Nerve-guided laparoscopic total mesorectal excision for distal rectal cancer.

BACKGROUND: Urogenital dysfunctions are well-recognized problems after rectal cancer surgery and are often due to autonomic nerve damage. Although following holy planes during total mesorectal excision (TME) reduces the possibility of damage to the autonomic nerve fibers, these could still be affected in some critical areas.1 (,) 2 To improve the quality of surgery and prevent nerve damage, accurate intraoperative anatomical orientation of autonomic nerve is essential.3 Thanks to advancement of the high-definition laparoscopic technology, even the finest nerve fibers deep in the pelvic cavity

Mastering Robotic SMA Dissection: How the Correct Anatomical Concept Refines Technical Steps in the Left Posterior Approach.

BACKGROUND: The left posterior approach to the superior mesenteric artery (SMA) facilitates central vascular ligation and R0 resection during robotic pancreaticoduodenectomy (PD).1-4 Despite its advantages, embryological intestinal rotation creates a complex anatomy around the SMA.5,6 This study clarifies the three-dimensional (3D) anatomical considerations and surgical techniques essential for this approach. PATIENTS AND METHODS: The technique begins with a left-sided scope orientation. The transverse mesocolon was cranially retracted using a Nathanson retractor to create a stable, wide oper

Laparoscopic resection for a mesenteric lipoma of the ascending colon: a case report.

INTRODUCTION: Lipomas are benign tumors that commonly occur in the arms, legs, and trunk of adults. Mesenteric lipomas are rare, particularly in the mesocolon. CASE PRESENTATION: A 32-year-old puerperal woman developed abdominal pain 1 day after a Cesarean section. Computed tomography revealed a 10-cm, poorly enhanced, smoothly contoured mass dorsal to the ascending colon and ventral to the right kidney. Magnetic resonance imaging showed a homogeneous mass with clear borders and a thin peripheral capsule. A retroperitoneal sarcoma and well-differentiated liposarcoma were suspected. The mass w

Association between Timed Up and Go Test and Subsequent Functional Dependency.

OBJECTIVE: Metastases in the supragastric lesser sac (SGLS) are not only occult but are also barriers to complete resection of ovarian cancer. We describe a cohort of patients with SGLS disease undergoing debulking surgery. METHODS: We identified all patients who underwent evaluation and eventual resection of SGLS disease as part of cytoreductive surgery for stage IIIC-IVB high-grade epithelial ovarian cancer at our institution from January 2018 to August 2022. RESULTS: Thirty-three of 286 patients (11.5%) underwent resection of SGLS disease. Metastases in the SGLS were identified by preoper

Internal Hernia Following Robotic Assisted Pancreaticoduodenectomy.

BACKGROUND Robotic assisted pancreaticoduodenectomy (RPD) is reported to be safe and feasible. Internal hernia (IH) after RPD is a serious but rarely reported complication. MATERIAL AND METHODS We retrospectively reviewed data of 231 patients who underwent RPD from October 2010 to December 2016. The incidence, symptoms, time of presentation, and outcome were investigated. RESULTS Five patients (2.6%) were diagnosed with IH. Significant correlation (P<0.001) between IH and transverse mesocolon defect was confirmed. In patients without defect closure, the incidence of IH was 62.5%, while patient

Gallbladder carcinoma - a rare cause of pyloric-duodenal stenosis.

Pyloric duodenal stenosis is usually caused by pyloric, juxtapyloric or duodenal ulcer, or by postbulbar ulcer. Gallbladder cancer (GBC), duodenal diverticula, annular pancreas and superior mesenteric artery syndrome (Wilkie's syndrome) are rare causes of pyloric duodenal stenosis. The case of a 66-year-old female patient is presented. The patient was admitted to hospital presenting anorexia, repeated alimentary vomiting, epigastric pain, and weight loss. Objective clinical examination upon admission: clapotage à jeun is present, triggered by tapping the epigastric region. Laboratory tests rev

Intestinal perforation secondary to intestinal Burkitt lymphoma.

INTRODUCTION AND IMPORTANCE: Small intestinal perforation in patients with Burkitt lymphoma is extremely rare. We present the first report of such a case. CASE PRESENTATION: A 53-year-old woman was admitted with abdominal pain and vomiting. Abdominal examination revealed rigidity and tenderness in the upper abdomen. Computed tomography scan showed thickening of the wall of the jejunum, intra-abdominal free gas, and ascites; the patient was diagnosed with small intestinal perforation, and underwent emergency surgery on the same day. Laparoscopic findings were a 50 mm jejunal perforation and pe

Chylous content might determine the optimal surgical approach for mesenteric lymphatic malformations in childhood.

BACKGROUND: Mesenteric lymphatic malformations (LMs) represent rare congenital anomalies that can include chylous or nonchylous content. The pathologic mechanisms explaining this phenomenon are poorly understood and not yet described. Furthermore, the current management approach does not consider the contents of the mesenteric LMs. In the present study, we have defined the relationship between the lymphatic mesenteric cyst content and the histologic evidence of LMs within the bowel wall. METHODS: We retrospectively investigated all patients with mesenteric LMs treated surgically at our depart

Outermost layer-oriented medial approach for infrapyloric nodal dissection in laparoscopic distal gastrectomy.

