[Primary surgery for base of tongue and tonsillar regions carcinoma: indications, surgical approaches and resections].

The oropharynx is a complex anatomical area gathering different subsites. Optimal treatment of oropharyngeal carcinoma remains still controversial. In early stages, surgery and radiation therapy are known to provide comparable oncologic results. Till recently, advanced tumors were best treated using primary radical surgery followed by postoperative radiotherapy, but new "organ sparing" approaches like concurent chemoradiotherapy seem now able to challenge primary surgery with a better functional outcome. This paper is focused on the role of primary surgery for cancer of the base of tongue and

Surgical management for snoring and sleep apnea.

Obstructive sleep apnea is a serious medical problem producing both physical and behavioral derangement. It is essential to provide a thorough workup and evaluation of all patients seeking care for snoring or OSA. Polysomnography is the standard for evaluation and assessment of the severity of OSA in every patient. The evaluation and workup for surgical intervention should include a thorough history, complete head and neck evaluation, nasopharyngeal laryngoscopy with a flexible fiberoptic endoscope, and appropriate imaging (e.g., cephalometrics). This workup allows pathologic entities of the u

Histopathological Parameters predicting Occult Nodal Metastases in Tongue Carcinoma Cases: An Indian Perspective.

BACKGROUND: It is a well-established fact that in squamous cell carcinoma cases, the presence of lymph node metastases decreased the 5-year survival rate by 50% and also caused the recurrence of the primary tumor with development of distant metastases. Till date, the predictive factors for occult cervical lymph nodes metastases in cases of tongue squamous cell carcinoma remain inconclusive. Therefore, it is imperative to identify patients who are at the greatest risk for occult cervical metastases. This study was thus performed with the aim to identify various histopathologic parameters of the

Expression of Oct3/4 and Nanog in the head and neck squamous carcinoma cells and its clinical implications for delayed neck metastasis in stage I/II oral tongue squamous cell carcinoma.

BACKGROUND: The side population (SP) of cancer cells is reportedly enriched with cancer stem cells (CSCs), however, the functional role and clinical relevance of CSC marker molecules upregulated in the SP of head and neck squamous carcinoma (HNSCC) cells are yet to be elucidated. Patients with clinical stage I/II (T1-2N0M0) tongue squamous cell carcinoma (TSCC) typically undergo partial glossectomy; however, development of delayed neck metastasis (DNM) tends to reduce their survival. In the present study, we aimed to determine the CSC markers in the SP of HNSCC cells along with their functions

Efficacy of Coblation Endoscopic Lingual Lightening in Multilevel Surgery for Obstructive Sleep Apnea.

IMPORTANCE: Multilevel obstruction is involved in the pathogenesis of obstructive sleep apnea (OSA). Coblation endoscopic lingual lightening (CELL) is a variation of glossectomy to reduce tongue collapse and can be considered in the surgical management of adults with OSA. OBJECTIVE: To evaluate the clinical and polysomnographic outcomes of tongue base reduction using CELL in multilevel surgery for OSA. DESIGN, SETTING, AND PARTICIPANTS: A retrospective case-control study was performed to review the medical records of adults with OSA (apnea-hypopnea index [AHI], >20) and Friedman stage III (t

Three-Dimensional Tongue Reconstruction Using a Trishul-Pattern Sensate Suprafascial Anterolateral Thigh Flap: Technique and Functional Outcomes.

BACKGROUND: Tongue reconstruction after oncologic glossectomy requires restoration of 3-dimensional contour, palatal contact, sensation, and floor-of-mouth support. A flat fasciocutaneous inset may close the defect but may not recreate the functional anatomy of the tongue. We describe a Trishul-pattern sensate suprafascial anterolateral thigh flap for subtotal and near-total tongue reconstruction, with selective use of a chimeric motor-innervated vastus lateralis component for floor-of-mouth reconstruction. METHODS: This descriptive case series was conducted from March 2023 to March 2025. Six

Association of Lymphovascular Space Invasion With Locoregional Failure and Survival in Patients With Node-Negative Oral Tongue Cancers.

IMPORTANCE: The indications for adjuvant therapy in resected oral tongue cancers are based on both clinical and pathological factors, with clear evidence for adjuvant radiation in patients with pathologically positive neck lymph nodes, positive margins, and extracapsular extension, but the data for patients with no nodal disease are sparse. OBJECTIVE: To investigate determinants of failure and survival in patients with node-negative oral tongue cancer. DESIGN, SETTING, AND PARTICIPANTS: Medical records for patients with oral tongue cancer treated with definitive surgery from 2003 to 2013 wer

Functional results of microvascular reconstruction after hemiglossectomy: free anterolateral thigh flap versus free forearm flap.

