Development, Feasibility, Acceptability, and Usability of an Artificial Intelligence-Powered Chatbot (Suzy) to Support Patients in Substance Use Disorder Recovery: Multiphase Study.

BACKGROUND: Substance use disorder (SUD) remains a major public health crisis in the United States, with significant challenges in treatment access, retention, and workforce capacity. SUD care teams, including addiction medicine physicians and peer recovery coaches (PRCs), support patients receiving SUD treatment but face heavy workloads and burnout. Artificial intelligence (AI) innovations, particularly large language model (LLM)-based chatbots, may extend PRC support and provide patients with on-demand recovery support between clinic visits and PRC contacts. However, evidence on their develo

Future Perspectives in Radiology: Artificial Intelligence for Responsible Imaging (AIRI).

Artificial intelligence (AI) has transformed the healthcare industry. In the field of radiology, AI has shown great promise in medical imaging, as it can enhance imaging accuracy, reduce diagnostic errors, and optimize workflow. However, the overuse and misuse of imaging is still a major ongoing problem, generating unnecessary costs, radiation exposure to patients, and delays in diagnosis. To address these challenges, we propose AI for responsible imaging (AIRI), a multimodal AI system to aid clinicians in making informed imaging decisions. AIRI is envisioned as a medical resource that would b

[Recent developments of pesticide adsorbents based on cyclodextrins].

The invention and application of pesticides have greatly increased the yield of crops, greatly contributing to ensuring people's basic livelihoods and gradually improving their livelihoods to a well-off level. However, foods, water sources, and soil, containing high levels of pesticide residues, result in increasingly serious pollution. Pesticide residues usually have the characteristics of micro toxicity, difficult biodegradation, and bioaccumulation, and thus pose serious threat to living organisms and ecosystems. In recent years, pesticide pollution has earned worldwide focus. Thus, methods

Patients' and Nurses' Experiences and Perceptions of Remote Monitoring of Implantable Cardiac Defibrillators in Heart Failure: Cross-Sectional, Descriptive, Mixed Methods Study.

BACKGROUND: The new generation of implantable cardioverter-defibrillators (ICDs) supports wireless technology, which enables remote patient monitoring (RPM) of the device. In Sweden, it is mainly registered nurses with advanced education and training in ICD devices who handle the arrhythmias and technical issues of the remote transmissions. Previous studies have largely focused on the perceptions of physicians, and it has not been explored how the patients' and nurses' experiences of RPM correspond to each other. OBJECTIVE: Our objective is to describe, explore, and compare the experiences an

Keep recycling going: New approaches to reduce LDL-C.

Hypercholesterolemia represents a leading cause in the development of atherosclerotic plaques, increasing the risk for ACVS. It actually counts as a major cause of cardiovascular disease etiopathogenesis. The causes of hypercholesterolemia are multifactorial, spanning from genetic constitution, age, sex, to sedentary lifestyle and diets rich in sugars and lipids. Although dietary restriction in saturated fats, increased exercise, and other modification in lifestyle represent a first-line approach to treat very initial stages in hypercholesterolemia, most patients will require the addition of p

Preparing Japanese surgeons for potential mass casualty situations will require innovative and systematic programs.

INTRODUCTION: The ongoing state of global geo-political instability means that it is prudent to prepare civilian surgeons to manage major military-type trauma. Japan has enjoyed a prolonged period of peace and consequently it is unlikely that surgeons will have been exposed to a sufficient volume of cases. This study reviews the state of trauma training and preparedness in Japan and reviews the trauma workload of a major Japanese emergency medical center and compared with a major South African trauma center with the intention of quantifying and comparing the time needed to gain adequate exposu

Ventriculo-arterial discordance: switching the morphologically left ventricle into the systemic circulation after 3 months of age.

OBJECTIVE: To retrospectively examine a 4 year policy of restoring the morphologically left ventricle to the systemic circuit in patients presenting after 3 months of age with ventriculo-arterial discordance with or without associated atrio-ventricular discordance. This policy was stimulated by the known tendency of the morphologically right ventricle to develop dysfunction sooner or later when left in the systemic circuit. Such a policy dictates a more complex surgical approach and, at this point, it remains controversial whether or not the increased surgical complexity is warranted. METHODS

Initial Insights Into an Institutional Secure Large Language Model for Magnetic Resonance Imaging Examination Requests: Retrospective Study.

