Computed tomography angiography-guided conservative management of a penetrating ballistic neck injury involving the carotid sheath: A case report.

Penetrating ballistic neck injury carries a high risk of vascular injury, particularly to the common carotid artery (CCA), due to its superficial location and critical role in cerebral perfusion. While surgical intervention is indicated in the presence of "hard signs" of vascular injury, the role of conservative management in asymptomatic patients remains debated. We report the case of a 45-year-old male who sustained a penetrating ballistic injury while seated and cleaning his 5-shot hunting rifle (shotgun). The accidental discharge occurred at less than 1 meter, producing a short-range, low-

Transoral robotic thyroidectomy using Versius platform: a preclinical feasibility study in cadavers.

Remote-access thyroidectomy techniques have been developed to address esthetic concerns related to cervical scarring following conventional thyroidectomy. Transoral robotic thyroidectomy offers a scarless vestibular access combined with the technological advantages of robotic systems. To date, only the Da Vinci platform has been clinically utilized for this procedure. The Versius robotic system is a new robotic platform under development for head and neck surgery. This study aims to evaluate the feasibility and safety of performing transoral thyroidectomy using the Versius robotic system in ca

Patterns of lymph node metastasis and treatment outcomes of parotid gland malignancies.

BACKGROUND: This study aimed to characterize the pattern of cervical lymph node spread and evaluate prognostic factors and outcomes of surgery and postoperative adjuvant therapy in primary parotid carcinoma (PPC). METHODS: We retrospectively enrolled 136 patients with PPC. The primary outcomes were disease-free survival (DFS) and overall survival (OS). We used Cox hazards models to assess variables influencing survival, and chi-square tests and logistic regression models to evaluate correlations between pN + and clinicopathological factors. RESULTS: Pathology-confirmed lymph node metastasis

Cancer-Specific Mortality of Partial Cystectomy Versus Radical Cystectomy in T2N0M0 Urothelial Bladder Cancer.

BACKGROUND: The authors hypothesized that for T2N0M0 urothelial carcinoma of urinary bladder (UCUB) patients, partial cystectomy (PC) does not differ from radical cystectomy (RC) regarding cancer-specific mortality (CSM). METHODS: Within the Surveillance, Epidemiology and End Results (SEER) database (2004-2021), rates of PC versus RC for T2N0M0 UCUB patients were tabulated. Nearest-neighbor 1:1 propensity score-matching (PSM) for age, sex, tumor size and site, race/ethnicity, chemotherapy, and pelvic lymph node dissection status was applied. Frozen Section Analysis in Submandibular Gland Tumors: Optimizing Intraoperative Decision-Making.

Introduction: With around 25 different salivary gland tumor entities described by the World Health Organization, the correct preoperative identification of masses as benign or malignant remains a challenge. If preoperative needle biopsy is inconclusive, frozen section analysis is a possible alternative for accurate histological identification. The purpose of our study was to evaluate the diagnostic effectiveness of frozen section performed for primary submandibular gland masses. Methods: In addition to acquiring epidemiological data from patients who underwent submandibular gland excision over

Cervicothoracic lipoma in a child: A case report.

INTRODUCTION: Lipomas are typically slow-growing tumors with the highest incidence in the fourth through sixth decades of life, less commonly occurring in pediatric patients, particularly extending across multiple body compartments. Excision is often reserved for cases that cause cosmetic or compressive symptoms and, due to the slow growth pattern, is less likely to be necessary in younger patients. CASE PRESENTATION: A 2-year-old female with congenital albinism presented with a painless but visible 3 × 4 cm non-mobile left lateral neck mass that had been present for 2 months. MRI without con

External carotid artery ligation in the management of carotid body tumors: A case report.

INTRODUCTION AND IMPORTANCE: Carotid body tumors (CBTs) are rare paragangliomas located at the bifurcation of the carotid artery. These tumors present significant clinical challenges due to their potential for local extension, high vascularity, and proximity to critical neurovascular structures. The rarity of complete resections requiring external carotid artery ligation (ECAL) underscores the clinical significance of this case, highlighting both the technical challenge and the functional outcome. CASE PRESENTATION: We report a case of a CBT in a middle-aged patient presenting with a progress

Tumor-width-to-tongue-width ratio: A novel predictor of cervical lymph node metastasis and prognosis in squamous cell carcinoma of the tongue.

OBJECTIVES: Squamous cell carcinoma of the tongue (SCCOT) with midline involvement has a higher risk of contralateral cervical metastasis. The aim of this study is to investigate the association between the tumor-width-to-tongue-width ratio (TWR) and unilateral/ contralateral cervical metastasis and prognosis in SCCOT. MATERIALS & METHODS: 221 patients with SCCOT from 2002 to 2025 were enrolled. Eligible patients underwent magnetic resonance imaging and primary surgical treatment. The TWR was measured and its predictive value for cervical metastasis was analyzed using logistic regression. Sur

Are high-dose CT examinations on the rise?

OBJECTIVE: To analyse high-dose CT examinations (≥50 mSv) over a 10-year period to study temporal changes and identify causes. METHODS: CT dose provided by the dose management system (Radimetrics) was assessed for the years 2013-2022. CT protocols that led to ≥50 mSv exams were identified. Using body mass index (BMI), patients were classified into underweight, normal weight, overweight, and obese, and correlation with dose was studied. Stratified analysis was performed for diagnostic and interventional examinations according to procedure type, patient age, and protocol. RESULTS: Over 10 year

Predicting nodal metastasis progression of oral tongue cancer using a hidden Markov model in MRI.

