An Unusual Case of Rabies Encephalitis.

Human rabies encephalitis is rare in Canada, with only five cases reported in the past 30 years. The first and only patient who contracted rabies encephalitis in British Columbia died in 2003. Here we provide the first detailed clinical and pathological description of that case, which had several unusual features, including preexisting immunosuppression, paralytic presentation, prolonged survival, focal lesions on neuroimaging and severe neuropathology with focal necrosis, intense inflammation, and abundant viral inclusion bodies.

Developmental dyslexia: dysfunction of a left hemisphere reading network.

This mini-review summarizes and integrates findings from recent meta-analyses and original neuroimaging studies on functional brain abnormalities in dyslexic readers. Surprisingly, there is little empirical support for the standard neuroanatomical model of developmental dyslexia, which localizes the primary phonological decoding deficit in left temporo-parietal (TP) regions. Rather, recent evidence points to a dysfunction of a left hemisphere reading network, which includes occipito-temporal (OT), inferior frontal, and inferior parietal regions.

Regional cerebral blood flow in a patient with Nasu-Hakola disease.

We report a functional neuroimaging study of a 43-year-old woman with Nasu-Hakola disease (NHD). Regional cerebral blood flow (rCBF) images were measured with technetium-99m ethyl cysteinate dimer single photon emission computed tomography (SPECT). rCBF was decreased in the bilateral frontal lobes and thalamus. This finding was consistent with the known underlying neuropathology in patients with NHD. Brain SPECT is useful for demonstrating the pathophysiologic brain region in patients with NHD.

Neuromyelitis optica in a child with atypical onset and severe outcome.

We report on a seven-year-old boy with inflammatory relapsing-remitting CNS disease, involving the optic nerves and spinal cord, with increasingly severe sequelae after each relapse. Clinical course, neuroimaging and laboratory findings were consistent with neuromyelitis optica. Biopsy of leptomeninges and underlying nervous tissue showed increased vascularization and thickened hyalinized vessel walls, reported as suggestive for neuromyelitis optica. Clinical features at onset were atypical, rendering the case highly unusual and the diagnosis tentative.

Migraine-like visual hallucinations in occipital lesions of cysticercosis.

Four Indian patients with occipital lesions of cysticercosis presented with visual hallucinations. Neuro-ophthalmic and systemic examinations were normal in all cases except for one patient who had a partial homonymous hemianopia. Electroencephalography was normal in all cases. Neuroimaging revealed ring-enhancing lesions in the occipital lobe typical of neurocysticercosis. In endemic regions like India, neurocysticercosis should be suspected in patients presenting with visual hallucinations, even when there are no other clinical findings.

Vertebrobasilar insufficiency and stroke.

Vertigo is the most common symptom of transient ischemia within the vertebrobasilar system. Episodes typically come on abruptly and last minutes. Although there are usually associated symptoms, isolated attacks of vertigo can occur. Vertigo is also a common symptom with brain stem and cerebellar strokes. Isolated cerebellar infarction can mimic peripheral vestibular disorders because vertigo and imbalance are the main symptoms with both disorders. Modern neuroimaging techniques have markedly improved diagnostic capabilities.

Imaging of neurodegenerative diseases.

Neuroimaging of degenerative brain disorders has continued to advance, primarily with MR imaging, single-photon emission CT, and positron emission tomography. Information gained from these morphologic and functional imaging studies provides insight into the pathologic and pathophysiologic changes associated with these diseases. Recent papers relating to various neurodegenerative disorders such as Alzheimer's disease, vascular dementias, acquired immunodeficiency syndrome-related dementia, Pick's disease, Huntington's disease, progressive supranuclear palsy, and others are reviewed and discusse

Evaluating the Association of Calcified Neurocysticercosis and Mesial Temporal Lobe Epilepsy With Hippocampal Sclerosis in a Large Cohort of Patients With Epilepsy.

BACKGROUND: Neurocysticercosis (NCC) is a parasitic infection of the central nervous system that has been associated with mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS). However, this association has not been completely established. OBJECTIVE: To evaluate the prevalence of calcified NCC (cNCC), its characteristics and a possible association between cNCC and MTLE-HS in a cohort of 731 patients with epilepsy. METHODS: We review clinical, EEG and neuroimaging findings of 731 patients with epilepsy. From these, 659 had CT-scans and 441 patients had complete neuroimaging with

The Mechanism of Macular Sparing.

Patients with homonymous hemianopia sometimes show preservation of the central visual fields, ranging up to 10°. This phenomenon, known as macular sparing, has sparked perpetual controversy. Two main theories have been offered to explain it. The first theory proposes a dual representation of the macula in each hemisphere. After loss of one occipital lobe, the back-up representation in the remaining occipital lobe is postulated to sustain ipsilateral central vision in the blind hemifield. This theory is supported by studies showing that some midline retinal ganglion cells project to the wrong h

Evaluation of CT and MRI Findings among Patients Presented with Chief Complaint of Headache in Central India.

