An inventory of mandibular fractures associated with implants in atrophic edentulous mandibles: a survey of Dutch oral and maxillofacial surgeons.

PURPOSE: The present study provides an inventory of the number of fractures that occurred in conjunction with implant placement in edentulous patients in the Dutch population from 1980 to 2007 and estimates the incidence with which this might occur. The study also sought to define the factors that increase the risk of fracture. MATERIALS AND METHODS: Questionnaires were sent to all 198 oral and maxillofacial surgeons working in 56 hospitals in the Netherlands. Questions were asked regarding the causes of fractures, the height of the edentulous mandible, and the methods of fracture treatment.

The use of a coded healing abutment as an impression coping to design and mill an individualized anatomic abutment: a clinical report.

A coded implant healing abutment makes an impression at the implant level no longer necessary. An impression is made of the healing abutment, which is placed onto the implant directly after implant placement. The codes embedded in the occlusal surface of the healing abutment provide essential information for the computer software to place the implant analog in the definitive cast and to design and mill the definitive abutment.

Feasibility of digitizing multiple tissue-level implants without scan bodies for fixed dental prostheses.

OBJECTIVE: This in vitro study evaluated the feasibility of intraoral scanning of two tissue-level implants for a three-unit fixed dental prosthesis (FDP) without using scan bodies, with the aim of potentially improving clinical cost efficiency. METHODS: A mandibular typodont model with two tissue-level implants - a regular-platform (RP) implant in the premolar region and a wide-platform (WP) implant in the molar region - was digitized. Three intraoral scanners (TRIOS 5, Primescan, DCS Scan) and one laboratory scanner (E4; reference scanner) were used to acquire 16 scans each, with and withou

Type II autosomal dominant osteopetrosis (Albers-Schönberg disease): clinical and radiological manifestations in 42 patients.

Type II autosomal dominant osteopetrosis (ADO II, Albers-Schonberg disease) is a genetic condition characterized by generalized osteosclerosis predominating in some skeletal sites such as the spine and pelvis. ADO II is rare, and most available clinical descriptions are based on small numbers of patients. We report the clinical and radiological manifestations in 42 ADO II patients. To our knowledge, this is the largest series reported so far. Our inclusion criterion was presence on radiographs of the spine of vertebral endplate thickening, producing the classic sandwich vertebra appearance. We

A scanning electron microscope characterisation of biofilm on failed craniofacial osteosynthesis miniplates.

INTRODUCTION: Between 3 and 18% of craniofacial osteosynthesis plates are removed due to chronic infection. Removal of the plate is necessary to manage the chronic infective state i.e. miniplate removal results in resolution of the infection. These observations are suggestive of a biofilm-related infection. The aim of this retrospective study was to characterise the presence of biofilm on the removed miniplates from oral and maxillofacial surgery. MATERIALS AND METHODS: A total of 12 plates and associated screws were recovered from eleven patients suffering from persistent, trauma site infect

[Surgical management of extracranial internal carotid artery aneurysms].

Aneurysms of the extracranial internal carotid artery are rare but may present as a mass, with ischemic symptoms, or with fatal hemorrhage. We operated on aneurysms in four patients, two males and two females, whose ages ranged from 47 to 57 years. While a lot of etiological factors for the aneurysms have been known to include trauma, vascular dysplasia, infection or surgery using patch graft for carotid endarterectomy, three aneurysms in our series were atherosclerotic and one was spontaneously dissecting. One patient had focal neurological deficit due to embolism, two presented with a growin

Problems associated with the atrophic mandible.

A range of treatment options has been presented regarding the use of dental implants to rehabilitate patients who otherwise have compromised function using conventional tissue-borne prostheses. This patient population offers significant challenges to both the surgeon and the prosthodontist. Generally speaking, IODs retained on two or more endosteal osseointegrated implants; transosteal, subperiosteal, ramus frame implant-supported and implant-retained prostheses, and a totally implant-supported design offer options to treat the severely atrophic mandible. When bone support or volume is so lack

Analysis of complications after reconstruction of bone defects involving complete mandibular resection using finite element modelling.

BACKGROUND: In a retrospective study, risk factors for complications after the bridging of mandibular defects using reconstruction plates were reviewed. Especially the loosening of the plate-screw-mandible complex should be analyzed with a finite element model in order to reduce plate complications in future. PATIENTS AND METHODS: We examined 60 patients who underwent a treatment with reconstruction plates after tumor resection during a period of 10 years. The problem of screw loosening was additionally reviewed by means of a finite element study, and a model for the loosening process was dev

Complete oral rehabilitation of a postresection ameloblastoma patient: a clinical case report.

Conventional ameloblastomas are rare, benign, epithelial odontogenic tumors that tend to grow slowly in the mandible or maxilla, but are locally invasive and can be highly destructive of the surrounding dental anatomy. Aggressive resection is the most effective method of eliminating the tumors, but treatment can further contribute to patient deformity and malfunction. Ameloblastomas also have a high rate of recurrence, which significantly diminishes 8 years after resection, but still requires vigilant clinical monitoring. This case report describes the complete oral rehabilitation of a postres

Influence of the implant cervical topography on the crestal bone resorption and immediate implant survival.

The aim of the study was the investigation of the survival rate of immediate implants in addition to the evaluation of the level of the alveolar bone around the neck region of immediately placed implants of different macrodesigns. To address the question whether the "biological", highly polished area of the implant neck is more associated with crestal resorpiton than the rough neck region, the influence of the surface characters of the implant cervical region was studied. The survival rate of 129 implants from 52 patients was evaluated on different time points after immediate implantation. The

Osseointegration dynamics of mandibular patient-specific implants: A mechanoregulation-based finite element study.

STATEMENT OF PROBLEM: Achieving strong and durable fixation remains a major challenge in mandibular reconstruction with patient-specific implants (PSIs). Most prior studies have focused on initial mechanical stability, with limited attention to the time-dependent progression of biological fixation at the bone-implant interface. PURPOSE: The objective of this single-patient study was to investigate the osseointegration dynamics of a mandibular PSI using a mechanoregulation-based finite element model (FEM) to explore how the predicted biological fixation behavior relates to the specific design

Undesirable course of an oral implant rehabilitation in a patient with a long history of bulimia nervosa: case report and review of the literature.

BACKGROUND: Bulimia nervosa is an eating disorder resulting in an intended weight loss due to decreased food intake, induced vomiting, or hyperactivity, and is observed frequently between 12 and 25 years of age. One of the complications is early tooth loss. Moreover, since bulimia nervosa patients suffer from increased atrophy of the alveolar processes, oral rehabilitation even with short dental implants may be impossible. In these cases, lateralization or transposition of the inferior alveolar nerve (IAN) followed by implant placement can be useful. CASE PRESENTATION: A 40-year-old woman wit

Correction of hemifacial microsomia with the help of mirror imaging and a rapid prototyping technique: case report.

A 23-year-old man presented with an 8-year history of unilateral hemifacial microsomia. A three-dimensional model of the maxillofacial bones was generated after acquisition of helical computed tomographic data. A customised implant model was designed by projecting a mirror image of the healthy mandible on to the three-dimensional model. A resin model of the implant was then made using a rapid prototyping machine. A polymeric biomaterial was sculpted according to the model and implanted into the affected side of the mandible to restore his facial symmetry. The hemifacial microsomia was correcte
