Noise Induced Hearing Loss and Tinnitus-New Research Developments and Remaining Gaps in Disease Assessment, Treatment, and Prevention.

Long-term noise exposure often results in noise induced hearing loss (NIHL). Tinnitus, the generation of phantom sounds, can also result from noise exposure, although understanding of its underlying mechanisms are limited. Recent studies, however, are shedding light on the neural processes involved in NIHL and tinnitus, leading to potential new and innovative treatments. This review focuses on the assessment of NIHL, available treatments, and development of new pharmacologic and non-pharmacologic treatments based on recent studies of central auditory plasticity and adaptive changes in hearing.

Directed Differentiation of Human Embryonic Stem Cells Toward Placode-Derived Spiral Ganglion-Like Sensory Neurons.

The ability to generate spiral ganglion neurons (SGNs) from stem cells is a necessary prerequisite for development of cell-replacement therapies for sensorineural hearing loss. We present a protocol that directs human embryonic stem cells (hESCs) toward a purified population of otic neuronal progenitors (ONPs) and SGN-like cells. Between 82% and 95% of these cells express SGN molecular markers, they preferentially extend neurites to the cochlear nucleus rather than nonauditory nuclei, and they generate action potentials. The protocol follows an in vitro stepwise recapitulation of developmental

[A new cerebellar neuron: the brush or monopolar cell. Characteristics and possible function].

The basic neuronal structure and circuitry of the cerebellum has been well known since Cajal's time. In recent years, however, a number of new neuronal connections and new immunohistochemically defined neuronal subtypes and functional cerebellar modules have been described. This new morphofunctional concept of the cerebellum seems to be in agreement with its newly assumed roles in learning and memory. In this new functional structure, a new cell (the brush cell, monopolar cell or monodendritic cell, Altman and Bayer, 1977; Mugnaini, 1994) specific to the cerebellar cortex and cochlear nucleus,

Response properties of inferior colliculus neurons in middle-aged C57BL/6J mice with presbycusis.

Extracellular recordings were obtained from inferior colliculus (IC) neurons in young (2-month) and middle-aged (7-month; 12- to 13-month) C57BL/6J mice in response to contralateral tone and noise stimuli. An age-related progressive loss of spiral ganglion cells, most pronounced near the cochlear base, was observed in the mice, accompanied by severe high frequency hearing loss manifested as elevation of neuronal thresholds, especially in the ventromedial half of the IC. There was a small age-related increase (2% to 11%) in 'sluggish' neurons (auditory, but poorly driven by sound); however, mos

Effectiveness of bimodal stimulation of the auditory-somatosensory system in the treatment of tonal tinnitus.

BACKGROUND AND OBJECTIVES: The dorsal cochlear nucleus (DCN) is the interaction site of auditory and somatosensory system inputs. According to the stochastic resonance theory, hearing loss increases the neural activity of the somatosensory system in the DCN and causes tinnitus. it is possible to modulate this neural hyperactivity by applying random noise through the auditory and somatosensory systems (bimodal stimulation). Therefore, this study aimed to investigate the effectiveness of the bimodal intervention based on the theory of stochastic resonance. METHODS: The study divided 34 particip

Decreases in metabolic ATP open KATP channels and reduce firing in an auditory brainstem neuron: A dynamic mechanism of firing control during intense activity.

Cartwheel (CW) neurons are glycinergic interneurons in the dorsal cochlear nucleus (DCN) that exhibit spontaneous firing, resulting in potent tonic inhibition of fusiform neurons. CW neurons expressing open ATP-sensitive potassium (KATP) channels do not fire spontaneously, and activation of KATP channels halts spontaneous firing in these neurons. However, the conditions that regulate KATP channel opening in CW neurons remain unknown. Here, we tested the hypothesis that fluctuations in metabolic ATP levels modulate KATP channels in CW neurons. Using whole-cell patch-clamp recordings in CW neuro

Medial olivocochlear efferent activity in awake guinea pigs.

Cochlear outer hair cells receive numerous connections from the medial olivocochlear efferent neurons. Medial olivocochlear efferent activity is highly dependent on the level of anesthesia. The present study was thus designed to investigate the efficiency of contralateral white noise stimulation on the distortion product otoacoustic emissions (DPOAEs) in a large number of awake guinea pigs, and to compare in the same animals the effect of urethane- and pentobarbitone-anesthesia. The monitoring of DPOAEs during contralateral white noise stimulation in awake animals requires the development of a

Clinical relevance of medial efferent auditory pathways.

