High rate of Simkania negevensis among Canadian inuit infants hospitalized with lower respiratory tract infections.

To determine the prevalence of Simkania negevensis in causing pulmonary infections in children, nasopharyngeal washes were obtained from 22 infants hospitalized with acute bronchiolitis in the Baffin Island, Canada. 14 (63.6%) were positive for S. negevensis. Mixed infections with other respiratory viruses were common. All patients recovered without specific antibiotic treatment. Even though a high prevalence of S. negevensis was found, this organism may potentially well be an opportunistic agent rather than a true pathogen.

Upper respiratory infection: helpful steps for physicians.

Upper respiratory infections (URIs) are extremely common in adults. Most adults have multiple URIs each year, with athletes being more or less susceptible, depending on training intensity. Most URIs are viral nasopharyngeal infections that will improve in 7 to 10 days with symptomatic treatment and result in little time lost from athletic participation. Physicians must recognize the symptoms consistent with bacterial pharyngitis, infectious mononucleosis, and sinusitis to provide appropriate diagnostic testing, treatment, and return-to-play recommendations.

Adult diseases in children.

The classic teaching in performance and interpretation of diagnostic studies is that "children are not small adults." The purpose of this article is to show a spectrum of "adult" disease entities that can occur, but are not usually considered in the pediatric population: cricopharyngeal achalasia, Schatzki's ring, achalasia, Helicobacter pylori, pancreatic carcinoma, adenocarcinoma of the colon, nasopharyngeal carcinoma, thyroid carcinoma, malignant melanoma, hydatidiform mole, renal cell carcinoma, leiomyosarcoma of the ovary and sarcoidosis. Radiologists interpreting pediatric imaging should

[Epstein-Barr virus and cellular immortalization].

Epstein-Barr virus has been associated to several forms of neoplasia including Burkitt's lymphoma, B lymphomas in immuno-compromised patients and undifferentiated nasopharyngeal carcinoma. Immunodepression, genetic and/or environmental factors and the expression of several viral genes (latent genes mainly) may contribute to these pathologies. In vitro, several latent proteins (EBNA 1, 2, 3A, 3C, 5 et LMP-1) directly or indirectly contribute to the initiation and maintenance of the transformation process. The role of these proteins is discussed in the present article.

Massive lingual swelling following palatoplasty. An unusual cause of upper airway obstruction.

We report a case of upper airway obstruction as a result of delayed massive lingual swelling following routine cleft palate repair in an otherwise healthy 12-month-old girl. We believe that ischaemia and venous congestion were the causes of macroglossia, after prolonged use of the Digman Dott tongue retractor. In any difficult and lengthy repair, we recommend the prophylactic insertion of a nasopharyngeal airway under direct vision by the surgeons after surgery to prevent potential upper airway obstruction.

Respiratory syncytial virus (RSV) bronchiolitis: comparative study of RSV groups A and B infected children.

The grouping characteristics of 29 respiratory syncytial virus (RSV) present in nasopharyngeal cells collected from hospitalized children with bronchiolitis during the 1990 RSV season in Porto Alegre, RS, were analysed. Twenty-two were grouped as belonging to group A and 7 to group B. Cyanosis, oxygen therapy, cough, length of hospitalization and atelectasis were observed to be more frequently found within group B infected children. Other clinical signs and symptoms were similarly found in both groups.

CT-based radiotherapy treatment planning for cancer of the nasopharynx.

We analysed 32 cases of nasopharyngeal carcinoma (NPC) treated with radiotherapy trying to define the role of CT scan during radiotherapy planning, as well as radiation treatment and the follow-up of the patients. CT scan was found to help 15 cases with low-parapharyngeal involvement and/or bone erosion. Persistent abnormal CT findings at 2 mo after the end of radiation treatment were related to a 30% local recurrence rate. This rate went up to 100% in case of skull-base destruction.

Radiation-induced cranial nerve palsy: hypoglossal nerve and vocal cord palsies.

Cranial nerve palsies are an unexpected complication of radiotherapy for head and neck tumours. We present a case of this radiation-induced cranial palsy. An 18-year-old female with nasopharyngeal carcinoma developed a right hypoglossal nerve palsy 42 months after cancericidal doses of radiotherapy. In addition, she developed a bilateral vocal cord palsy 62 months after therapy. Follow-up over four years has demonstrated no evidence of tumour recurrence and no sign of neurological improvement.

[Sphenoidal sinus mucocele. Diagnosis and endoscopic surgery].

