Unilateral C-1 lateral mass sagittal split fracture: an unstable Jefferson fracture variant.

OBJECT: The object of this study was to describe an unusual fracture subtype within C-1 injuries with a propensity to result in late deformity and pain. Most patients with C-1 injuries are nonsurgically treated using external immobilization unless there is an injury of the transverse atlantal ligament. The authors describe an unusual variant involving a unilateral sagittal split with a high tendency to late deformity and pain. They also review the literature and treatment of C-1 fractures. METHODS: A retrospective review of 12,671 CT scans from a Level I trauma center over a 6-year period yie

Vertebral artery injury in cervical spine surgery: anatomical considerations, management, and preventive measures.

BACKGROUND CONTEXT: Vertebral artery (VA) injury can be a catastrophic iatrogenic complication of cervical spine surgery. Although the incidence is rare, it has serious consequences including fistulas, pseudoaneurysm, cerebral ischemia, and death. It is therefore imperative to be familiar with the anatomy and the instrumentation techniques when performing anterior or posterior cervical spine surgeries. PURPOSE: To provide a review of VA injury during common anterior and posterior cervical spine procedures with an evaluation of the surgical anatomy, management, and prevention of this injury.

Early stabilization and decompression for incomplete paraplegia due to a thoracic-level spinal cord injury.

All patients treated between 1985 and 1990 for acute incomplete spinal cord injury between T2 and T11 were retrospectively studied. This level was chosen for study because by excluding cervical cord, conus, and cauda equina injuries, neurologic improvement could be attributed to improvement of spinal cord function. Only 14 patients with incomplete thoracic level paraplegia were identified, representing 1.2% of all spinal injuries. All 14 patients were treated by early operative reduction, stabilization, or decompression. Twelve patients had surgery within 24 hours of neurologic injury, one at

The evolving concept of conversion surgery for upfront unresectable upper gastrointestinal and hepato-pancreato-biliary cancers: comprehensive review.

BACKGROUND: In the absence of a commonly accepted definition, conversion surgery is generally considered as surgical resection with the intent of prolonging survival after non-surgical induction therapy in patients with upfront unresectable disease at diagnosis. Despite the heterogeneity of possible targets, conversion surgery is a quickly evolving concept, with commonalities for upper gastrointestinal (UGI) and hepato-pancreato-biliary (HPB) malignancies. METHODS: A comprehensive narrative review of the most recent and relevant literature was conducted by experts in the field of different UG

Endoscope-Assisted Neuroportal Transcerebellar Approach to the Fourth Ventricle: An Anatomical Study.

BACKGROUND AND STUDY AIMS: Minimally invasive approaches to deep-seated lesions still represent a fundamental issue in modern neurosurgery. Tubular retractors allow to enhance the operability of intraventricular lesions, minimizing the risk of damages to brain parenchyma. Increasing interest for portal devices has been mainly focused on supratentorial pathologies, while transportal approaches in the posterior cranial fossa have been rarely described. In the present study, the authors aimed to investigate the surgical exposure and operability obtained with a microsurgical neuroportal transcereb

Decision-centered artificial intelligence for perioperative care outside the operating room: a practical review for surgeons.

Artificial intelligence (AI) is increasingly being applied across the spectrum of surgical care; however, most existing review articles have organized prior studies primarily by algorithmic type or predicted outcomes. Consequently, a structured understanding of "when" and "why" AI is integrated into real-world clinical workflows, and how its outputs inform surgical decisionmaking, remains insufficiently developed. Although both the preoperative and postoperative phases involve risk prediction, they differ fundamentally in their decision contexts, data characteristics, and modes of clinical app

Comparison of Perioperative Complication Rates in Congenital Scoliosis Patients With and Without Tethered Spinal Cord.

BACKGROUND: Congenital early onset scoliosis (C-EOS) often co-occurs with tethered spinal cord syndrome (TSCS), necessitating surgical intervention to address both conditions to prevent worsening neuromuscular function. Detethering can be done concurrently with spinal deformity correction (SDC), before SDC, or not done at all. This study explores perioperative complications in C-EOS patients with and without TSCS who underwent SDC with growing instrumentation or fusion. We hypothesize that C-EOS patients with a history of TSCS who underwent SDC with either growing instrumentation or fusion exp

Center of rotation analysis for thoracic and lumbar 3-column osteotomies in patients with sagittal plane spinal deformity: insights in geometrical changes can improve understanding of correction mechanics.

OBJECTIVE: Three-column osteotomy (3CO) is used for severe spinal deformities. Associated complications include sagittal translation (ST), which can lead to neurological symptoms. Mismatch between the surgical center of rotation (COR) and the concept of the ideal COR is a potential cause of ST. Matching surgical with conceptual COR is difficult with pedicle subtraction osteotomy (PSO) and vertebral column resection (VCR). This mismatch influences correction geometry, which can prevent maximum possible correction. The authors' objective was to examine the sagittal correction geometry and surgic

Treatment Guideline for Patients with Native Culture-negative Pyogenic Vertebral Osteomyelitis.

BACKGROUND: With the development of evidence-based guidelines for the diagnosis and antibiotic treatment of patients with pyogenic vertebral osteomyelitis, various nonsurgical and surgical treatment strategies have achieved favorable results. However, sufficient administration of appropriate antibiotics is a prerequisite for treatment success, which cannot be guaranteed in patients with culture-negative pyogenic vertebral osteomyelitis. Unfortunately, previous studies on culture-negative pyogenic vertebral osteomyelitis were limited by small patient groups, short follow-up periods, varied trea

Surgical site infections in early onset scoliosis: what are long-term outcomes in patients with traditional growing rods?

PURPOSE: Deep surgical site infections (SSIs) are a common and potentially severe complication in early onset scoliosis (EOS) patients. We sought to identify the long-term outcomes following SSI, specific risk factors associated with recurrent infections, and if instrument retention is a prudent SSI management strategy in EOS. METHODS: We performed a retrospective review of all EOS patients who underwent traditional growing rod spine procedures from 2003 to 2017. Infections were categorized as single or multiple SSIs. All infections were treated with operative irrigation and debridement (I&D)

Fingertip Forces and Thenar Pressure During Instrument Use in Simulated Meniscectomy: A Comparative Study of Rongeur and Forceps.

BACKGROUND: Hand-intensive surgical tasks expose the thumb and surrounding musculature to repetitive mechanical loading. Instrument design may influence the distribution of forces across the hand, yet quantitative data characterizing these loads during routine orthopedic procedures remain limited. This study quantified fingertip forces and thenar pressure during simulated meniscectomy using a pituitary rongeur and Ferris-Smith forceps. METHODS: Orthopedic residents performed simulated meniscectomy on a porcine knee model while wearing TekScan pressure sensors. Each participant used both a pit