BACKGROUND: Incomplete clinical details on magnetic resonance imaging (MRI) examination requests (MERs) can lead to suboptimal protocol selection. An institutional secure large language model (sLLM) with access to manually retrieved salient data from the electronic medical record (EMR) may improve request completeness and protocol accuracy across multiple MRI subspecialties. OBJECTIVE: The objective of this study was to compare clinician MERs with sLLM-augmented MERs for information quality and to evaluate the protocoling accuracy of the sLLM versus board-certified radiologists across body, m

Efficient and accurate commissioning and quality assurance of radiosurgery beam via prior-embedded implicit neural representation learning.

BACKGROUND: Dosimetric commissioning and quality assurance (QA) for linear accelerators (LINACs) present a significant challenge for clinical physicists due to the high measurement workload and stringent precision standards. This challenge is exacerbated for radiosurgery LINACs because of increased measurement uncertainty and more demanding setup accuracy for small-field beams. Optimizing physicists' effort during beam measurements while ensuring the quality of the measured data is crucial for clinical efficiency and patient safety. PURPOSE: To develop a radiosurgery LINAC beam model that emb

The Impact of Additional Exercise Interventions on Physical Performance and Muscle Strength of Frail Patients After Open-Heart Surgery: A Randomized Trial.

Background and Objectives: Frail patients after open-heart surgery often experience worse treatment outcomes in improving physical performance and muscle strength. As the functional recovery of frail patients after open-heart surgery is slower, conventional rehabilitation is frequently insufficient to achieve treatment goals. Therefore, the inclusion of additional exercise interventions in cardiac rehabilitation is becoming more relevant. The aim of this study was to assess and compare the effectiveness of additional exercise interventions-multicomponent and computer-based programs-applied alo

Radiofrequency catheter modification of the sinus node for "inappropriate" sinus tachycardia.

BACKGROUND: Radiofrequency catheter ablation is the treatment of choice for patients with paroxysmal supraventricular tachycardias refractory to medical therapy. However, in symptomatic patients with inappropriate sinus tachycardia resistant to drug therapy, catheter ablation of the His' bundle with permanent pacemaker insertion is currently applied. We evaluated the safety and efficacy of radiofrequency modification of the sinus node as alternative therapy for patients with inappropriate sinus tachycardia. METHODS AND RESULTS: Sixteen patients with disabling episodes of inappropriate sinus t

Exercise training improves left ventricular diastolic filling in patients with dilated cardiomyopathy. Clinical and prognostic implications.

BACKGROUND: Patients with dilated cardiomyopathy (DCM) often have left ventricular (LV) diastolic dysfunction that can precede the development of systolic dysfunction. Recent reports showed that exercise training (ET) improves the exercise capacity of these patients. Although this improvement is primarily due to peripheral adaptations, the contribution of LV diastolic filling has not been well defined. The purpose of this study was to determine whether ET can induce changes in LV diastolic filling that can account for an increase in exercise capacity and whether these changes can influence pro

Improving Sensitivity of Tinnitus Screening in Hearing Conservation Programs.

INTRODUCTION: Tinnitus is prevalent among U.S. military Service Members (SMs). Current Department of Defense (DOD) Hearing Conservation Program (HCP) protocols use a standardized annual screening question to identify bothersome tinnitus. However, this question fails to distinguish transient ear noise-a benign, short-lived auditory phenomenon-from tinnitus. The Department of Veterans Affairs (VA)/DOD Tinnitus Working Group consequently modified the screening question to integrate symptom duration, frequency, and impact on life. The purpose of this study was to evaluate whether the modified scre

Multi-view constrained semi-supervised vertebra detection for 3D ultrasound spine volume.

BACKGROUND: Scoliosis is a spinal disorder characterized by a three-dimensional (3D) deformity of the vertebral column. 3D ultrasound imaging has been used for scoliosis assessment, and automatic vertebra detection is important for objective evaluation of spinal deformity. However, existing automatic assessment methods are mainly based on two-dimensional (2D) projection images and require large amounts of labeled data, leading to incomplete use of volumetric ultrasound information and substantial annotation effort. PURPOSE: To develop a multi-view constrained semi-supervised learning (SSL) me