BACKGROUND: Based on our experience of suprapancreatic nodal dissection in laparoscopic gastrectomy, we developed an outermost layer-oriented medial approach for infrapyloric nodal dissection. The objective of this single-institution retrospective study was to determine the feasibility, safety, and reproducibility of this novel and unique dissection procedure. METHODS: This approach can be performed in the same manner as suprapancreatic nodal dissection but by replacing the left gastric artery with the right gastroepiploic artery (RGEA), the common hepatic artery with the anterior superior pa

Internal hernias after laparoscopic Roux-en-Y gastric bypass: incidence, treatment and prevention.

BACKGROUND: Laparoscopic Roux-en-Y gastric bypass (RYGBP) has been shown to be a safe and effective alternative to traditional "open" RYGBP. Although lack of postoperative adhesions is one advantage of minimally invasive surgery, this is also responsible for a higher incidence of internal hernias. These patients often present with intermittent abdominal pain or small bowel obstruction with completely normal contrast radiographs. METHODS: Data was obtained concurrently on 2,000 consecutive patients from February 1998 to October 2001 and analyzed retrospectively. Radiographs, when available, we

[Stomal stenosis following gastrectomy in the elderly].

Seven hundred and thirty seven patients over 65 years of age (mean 76 years) undergoing gastrectomies from 1979 to 1991 were reviewed to evaluate the cause of stomal stenosis in the early postoperative period. Fifty seven (7.7%) patients, 24 males (5.6%) and 33 females (10.6%), had overt stomal stenosis or obstruction documented by radiological and endoscopic findings. The incidence of stenosis in females was significantly higher than in males (p < 0.05). Complications developed in 19 (20.0%) of 95 patients after gastroduodenostomy (Billroth-I), 29 (6.2%) of 465 after gastrojejunostomy (Billro

[Duplication of the alimentary tract: report of three cases].

Three cases of duplications of the alimentary tract are presented. Case 1 was a 5-month-old male baby. He was admitted due to copiously painless rectal bleeding. Tc-99m pertechnetate scanning revealed unusual tubular and spherical lesions. Laparotomy findings were that a tubular ileal duplication m assured 100cm or so in length and communicated with ileal lumen by its distal orifice. Moreover, there were 2 spherical duplications like ping-pong ball in morphology located in neighboring mesentery. Case 2 was a 2-day-old male newborn. He was admitted because of persistent bilious vomiting on his

Influence of Anatomical Variations due to Horseshoe Kidney on Colorectal Cancer Surgery.

BackgroundHorseshoe kidney is a rare renal fusion anomaly often associated with vascular and ureteral variations. Previous case reports have described technical considerations during colectomy in patients with horseshoe kidney. However, studies systematically evaluating the impact of anatomical anomalies associated with horseshoe kidney on surgical procedures remain limited.MethodsWe retrospectively analyzed 13 patients with horseshoe kidney who underwent colectomy for primary colorectal adenocarcinoma at a single center between April 2007 and December 2024. Venous anomalies were primarily eva

Role of Mesenteric Defect Closure in Preventing Internal Hernias After Ileocecectomy With Kono-S Anastomosis.

Crohn's disease is a chronic inflammatory bowel condition that frequently leads to complications such as strictures and bowel obstruction, often necessitating surgical intervention. Surgical approaches like ileocecectomy and right colectomy are commonly performed, with an ongoing debate about whether to close the mesenteric defect during these procedures to prevent internal hernias and small bowel obstruction. In this case, a 39-year-old male individual with longstanding Crohn's disease underwent robotic-assisted colon mobilization, ileocecectomy with Kono-S anastomosis, and gastrojejunostomy

Primary Pancreatic GIST - A-Single Centre Case Series and Systematic Review of Literature.

PURPOSE: GISTs arising from organs outside GI tract are defined as extragastrointestinal GISTs (EGIST). The majority of EGISTs arise from small intestinal mesentry, mesocolon, omentum, retroperitoneum, abdominal wall, liver and pancreas with pancreas comprising less than 5% of it. Due to limited data, it is unknown if the results of GIST can be generalised for EGISTs. We thereby present the largest single-centre case series of primary pancreatic GIST so far with review of existing literature. METHODS: A total of 9 patients of primary pancreatic GIST were treated at our institute from Septembe

Delayed development of inferior mesenteric vein in human foetuses.

BACKGROUND: The superior mesenteric vein appears as a fusion between irregularly-shaped slits of the midgut mesentery tissue at 5-6 weeks. In contrast, there has been no report identifying when and how the inferior mesenteric vein (IMV) develops. So we aimed to find the human initial IMV. MATERIALS AND METHODS: We examined 1) sagittal histological sections of seven human foetuses with 45-75 mm crown rump length or CRL (10-12 weeks); 2) horizontal sections of 15 foetuses with 70-155 mm CRL (12-18 weeks); and 3) horizontal sections of 12 late-term foetuses with 225-328 mm CRL (28-41 weeks). Two case of bilateral approach in laparoscopic pancreas-sparing distal duodenectomy for duodenal neoplasms arising from the distal duodenum.

INTRODUCTION: Laparoscopic pancreas-sparing distal duodenectomy is a less invasive surgical therapy; however, the anatomical complexity of the duodenum increases the difficulty of laparoscopic procedures. We introduce our technique for laparoscopic pancreas-sparing distal duodenectomy for distal duodenal tumors. PRESENTATION OF CASES: A first patient was 47-year-old woman who had 30 mm of duodenal tumor which located in third portion of duodenum. A second patient was 66-year-old man who had 35 mm of submucosal tumor which located in the third portion of duodenum. Laparoscopic pancreas-sparing
