possible cause of pain and breastfeeding cessation in lactating women. However, there are still few studies on the characterization of this manifestation. Thus, we aim to develop a systematic review of the literature carried out between January 1992 and January 2024 in PubMed, Scopus, Web of Science, Virtual Health Library (VHL), and Portal de Periódicos da CAPES. Of the 438 articles, 19 met the eligibility criteria. The findings were divided by heuristic questions into two groups: "Epidemiological, pathophysiological, and clinical characterization of Ra

Anticholinergic Medication Burden and Cognitive Subtypes in Parkinson's Disease without Dementia.

OBJECTIVE: Cognitive changes are heterogeneous in Parkinson's disease (PD). This study compared whether anticholinergic burden drives differences in cognitive domain performance and empirically-derived PD-cognitive phenotypes. METHOD: A retrospective chart review contained participants (n = 493) who had idiopathic PD without dementia. Participants' medications were scored (0-3) and summed based on the anticholinergic cognitive burden scale (ACBS). We examined the ACBS' relationship to five cognitive domain composites (normative z-scores) and three (K-means clustering based) cognitive phenotyp

Emicizumab: the hemophilia A game-changer.

In hemophilia, the unmet needs regarding adherence to prophylaxis and lack of effective long-term prophylaxis regimens, especially in patients with inhibitors, led to the production of emicizumab, the first non-factor medicine for subcutaneous administration in patients with severe and moderate hemophilia A with or without factor VIII inhibitors. This review describes the research steps behind the development of this game-changing medication as well as its success in the prophylaxis of bleeding episodes, as witnessed by the results of pivotal clinical trials but also by real-life use in the fr

Therapeutic Outcomes of Treatment With Long-Acting Injectable Antipsychotics in Forensic Populations With Schizophrenia-Spectrum Diagnoses.

The advantages of long-acting injectable antipsychotics (LAIs) in schizophrenia are well studied. However, forensic involvement is common in schizophrenia, and incarcerated individuals are often excluded from clinical trials. Nonadherence and oral medication diversion in forensic populations with schizophrenia, and the relationship between antipsychotic nonadherence and crime support LAI utilization in this subset of patients. Yet, federal regulations limit data generation in forensic populations. This review characterizes data on therapeutic outcomes of LAIs in correctional populations with s

Mobile phone interventions to improve health outcomes among patients with chronic diseases: an umbrella review and evidence synthesis from 34 meta-analyses.

This umbrella review of 34 meta-analyses, representing 235 randomised controlled trials done across 52 countries and 48 957 participants and ten chronic conditions, aimed to evaluate evidence on the efficacy of mobile phone interventions for populations with chronic diseases. We evaluated the strengths of evidence via the Fusar-Poli and Radua methodology. Compared with usual care, mobile apps had convincing effects on glycated haemoglobin reduction among adults with type 2 diabetes (d=0·44). Highly suggestive effects were found for both text messages and apps on various outcomes, including med

Natural Products as Monoamine Oxidase Inhibitors: Potential Agents for Neurological Disorders.

The role of medicinal plants has been advantageous due to their manifestation through various cellular and molecular mechanisms. Inhibition of the monoamine oxidase enzyme is suspected to be a highly effective treatment for various neurological illnesses like Alzheimer's disease, Parkinson's disease, depression, social phobia, and panic disorders. The study of phytochemicals and plant extracts used as a traditional source of medication revealed that they possess the vast potential for monoamine oxidase inhibition. Thus, the article focuses on the potential use of plant extracts and phytochemic

Long-acting antiretroviral therapy in low-income and middle-income countries: considerations for roll-out.

PURPOSE OF REVIEW: Long-acting ART (LA-ART) in low-income and middle-income countries (LMICs) may address specific issues that affect people living with HIV (PWH) and people at substantial risk of HIV infection. We reviewed products in use and under consideration in LMICS, current and anticipated challenges for implementation, and offer strategies for effective rollout. RECENT FINDINGS: Factors to consider for effective implementation of LA-ART in LMICs are: managing co-conditions (pregnancy) and comorbidities (TB and hepatitis B); medication access, both cost and supply-related; and health s

Drug induced hypoprolactinemia.

Prolactin levels can be influenced by multiple medications primarily through the interaction with dopamine receptors which regulate its secretion. Unlike hyperprolactinemia which has a well-defined clinical phenotype, the effects of hypoprolactinemia beyond inability to lactate are incompletely understood. Recent studies have raised concerns regarding detrimental changes in glucose metabolism, sexual function and psychological profile in patients with low prolactin levels. In contrast with anatomic and genetic etiologies, drug-induced hypoprolactinemia is usually reversible after dose reductio

Therapeutic plasma exchange as an intervention for gemtuzumab ozogamicin impaired hemoglobin scavenging: A case and systematic review.

Gemtuzumab ozogamicin (GO) is a CD33 monoclonal antibody-drug conjugate currently in use to treat myeloid malignancies. A unique adverse effect of this medication is destruction of CD33 positive macrophages resulting in reduced clearance of free hemoglobin leading to grossly red plasma. This build-up of free hemoglobin can potentially lead to end organ damage and prevent performance of clinically necessary laboratory evaluation. We present a case of a pediatric patient who developed this adverse effect and was successfully treated with therapeutic plasma exchange (TPE). We also present results

Non-Infectious Uveitis and Pregnancy, is There an Optimal Treatment? Uveitis Course and Safety of Uveitis Treatment in Pregnancy.

In pregnancy, a plethora of factors causes changes in maternal immunity. Uveitis flare-ups are more frequent in the first trimester and in undertreated patients. Management of non-infectious uveitis during pregnancy remains understudied. A bibliographic review to consolidate existing evidence was performed by a multidisciplinary group of Ophthalmologists, Gynaecologists and Rheumatologists. Our group recommends initial management with minimum-required doses of corticosteroids, preferably locally, to treat intraocular inflammation whilst ensuring good neonatal outcomes. If ineffective, clinicia

Homeobox genes in the human placenta: Twists and turns on the path to find novel targets.

Fetal growth restriction (FGR) is a clinically important human pregnancy disorder that is thought to originate early in pregnancy and while its aetiology is not well understood, the disorder is associated with placental insufficiency. Currently treatment for FGR is limited by increased surveillance using ultrasound monitoring and premature delivery, or corticosteroid medication in the third trimester to prolong pregnancy. There is a pressing need for novel strategies to detect and treat FGR at its early stage. Homeobox genes are well established as master regulators of early embryonic developm

Native nephrectomy in polycystic kidney disease patients on transplant lists: how and when?

Autosomal dominant polycystic kidney disease (ADPKD), the most common hereditary kidney disease, accounts for approximately 10% of the patients on kidney transplantation waitlists. High rates of complications including hemorrhage, infections, nephrolithiasis and kidney size-related compressive complaints have been reported among ADPKD patients. Therefore, the need for routine native nephrectomy and timing of such procedure in ADPKD patients being prepared for transplantation are debated. Even though pre-transplant nephrectomy has the potential to provide fewer infectious complications due to l
