[Type V floating elbow with type III A3 exposure and nerve injury. Initial and definitive treatment with a minimally invasive technique. A case report].

BACKGROUND: The increased number of high energy accidents has led to the occurrence of several injuries in a single extremity, particularly among youths. Stanitski and Micheli defined the floating elbow as a forearm fracture plus an ipsilateral supracondylar fracture. Its incidence ranges from 3% to 13%, predominant ages are 7 to 11 years, with a 2:2 male:female ratio. The mechanism of injury is as follows: fall from a height with elbow hyperextension and wrist dorsiflexion and pronation of the forearm. OBJECTIVE: To present the case of a child with a type V left open floating elbow injury, s

Pre-eclampsia and partial uterine denervation.

Pre-eclampsia is characterised by a maternal syndrome of hypertension and proteinuria, that is frequently associated with reduced fetal growth. The characteristic histopathological observation in the placental bed is narrowing and atherosis of the distal branches of the uterine arteries at, and around, the deciduo-myometrial interface. In the maternal kidneys there is swelling of the glomerular capillaries and mesangium with some inclusions in the basement membrane ("glomeruloendotheliosis") and evidence of vasoconstriction in many other organs. Untreated maternal hypertension leads to convuls

Increased expression and localization of the RNA-binding protein HuD and GAP-43 mRNA to cytoplasmic granules in DRG neurons during nerve regeneration.

The neuronal-specific RNA-binding protein, HuD, binds to a U-rich regulatory element of the 3' untranslated region (3' UTR) of the GAP-43 mRNA and delays the onset of its degradation. We have recently shown that overexpression of HuD in embryonic rat cortical cells accelerated the time course of normal neurite outgrowth and resulted in a twofold increase in GAP-43 mRNA levels. Given this evidence, we sought to investigate the involvement of HuD during nerve regeneration. It is known that HuD protein and GAP-43 mRNA are expressed in the dorsal root ganglia (DRG) of adult rat and that GAP-43 is

Expression of gicerin, a novel cell adhesion molecule, is upregulated in the astrocytes after hypoglossal nerve injury in rats.

Gicerin is an integral membrane glycoprotein which mediates cell-cell and cell-extracellular matrix (ECM) interactions in the nervous system. We studied gicerin expression in the hypoglossal nucleus post transection using in situ hybridization and immunocytochemistry. We found that hypoglossal nerve injury resulted in a significant increase in gicerin expression. Its expression levels reached peak values in reactive astrocytes surrounding axotomized motoneurons of the ipsilateral hypoglossal nucleus 14 days after hypoglossal nerve injury. The results indicate that gicerin is up-regulated durin

A long-acting injectable nanoparticle-hydrogel depot enables sustained IGF-1 delivery to reverse chronic denervation.

Chronic denervation following peripheral nerve injury leads to progressive muscle atrophy and Schwann cell dysfunction that severely limit functional recovery, yet no therapies effectively sustain trophic support over the prolonged regenerative period. Here, we develop a long-acting injectable (LAI) biomaterial platform that enables sustained local delivery of insulin-like growth factor-1 (IGF-1). The system encapsulates IGF-1 in poly(ethylene glycol)-b-polycaprolactone nanoparticles and embeds nanoparticles within an injectable nanofiber-hydrogel composite, enabling six-week local IGF-1 deliv

Tumor-associated macrophages enhance peripheral nerve tumor infiltration and spinal cord repair.

Tumor-associated macrophages (TAMs) enhance cancer progression by promoting angiogenesis, extracellular matrix remodeling, and immune suppression. Nerve infiltration also contributes to tumor growth. However, the role of TAMs in promoting intratumoral nerve growth remains unclear. In this study, we have shown that TAMs express a distinct neural growth gene signature. TAMs actively enhanced neural growth within tumors and directly promoted in vitro neurite outgrowth. We identified secreted phosphoprotein 1 (SPP1) as a required mediator of TAM-driven neural growth and mTORC2 activation. Leveragi

Exploring the Regenerative Role of Exosomes as a Potential Therapy for Peripheral Nerve Injury: A Systematic Review.

Exosomes are nano-sized extracellular vesicles loaded with bioactive cargo that mediate intercellular communication. Their emerging role in tissue regeneration has garnered increasing attention, especially in the context of peripheral nerve injury (PNI), which poses a significant therapeutic challenge. Although many studies have explored exosome-based therapies for PNI, the findings are often fragmented and lack a comprehensive synthesis. This systematic review aims to synthesize the current evidence and elucidate the molecular mechanisms through which exosomes contribute to neural regeneratio

Anteromedial Knee Pain Unmasked: Schwannoma of the Infrapatellar Branch of the Saphenous Nerve - A Rare Case Report.

INTRODUCTION: Schwannomas are benign, encapsulated peripheral nerve sheath tumors derived from Schwann cells. They are rarely seen around the knee, accounting for <1% of all schwannomas, and can mimic other benign swellings such as ganglion cysts or bursae. CASE REPORT: We report a 32-year-old female who presented with anteromedial right knee pain and a tender swelling for 2 years. Radiographs were normal, and a provisional diagnosis of a ganglion cyst was made. The lesion was excised under local anesthesia through a vertical incision and found to be a well-encapsulated mass measuring approxi

Effects of Acute Photobiomodulation on Heart Rate Variability in Physically Active Individuals: A Randomized and Controlled Clinical Trial.

