Dosimetric Comparisons of Volumetric Modulated Arc Therapy and Tomotherapy for Early T-Stage Nasopharyngeal Carcinoma.

PURPOSE: To compare the dosimetric differences between volumetric modulated arc therapy (VMAT) and helical tomotherapy (HT) in treating early T-stage nasopharyngeal carcinoma (NPC). METHOD: Ten patients with early T-stage NPC who received tomotherapy using simultaneously integrated boost (SIB) strategies were replanned with VMAT (RapidArc of Varian, dual-arc). Dosimetric comparisons between the RapidArc plan and the HT plan included the following: (1) D98, homogeneity, and conformity of PTVs; (2) sparing of organs at risk (OARs); (3) delivery time and monitor units (MUs). RESULTS: (1) Compar

Do recurrent seizures cause neuronal damage? A series of studies with MRI volumetry in adults with partial epilepsy.

Despite optimal treatment, 30% of epilepsy patients develop intractable epilepsy and continue to have recurrent seizures or other symptoms of epileptic syndrome restricting their ability to lead a full life. Hippocampal sclerosis is found in 60-70% of patients with intractable temporal lobe epilepsy (TLE). However, it is not known whether the damage in the hippocampus is the cause or the consequence of TLE. The purpose of the present series of studies was to investigate with magnetic resonance imaging (MRI) the appearance of medial temporal lobe damage during the course of partial epilepsy, an

Dynamic face-related eye movement representations in the human ventral pathway.

Multiple brain areas along the ventral pathway have been known to represent face images. Here, in a magnetoencephalography (MEG) experiment, we show dynamic representations of face-related eye movements in the ventral pathway in the absence of image perception. Participants followed a dot presented on a uniform background, the movement of which represented gaze tracks acquired previously during their free-viewing of face and house pictures. We found a dominant role of the ventral stream in representing face-related gaze tracks, starting from the orbitofrontal cortex (OFC) and anterior temporal

Transcranial ultrasound stimulation selectively enhances fronto-temporal context-guided memory.

There is substantial scientific interest in improving approaches that can enhance cognition through brain stimulation. We implemented a non-invasive focal Transcranial Ultrasound Stimulation (TUS) approach with known longer-lasting post-stimulation effects in two rhesus macaques performing a context-dependent memory-sequencing task implemented on multiple touchscreens within their home units. Consistently in both monkeys, TUS to the anterior - but not posterior - medial temporal lobe enhanced performance under stable memory-sequencing contexts. TUS to the medial prefrontal cortex, on the other

Urea cycle disorder presenting as bilateral mesial temporal sclerosis - an unusual cause of seizures: a case report and review of the literature.

BACKGROUND: Urea cycle disorders are secondary to defects in the system converting ammonia into urea, causing accumulation of ammonia and other byproducts which are neurotoxic. Ornithine transcarbamylase deficiency is the most common of the urea cycle disorders and frequently presents with coma or seizures during hyperammonemia. However, seizures can also occur without metabolic decompensation. CASE PRESENTATION: We describe a 23-year-old Chinese woman with urea cycle disorder who presented with confusion due to focal seizures arising from the left frontotemporal region. Interestingly, her am

Structural versus functional prediction of memory change following anterior temporal lobectomy.

Decline in recent memory function is a significant risk for patients undergoing anterior temporal lobectomy. We report a patient with a febrile seizure history, complex partial seizures arising from the left anterior temporal lobe, and MRI evidence suggesting left hippocampal sclerosis, all of which indicate a low likelihood of significant postoperative memory decline. However, high normal verbal memory on neuropsychological testing and bilaterally normal Wada memory scores indicated increased risk for postoperative memory decline. Following left anterior temporal lobectomy, the patient displa

Whole-brain gray matter volume mediates the relationship between psychological distress and job satisfaction.

Individual differences in brain structure are not only seen between healthy and diseased individuals, or between young and elderly people, but also in middle-aged men and women. Previous studies have shown that the cause is related to psychological distress and stress, and that it is accompanied by a decline in performance. This suggests that among middle-aged male and female workers who consider themselves healthy, the brain may atrophy depending on the level of psychological distress, which may reduce job satisfaction. Therefore, in this study, we analyzed the relationship between brain stru

Dysembryoplastic Neuroepithelial Tumor: A Case Report of A Benign Intracranial Lesion Masquerading as Seizure Disorder.

The uncommon, benign dysembryoplastic neuroepithelial tumor (DNET, WHO grade 1) is frequently linked to epilepsy. It is a glioneuronal neoplasm in the cerebral cortex of children or young adults defined by the presence of a pathognomonic glioneuronal element that may be linked to glial nodules and activating mutations of fibroblast growth factor receptor 1 (FGFR1) (CNS WHO grade 1 according to WHO classification of CNS and pituitary tumors, 2021 ). The cerebral cortex is primarily affected. The most frequent areas are the temporal lobe, particularly the medial lobe, frontal lobe, and other cor

[A case of primary central nervous system vasculitis diagnosed by second brain biopsy and treated successfully].

We report a case of primary central nervous system vasculitis (PCNSV) diagnosed by second brain biopsy. A 53-year-old man initially presented with left lateral gaze diplopia. Brain MRI revealed multiple enhanced lesions in the bilateral frontal lobe, bilateral basal ganglia, left cerebellum and brainstem. An initial brain biopsy of the right frontal lobe suggested immune-related encephalitis with angiocentric accumulation of chronic inflammatory cells, while malignant lymphoma could not be completely ruled out. The patient deteriorated despite being treated with repeated methylprednisolone pul

Single-photon emission computed tomographic investigation of patients with motor neuron disease.