The aim of the present study is to assess functional outcomes after hemiglossectomy and microvascular reconstruction. Twenty-six patients underwent primary tongue microvascular reconstruction after hemiglossectomy. Twelve patients were reconstructed using a free radial forearm flap and 14 with an anterolateral thigh flap. Speech intelligibility, swallowing capacity and quality of life scores were assessed. Factors such as tumour extension, surgical resection and adjuvant radiotherapy appeared to be fundamental to predict post-treatment functional outcomes. The data obtained in the present stud

Translocation (12;14) and other chromosome abnormalities in squamous cell carcinoma of the tongue.

Tongue squamous cell carcinoma (SCC) has an increasing incidence, a high morbidity rate, and a 50% 5-year survival rate. The prognosis of tongue SCC is poor compared to SCC originating at other sites in the oral cavity, because they represent different biological subentities. Cytogenetic studies of head and neck SCC showed more losses than gains of various chromosomes or chromosomal segments. Among the frequent alterations there are losses of -4, -10, -13, -14, -18, -19, -21, -22, gains of +7, +8, +9, +16, +18, +20, and isochromosomes i(1q), i(3q), i(5p), i(8q). We are unaware of cytogenetic r

Squamous cell carcinoma of the anterior tongue: is tumour thickness an indicator for cervical metastasis?

The incidence of squamous cell carcinoma (SCC) of the tongue accounts for 90% of all malignancies affecting the oral cavity and oropharynx. The distribution between the anterior and posterior tongue is equal. Nodal metastasis is dependent on various factors including tumour thickness, site, size, differentiation, and perineural and perivascular invasion. There is increasing evidence of a close correlation between tumour thickness and metastasis. A retrospective study covering the 16-year period from 2000 to 2016 was performed. Eighty-one patients with anterior tongue SCC were included. The onl

Superficial ulnar artery perforator flap.

Superficial ulnar artery is a rare finding but shows significant surgical implications. Its thinness and pliability make this flap an excellent solution for soft tissue reconstruction, especially in the head and neck region. We hereby report a successful free superficial ulnar artery perforator forearm flap transfer for tongue reconstruction. A 64-year-old man presenting with a squamous cell carcinoma of the left tongue underwent a wide resection of the tumor, left radical neck dissection, and reconstruction of the tongue and the left tonsillar pillar with the mentioned flap. No complications

Does swallowing function recover in the long term in patients with surgically treated tongue carcinomas?

PURPOSE: The present study aimed to measure postsurgical swallowing function in patients 5 years after the surgical treatment of tongue carcinoma. PATIENTS AND METHODS: Using a retrospective cohort study design, the investigators enrolled postsurgical patients treated for tongue carcinomas in Hokkaido University Hospital. The primary outcome variable was oropharyngeal swallow efficiency (OPSE) determined by videofluoroscopic evaluation, and OPSE at follow-up was compared with that at discharge. Other variables included current nutritional status (body mass index, serum albumin), dietary intak

Early tongue carcinoma: analysis of failure.

BACKGROUND: Failure rate of surgery for early tongue carcinoma remains high. We sought to identify patterns of failure and recurrence risk factors. METHODS: Data review was carried out on 50 patients treated for early tongue carcinoma (T1/2N0M0); surgery was unsuccessful in 11 of these patients. All patients underwent transoral resection of the tongue tumor and prophylactic neck dissection (supraomohyoid). RESULTS: Tumor recurred within 3 to 18 months. Nine died of disease. Four had failure in neck level 4, 6 in level 1, and 1 simultaneously in level 1 and locally. Most tumors were moderatel

Microvascular reconstruction of the tongue using a free anterolateral thigh flap: Three-dimensional evaluation of volume loss after radiotherapy.

The goal of tongue microvascular reconstruction is to maximise oral function with less morbidity while preserving speech and swallowing. This kind of reconstruction often requires a bigger flap volume than is actually needed to repair the defect. This is because every reconstructive flap is subject to a shrinking process due to oedema reduction and differences among individual tissue healing processes. Moreover, patients with advanced cancers often need adjuvant radiation therapy, which can result in further flap volume loss. For these reasons, we designed this study to assess the three-dimens
