We report two rare cases of prominent ossified ligamenta flava of the thoraco-lumbar junction. The first case presented with thoracic myelopathy diagnosed by spinal MRI. The definite diagnosis and the nature of the T10-T11 compressing process was established by spiral CT with multiaxial reconstruction and the patient was treated by laminectomy. In the second case the ossifications were found on a lateral lumbar X-ray and confirmed by spiral CT with multiplanar reconstructions. The pathology was essentially confined to the lateral portion of the T12-L1 ligamenta flava resulting in stenosis of t

RATIONALE: Paragangliomas are rare neuroendocrine tumors that originate in specialized cells derived from the neural crest with metastasis to the thoracic spine being among the rarest forms. Here, we are presenting a detailed analysis of a case of malignant paraganglioma in the thoracic spinal region in a 14-year-old boy. Our focus is to emphasize the importance of considering malignant paraganglioma as a diagnosis and guiding the perioperative management upon surgical treatment. The management of these unique cases has yet to be well-documented.
PATIENT CONCERNS: A 14-year-old boy presented

STUDY DESIGN: A retrospective study.
OBJECTIVE: To evaluate the feasibility, efficacy, and safety of laminoplasty with plate fixation at alternating levels through radiologic analysis of the enlarged spinal canal and clinical follow-up.
SUMMARY OF BACKGROUND DATA: Laminoplasty is commonly used to manage cervical myelopathy. Because of the absence of rigid fixation, traditional laminoplasty commonly results in complications, including secondary narrowing of the spinal canal and neurological deterioration. At present, miniplate fixation is promising to prevent reclosure of the opened lamina ef

PURPOSE OF THE STUDY: Extensive cervical laminectomy has been widely used in the treatment of progressive myelopathies secondary to stenotic conditions. Complications of this procedure such as spinal instability, accelerated spondylotic changes, postoperative spinal deformity and constriction of the dura mater by extradural scar tissue formation have been recognized. However, the frequency of these complications is probably overestimated and their consequences on the clinical outcome remain unknown. The purpose of this report was to describe the incidence and consequences of cervical spinal de

Background Cervical myelopathy is a progressive condition caused by spinal cord compression, often requiring timely surgical decompression. Although clinical improvement is generally expected after surgery, the role of immediate postoperative magnetic resonance imaging (MRI) in assessing decompression adequacy and predicting neurological recovery remains underexplored. This study examines the value of early postoperative MRI in identifying residual stenosis, missed levels, and intramedullary signal changes. Methods A total of 181 patients who underwent cervical decompression surgery for cervic

STUDY DESIGN: A prospective interventional case-series study.
OBJECTIVE: To provide clinical results of CyberKnife fiducial-free spinal radiosurgery. The study focused on patients with no more than 2 malignant spinal tumors.
SUMMARY OF BACKGROUND DATA: Progress in frameless radiosurgical technology has enabled the application of radiosurgery to the spine. The CyberKnife System has been used extensively for spine radiosurgery. Until recently, the system required metallic fiducial implants for precise target tracking. Fiducial-free spinal radiosurgery with the CyberKnife has recently become po

Objectives: Among the spectrum of spinal injuries, Spinal Cord Injury Without Radiographic Abnormality (SCIWORA) occupies a unique and challenging position. SCIWORA presents diagnostic and therapeutic challenges due to its broad clinical and radiological heterogeneity. While most children recover favorably with conservative treatment, a subset may require surgery based on imaging findings. The findings underscore the need for standardized diagnostic criteria, MRI-based classification systems, and evidence-based treatment algorithms to improve consistency in care and long-term neurological outc

INTRODUCTION: Ossification of the posterior longitudinal ligament (OPLL) is a hyperostotic spinal condition resulting from progressive endochondral ossification of the posterior longitudinal ligament, leading to spinal canal narrowing and spinal cord compression. Primarily seen in East Asian countries, OPLL has been increasingly recognized in North American cohorts. This study aims to evaluate the demographic characteristics and outcomes of patients undergoing surgery for OPLL in a North American cohort.
METHODS: This was a single-center retrospective cohort study of patients with OPLL underg

BACKGROUND: Surgical treatment is mainly used for atlantoaxial tuberculosis with neurological damage. However, the anatomic structure around the atlantoaxial joint is complex, and the position of vertebral body is deep, which increases the difficulty of the operation and it is challenging for the surgeon to develop surgical strategy. The purpose of this study was to evaluate the clinical outcomes of one-stage combined anterior and posterior surgical treatment approach for atlantoaxial tuberculosis with neurological impairment.
METHODS: From January 2005 to January 2015, 12 patients suffering

OBJECTIVE Increased survival time after diagnosis of neoplastic disease has resulted in a gradual increase in spine tumor incidence. Radiosurgery is frequently a viable alternative to operative management in a population with severe medical comorbidities. The authors sought to assess the histopathological consequences of radiosurgery in the subset of patients progressing to operative intervention. METHODS Eighteen patients who underwent radiosurgery for spine tumors between 2008 and 2014 subsequently progressed to surgical treatment. A histopathological examination of these cases was performed

Surgical access to the cervicothoracic junction (CTJ) is challenging. The aim of this study was to assess technical feasibility, early morbidity, and outcome in patients undergoing anterior access to the CTJ via partial sternotomy. Consecutive cases with CTJ pathology treated via anterior access and partial sternotomy at a single academic center from 2017 to 2022 were retrospectively reviewed. Clinical data, perioperative imaging, and outcome were assessed with regards to the aims of the study. A total of eight cases were analyzed: four (50%) bone metastases, one (12.5%) traumatic instable fra

Purely extradural spinal cavernous malformations (ESCMs) are rare, but the number of cases reported seems to be increasing. A 44-year-old woman presented with progressive cervicothoracic back pain, lower limb paraesthesia and hypertonia. MRI showed a well-circumscribed, convex lesion in the T1-4 extradural space causing cord compression and encroaching into the left T2/3 neural foramen. Selective spinal angiography demonstrated a vascular blush at the T2/T3 level corresponding with the mass. A feeding vessel arising from the base of the T1 spinous process was identified and successfully cathet
