BACKGROUND: Injuries to the ulnar collateral ligament (UCL) are increasing among baseball players at all levels. Elbow forces during pitching routinely exceed levels that cause failure in UCL cadaver studies, highlighting the importance of identifying protective measures against UCL injury. The flexor-pronator mass has been demonstrated as a dynamic stabilizer of the elbow against valgus loads. Increasing dynamic stability of the elbow may help mitigate medial elbow gapping, a known risk factor for UCL injury. The purpose of this study was to determine the effect of an 8-week forearm-strengthe

BACKGROUND: Ulnar collateral ligament (UCL) injuries of the elbow are uncommon in the general population but prevalent in the athletic community, particularly among baseball players. Platelet-rich plasma (PRP) injection therapy has become a popular nonoperative adjuvant treatment for such injuries to help reduce recovery time and avoid surgery.
PURPOSE/HYPOTHESIS: To analyze patient outcomes by injury severity and identify injury types that responded most favorably and unfavorably to PRP treatment. It was hypothesized that PRP therapy would prove to be most beneficial in the treatment of lowe

OBJECTIVE: Nanotechnology has been increasingly incorporated into shoulder and elbow surgery, from bioinductive collagen scaffolds for rotator cuff repair to nanostructured implant surfaces for arthroplasty and nanoparticle-functionalized sutures for elbow tendon repair. This review evaluates the current evidence base for these applications and clarifies which technologies are supported by clinical data versus preclinical/mechanistic evidence alone.
METHODS: A narrative review was conducted using PubMed/MEDLINE, Embase, Cochrane Library, Web of Science, and Scopus (January 2005-September 2026

BACKGROUND: Valgus stress on the elbow during pitching can exceed the tensile strength of the ulnar collateral ligament and is the primary cause of throwing-related elbow injuries. The forearm flexor-pronator muscles act as dynamic stabilizers against elbow valgus stress. However, the temporal changes in activity during repetitive pitching and their relationship with valgus stabilization remain unclear. This study aimed to investigate changes in the activity of the forearm flexor-pronator muscles and medial elbow joint gapping during repetitive pitching, and to examine their association.

BACKGROUND: Pitch counts are tightly monitored in Major League Baseball (MLB). Hidden pitches-including warm-up pitches before and between innings and pitches before the starting/relieving pitcher's appearance in a game-are not as closely monitored.
PURPOSE/HYPOTHESIS: To report the number of hidden pitches thrown per game and over the course of a season for a single organization. We hypothesized that players who threw more hidden pitches would be at an increased risk of injury compared with those who threw fewer hidden pitches.
STUDY DESIGN: Case-control study; Level of evidence, 3.

BACKGROUND: Although the onset mechanism of an olecranon stress fracture (OSF) due to a throwing disorder is believed to involve valgus extension overload, olecranon posteromedial impingement, or triceps traction force, this issue is still debated in the literature.
PURPOSE: To establish a classification system for the different types of OSFs to improve diagnosis and clarify the onset mechanism.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: A total of 200 baseball players (198 male, 2 female; mean age, 16.1 years; age range, 13-27 years) who were diagnosed with an OSF from Janua

BACKGROUND: The use of a metal radial head prosthesis to help stabilize an elbow with traumatic instability is appealing because internal fixation of multifragment, displaced fractures of the radial head is susceptible to either early or late failure. The newer modular prostheses are easier to size and implant, but their effectiveness has not been investigated, to our knowledge.
METHODS: Twenty-seven patients in whom a radial head replacement with a modular metal spacer prosthesis had been performed to treat traumatic elbow instability were evaluated with use of the Mayo Elbow Performance Ind

BACKGROUND: Elbow injuries are common among baseball players, often involving the medial epicondyle of the humerus in childhood and the ulnar collateral ligament in adulthood. During skeletal immaturity, excessive valgus stress may lead to fragmentation or hypertrophy of the medial epicondyle. If such morphological abnormalities persist into adulthood, they may compromise elbow stability. The forearm flexor-pronator muscles contribute to dynamic elbow stability under valgus stress during throwing. However, the influence of medial epicondyle morphology on this stabilization remains unclear. Thi

BACKGROUND: Medial elbow injuries, particularly ulnar collateral ligament injuries, are a major concern in youth and adolescent pitchers. Elbow varus torque (EVT) is commonly used as an index of medial valgus load. We examined whether the Forearm Whip Angle (FWA)-defined as the sagittal-plane inclination of the throwing-forearm segment relative to the horizontal-relates independently to EVT and which phase-specific lower-extremity/trunk motions determine FWA.
METHODS: Seventeen healthy high-school pitchers were analyzed with 3-dimensional motion capture. Across the pitching cycle (0%-100%, 1%

BACKGROUND: Ulnar collateral ligament (UCL) injuries in youth pitchers continue to be concerning despite the institution of pitch count limits. Flexor-pronator mass fatigue can lead to diminished dynamic stability, resulting in greater stress on the UCL.
PURPOSE/HYPOTHESIS: To evaluate fatigue of the flexor-pronator mass by assessing changes in medial elbow laxity; noninvasively characterizing alterations in muscle glycogen; and identifying changes in subjective fatigue, strength, range of motion (ROM), pitching velocity, and accuracy with increasing pitches thrown by youth pitchers to their

BACKGROUND: Overhead athletes are particularly susceptible to elbow valgus extension overload syndrome and development of pathologic changes in the posteromedial elbow. Though arthroscopic débridement/osteophyte resection is frequently performed, few studies have analyzed the outcomes of this procedure and none have specifically addressed professional level athletes.
HYPOTHESIS: We hypothesized that following posteromedial elbow débridement, Major League Baseball (MLB) pitchers would exhibit a higher rate of ulnar collateral ligament (UCL) reconstruction than baseline incidence in the existin

BACKGROUND: Previous studies have described various techniques and confirmed the clinical utility of valgus stress radiography and stress ultrasound in overhead athletes. The addition of valgus stress and a high-resolution anatomic assessment of the elbow with magnetic resonance imaging (MRI) in the active throwing position (flexed elbow valgus external rotation [FEVER] view) can add valuable diagnostic or prognostic information in throwing athletes.
PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate findings on MRI and subsequent performance in professional throwing athletes. It w

PURPOSE OF THE STUDY: A hinged external fixator of the elbow provides stable fixation of the joint while maintaining the range of its motion. The aim of the study was to evaluate a group of patients in whom an external fixator was used to manage severe injuries to the elbow, namely, traumatic unstable dislocation, unstable fracture-dislocation, and elbow fractures not permitting management by primary osteosynthesis. This also involved assessment of early elbow mobilisation and a comparison of this group with a group of patients treated conservatively for less severe elbow injuries.