Photobiomodulation (PBM) therapy is widely investigated for tissue recovery, performance enhancement, and post-exercise optimization. This study assessed whether PBM applied to the vagus nerve in the infra-auricular region, combined with resistance exercise, modulates cardiac autonomic function in healthy, physically active adults. In this acute randomized controlled trial, 34 volunteers were enrolled: 17 underwent PBM or sham PBM in a crossover design, while a control group (n = 17) received PBM without exercise. PBM was delivered with a total energy dose of 12 J. Heart rate variability (HRV)

A transient receptor potential vanilloid 1-dependent corneal-trigeminal neuroinflammatory circuit promotes corneal neuropathy.

Corneal neurosensory abnormalities cause pain and discomfort in ocular surface disease, yet their pathophysiology is poorly understood. Here we show that in a mouse dry eye model, the ocular (over)activation of transient receptor potential vanilloid 1 (TRPV1) channels in response to tear deficiency and tissue damage promotes neuroinflammatory gene expression and macrophage reactivity in the trigeminal ganglion, where the cornea-innervating sensory neurons are located. This is accompanied by ocular surface macrophage activation, impaired corneal sensitivity to mechanical and non-TRPV1-mediated

A causal link between autoantibodies and neurological symptoms in long COVID.

Acute SARS-CoV-2 infection triggers the de novo production of diverse, functional autoantibodies (AABs) that remain elevated in long COVID (LC), but their pathogenic role remains unclear. Using tissue-based immunofluorescence, ELISA, human protein array, and mass spectrometry assays, we identified a broad range of AAB targets among individuals with LC. Individuals with neurocognitive symptoms showed increased AABs against central nervous system (CNS) and peripheral nervous system proteins. Purified immunoglobulin G (IgG) reacted with human locus coeruleus, thalamus, adrenal gland, and thyroid

Lateral skull base metastasis from cervical adenocarcinoma: the masquerading trespasser - a case report.

BACKGROUND: Metastatic lesions of the lateral skull base are rare and often diagnostically challenging, especially when originating from cervical adenocarcinoma - a malignancy not commonly associated with skull base involvement. CASE PRESENTATION: We report the case of a 52-year-old postmenopausal woman who presented with right-sided facial nerve palsy, tongue deviation, and multiple cranial nerve deficits. Clinical examination and imaging suggested a skull base mass involving the jugular foramen, petrous bone, and hypoglossal canal. Radiological evaluation revealed a soft tissue lesion with

Capsaicin in the Management of Peripheral Neuropathy: A Review.

PURPOSE OF REVIEW: The purpose of this review is to provide an update on the efficacy and safety of capsaicin in the treatment of neuropathic pain. RECENT FINDINGS: Capsaicin can provide prolonged relief from pain in various neuropathies. One proposed mechanism is the ability to cause degeneration of hypersensitized nerve tissue and incite regeneration of healthy nerve fibers. Capsaicin provides more significant relief than placebo and is non-inferior to pregabalin in the treatment of peripheral neuropathy. It is emerging as an alternative but effective and well-tolerated treatment for neurop

PTEN regulates vagal-insulin signaling to optimize autonomic output determining peripheral inflammatory and metabolic homeostasis.

The vagus nerve (VN) is a major component of the parasympathetic nervous system that regulates glucose and energy homeostasis. However, the specific molecular signaling pathways within the VN that regulates this homeostasis remain unclear. Here, we show that vagal neuron-specific deletion of phosphatase and tensin homolog (Pten), the endogenous negative regulator of PI3K, led to increased vagal output. Intriguingly, dopaminergic signaling genes were upregulated, correlating with elevated sympathetic nerve density and increased norepinephrine levels in adipose tissue of vagal Pten-deficient mic

Sleep fragmentation exacerbates myocardial ischemia‒reperfusion injury by promoting copper overload in cardiomyocytes.

Sleep disorders increase the risk and mortality of heart disease, but the brain-heart interaction has not yet been fully elucidated. Cuproptosis is a copper-dependent type of cell death activated by the excessive accumulation of intracellular copper. Here, we showed that 16 weeks of sleep fragmentation (SF) resulted in elevated copper levels in the male mouse heart and exacerbated myocardial ischemia-reperfusion injury with increased myocardial cuproptosis and apoptosis. Mechanistically, we found that SF promotes sympathetic overactivity, increases the germination of myocardial sympathetic ner

Thymoma-Associated Myasthenia Gravis With Myocarditis.

Myasthenia gravis (MG) complicated by myocarditis is a rare autoimmune manifestation. We present a patient who initially presented with a suspected ST-segment elevation myocardial infarction (STEMI) with angiographically normal coronary arteries. A chest CT scan revealed a large homogenous soft-tissue density anterior mediastinal mass suspicious of thymoma. Neurological deterioration in the hospital suggested a diagnosis of MG with subsequent electromyography and nerve conduction studies (EMG/NCS) and repetitive nerve stimulation (RNS) confirmation. A cardiac magnetic resonance imaging study (

Recent Advances in Biomaterials-Based Therapies for Alleviation and Regeneration of Traumatic Brain Injury.

Traumatic brain injury (TBI), a major public health problem accompanied with numerous complications, usually leads to serve disability and huge financial burden. The adverse and unfavorable pathological environment triggers a series of secondary injuries, resulting in serious loss of nerve function and huge obstacle of endogenous nerve regeneration. With the advances in adaptive tissue regeneration biomaterials, regulation of detrimental microenvironment to reduce the secondary injury and to promote the neurogenesis becomes possible. The adaptive biomaterials could respond and regulate biochem
