Paraneoplastic Dermatomyositis Preceding Gastroesophageal Junction Adenocarcinoma: A Case Emphasizing the Importance of Early Malignancy Screening.

Dermatomyositis (DM) is an idiopathic inflammatory myopathy marked by proximal muscle weakness and pathognomonic rashes such as heliotrope rash and Gottron papules. DM is associated with an increased risk of malignancy compared to the general population, with 15-30% of cases being paraneoplastic. DM-associated malignancies are often diagnosed at late stages, resulting in cancer being a leading cause of death in this condition. Additionally, DM can mask malignancy by presenting with similar symptoms as the inciting cancer, further emphasizing the importance of early, disease-specific cancer scr

Rivaroxaban Plasma Concentrations Cannot be Predicted from Coagulation Assays: Real-Life Experience From a Tertiary Hospital in New Zealand.

BACKGROUND: The use of direct oral anticoagulants, such as rivaroxaban, is preferred over vitamin K antagonists. Several clinical scenarios highlighting potential benefits of measuring plasma concentrations have been reported. This study aimed to describe the clinical experience with rivaroxaban in adult patients and develop a mathematical model to predict rivaroxaban concentrations using routine coagulation screening assays. METHODS: Data were prospectively collected over a 4-year-period, including clinical patient characteristics, total plasma rivaroxaban concentrations, and screening coagu

Quality improvement project to improve cardiovascular risk screening in patients with rheumatoid arthritis.

SIGNIFICANCE AND BACKGROUND: Rheumatoid arthritis (RA) is a chronic inflammatory joint disease that is associated with an increased cardiovascular (CV) risk.Additional CV risk screening is advised in several national and international guidelines, but is performed inadequately. The CV risk screening in the Netherlands is performed by the general practitioner (GP). PURPOSE: The aim of our project was to increase the CV risk screening of patients with RA, performed by the GP, from 46% to 65% of the selected patients of 40 years and older, within the time frame of our project (18 months). Rapid hearing threshold assessment with modified auditory brainstem response protocols in dogs.

INTRODUCTION: Auditory brainstem response (ABR) is the gold standard for hearing testing in dogs. ABR is commonly used in puppies to diagnose congenital sensorineural deafness. Long test times limit the use for a more comprehensive hearing screening in veterinary practice. This study aimed to establish a super-fast hearing screening protocol in dogs. METHODS: Hearing thresholds were routinely measured with a mobile device designed for newborn hearing screening in 90 dogs. We introduced modifications of the ABR protocol, e. g., a binaural test mode, higher stimulus rates, a broadband chirp sti

Transmission of Brucella canis in a canine kennel following introduction of an infected dog.

Brucella canis is a zoonotic pathogen and the main causative agent of canine brucellosis. In the Netherlands, B. canis had previously only been detected in individual cases of imported dogs. However, an outbreak of B. canis occurred for the first time in a cohort of autochthonous dogs in a breeding kennel in 2019. The outbreak began with a positive serological test result of an imported intact male dog showing clinical symptoms of brucellosis. Consequently, urine and blood samples were collected and tested positive for B. canis by culture, matrix-assisted laser desorption/ionization - time of

Investigation of Factors Associated with Subclinical Infections of Giardia duodenalis and Cryptosporidium canis in Kennel-Housed Dogs (Canis lupus familiaris).

Giardia duodenalis and Cryptosporidium spp. are zoonotic protozoal pathogens, spread by a fecal-oral route, which can infect a wide range of hosts including but not limited to dogs and humans. Giardia was recently estimated to be present in 37% to 50% of kennel-housed dogs. Cryptosporidium infections in kennel-housed dogs have been reported in 7% to 21% of the population. The goal of this study was to define demographic factors and fecal scores associated with positive screening test cases of Giardia and Cryptosporidium in kennel-housed laboratory dogs in the state of Texas. Fecal samples were

Prevention of spontaneous preterm delivery - an update on where we are today.

Spontaneous preterm birth (delivery before 37 completed weeks) is the single most important cause of perinatal morbidity and mortality. The rate is increasing world-wide with a great disparity between low, middle and high income countries. It has been estimated that the cost of neonatal care for preterm babies is more than 4 times that of a term neonate admitted into the neonatal care. Furthermore, there are high costs associated with long-term morbidity in those who survive the neonatal period. Interventions to stop delivery once preterm labor starts are largely ineffective hence the best app

Patient factors affecting successful linkage to treatment in a cervical cancer prevention program in Kenya: A prospective cohort study.

