Reoperation of the nose is challenging and sometimes emotionally difficult for the surgeon and patient. There are multiple pitfalls to be avoided and it is always best to carefully diagnose and establish a surgical treatment plan. Even among the best of plans and surgical techniques, revision may be necessary. The patient and surgeon should understand the limitations of the surgical techniques and the individual anatomy.

The Ilizarov method for leg lengthening was used for cosmetic reasons in 54 patients with constitutional short stature. A mean lengthening of 7 cm with a low rate of complications produced an excellent or good outcome in all the patients, including improvement in psychological disturbances related to short stature. The orthodontic management of patients with profound learning disability is described including a report of two cases. Close co-operation between the orthodontist and other providers of dental treatment is essential for the management of this group of patients, and a general anaesthetic may be needed to allow fixed appliances to be placed. The orthodontic treatment is co-ordinated with any necessary restorative or periodontal treatment. Many surgeons will find themselves faced with a claim of malpractice. At times this may be due to a less-than-perfect outcome. Well over half of such claims are preventable, as most are based on failures of communication and/or patient selection criteria. Attention to legal principles such as standard of care, warranty and disclosure, knowledge of patient factors, and the ability to communicate clearly and effectively will hopefully maximize the possibility of remaining a claims-free surgeon.