OBJECTIVES: The presence of occult nodal metastases in patients with oral tongue squamous cell carcinomas (OTSCCs) has implications for treatment. More than 30% of patients will have occult nodal metastases, yet a considerable number of patients undergo unnecessary invasive neck dissection to confirm nodal status. In this work, we propose a probabilistic model for lymphatic metastatic spread that can quantify the risk of microscopic involvement at the lymph node level (LNL) given the location of macroscopic metastases and the tumor stage using the MRI method. MATERIALS AND METHODS: A total of

University of Washington Quality of Life subdomain outcomes after treatment of sinonasal malignancy: A prospective, multicenter study.

PURPOSE: Sinonasal malignancies (SNMs) adversely impact patients' quality of life (QOL) and are frequently identified at an advanced stage. Because these tumors are rare, there are few studies that examine the specific QOL areas that are impacted. This knowledge would help improve the care of these patients. METHODS: In this prospective, multi-institutional study, 273 patients with SNMs who underwent definitive treatment with curative intent were evaluated. We used the University of Washington Quality of Life (UWQOL) instrument over 5 years from diagnosis to identify demographic, treatment, a

Predictive Factors of Athyroglobulinemia After Total Thyroidectomy for Papillary Thyroid Cancer.

Background: Thyroglobulin (Tg) is the specific tumor marker for epithelial thyroid cancer. It holds significant value in the postoperative period, and somehow, the goal of surgery in papillary thyroid cancer (PTC) undergoing total thyroidectomy is to achieve undetectable levels of postoperative thyroglobulin (uTg). Methods: This is a retrospective single-center study in which first basal Tg values were evaluated post-surgery in PTC patients undergoing total thyroidectomy. Patients with elevated antithyroglobulin antibodies were excluded. The impact of various demographic, clinical, therapeutic

Enhanced Surgical Techniques in Orthopedics: A Comprehensive Guide for Surgeons Based on Modification of the McFarland and Osborne Approach to the Hip.

INTRODUCTION: The implementation of various approaches in hip arthroplasties introduces distinct advantages and complications. Notably, widely adopted methods such as the posterior approach have been linked to elevated rates of posterior hip dislocations and iatrogenic sciatic nerve injuries, while the lateral approach has been associated with superior gluteal nerve injuries. In this study, we propose a refined modification of the McFarland and Osborne approach, aiming to amalgamate the most favorable aspects of prior modifications of the lateral approach to the hip. Additionally, our contribu

Retrieval of patent ductus arteriosus device embolization using hybrid approach: a case report.

BACKGROUND: Patent ductus arteriosus (PDA) is a congenital heart defect that requires closure to prevent complications like heart failure and pulmonary hypertension. Catheter-based closure using devices such as the Amplatzer duct occluder is the preferred method due to its minimally invasive nature. However, device embolization is a rare but recognized complication, particularly in small children or high-flow PDAs. CASE PRESENTATION: We report a rare and complex case of spontaneous embolization of a PDA closure device into the descending aorta in an 11-month-old female. The patient, with a hi

Primary Mast Cell Sarcoma of the Maxillary Sinus and Gingiva Mimicking Malignant Neuroendocrine Tumor: A Case Report.

Mast cell sarcoma (MCS) is an extremely rare and aggressive malignancy primarily affecting bones, with limited literature associating it with neuroendocrine marker expression. This report presents a rare case of MCS arising in the maxillary sinus and gingiva. A 74-year-old man presented with a progressively enlarging ulcer on the right-sided upper gingiva. Magnetic resonance imaging revealed a 3.4 cm tumor on the floor of the right maxillary sinus. The patient underwent an inferior maxillectomy and right-sided neck dissection. Microscopically, the tumor consisted of monotonous round cells with

Clipping of Anterior Circulation Aneurysms: Operative Instructions and Safety Rules for Young Cerebrovascular Surgeons.

INTRODUCTION: Due to the constant development of the technique, in the last 30 years, the endovascular treatment of the intracranial aneurysms (IAs) has gradually superseded the traditional surgery in the majority of centers. However, clipping still represents the best treatment for some anterior circulation IAs according to their angioarchitectural, topographical, and hemodynamic characteristics. Thus, the identification of residual indications for clipping and the maintenance of training programs in vascular neurosurgery appear nowadays more important than ever. MATERIALS AND METHODS: We re

End-to-end revascularization between the occipital Artery(OA) and the p1 segment of posterior inferior cerebellar Artery(PICA) for a patient with posterior circulation ischemia via a far-lateral approach:2-Dimensional Operative video.

Vertebral artery (VA)Aneurysms involving the origin of the posterior inferior cerebellar artery (PICA) ,occasionally, induce cerebellum and brainstem infarction due to intraluminal thrombus and calcific VA stenosis. At times, vessel occlusion and revascularization is necessary for successful obliteration of these aneurysms.2 The occipital artery (OA) is often the preferred donor graft for lesions of the posterior fossa. Although most OA-PICA bypasses can be performed using the p3 segment as the recipient site for an end-to-side anastomosis, a more feasible alternative to conventional OA-p3 PIC