INTRODUCTION: Headache is one of the most common presenting complaints in day to day medical practice however the secondary causes of headache are uncommon. Thus, appropriate selection of headache patient (Pt) is important to determine those that require neuroimaging due to likely secondary cause. Red flags and Clinical warning criterion (CWC) act as a screening tool to help in identifying those who may get benefit from neuroimaging. AIM: To evaluate the findings of computed tomography (CT) and Magnetic Resonance Imaging (MRI) among patients presented with the chief complaint of headache and

Integrating functional network topology, synaptic density, and tau pathology to predict cognitive decline in amnestic mild cognitive impairment.

BACKGROUND: Amnestic mild cognitive impairment (aMCI) represents a prodromal stage of Alzheimer's disease and carries a high risk of progression to dementia. Disruptions in functional brain networks, synaptic degeneration, and tau pathology each serve as neural markers of cognitive decline in aMCI. In this study, we employed a multimodal neuroimaging approach to determine whether integrating these markers provides a more powerful explanation of cognitive deterioration than examining them individually. METHODS: Twenty-eight aMCI patients and 24 healthy controls underwent cognitive assessment,

Diagnostic association of ante-mortem biomarkers with cerebral amyloid angiopathy in a cohort with high Alzheimer's disease comorbidity.

BACKGROUND: Cerebral amyloid angiopathy (CAA) frequently co-occurs with Alzheimer's disease (AD), complicating the interpretation of biomarkers. Current diagnostic approaches rely largely on neuroimaging but remain constrained by limited accuracy. OBJECTIVE: This study aimed to evaluate the diagnostic discrimination and pathological association of neuropathological scoring systems and fluid biomarkers for CAA using pathologically confirmed samples. METHODS: We examined 100 participants from the Alzheimer's Disease Neuroimaging Initiative with complete neuropathological assessments. Associati

Dimethylarginine Dimethylaminohydrolase 1 (DDAH1)-Arginine Metabolic Axis Is Independently Associated With Alzheimer's Disease Risk in the UK Biobank.

Background Altered arginine metabolism and nitric oxide (NO) signaling have been implicated in Alzheimer's disease (AD), but the role of dimethylarginine dimethylaminohydrolase 1 (DDAH1), a regulator of nitric oxide bioavailability, remains unclear. We investigated whether the DDAH1-arginine metabolic axis is independently associated with AD risk in the UK Biobank (UKB). Methods We analyzed 50,988 UK Biobank participants with available proteomic, metabolomic, clinical, genetic, and neuroimaging data. Plasma DDAH1 levels were measured using the Olink Explore 3072 platform (Olink Proteomics AB,

Near-care assay of plasma glial fibrillary acid protein and ubiquitin carboxyl-terminal hydrolase isozyme L1 with shorter and prolonged duration exercise.

Neurobiomarkers measured in peripheral blood can supplement management strategies following traumatic brain injury (TBI). Dual-assay of glial fibrillary acid protein (GFAP) and ubiquitin carboxyl-terminal hydrolase isozyme L1 (UCHL1) is FDA-approved to inform a decision threshold approach (GFAP > 30 µg.L- 1 and/or UCHL1 > 360 µg.L- 1) for post-TBI neuroimaging. As physical activity and thermal strain often accompany TBI-prone activities, we investigated whether each molecule's quantification - and, by extension, clinical decisions - could be influenced by exercise-heat stress. Neurodevelopmental regression Due to PLA2G6-associated neurodegeneration despite normal brain MRI: a case report.

BACKGROUND: Infantile neuroaxonal dystrophy is a rare autosomal recessive neurodegenerative disorder within the spectrum of PLA2G6-associated neurodegeneration. It is typically characterized by early psychomotor regression, progressive motor impairment, bulbar dysfunction, and characteristic neuroimaging abnormalities, including cerebellar atrophy and, in some cases, brain iron accumulation. CASE REPORT: We report the case of a 4-year-11-month-old girl with progressive neurodevelopmental regression beginning at approximately 18 months of age. The patient presented with loss of previously acqu

Treatment-related associations of nucleus accumbens connectivity within mesocorticolimbic circuits in depression.

Pharmacological treatment remains a mainstay in managing depression, yet the neural correlates associated with treatment response remain incompletely understood. This study used multimodal neuroimaging to examine nucleus accumbens (NAc)-centered structural and functional alterations associated with fluoxetine and Shugan Jieyu Capsule (SG), a traditional Chinese medicine, in patients with mild-to-moderate depression (MMD). Sixty patients were randomized to an 8-week course of fluoxetine or SG. Depression severity was assessed using the 24-item Hamilton Depression Rating Scale (HAMD-24), and str

Early diagnosis of Alzheimer's Disease: Graph theoretical analysis of cerebellar network features based on 18F-AV45 PET.

Pathological and neuroimaging changes in the cerebellum of Alzheimer's disease (AD) patients have been well documented. However, the changes in cerebellar amyloid plaque deposition connectivity networks during AD progression based on positron emission tomography (PET) imaging remain unclear. We selected 18F-florbetapir PET (18F-AV45 PET) imaging data from the Alzheimer's disease neuroimaging initiative (ADNI) dataset (n = 612) and employed graph theoretical analysis to examine amyloid plaque deposition connectivity, comparing the connectivity differences across cognitively normal (CN), early m
