Spinal Arachnoiditis as a Complication of Cryptococcal Meningoencephalitis in Non-HIV Previously Healthy Adults.

BACKGROUND: Cryptococcus can cause meningoencephalitis (CM) among previously healthy non-HIV adults. Spinal arachnoiditis is under-recognized, since diagnosis is difficult with concomitant central nervous system (CNS) pathology. METHODS: We describe 6 cases of spinal arachnoiditis among 26 consecutively recruited CM patients with normal CD4 counts who achieved microbiologic control. We performed detailed neurological exams, cerebrospinal fluid (CSF) immunophenotyping and biomarker analysis before and after adjunctive immunomodulatory intervention with high dose pulse corticosteroids, affordin

Identification of bladder and colon afferents in the nodose ganglia of male rats.

The sensory neurons innervating the urinary bladder and distal colon project to similar regions of the central nervous system and often are affected simultaneously by various diseases and disorders, including spinal cord injury. Anatomical and physiological commonalities between the two organs involve the participation of shared spinally derived pathways, allowing mechanisms of communication between the bladder and colon. Prior electrophysiological data from our laboratory suggest that the bladder also may receive sensory innervation from a nonspinal source through the vagus nerve, which inner

When gout involves the spine: five patients including two inaugural cases.

UNLABELLED: Spinal involvement is uncommon during gout and may raise diagnostic challenges. We describe five cases seen at a single center. METHODS: We retrospectively reviewed the medical charts of the five patients with spinal gout seen over a 3-year period. RESULTS: There were four men and one woman with an age range of 52 to 87 years. One patient presented with acute neck pain and visualization by imaging studies of a discovertebral tophus, another had febrile arthritis of a lumbar facet joint, and a third presented with a synovial cyst in a lumbar facet joint. The remaining two patients

Differential localization of gamma-aminobutyric acid type A and glycine receptor subunits and gephyrin in the human pons, medulla oblongata and uppermost cervical segment of the spinal cord: an immunohistochemical study.

Gephyrin is a multifunctional protein responsible for the clustering of glycine receptors (GlyR) and gamma-aminobutyric acid type A receptors (GABA(A)R). GlyR and GABA(A)R are heteropentameric chloride ion channels that facilitate fast-response, inhibitory neurotransmission in the mammalian brain and spinal cord. We investigated the immunohistochemical distribution of gephyrin and the major GABA(A)R and GlyR subunits in the human light microscopically in the rostral and caudal one-thirds of the pons, in the middle and caudal one-thirds of the medulla oblongata, and in the first cervical segmen

The pathophysiology and nonsurgical treatment of lumbar spinal stenosis.

Lumbar spinal stenosis, which affects an ever-increasing number of patients, is best defined as a collection of clinical symptoms that includes low back pain, bilateral lower extremity pain, paresthesias, and other neurologic deficits that occur concomitantly with anatomic narrowing of the neural pathway through the spine. The narrowing may be centrally located in the spinal canal or more laterally in the lateral recesses or neuroforamina. Lumbar spinal stenosis can have a congenital or acquired etiology, and the origin of acquired lumbar stenosis is classified as degenerative, posttraumatic,

Spinal cord injury-induced plasticity in the mouse--the crossed phrenic phenomenon.

The crossed phrenic phenomenon (CPP) describes respiratory functional plasticity that arises following spinal cord injury. Cervical spinal cord hemisection rostral to the phrenic nucleus paralyzes the ipsilateral hemidiaphragm by interrupting the descending flow of respiratory impulses from the medulla to phrenic motoneurons in the spinal cord. This loss of activity converts some synapses on phrenic motoneurons from a "functionally ineffective" state pre-hemisection to a "functionally latent" state post-hemisection. If the animal is subjected to respiratory stress by transecting the contralate

Intra-operative spinal cord neuromonitoring in patients operated on for intramedullary tumors and syringomyelia.

Intra-operative neuromonitoring is of great help to the neurosurgeon and enables him to operate more precisely and with less risk of post-operative neurological deficit. The purpose of the study was to define the changes of the somatosensory evoked potentials (SEP) elements in intramedullary tumors and syringomyelia, and to show the correlation between the changes of the elements and the location of the lesions in the spinal cord. Thirty patients with pain syndromes, intramedullary tumors and syringomyelia were operated on the spinal cord. The methodology of the intra-operative neuromonitoring

Hyperactive pectoralis reflex as an indicator of upper cervical spinal cord compression. Report of 15 cases.

Myelopathy from cervical spondylosis is often accompanied by hyperreflexia of the upper-extremity deep tendon reflexes (DTRs). Reflexes such as the pectoralis jerk and the deltoid jerk may only be apparent in the context of hyperreflexia. Although the nerve roots involved in the reflex arcs are well described, levels of cervical spinal cord compression that lead to the hyperreflexia are not as clear. This is of particular significance for patients with multilevel cervical spondylosis in determining the levels responsible for their symptoms. The authors examined 15 consecutive patients who pres

Anatomical significance of the nerve to the pyramidalis muscle: a morphological study.

