An international perspective on grief in neurosurgery: a survey study on how neurosurgeons deal with complications.

OBJECTIVE: Complications in neurosurgery are virtually inevitable due to the specialty's high-stakes nature. The emotional impact of these events on neurosurgeons is profound, yet little is known about how surgeons cope across different healthcare and cultural systems. Understanding these coping mechanisms is critical to support well-being and reduce burnout. Hence, the authors aimed to characterize the emotional and behavioral responses of neurosurgeons to complications and to examine differences in coping strategies between practitioners in Europe and the US. METHODS: The authors conducted

Does financial compensation matter for research participation in prevention trials in Tanzania? Perspectives of healthy volunteers.

BACKGROUND: Financial compensation is an important aspect of respect for people enrolled in clinical research, yet continues to raise ethical questions. Respondents' characteristics, shaped by contextual and geographic factors, may influence perceptions of the ethical implications of financial compensation. This study explored the relationship between respondents' characteristics and their ethical perspective on the role of financial compensation in prevention trials in Tanzania. MATERIALS AND METHODS: A cross-sectional survey of 537 participants from two prevention studies within Tanzania wa

[The influence of relationship dynamics on the psychosocial outcome of genetically related and unrelated living kidney donor-recipient pairs].

OBJECTIVES: Renal diseases and their treatment can cause distress and psychosocial problems for both patients and relatives. METHODS: Relationship dynamics and parameters of quality of life were assessed in 10 genetically related (brothers) and 14 genetically unrelated donor-recipient pairs (spouses) involved in living kidney donation (LKD). RESULTS: LKDs were described by related donors and recipients as fundamentally positive and in many cases led to an intensification of the relationship between donor and recipient. LKDs between unrelated donors and recipients appeared to achieve the same

Patient values in patient-provider communication about participation in early phase clinical cancer trials: a qualitative analysis before and after implementation of an online value clarification tool intervention.

BACKGROUND: Patients with advanced cancer who no longer have standard treatment options available may decide to participate in early phase clinical trials (i.e. experimental treatments with uncertain outcomes). Shared decision-making (SDM) models help to understand considerations that influence patients' decision. Discussion of patient values is essential to SDM, but such communication is often limited in this context and may require new interventions. The OnVaCT intervention, consisting of a preparatory online value clarification tool (OnVaCT) for patients and communication training for oncol

"Free Testing and PrEP without Outing Myself to Parents:" Motivation to participate in oral and injectable PrEP clinical trials among adolescent men who have sex with men.

BACKGROUND: Adolescent men who have sex with men (AMSM) account for disproportionately high numbers of new HIV diagnoses. Non-adherence to daily use limiting the effectiveness of oral PrEP (Truvada) has led to current trials with adult MSM testing Cabotegravir, a long-term injectable medication. Once comparative studies with young adult MSM have established relative safety and efficacy of these medications, there will be a need for such comparative trials involving adolescents. Trends in state laws and IRB protocol review indicate that many of these studies will permit youth to provide indepen

Vaccination Against Seasonal or Pandemic Influenza in Emergency Medical Services.

INTRODUCTION: Influenza is a major concern for Emergency Medical Services (EMS); EMS workers' (EMS-Ws) vaccination rates remain low despite promotion. Determinants of vaccination for seasonal influenza (SI) or pandemic influenza (PI) are unknown in this setting. HYPOTHESIS: The influence of the H1N1 pandemic on EMS-W vaccination rates, differences between SI and PI vaccination rates, and the vaccination determinants were investigated. METHODS: A survey was conducted in 2011 involving 65 Swiss EMS-Ws. Socio-professional data, self-declared SI/PI vaccination status, and motives for vaccine ref

Assessment of the Information Sources and Interest in Research Collaboration Among Individuals with Vascular Ehlers-Danlos Syndrome.

BACKGROUND: Patient-centered research requires active engagement of patients. The vascular Ehlers-Danlos Syndrome (vEDS) research collaborative was established to ascertain patient-centered vEDS research priorities and to engage affected individuals as research partners. Evaluation of access to information and interest in research among individuals with vEDS was the first step undertaken as part of this work. METHODS: A 28-question survey was created to evaluate 4 domains of interest: diagnostic and clinical care history, vEDS experience, information resources, and willingness to collaborate

Altruistic and self-serving goals modulate behavioral and neural responses in deception.

People tell lies not only for their own self-interests but sometimes also to help others. Little is known about the ways in which different types of goals modulate behaviors and neural responses in deception. The present study investigated the neural processes associated with spontaneous deception that occurs with altruistic reasons (i.e. the money would be donated to charity), self-serving reasons (i.e. the participant receives all of the money) and mixed goals (i.e. the money would be equally split between the participant and the charity). Altruistic motivation for deception reduced the inte

Motivations, outcomes, and characteristics of unspecified (nondirected altruistic) kidney donors in the United Kingdom.

BACKGROUND: Unspecified (nondirected altruistic) kidney donation is becoming increasingly common in the United Kingdom. Questions regarding motivation and characteristics of these donors persist, alongside concerns about regret and long-term psychosocial outcomes. The aims of this study were to compare psychosocial and physical outcomes in unspecified kidney donors (UKDs) versus specified kidney donors (SKDs). METHODS: We performed a cross-sectional study, in which a detailed assessment of psychosocial outcomes was made using validated questionnaires. Additional questions specific to donation

Financial compensation for deceased organ donation in China.

