Giant Ascending Aortic Aneurysm With Atypical Presentation as Abdominal Pain.

BACKGROUND: Ascending aortic aneurysms typically present with chest or back pain; however, atypical presentations may delay diagnosis. CASE SUMMARY: A 58-year-old woman presented with acute lower abdominal pain and diaphoresis. Hypertension and a new diastolic murmur prompted cardiology evaluation, revealing clinical signs of chronic severe aortic regurgitation. Imaging confirmed a giant ascending aortic aneurysm (89 × 83 mm) with severe aortic regurgitation. DISCUSSION: This case emphasizes atypical presentations of thoracic aortic disease, compensated high-risk anatomy, and timely referral

Atypical Atrial Flutter Unveiling Cor Triatriatum Sinister in a Stable Adult.

BACKGROUND: Cor triatriatum sinister (CTS) is a rare congenital anomaly, infrequently identified in asymptomatic adults. CASE SUMMARY: A 64-year-old man presented with asymptomatic atrial flutter detected on routine electrocardiogram. Rate control was suboptimal, and because of unknown arrhythmia duration, transesophageal echocardiography (TEE) was performed before cardioversion. TEE excluded intracardiac thrombus and incidentally revealed a left atrial membrane. Cardiac magnetic resonance imaging confirmed nonobstructive CTS. The patient underwent successful TEE-guided electrical cardioversi

Acral Fibrochondromyxoid Tumor: Diagnostic Challenges and Multimodal Characterization in a Hand Mass Case.

Acral fibrochondromyxoid tumor (AFCMT) is a rare and recently characterized matrix-rich soft tissue occurring on acral sites. Here, we conducted a multi-dimensional analysis of AFCMT that occurred in the hands from clinical, image, pathological, and molecular perspectives, and discussed its clinicopathological features and diagnostic strategies. In general, the hand appears as a clearly demarcated lump. In the image, scattered calcifications can be observed, along with the proliferation of mucinous cartilage matrix. Microscopically, pale eosinophilic chondrocyte-like cells are suspended within

Fulminant Myocarditis With Normal Inflammatory Markers.

BACKGROUND: Fulminant myocarditis is a rare, rapidly progressive and life-threatening condition requiring early diagnosis and management. CASE SUMMARY: We present the case of a 31-year-old man who presented with sudden onset chest pain; though his initial echocardiography was largely unremarkable, and angiography ruled out acute coronary syndrome, the patient developed worsening pain with rising high-sensitivity troponin markers. Cardiac magnetic resonance imaging revealed widespread transmural late gadolinium enhancement throughout the left ventricle, which prompted urgent transfer to a quat

Real-Time Ultrasound Diagnosis of Developmental Dysplasia of the Hip Using an Attention-Enhanced YOLOv11 Model.

Developmental dysplasia of the hip (DDH) is a common pediatric orthopedic disorder that can lead to lifelong disability if undetected. Ultrasound is the primary diagnostic modality but is subject to operator dependence and inter-observer variability. To address this challenge, we propose an attention-enhanced YOLOv11 framework for automated DDH classification. A dataset of 6,075 hip ultrasound images was preprocessed with augmentation and dimensionality reduction via UMAP. The model integrates Cross-Stage Partial (CSP) modules and C2PSA spatial attention to improve feature extraction, and was

Breaking the Barriers: Transseptal Puncture in Patients with Atrial Septal Defect Closure Devices.

Atrial septal defect and patent foramen ovale are common congenital heart defects in adults, often treated with atrial septal occluders (ASOs) via percutaneous closure. Transseptal puncture (TSP) for left atrial interventions in patients with ASOs presents procedural challenges, requiring detailed pre-procedural planning and imaging. Transoesophageal echocardiography and cardiac CT help identify optimal puncture sites, either through the native septum or across the ASO. Advanced techniques, such as balloon-assisted tracking and radiofrequency energy delivery, aid in navigating the device. TSP

Management of Mechanical Prosthetic Heart Valve Thrombosis: JACC Review Topic of the Week.

Mechanical prosthetic heart valves, though more durable than bioprostheses, are more thrombogenic and require lifelong anticoagulation. Mechanical valve dysfunction can be caused by 4 main phenomena: 1) thrombosis; 2) fibrotic pannus ingrowth; 3) degeneration; and 4) endocarditis. Mechanical valve thrombosis (MVT) is a known complication with clinical presentation ranging from incidental imaging finding to cardiogenic shock. Thus, a high index of suspicion and expedited evaluation are essential. Multimodality imaging, including echocardiography, cine-fluoroscopy, and computed tomography, is co

Tricuspid valve endocarditis: Cardiovascular imaging evaluation and management.

Right-sided infective endocarditis is an increasingly recognized disease entity, with tricuspid valve being most frequently involved. Risk factors for tricuspid valve endocarditis (TVIE) include intravenous drug use, cardiac implantable electronic devices and indwelling catheters. Staphylococcus aureus is the predominant causative organism in TVIE. The diagnosis of infective endocarditis (IE) is based on clinical manifestations, blood cultures, and the presence of valvular vegetations detected by echocardiography. Complementary imaging is helpful when there is ongoing clinical suspicion for IE

Diabesity: the combined burden of obesity and diabetes on heart disease and the role of imaging.