To investigate the correlation between involvement of the CNS in motor neuron disease (MND) and neuroimaging abnormalities, we studied 18 patients with MND by single-photon emission computed tomography (SPECT) and MRI. Patients were divided into four groups according to the results of SPECT. Group 1 consisted of four patients with reduced isotope uptake in the frontal lobe, including the motor area, and in the anterior part of the temporal lobe; group 2 consisted of two patients with reduced isotope uptake in the motor area spreading to the adjacent frontal lobe; group 3 consisted of eight pat

Temporal Base Transdural Anastomosis in Moyamoya Disease: A Potential Association with Posterior Cerebral Artery Involvement and Clinical Importance.

Moyamoya disease is associated with the formation of collateral pathways, including transdural anastomosis, such as vault and ethmoidal moyamoya. However, temporal base transdural anastomosis remains unrecognized. This study investigates the imaging characteristics and features of temporal base transdural anastomosis in moyamoya disease. This retrospective review was conducted on 164 hemispheres from 82 patients with moyamoya disease admitted to our institution between 2012 and 2024. Digital subtraction angiography helped identify temporal base transdural anastomosis, which is classified as "o

Neural activities during the Processing of unattended and unseen emotional faces: a voxel-wise Meta-analysis.

Voxel-wise meta-analyses of task-evoked regional activity were conducted for healthy individuals during the unconscious processing of emotional and neutral faces with an aim to examine whether and how different experimental paradigms influenced brain activation patterns. Studies were categorized into sensory and attentional unawareness paradigms. Thirty-four fMRI studies including 883 healthy participants were identified. Across experimental paradigms, unaware emotional faces elicited stronger activation of the limbic system, striatum, inferior frontal gyrus, insula and the temporal lobe, comp

What bilateral damage of the superior parietal lobes tells us about visual attention disorders in developmental dyslexia.

Neuroimaging studies have identified the superior parietal lobules bilaterally as the neural substrates of reduced visual attention (VA) span in developmental dyslexia. It remains however unclear whether the VA span deficit and the deficits in temporal and spatial attention shifting also reported in dyslexic children reflect a unitary spatio-temporal deficit of attention - probably linked to general posterior parietal dysfunction- or the dysfunction of distinct attentional systems that relate to different neural substrates. We explored this issue by testing an adult patient, IG, with a specifi

Giant cell tumor of temporomandibular joint masquerading as temporomandibular joint pain dysfunction syndrome: a rare case report.

Giant cell tumor (GCT) of the craniofacial bones has been reported but they are not common. This tumor occurs more often in women than in men and predominantly affects patients around the third to fifth decade of life. GCTs are generally benign but can be locally aggressive as well. We report a case of GCT involving the temporomandibular joint (TMJ), which was initially thought to be temporomandibular disorder (TMD). A 22-year-old female presented with swelling and pain over the right temporal region for 18 months associated with jaw locking and clicking sounds. On examination, her jaw deviate

Preserved mid-fusiform activation for visual words in a patient with a visual word recognition impairment.

Previous functional imaging studies have highlighted the role of left ventral temporal cortex in processing written word forms. We explored activation and anatomical connectivity of this region in HE, a professional writer with alexia as a result of stroke affecting primarily white matter in the left inferior temporal lobe. We used a one-back visual recognition task and functional Magnetic Resonance Imaging to elicit automatic activation to various orthographic and non-orthographic stimuli. Surprisingly, HE showed cortical activation in the left mid-fusiform area during the presentation of wor

The usefulness of repeated ictal SPET for the localization of epileptogenic zones in intractable epilepsy.

This study investigated whether repeated ictal single-photon emission tomography (SPET) is helpful in the localization of epileptogenic zones and whether it can provide information confirming that an area of increased perfusion is really the culprit epileptogenic lesion. Fifty-four repeated ictal SPET studies were performed in 24 patients with ambiguous or unexpected findings on the first ictal SPET study. These patients were enrolled from among 502 patients with intractable epilepsy in whom pre-operative localization of epileptogenic zones was attempted with a view to possible surgical resect

Surgical management of brain tissue herniation into the middle ear and mastoid.

In the well pneumatized temporal bone, the temporal lobe of the brain is separated from the middle ear and mastoid process by a thin layer of bone known as the tegmen. Congenital defects, infection, and trauma can alter this structure in such a way that cerebral tissue herniates into the ear. This unusual condition may precipitate numerous otologic problems such as hearing loss, trapped squamous epithelium, and the potential for meningitis or encephalitis. The purpose of this paper is to define the problem as well as discuss the diagnosis and surgical management. The authors feel this should b

Bringing psychiatrists into the picture: Automated measurement of regional MRI brain volume in patients with suspected dementia.

OBJECTIVE: The volumes of various brain regions can be rapidly quantified using automated magnetic resonance imaging tools. While these appear to be useful at face value, their formal clinical utility is not yet understood, particularly for non-neuroradiologists and in patients presenting with suspected dementia. This study investigated the utility of an automated normative morphometry tool on determinations of brain atrophy by psychiatrists and radiologists in a tertiary hospital. METHODS: Consecutive magnetic resonance scans (n = 110) of patients referred with suspected neurodegenerative di

Crossed cerebellar diaschisis: risk factors and prognostic value in focal cortical dysplasia by 18F-FDG PET/CT.

PURPOSE: Crossed cerebellar diaschisis (CCD) has been widely studied in hemispheric stroke but is less characterized in epilepsy. In this study, we used 18F-FDG PET/CT to investigate the risk factors for CCD and its prognostic value for intractable epilepsy caused by focal cortical dysplasia (FCD). METHODS: One hundred medically intractable epilepsy patients pathologically diagnosed with FCD postoperatively were included and classified into two groups: CCD+ and CCD-. PET/CT images were analyzed qualitatively by visual assessment and semi-quantitatively using the absolute asymmetry index (|AI|