Evoked otoacoustic emissions have been shown to be suppressed by sounds applied in the contralateral ear and this effect can be largely explained by the involvement of medial olivocochlear efferent fibers. Thus, EOAE recording during contralateral stimulation provides a non-invasive means of investigating auditory efferent system functioning in humans. The question remains, however, as to whether this test provides a tool, which could be useful in a variety of clinical applications. This review describes current clinical applications for this test, showing that it may prove useful for improvin

The function of the tensor tympani muscle: a comprehensive review of the literature.

The tensor tympani muscle is structurally important in the middle ear, specifically through its involvement in the impedance of sound in response to intense auditory and non-auditory stimuli. Despite numerous studies, its true function has been debated for many years; questions still remain about its role in auditory and non-auditory reflexes and in sound damping. Some studies suggest that the tensor tympani muscle contracts as a result of non-auditory stimulation such as facial or head movements; others suggest that it contracts due to input from the cochlear nucleus, therefore by way of audi

Musical pitch perception with electrical stimulation of the cochlea.

Studies were undertaken to investigate the ability of a user of the Nucleus multi-electrode cochlear implant to judge pitch in the context of musical intervals. The subject had qualified as a musical instrument tuner before he received his implant, and was able to judge the intervals between electrical sensations with neither training nor the guidance of familiar melodies. The procedures used were interval estimation, and interval production by the method of adjustment. The pitch of the electrical stimulation was controlled by varying the pulse repetition rate, the active electrode position, o

Afferent influences on brain stem auditory nuclei of the chicken: effects of conductive and sensorineural hearing loss on n. magnocellularis.

Nucleus magnocellularis is the avian homologue of the spherical cell region of the mammalian anteroventral cochlear nucleus. Its primary excitatory synaptic input is from large end bulbs of Held from the eighth nerve ganglion cells. We have examined the effects of three peripheral manipulations--middle ear ossicle (columella) removal (monaural and binaural), columella removal and oval window puncture (monaural), and monaural earplug--on cross-sectional cell area ("cell size") of second-order auditory neurons in n. magnocellularis of the chicken. Manipulations were performed between embryonic d

Development of synaptic fidelity and action potential robustness at an inhibitory sound localization circuit: effects of otoferlin-related deafness.

Sound localization involves information analysis in the lateral superior olive (LSO), a conspicuous nucleus in the mammalian auditory brainstem. LSO neurons weigh interaural level differences (ILDs) through precise integration of glutamatergic excitation from the cochlear nucleus (CN) and glycinergic inhibition from the medial nucleus of the trapezoid body (MNTB). Sound sources can be localized even during sustained perception, an accomplishment that requires robust neurotransmission. Virtually nothing is known about the sustained performance and the temporal precision of MNTB-LSO inputs after

Metabolomic and Cellular Mechanisms of Drug-Induced Ototoxicity and Nephrotoxicity: Therapeutic Implications of Uric Acid Modulation.

Certain medications, including cisplatin and neomycin, often cause both hearing loss and renal dysfunction. This study aims to uncover the common mechanisms behind drug-induced ototoxicity and nephrotoxicity to aid early diagnosis and treatment. Metabolomic analyses reveal simultaneous disruptions in endogenous metabolic networks in the kidney, inner ear, and serum after administrating cisplatin or neomycin. Notably, a marked elevation in uric acid (UA), a recognized indicator of renal tubular injury, is identified. Supplementing UA and inhibiting its renal excretion worsen hearing loss and ha

Tonic efferent-induced cochlear damping in roosting and echolocating mustached bats.

The activity of the medial olivocochlear (MOC) efferent system in mustached bats, Pteronotus p. parnellii, was studied by monitoring changes in the mechanical properties of the cochlea. The changing properties were expressed by the decay time (DT) of cochlear microphonic potentials produced by transient-induced ringing (Henson et al., 1995). Tape-recorded roost noise (biosonar and communication sounds) produced sudden, marked decreases in DT when presented to the contralateral ear of animals adapted to the quiet. When the animals were first removed from their roosts the DT was relatively short
