[Brachyonychia of different types in a mother and daughter].

Different types of brachyonychia were observed in a mother and her daughter. The mother presented a unilateral racket thumb-nail (localized brachyonychia "en raquette" of Basset). In the case of the daughter the thumbs and three middle fingers of both hands were involved, the nails of the little fingers being normal (generalized brachyonychia "en raquette" of Basset). Also, the daughter's toe-nails were short and wide. There was a family history of short nails "on all fingers" occurring in the two sisters of the mother's father. We believe that localized and generalized types of racket nails a

Modern trends in internal fixation of femoral shaft fractures in children. A critical review.

Elastic intramedullary (IM) devices, which avoid the physeal region like Nancy and Ender nails do, can be used in children younger than even 4 years without compromising the trochanter physis or risking an avascular necrosis. Intramedullary fixation of femoral shaft fractures leads to satisfactory results of alignment and union. Prerequisites for intramedullary closed nailing are a traction table, especially for older children and adolescents, and an image intensifer. Interlocking IM, Ender, and Nancy nails all have the equal advantage of early weight bearing and reduced hospitalization time c

Extracorporal noninvasive acute retraction of STRYDE® for continued lengthening in cases with limited nail stroke: a technical less invasive solution to reload the STRYDE®.

With STRYDE® nails (NuVasive Specialized Orthopedics, San Diego, CA), lengthening nails for full weight-bearing during callus distraction have been available CE-certified since February 2019 in Europe. At present only antegrade femur nails and tibia nails with various lengths and diameters are available. Due to a mismatch of bone length and realizable (implantable) nail length e.g., in cases of skeletal dysplasia or achondroplastic dwarfism, it may happen that a desired lengthening of 6-8 cm cannot be realized because of a nail stroke from only 5 cm. Retrograde usage of antegrade piriformis na

Combination therapy for onychomycosis using a fractional 2940-nm Er:YAG laser and 5 % amorolfine lacquer.

Onychomycosis remains difficult to cure by traditional methods. The aim of this study was to evaluate the efficacy of combination therapy with a fractional erbium yttrium aluminum garnet (Er:YAG) laser and 5 % amorolfine lacquer on onychomycosis. Nine patients with bilateral nails affected by distal and lateral subungual onychomycosis were included. The bilateral nails of each patient were divided into two groups. The 20 affected nails on one side of each patient as group 1 were treated with a fractional Er:YAG laser once a week and 5 % amorolfine lacquer twice weekly, while the 20 nails on th

Retrospective Cohort Study Comparing Rates of Symptomatic Implants Requiring Removal between Standard versus Low-Profile Tibial Nail Interlocking Screws.

This study aimed to compare the rates of symptomatic implants requiring removal between intramedullary tibia nails using standard (SD) versus low-profile (LP) screws. We hypothesize that LP screws will demonstrate lower rates of symptomatic screw removal compared with SD interlocking screws with no difference in secondary outcomes. We performed a retrospective cohort study of all patients 18-89 years old who underwent IMN of tibia fractures from November 2018 to February 2023 at a single, academic level one trauma center. The primary outcome analyzed was symptomatic screws requiring removal. S

Comparative clinical outcomes of suprapatellar intramedullary nailing vs. minimally invasive plate osteosynthesis fixation for distal tibial fractures: a retrospective cohort study.

INTRODUCTION: Distal tibial fractures are challenging injuries due to limited soft tissue coverage and complex biomechanics. Suprapatellar intramedullary nailing (IMN) and minimally invasive plate osteosynthesis (MIPO) are widely used; however, their comparative effectiveness remains controversial. This study compared perioperative parameters, functional outcomes, radiographic results, and complications to inform surgical decision-making. METHODS: This retrospective cohort study included 70 patients with distal one-third tibial fractures treated with suprapatellar IMN (n = 35) or MIPO (n = 35

Revision and postoperative complication rates of conversion total hip arthroplasty after cephalomedullary nailing of intertrochanteric femur fractures: a systematic review and meta-analysis.

PURPOSE: Management of failed intertrochanteric (IT) fracture fixation often requires conversion total hip arthroplasty (cTHA), but complication and reoperation rates remain poorly characterized in the literature. This study aimed to characterize revision and postoperative complication rates for cTHA after cephalomedullary nailing of IT fractures. METHODS: A systematic review and meta-analysis was conducted following PRISMA guidelines. PubMed and Embase were queried for studies examining cTHA outcomes after prior cephalomedullary nailing for IT fractures. The primary outcome was revision rate,

Impact of limited intramedullary reaming versus unreamed technique in short proximal femoral nail anti-rotation for intertrochanteric fractures: a matched retrospective study.

PURPOSE: Short proximal femoral nail antirotation (PFNA) fixation is commonly performed for intertrochanteric fractures and is typically inserted without reaming. However, limited reaming may be required in cases of a narrow femoral canal, severe femoral bowing, or inadvertent selection of an oversized nail. Unlike long nails, reaming for short PFNA is confined to the isthmus, and its effects have not been well described. This study aimed to evaluate the association between limited reaming during short PFNA fixation and clinical outcomes. METHODS: We conducted a retrospective matched cohort s

Finite element analysis of long proximal femoral bionic nail (PFBN) fixation for femoral shaft fractures combined with ipsilateral femoral neck fractures.

