Atlantoaxial fixation in which C1-2 screw-rod fixation is performed is a relatively new method. Because reports about this technique are rather scant, little is known about its associated complications. In this report the authors introduce hypoglossal nerve palsy as a complication of this novel posterior atlantoaxial stabilization method. A 67-year-old man underwent a C1-2 screw-rod fixation for persistent neck pain resulting from a Type 2 odontoid fracture that involved disruption of the transverse atlantal ligament. Posterior instrumentation in which a C-1 lateral mass screw and C-2 pedicle

AIM: We present a case report and the histological analysis of cage failure following vertebral body replacement in Hodgkin's disease.
METHOD: In a 35-year-old patient with a single metastasis of Hodgkin's disease replacement of the third lumbar vertebral body (Harms-titanium-mesh-cage, DePuy Acromed, completely filled with autogenous spongiosa from the iliac crest) was performed. 51 months postoperatively, the patient presented with acute back pain without trauma. The X-ray showed a collapse of the Harms-titanium-mesh-cage. The cage fragments had caused ventral dislocation of the main blood

The author presents a case in which a severe chronic thoracolumbar kyphotic posttraumatic deformity was corrected by a one-block spondylectomy with spine shortening, using a posterior approach. A 19-year-old paraplegic woman was admitted with a chronic deformity of the thoracolumbar spine due to a telescoped posterior dislocation of L1 on T12, which had occurred 8 months earlier. Single reduction by axial traction was impossible. Surgical correction was achieved by means of a total, one-block, L1 spondylectomy and end-to-end apposition of the T12 inferior platform onto the L2 superior platform

This study evaluated the bony union obtained through posterior lumbar interbody fusion (PLIF) using Ray's threaded fusion cage (TFC) without other instrumentation. We assessed 25 consecutive patients who underwent treatment using this method. A bone graft was placed in the cages only. No additional instrumentation, such as a pedicle screw system, was used. The study group consisted of 12 men and 13 women. The mean age at the time of operation was 48.6 years, and the mean follow-up period was 44 months. The extent of facet joint destruction, bony union, and correction loss of spondylolisthesis

Traumatic spondylolisthesis in the lower cervical spine is rare and only a few cases have been reported. We present a 56-year-old man who had severe C6-C7 spondylolisthesis without major neurological complications, caused by a traffic accident. Plain CT images showed a pedicular fracture on the right side and a laminar fracture on the left side at C6, but magnetic resonance images revealed no spinal cord compression. Application of a halo brace and maintaining the neck in slight flexion without traction resulted in reduction of the spondylolisthesis to nearly normal alignment. Anterior fusion

STUDY DESIGN: In this cadaveric study, a computer-assisted image guidance system was tested for accuracy of thoracic pedicle screw placement.
OBJECTIVES: Evaluate the system's accuracy for thoracic pedicle screw placement in vitro.
SUMMARY OF BACKGROUND DATA: The effective use and reliability of pedicle screw instrumentation in providing short-segment stabilization and correction of deformity is well known in the lumbar spine. Pedicle screw placement in the thoracic spine is difficult because of the small dimensions of the thoracic pedicles and risk to the adjacent spinal cord and neurovascu

This is a retrospective study aimed to analyse the clinical outcomes of patients following anterior cervical decompression and fusion for radiculopathy in worker's compensation, third party and non-compensable group. The outcome of 224 cases operated between 1991 to 1998 were analysed. Only patients with radiculopathy due to a cervical disc protrusion and spondylosis were included. There were 140 non-compensable patients, 58 worker's compensation and 26 third party. There was no statistical difference in radiological fusion between the three groups (P=0.46). The worker's compensation and third

Skeletal sarcoidosis is rare. This report describes a 31-year-old African American man who presented with a destructive osteolytic lesion of C2 and another lesion in a rib. The lesion at C2 was treated with corpectomy and bone graft. Four months later the lesion recurred and a new lesion was discovered in the cervical vertebral column. The patient declined surgery for instability for another 3 months, choosing to remain in a halo. Seven months following the initial operation, a technetium bone scan showed spread of the disease to the calvarium and thoracic and lumbar vertebrae. The patient had

The vertebral abnormalities present in 35 patients diagnosed with Goldenhar's syndrome are reported. There was no absolute correlation between the presence of any of the formative or segmentation vertebral defects and that of other concomitant malformations. This study supports the conclusion that patients with various groupings of Goldenhar-related anomalies should be considered as a single entity, to which the name Goldenhar association could be applied.

50 patients with a history of distortion injury of the cervical spine were examined with static and functional MRI. Functional MRI consisted of different patient's positions from maximal extension to maximal flexion (30 degrees, 0 degrees, 25 degrees, 40 degrees, 50 degrees). T2*-weighted gradient echo sequences were performed in a sagittal view for the different positions. Ligamentous instabilities and disc protrusions were seen only in functional MRI in 17 patients. These findings correlated with the neurological symptoms. Two patients were treated by operative fusion because of these findin

A simplified method of posterior atlantoaxial arthrodesis allowing immediate stabilization and mobilization is described. This method of fixation has been used since 1984 in 33 cases, with the indications being nonunion of odontoid peg fractures, atlantoaxial dislocation, and other traumatic conditions about the axis. A solid fusion was noted in all 33 patients by 3 months postsurgery, and the complication rate was minimal. This method of atlantoaxial arthrodesis is simple, allows immediate mobilization, controls rotation and lateral bending, and is inexpensive.

Short-segment stability problems of the spine, such as fracture, pseudoarthrosis, tumor, degenerative disc, and spondylolisthesis should be resolved by short-segment stabilization methods. The "rod long, fuse short" technique using traditional Harrington or Luque rods still have unavoidable problems of facet arthritis at the segments temporarily instrumented but not grafted. Transpedicular fixation of the spine is currently being used in many systems. Biomechanical studies have shown that three-level transpedicular fixation is more rigid than five-level fixation of Harrington or Luque rods. Re

Ten patients with symptomatic spondylolysis or Grade I spondylolisthesis were treated with the Buck method. At follow-up, nine patients were graded as successful. All patients fused. Pain relief, level of function, and likelihood of return to work were higher in patients preoperatively selected by lidocaine infiltration of the pars defect. Pars infiltration gives an accurate prediction of successful outcome following pars repair.

The paraspinal approach was described by our group in 1968. It differs from the approach described by Melvin Watkins in 1953 in that it is a longitudinal separation of the sacrospinalis group between the multifidus and longissimus, and not between the lateral border of the entire sacrospinalis group and quadratus lumborum. Also, Watkins removed a flake of the iliac crest with muscles attached, which he swung cranially and medially. This article also describes several refinements not mentioned in the original article and gives several new uses for the approach. Specifically, its use for removin

We are reporting the cases of five patients who had occipito-atlantal instability, a rare condition that may be due to either trauma or congenital abnormalities. In three of the patients the instability was secondary to trauma. The clinical and neurological manifestations were varied and included cardiorespiratory arrest, motor weakness, quadriplegia, torticollis, pain in the neck, vertigo, and projectile vomiting. All of the patients underwent posterior arthrodesis of the occiput to the first or second cervical vertebra. In the patients who had trauma-related instability, surgery was performe