Diabesity is a term used to describe the combined adverse health effects of obesity and diabetes mellitus. The worldwide dual epidemic of obesity and type 2 diabetes is an important public health issue. Projections estimate a sixfold increase in the number of adults with obesity in 40 years and an increase in the number of individuals with diabetes to 642 million by 2040. Increased adiposity is the strongest risk factor for developing diabetes. Early detection of the effects of diabesity on the cardiovascular system would enable the optimal implementation of effective therapies that prevent at

Multimodal evaluation and etiology-oriented management of chronic intracranial venous hypertension.

Chronic intracranial venous hypertension (CIVHS) comprises a heterogeneous group of disorders characterized by impaired cerebral venous outflow and symptoms related to elevated intracranial pressure. Although venous sinus stenting is increasingly used for venous sinus stenosis with elevated trans-stenotic pressure gradients, venous sinus stenosis is an imaging phenotype rather than a single diagnosis. We retrospectively reviewed 13 consecutive patients with CIVHS who underwent clinical, imaging, and hemodynamic evaluation, including conventional neuroimaging, MR or CT venography, digital subtr

[18F]FDG PET/MRI for axillary staging in node positive breast cancer patients receiving primary systemic therapy. A prospective single arm trial.

BACKGROUND: Management of axillary nodes in breast cancer (BC) remains controversial. Increasingly, node-positive patients receive primary systemic therapy (PST), achieving axillary downstaging in up to 70% of cases. However, current imaging methods, despite being pivotal in the choice for PST, show limited accuracy in assessing treatment response. This study evaluated the diagnostic performance of hybrid [18F]FDG PET/MRI as non-invasive, single-step tool for post-PST nodal assessment. METHODS: This prospective, single-arm, monocentric interventional trial (SNBvsPET/MRI1, ClinicalTrials.gov N

Criminal Behavior in Frontotemporal Dementia: A Multimodal MRI Study.

The behavioral variant of frontotemporal dementia (bvFTD) is related to a variety of social misbehaviors, including criminal behavior (CB) due to deep changes in cognition, behavior, and personality. Recent work suggests that impairment in emotional processing, along with disinhibition, constitutes the necessary elements for CB in bvFTD. However, the underlying neurobiological mechanisms are still unclear. Therefore, we aim at investigating structural and functional brain changes related to CB in bvFTD using magnetic resonance imaging (MRI) with the German Consortium for Frontotemporal Lobar D

18 F-FDG PET/CT and 68 Ga-DOTA-NOC PET/MRI in an Adolescent With Ectopic Adrenocorticotropic Hormone Syndrome From a Presacral Teratoma.

A 13-year-old girl with severe Cushingoid features had ectopic ACTH-dependent Cushing syndrome. 18 F-FDG PET/CT demonstrated diffusely FDG-avid bilateral adrenal hyperplasia, confirming ectopic ACTH stimulation, and detected a presacral mass with mild uptake. 68 Ga-DOTA-NOC PET/MRI revealed intense somatostatin-receptor activity (SUVmax: 56.1), and MRI identified fat and cystic elements, suggesting teratoma. Surgery proved a mature teratoma harboring an ACTH-secreting NET (G1). This rare case demonstrates that in pediatric patients with aggressive-appearing presacral masses and life-threatenin

Integrating clinical features with 11C-methionine PET-MRI radiomics for non-invasive prediction of H3 K27M mutation in brainstem gliomas.

BACKGROUND: Accurate determination of H3 K27M mutation status is critical for prognosis evaluation and treatment planning in brainstem gliomas (BSGs). We developed a clinical-radiomics model integrating 11C-methionine (11C-MET) positron emission tomography (PET), magnetic resonance imaging (MRI), and clinical features for non-invasive, preoperative prediction of H3 K27M mutation status in BSGs. METHODS: This retrospective study included 77 patients with newly diagnosed BSG who underwent preoperative 11C-MET PET imaging and MRI. Among them, 53 had a histologically confirmed H3 K27M mutation. Diagnostic performance of simultaneous PET-MR versus PET-CT in oncology with an overview on clinical utility and referral pattern of PET-MR: a single institutional study.

BACKGROUND: PET-Magnetic Resonance (PET-MR) imaging is an upcoming investigative modality with a few installations in Asia and only three in India. PET-Computed Tomography (PET-CT) is an established diagnostic cornerstone for oncological indications but with limited resolution for small lesions due to low soft-tissue contrast and additional radiation exposure. OBJECTIVE: Our primary objective was to evaluate the diagnostic performance of simultaneous PET-MR and PET-CT in lesion detection in oncological practice. Secondly to assess the referral pattern and study the clinical utility of PET-MR

A PET/MRI study on the effect of obesity and NAFLD on hepatic [18F]FDG uptake.

PURPOSE: The potential limitations of hepatic [18F]FDG-PET imaging for individuals with obesity and excessive liver fat (NAFLD) are being investigated. In this study, we aim to determine the reliability of standardized uptake values (SUVs) focusing on adjustment for liver fat content (LFC) derived from DIXON images and the effects of whole-body normalizations. METHODS: Lean and with obesity volunteers who underwent [18F]FDG-PET/MRI were reviewed retrospectively. DIXON fat images were used to determine LFC and for adjustment of SUVmean. The hepatic SUVs (mean, fat adjusted mean and max) were n