OBJECTIVE: Femoral reconstruction nails (RN) and three cannulated screws combined with a plate (TCP) are commonly employed for the treatment of ipsilateral femoral neck and shaft fractures (IFNSF). However, these fixation strategies are associated with a considerable incidence of postoperative complications, reflecting the biomechanical challenges of this fracture pattern. Therefore, this study aims to compare the biomechanical performance of a novel long proximal femoral bionic nail (PFBN) with these conventional fixation methods for the treatment of this complex fracture pattern. METHODS: C

Retrograde intramedullary nailing versus locked plate fixation for distal femur fractures: A systematic review and meta-analysis of randomised controlled trials only.

BACKGROUND: The optimal fixation method for distal femur fractures remains contentious. Retrograde intramedullary nailing (RIN) and locking plate fixation (PLATE) are the two principal surgical options, yet their comparative effectiveness has not been definitively established. This systematic review and meta-analysis aimed to comprehensively compare the operative, radiological, and functional outcomes of RIN versus PLATE for distal femur fractures exclusively from randomised controlled trials (RCTs). METHODS: A systematic search of MEDLINE, Embase, Scopus and Web of Science was performed from

Tibiotalocalcaneal arthrodesis using a retrograde intramedullary nail and augmented allograft after failed total ankle replacement: an exploratory retrospective matched cohort study.

INTRODUCTION: The high 10-year extraction rate of failed second- and third-generation total ankle replacements (TARs) has led to an increasing prevalence of revision procedures with substantial bone defect. In this context, revision technique utilizing tibiotalocalcaneal arthrodesis (TTCA) with massive bone allograft and autograft is a salvage procedure, providing variable outcomes that need to be compared to those after primary TTCA. This study compared fusion rates, reoperations, and clinical outcomes of two cohorts of TTCA. MATERIAL AND METHODS: A retrospective single-center cohort of pati

Serial comparison of a predefined ultrasonographic cortical bridging appearance and conventional radiography in tibial fractures after intramedullary nailing: A prospective comparative cohort study.

BACKGROUND: Conventional radiography is the standard method for monitoring tibial fracture healing, but radiographic mineralization may be inconspicuous when earlier sonographic changes are detectable. This study compared serial ultrasonographic and conventional radiographic detection of a predefined cortical bridging finding using an identical four-cortex threshold and assessed interobserver reliability of the Radiographic Union Score for Tibial fractures (RUST). MATERIALS AND METHODS: This prospective comparative cohort study included 269 patients with tibial fractures treated with intramed

Endovascular Stent Grafting for Iatrogenic Deep Femoral Artery Injury After Short Femoral Nailing: A Case Report With a Three-Year Follow-Up.

Injury to the deep femoral artery (DFA) is a rare but potentially life-threatening complication of intertrochanteric femoral fracture surgery. We describe a case in which this complication occurred during drilling for the distal screw insertion of a short femoral nail. A 4 cm pseudoaneurysm was treated with stent grafting. Hemostasis was promptly achieved, and no recurrence of the aneurysm was observed over three years. The stent remained intact without fracture or migration. Although the stent lumen became nearly occluded during follow-up, the patient remained asymptomatic without signs of lo

High Fixation Failure Rate of Cephalomedullary Nail Fixation in Patients with Low-Energy Basicervical Femoral Fractures: Do We Need Extramedullary Reduction?

Background and Objectives: A basicervical femoral fracture is a relatively uncommon type of proximal femoral fracture. However, as the proportion of proximal femoral fractures rises in conjunction with the aging of society, the absolute number of patients with basicervical femoral fractures is also increasing. Nevertheless, the optimal surgical methods for the treatment of basicervical femoral fractures remain a topic of debate. The aim of this study is to evaluate the failure rates of cephalomedullary nail fixation in basicervical femoral fractures based on reduction types. Methods: A retrosp

Comparative Study Between Minimally Invasive Plate Osteosynthesis and Distal Tip Locking Tibial Nailing in the Treatment of Lower Third Tibial Shaft Fractures.

BACKGROUND: Distal third tibial shaft fractures are challenging due to limited soft tissue coverage and poor vascularity. We compared minimally invasive plate osteosynthesis (MIPO) with distal tip-locking intramedullary nailing. METHODS: A prospective two-center cohort of 40 adults with extra-articular distal third tibial fractures was allocated to MIPO (n = 20) or intramedullary nailing (IMN) (n = 20). Primary outcomes were time to union and six-month functional results (AOFAS). Secondary outcomes included infections, malalignment, and secondary procedures. RESULTS: Mean age was similar (MI

Comparison of mechanical and clinical outcomes between cement-augmented and conventional cephalomedullary nailing in osteoporotic trochanteric fractures: a propensity score-matched cohort study.

BACKGROUND: Trochanteric fractures in the older population are challenging to treat due to osteoporotic bone and high risk of fixation failure. Cement augmentation (CA) of cephalomedullary fixation has been proposed to enhance implant anchorage and reduce complications. This study compared mechanical failure rates and clinical outcomes between cement-augmented and conventional cephalomedullary nailing in osteoporotic trochanteric fractures. METHODS: We performed a retrospective comparative study of patients with trochanteric fractures treated with either a CA or non-CA cephalomedullary nail f

The effects of surgeon handedness on the efficacy and safety of proximal femoral nail antirotation fixation for intertrochanteric femur fractures in elderly patients: A single Center's experience.

BACKGROUND: Handedness refers to the lateral preference of using one hand more than the other. Surgeon handedness has been widely discussed in operative surgery and could cause clinical discrepancy. However, in the use of proximal femoral nail antirotation (PFNA) for the treatment of intertrochanteric femur fracture (IFF), the effect of handedness on clinical outcomes is easily overlooked. In fact, when right-handed surgeons operate on right-sided IFF patients, due to the specific nature of the surgical site, they have to use their left hand to perform the opening at the proximal end of the fe