In March 2010, China launched a pilot programme of deceased donor organ donation in 10 provinces and cities. However, the deceased donor donation rate in China remains significantly lower than in Spain and other Western countries. In order to provide incentive for deceased donor organ donation, five pilot provinces and cities have subsequently launched a financial compensation policy. Financial compensation can be considered to include two main forms, the 'thank you' form and the 'help' form. The 'thank you' form is an expression of gratitude on behalf of the Red Cross Society of China for con

Economic games quantify diminished sense of guilt in patients with damage to the prefrontal cortex.

Damage to the ventromedial prefrontal cortex (VMPFC) impairs concern for other people, as reflected in the dysfunctional real-life social behavior of patients with such damage, as well as their abnormal performances on tasks ranging from moral judgment to economic games. Despite these convergent data, we lack a formal model of how, and to what degree, VMPFC lesions affect an individual's social decision-making. Here we provide a quantification of these effects using a formal economic model of choice that incorporates terms for the disutility of unequal payoffs, with parameters that index behav

The impact of a humanitarian crisis on the magnitude of malaria in Tigray, Northern Ethiopia from 2014 to 2024.

BACKGROUND: Though Ethiopia has made a remarkable effort towards malaria prevention and control activities, malaria is occurring at epidemic levels in different regions. In Tigray, the health system including the surveillance system was damaged due to the war that erupted in November 2020, which requested to assess the burden of malaria in the region. A noticeable lack of malaria is due to the humanitarian crisis (war). Therefore, this study aimed to describe the malaria situation in the Tigray region of Ethiopia from 2014 to 2024. METHODS: A descriptive cross-sectional study design was condu

Psychological and social interventions for the promotion of mental health in people living in low- and middle-income countries affected by humanitarian crises.

BACKGROUND: Because of wars, conflicts, persecutions, human rights violations, and humanitarian crises, about 84 million people are forcibly displaced around the world; the great majority of them live in low- and middle-income countries (LMICs). People living in humanitarian settings are affected by a constellation of stressors that threaten their mental health. Psychosocial interventions for people affected by humanitarian crises may be helpful to promote positive aspects of mental health, such as mental well-being, psychosocial functioning, coping, and quality of life. Factors associated with common mental health problems of humanitarian workers in South Sudan.

BACKGROUND: The latest data on major attacks against civilian aid operations have identified South Sudan as the most dangerous country for aid workers globally. Exposure to other traumatic events and chronic stress is also common in this population. No research exists on the mental health of humanitarian workers in South Sudan. OBJECTIVES: This study examined symptom burden and predictors of posttraumatic stress disorder (PTSD), depression, anxiety, hazardous alcohol consumption, and burnout among humanitarian workers in South Sudan. METHOD: We conducted a cross-sectional online survey with

Motivations for entering and remaining in volunteer service: findings from a mixed-method survey among HIV caregivers in Zambia.

BACKGROUND: A high burden of HIV in many sub-Saharan African countries has triggered renewed interest in volunteer-based community health programmes as a way to support treatment roll-out and to deliver services to children orphaned due to HIV. This study was undertaken as an evaluation of a USAID project implemented by a consortium of 7 NGOs operating in 52 Zambian districts. We aimed to examine motivations for becoming volunteer caregivers, experiences in service and commitment to continue volunteering in the future. METHODS: A mixed-method survey approach was adopted incorporating close- a

High transplant rates of highly sensitized recipients with virtual crossmatching in kidney paired donation.

BACKGROUND: In kidney paired donation (KPD), flexibility in the allocation of incompatible pairs is required if a critical mass of pairs to efficiently find matches cannot be reached. METHODS: In the Australian KPD program, virtual crossmatch is used for the allocation of suitable donors to registered recipients. Matching is based on acceptable mismatches, and donors are excluded from matching to recipients with donor-specific antibodies (DSAs) greater than 2000 mean fluorescence intensity (MFI). Match and transplant rates in the first year of the program were reviewed with respect to recipie

Imagining being the agent of actions that carry different moral consequences: an fMRI study.

To investigate the contribution of agency to neural processing involved in aggression and morality, participants viewed in the MRI scanner a series of short visual scenarios in which an individual was either intentionally harming another person or easing the other's pain. They were required to mentally simulate being the perpetrator or the recipient of those actions. Functional connectivity analyses demonstrate that positive agency (easing the pain of another) was associated with increased activity in ventral striatum, while negative agency (harming the other) resulted in a strong signal decre

Disaster relief in post-earthquake Haiti: unintended consequences of humanitarian volunteerism.

This article provides an overview of US humanitarian relief efforts in Haiti following the earthquake on January 12, 2010. Humanitarian aid arrived rapidly from many sources and was largely provided by organized and skilled humanitarian volunteers. There are however multiple impacts on the existing health care systems, as well as the pharmaceutical and medical supply chain created by massive relief efforts involving personnel, medicines, supplies and equipment that should be considered even in the immediate post-disaster period. Additionally the consequences of short-term medical missions by s

Altruistic decisions following penetrating traumatic brain injury.

The cerebral correlates of altruistic decisions have increasingly attracted the interest of neuroscientists. To date, investigations on the neural underpinnings of altruistic decisions have primarily been conducted in healthy adults undergoing functional neuroimaging as they engaged in decisions to punish third parties. The chief purpose of the present study was to investigate altruistic decisions following focal brain damage with a novel altruistic decision task. In contrast to studies that have focused either on altruistic punishment or donation, the Altruistic Decision Task allows players t