Characteristics of the nerve to the pyramidalis muscle (NPy), including its origin, course and distribution, were observed (macroscopically) in detail in the present study. The spinal segments that give rise to the nerve vary considerably and involve Th12-L2. The course and distribution of the nerve also vary widely. The NPy is given off from one of the following: (i) the anterior cutaneus branch (Rca) of the intercostals nerve; (ii) the ilioinguinal (li) nerve; or (iii) the genital branch (Rg) of the genitofemoral nerve. The NPy can be classified into nine types according to features of the c

[Local anomalies and secondary degenerative-dystrophic damage in the craniovertebral region].

Clinical and x-ray studies of patients with focal anomalies of the osteoarticular apparatus of the craniovertebral area demonstrated that half of the cases showed early complications in the form of secondary degenerative-dystrophic lesions. This process is aggravated proportionally to the degree of pronounced interrelation disturbances and overstrain in the craniovertebral area. The secondary degenerative-dystrophic lesions indicate disordered compensation and significantly aggravate the clinical state and working capacity of the patients.

Neuroblastoma of the lumbosacral canal in an adult: a case report and literature review.

Neuroblastoma (NB) is a leading cause of death in children. It usually occurs in the adrenal gland and rarely in the spinal canal. Here, we report the case of a 48-year-old male patient with abnormal thickening of the cauda equina nerve as revealed by lumbosacral magnetic resonance imaging. The patient's main clinical manifestations were numbness and pain in both lower limbs. The patient underwent surgical treatment; however, intraoperatively, an unclear border was observed between the cauda equina nerve and the tumor; therefore, the tumor was not forcibly excised. The postoperative pathologic

Ultrasound-guided peripheral nerve stimulation for neuropathic pain after brachial plexus injury: two case reports.

Brachial plexus injury (BPI) often causes severe neuropathic pain that becomes chronic and difficult to treat pharmacologically or surgically. Here, we describe two cases of successful treatment of BPI with peripheral nerve stimulation (PNS). Both patients had experienced severe neuropathic pain after incomplete BPI for a long time (32 and 17 years) and did not response to medication, radiofrequency neuroablation, or spinal cord stimulation. After PNS using ultrasound, their pain was relieved by more than 50% over the course of 1 year. Both patients were satisfied with their improved sleep and

Traumatic atlanto-occipital dislocation presenting with Dysphagia as the chief complaint: a case report.

We report a patient with traumatic atlanto-occipital dislocation who presented with dysphagia as the chief complaint. A 59-year-old man complained of swallowing difficulty for 2 months after trauma to the neck. On physical examination, there was atrophy of the right sternocleidomastoid and upper trapezius muscles, and the tongue was deviated to the right. In a videofluoroscopic swallowing study, penetration and aspiration were not seen, food residue remained in the right vallecula and pyriform sinus, and there was decreased motion of the soft palate, pharynx and larynx. Cavernous angioma of the cauda equina: a case report and systematic review of the literature.

Cavernous angioma is an uncommon vascular malformation of the central nervous system with a tumoural aspect. Spinal cavernous angioma mainly occurs within vertebral bodies; only 3-5% of tumours are located entirely in the vertebral canal. This case report describes a case of cavernous angioma, originating from the nerve roots of the cauda equina at the L1 level, in a 57-year-old woman presenting with acute lower back pain. The lesion was surgically resected 6 months after symptom onset and the structural integrity of the nerve root was maintained. Histopathological examination confirmed the di

Continuous spinal anesthesia for hip replacement in an elderly patient with an anterior mediastinal mass: a case report.

Incremental dosing of intrathecal local anesthetic for an indefinite duration performed during continuous spinal anesthesia (CSA) allows for better control of adverse effects caused by sympathetic nerve blockade. This case report describes a 91-year-old female with a left intertrochanteric fracture and a large thyroid mass causing contralateral anterior displacement of the trachea and the great vessels. We conducted a detailed investigation of the different techniques that can be used in such circumstances and the consequences each may have on a patient. We concluded that: better control of sy

Epidural haemorrhage of the cervical spinal cord: a post-mortem artefact?

Spinal epidural haemorrhage is a rare entity that occurs uncommonly in adults and rarely in children. It has a typical clinical presentation, although to date, the cause for the majority of cases remains unknown. We present a series of cases where epidural haemorrhage was identified at post-mortem, principly to the cervical cord, in cases outside the age range usually reported for clinical epidural haemorrhage, and with no underlying pathology to account for the finding. We present a hypothesis for a post-mortem cause for this finding and consider that, in the absence of any other identifiable
