Isolated congenital nail dysplasia is an autosomal dominant disorder recently observed in a large family from southern Germany. The disorder is characterized by longitudinal streaks, thinning, and impaired formation of the nail plates leading to increased vulnerability of the free nail margins. In most cases, all fingernails and toenails are similarly involved with some accentuation of the thumb and great toenails. Histologic changes include hypergranulosis of the nail matrix and epithelial outgrowths from the nail bed. Patients do not show any alterations of hair growth and dentition, no malf

BACKGROUND: The Morquio syndrome is a rare autosomal-recessive mucopolysaccharidosis. The Morquio syndrome is characterized by a reduced activity of N-acetylgalactosamine-6-sulfate-sulfatase (type A), or beta-galactosidase (type B). This deficiency leads to a lysosomal storage disease with accumulation of keratan sulfate und chondroitin-6-sulfate in connective tissue, skeletal system und teeth. Consequently, abnormalities of the skeletal system, aortic valvular disease and dental abnormalities occur. Ophthalmologically, diffuse corneal opacification and alterations of the trabecular meshwork--

We report the features of a group of 41 patients with Paget's disease (PD) who were identified in a cross sectional study in the "Sierra de la Cabrera" (Madrid). As the screening test, we used serum alkaline phosphatase quantification, and the diagnosis was confirmed by radiological study. This biochemical parameter shows an excellent diagnostic performance, with a sensitivity of 98% and a specificity of 85%. The prevalence in the population over 40 years of age is 6.37% (95% CI 4.79-7.95) which, comparing it with that from other areas, suggests that this region is a "focus" of PD. 51% of case

LGL leukemia results from a chronic, clonal proliferation of LGL. Chronic neutropenia with recurrent bacterial infection and splenomegaly are common clinical manifestations. Rheumatoid arthritis coexists in some of these patients, who thus resemble patients with Felty syndrome. Other hematologic abnormalities that may occur include pure red-cell aplasia and adult-onset cyclic neutropenia. Lymphoid infiltration of bone marrow, splenic red pulp cords, and hepatic sinusoids is characteristic; lymph node and skin involvement are rare. Multiple serologic abnormalities are frequently present, includ

The frequent occurrence of Kaposi's sarcoma (KS) in association with the acquired immune deficiency syndrome (AIDS) could be due to the fact that the etiological agent of this tumor is the same retrovirus causing AIDS, to another oncogenic virus frequently found in AIDS patients, or to the unmasking of the tumorigenic potential of KS cells by immunosuppression. We have therefore investigated the presence of DNA sequences homologous to the AIDS retrovirus, cytomegalovirus (CMV), and hepatitis B virus in 13 KS necropsies and biopsies from AIDS patients. All KS DNA samples were negative for AIDS

Dermatologists may also encounter patients presenting with skin lesions that reflect an underlying endocrine disorder not commonly seen in daily practice. Some of these endocrine disorders include glucagonoma, neurofibromatosis type 1, McCune-Albright syndrome, multiple endocrine neoplasia, the Carney complex, carcinoid tumors, and mastocytosis. The clinical syndrome classically associated with glucagonoma includes necrolytic migratory erythema, weight loss, diabetes mellitus, anemia, cheilitis, venous thrombosis, and neuropsychiatric symptoms. The hallmarks of neurofibromatosis type 1 are the

Genetic investigation of inherited skin disorders has informed the understanding of skin self-renewal, differentiation, and barrier function. Erythrokeratodermia variabilis et progressiva (EKVP) is a rare, inherited skin disease that is characterized by transient figurate patches of erythema, localized or generalized scaling, and frequent palmoplantar keratoderma. By using exome sequencing, we show that de novo missense mutations in GJA1 (gap junction protein alpha 1) cause EKVP. The severe, progressive skin disease in EKVP subjects with GJA1 mutations is distinct from limited cutaneous findin

The cytosolic protein APAF1, human homolog of C. elegans CED-4, participates in the CASPASE 9 (CASP9)-dependent activation of CASP3 in the general apoptotic pathway. We have generated by gene trap a null allele of the murine Apaf1. Homozygous mutants die at embryonic day 16.5. Their phenotype includes severe craniofacial malformations, brain overgrowth, persistence of the interdigital webs, and dramatic alterations of the lens and retina. Homozygous embryonic fibroblasts exhibit reduced response to various apoptotic stimuli. In situ immunodetection shows that the absence of Apaf1 protein preve

Polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, and skin changes (POEMS) syndrome is frequently mistaken for chronic inflammatory demyelinating polyneuropathy, as its clinical profile is highly variable and neurological symptoms often dominate the initial course. In the present case, the disorder initially presented as cardiovascular dysfunction, which delayed decision-making related to the original diagnosis. As the condition advanced, the patient developed progressive limb weakness, splenomegaly, and abnormalities in lipid metabolism. Whole-body bone scintigraphy revealed o

Thrombomodulin (THBD) is an endothelial membrane protein that has anticoagulant and anti-inflammatory actions, inhibits fibrinolysis, and modulates innate immunity and complement activation. We report the characterization of a novel THBD mutation, C256S, homozygously expressed in an adolescent boy. The patient presented with a wide range of symptoms, including life-threating epistaxis, thrombotic microangiopathy, massive intraoperative bleeding, venous thromboses, and skin necrosis. THBD-C256S is predicted to prevent the formation of a critical disulfide bond within epidermal growth factor-lik

BACKGROUND: Cardiac sarcoidosis (CS) can cause conduction disturbances and heart failure. Chronotropic incompetence due to atrioventricular (AV) conduction abnormalities may represent an early and potentially reversible manifestation of CS.
CASE SUMMARY: A 62-year-old woman with suspected CS presented with exertional dyspnea and exercise-induced second-degree AV block. Despite preserved left ventricular systolic function at rest, cardiopulmonary exercise testing combined with exercise echocardiography revealed a severely impaired chronotropic response and inadequate cardiac output augmentatio

Inflammatory breast cancer (IBC) is a rare but aggressive form of cancer; hence, early diagnosis is essential. Although predominantly occurring in older women, it can occur during pregnancy and lactation, making the diagnosis especially challenging. IBC is characterized by erythema and edema of the breast skin, with or without an underlying palpable mass. In pregnant and postpartum women, it can be easily confused with lactational mastitis. Awareness of the history and clinical signs of IBC, and how it differs from lactational mastitis, increases the likelihood that women will receive due care

Periodontal Ehlers-Danlos Syndrome (pEDS) is a rare autosomal dominant connective tissue disorder with fewer than 200 cases reported. In addition to typical Ehlers-Danlos Syndrome (EDS) features of joint hypermobility, skin fragility and tissue friability, it is characterized by periodontal abnormalities, leading to periodontal bone and teeth loss. The article presents a case of a 10-year-old boy diagnosed with pEDS, exhibiting typical symptoms of violaceous pretibial plaques, hypermobile joints, and periodontal disease with mobile teeth. Additionally, the patient displayed atypical features o

Rosacea is a chronic skin condition characterized by facial erythema, flushing, and telangiectasia. Abnormalities in the vascular responses are associated with the development of rosacea. Our analysis of the GSE65914 dataset revealed differential expression of angiogenesis-related genes in rosacea lesions classified rosacea samples with distinct angiogenic molecular patterns. Further investigation of immune infiltration characteristics across angiogenic patterns identified unique immune signatures associated with VEGFAhigh MMP9low and VEGFAlow MMP9high subtypes. Moreover, STAT2 proteins were h

Decompression sickness (DCS, "bends") is the clinical condition triggered by generation of bubbles in tissues or blood due to supersaturation of inert gas during or after a reduction in ambient pressure. The condition can occur in association with compressed gas diving, compressed air ("caisson") work or rapid decompression to high altitude or reduced cabin pressure such as extravehicular activity (EVA) in space suits. It can also be triggered by mild reduction in ambient pressure such as during commercial aircraft flight after scuba diving. Its manifestations range from joint or muscle pain,

Morphea, a rare autoimmune disorder characterized by progressive skin fibrosis and soft tissue atrophy, remains clinically challenging due to poorly understood pathogenesis. Here, we revealed that mechanical abnormalities in morphea tissues activate the transcriptional co-activator Yes-associated protein 1 (YAP1), which drives pathological fibrosis through glycolytic reprogramming. Mechanistically, YAP1 promotes glycolysis by upregulating the expression of phosphofructokinase platelet type (PFKP), thus creating a self-reinforcing profibrotic cycle. Consequently, pharmacological inhibition of Y

BACKGROUND: Upper lip scar adhesion frequently follows traumatic injuries, manifesting as skin-mucosal adhesion to the nasal column with philtral distortion and Cupid's bow deformity. This condition primarily stems from posttraumatic tissue defects and regional anatomical characteristics. Existing excision techniques may yield suboptimal esthetic/functional outcomes, including labial peak asymmetry and adhesion recurrence, necessitating secondary surgery. The authors describe a novel surgical approach employing local modification combined with triangular flap insertion.
METHODS: This retrospe