Two cases of sphenoid sinus mucocele are described. Diagnosis was based on insidious orbital symptoms, apex orbital syndrome, followed by CT and MRI examinations. In one case the mucocele was preceded by seven years of a successfully treated undifferentiated nasopharyngeal carcinoma. Simple transnasal endoscopic drainage of the mucoceles gave good results, although a monocular blindness of long standing was irreversible. A high level of suspicion, combined with modern imaging techniques and prompt surgical intervention, are the prerequisites for avoiding persisting sequelae.

[The management of pharyngeal cancer from the surgical viewpoint].

From experiences in Rostock nasopharyngeal carcinomas are a domain of radiotherapy, whereas surgical measures can be used in lymph node metastases additionally. Carcinomas of the oropharynx need a balanced radiosurgical combination therapy, which has been done by sandwich-procedure in Rostock for 10 years. The value of an additional chemo- or immunotherapy needs further securing yet, as generally a prospective randomized study is suggested for all the country. Carcinomas of hypopharynx are treated in combination by operation of different dimension and after-irradiation.

Corynebacterium diphtheriae in the environment of carriers and patients.

A total of 533 throat and nasopharyngeal cultures were obtained from 328 staff who nursed cases of diphtheria and carriers of Corynebacterium diphtheriae; none yielded C. diphtheriae. Only one out of 189 environmental samples relating to 22 patients and carriers was positive. In addition, one carrier with chronic skin disease was found to have widespread colonization with C. diphtheriae. Several samples both from his hospital room and home yielded C. diphtheriae, indicating that he was a 'disperser.'

Surgically correctable causes of sleep apnea syndrome.

This report presents six cases of obstructive sleep apnea associated with discrete anatomic abnormalities of the upper airway tract. All patients were relieved of their sleep apnea by surgical correction of the airway obstruction, and therefore permanent tracheotomy was avoided. The obstructive causative factors were nasal septal deformity, adenoidal hypertrophy, nasopharyngeal stenosis, lingual cyst, lingual tonsillar hypertrophy, and laryngeal cyst. The last-named four conditions have not previously been reported as correctable causes of sleep apnea.

Upregulation of IQGAP2 by EBV transactivator Rta and its influence on EBV life cycle.

Epstein-Barr virus (EBV) is a human oncogenic γ-herpesvirus that establishes persistent infection in more than 90% of the world's population. EBV has two life cycles, latency and lytic replication. Reactivation of EBV from latency to the lytic cycle is initiated and controlled by two viral immediate-early transcription factors, Zta and Rta, encoded by BZLF1 and BRLF1, respectively. In this study, we found that IQGAP2 expression was elevated in EBV-infected B cells and identified Rta as a viral gene responsible for the IQGAP2 upregulation in both B cells and nasopharyngeal carcinoma cell lines.

Carboxy-terminal sequence variation of LMP1 gene in Epstein-Barr-virus-associated mononucleosis and tumors from Serbian patients.

Seven strains of Epstein-Barr virus (EBV) are defined based on C-terminal sequence variations of the latent membrane protein 1 (LMP1). Some strains, especially those with a 30-bp deletion, are thought to be related to tumorigenic activity and geographical localization. The aims of the study were to determine the prevalence of different LMP1 strains and to investigate sequence variation in the C-terminal region of LMP1 in Serbian isolates. This study included 53 EBV-DNA-positive plasma and tissue block samples from patients with mononucleosis syndrome, renal transplantation, and tumors, mostly

Transverse maxillary dimensions and upper airway morphology in mouth- and nasal-breathing children aged 10-12 years: A CBCT-based study.

INTRODUCTION: Mouth breathing (MB) is a dysfunctional respiratory pattern that may affect craniofacial development by altering maxillary arch width and upper pharyngeal airway morphology. Early identification is critical to prevent long-term dentofacial and airway complications. This study aimed to compare maxillary arch width and upper airway morphology between mouth- and nasal-breathing children aged 10-12 years using Cone Beam Computed Tomography (CBCT). MATERIALS AND METHODS: In this cross-sectional study, 30 children (15 mouth breathers and 15 nasal breathers) underwent CBCT imaging. Tra

A preliminary evaluation of a fast, low-cost, and high-throughput nucleic acid extraction method for bacterial microbiota profiling in low-microbial biomass samples.

The respiratory tract is colonized with low-density microbial communities, which have been shown to impact human respiratory health through microbiota-host interactions. However, a lack of fast and cost-effective nucleic acid extraction method for low-microbial biomass samples hinders investigation of respiratory microbiota. Here, we performed a pilot study to assess the suitability of the NAxtra nucleic acid extraction protocol for profiling bacterial microbiota in respiratory samples. A small number of nasopharyngeal aspirate (n = 8), nasal swab (n = 8), and saliva samples (n = 8) were colle