OBJECTIVE: To identify patient factors associated with whether women who screened positive for high-risk human papillomavirus (hrHPV) successfully accessed treatment in a cervical cancer prevention program in Kenya. METHODS: A prospective cohort study was conducted as part of a trial of implementation strategies for hrHPV-based cervical cancer screening in western Kenya from January 2018 to February 2019. In this larger trial, women underwent hrHPV testing during community health campaigns (CHCs), and hrHPV+ women were referred to government facilities for cryotherapy. For this analysis, we l

High Number of Potential Transmitters Revealed in a Population-based Systematic Hepatitis C Virus RNA Screening Among Human Immunodeficiency Virus-infected Men Who Have Sex With Men.

BACKGROUND: The proportion of undiagnosed hepatitis C virus (HCV) infections in high-risk populations, such as human immunodeficiency virus (HIV)-infected men who have sex with men (MSM) is unclear. Identification of potential HCV transmitters is important to reach World Health Organization HCV elimination targets. METHODS: Between October 2015 and May 2016, we performed a systematic HCV RNA-based screening among HIV-infected MSM participating in the Swiss HIV Cohort Study (SHCS). HCV antibodies were measured from all HCV RNA-positive samples. RESULTS: Of 4257 MSM recorded in the SHCS databa

Abdominal Aortic and Visceral Artery Aneurysms.

Abdominal aortic aneurysms account for nearly 9000 deaths annually, with ruptured abdominal aortic aneurysms being the thirteenth leading cause of death in the United States. Abdominal aortic aneurysms can be detected by screening, but a majority are detected incidentally. Visceral artery aneurysms are often discovered incidentally, and treatment is guided by symptoms, etiology, and size. A timely diagnosis and referral to a vascular specialist are essential for timely open or endovascular repair and to ensure successful patient outcomes.

Use of a bleeding time determination in the evaluation of unexplained hematuria.

Although von Willebrand's disease is an unusual cause of gross hematuria in children, it is readily treatable with fresh frozen plasma or cryoprecipitate. We present 2 cases of recurrent, painless gross hematuria owing to this congenital factor VIII deficiency disorder. In each case the diagnosis was suggested first by the finding of a prolonged bleeding time. We suggest that the bleeding time determination be included as part of the screening hemostatic studies used in the evaluation of unexplained hematuria.

The Yield of Clinical Screening in Pediatric Relatives of Hypertrophic Cardiomyopathy With Negative Genetic Testing.

BACKGROUND: Serial clinical screening of first-degree relatives of individuals with hypertrophic cardiomyopathy (HCM) is recommended. A lower yield of screening in adult relatives with genotype-negative disease might mean a one-off assessment is appropriate, but it is unknown if these findings can be extrapolated to pediatric relatives. OBJECTIVES: This study aims to determine the yield of clinical screening in pediatric relatives of genotype-negative families. METHODS: Clinical and outcome data were collected from 404 children (≤18 years) from 227 genotype-positive (G+) families and 292 chi

Screening older adults for alcohol use in orthopaedic care: Healthcare team perspectives, current practices, and barriers.

BACKGROUND: Alcohol use disorder (AUD) is rising among older adults at a concerning rate and is considered an emerging public health problem. Despite this, validated screening is infrequently implemented in orthopaedic settings despite evidence that alcohol use negatively affects fracture risk and postoperative outcomes. Limited knowledge, training, and workflow integration may contribute to inconsistent practices. The purpose of this study is to evaluate orthopaedic healthcare professionals' perceptions of older adult alcohol use, current screening behaviors, familiarity with validated tools,

Influence of Comorbidities on Colorectal Cancer Screening Participation and Mortality.

AIMS: Organized screening programs improve colorectal cancer (CRC) screening participation, but outreach services can be improved. We sought to understand screening deferral by examining patient-level factors and how they relate to fecal immunochemical test (FIT) orders, completion rates, and long-term mortality. METHODS: Patients aged 50-75 years who were not up to date with CRC screening receiving usual care were followed over time (NCT02613260). Patient-level laboratory and cancer registry data were used to identify patients who met a specified laboratory threshold: albumin < 3 g/dL, HIV v
