Bi-maxillary protrusion in adolescent patients has traditionally been treated by extraction of the four first premolars and retraction ofthe anterior teeth. The ectopic eruption of the maxiIlary permanent canines is a frequently encountered clinical problem. Orthodontic treatment of the impacted teeth remains a challenge for clinicians. If it is associated with other dental and skeletalproblems, there will be further complications to the treatment plan. In such cases, if we extract canines, then problems with this approach are restricted to anatomical and functional limitations ofpremolars sub

OBJECTIVE: To evaluate and compare skeletal effects and the amount of molar distalization in maxilla using modified palatal anchorage plate (MPAP) vs headgear appliances in adolescent patients.
MATERIALS AND METHODS: Pre- and posttreatment lateral cephalograms of 45 Class II malocclusion patients were analyzed; 24 were treated with MPAP appliances (age, 12.4 years) and 21 with headgear (age, 12.1 years). Fixed orthodontic treatment started with the distalization process in both groups. Thirty-two variables were measured and compared between both groups using multivariate analysis of covariate

INTRODUCTION: When mini-implants fail during orthodontic treatment, there is a need to have a backup plan to either replace the failed implant in the adjacent interradicular area or wait for the bone to heal before replacing the mini-implant. We propose a novel way to overcome this problem by replacement with a miniplate so as not to interrupt treatment or prolong treatment time.
METHODS: The indications, advantages, efficacy, and procedures for switching from a mini-implant to a miniplate are discussed. Two patients who required replacement of failed mini-implants are presented. In the first

PURPOSE: To examine the relationship between cortical bone thickness, inter-root distance (horizontal space), distance from alveolar crest to the bottom of maxillary sinus (vertical space) at the prepared site, and implant placement torque and the success rate of mini-implants placed for orthodontic anchorage.
MATERIALS AND METHODS: After computerized tomography examination, mini-implants 1.6 mm wide and 8 mm long were placed in the posterior alveolar bone. The mini-implant was judged a success when orthodontic force could be applied for at least 6 months without pain or clinically detectable

INTRODUCTION: Deepbites can be corrected by intrusion of mandibular anterior teeth. Direct anchorage with miniscrews simplifies complex tooth movements; however, few studies have reported their use for mandibular anterior intrusion. The purpose of this study was to evaluate, by means of the finite element method, initial tooth displacement and periodontal stress distribution using various mandibular anterior intrusion mechanics. Miniscrews were used as skeletal anchorage devices.
METHODS: Cone-beam computed tomography scans were used for 3-dimensional reconstruction of the mandible and the ma

INTRODUCTION: Placing mini-screws between teeth can risk contacting or damaging dental roots and injuring adjacent nerves, blood vessels or maxillary sinus when local anatomy is not visualised precisely. The latest digital advances in orthodontics have enabled three-dimensional (3D) virtual protocol designing, allowing orthodontists to simulate the orthodontic treatments and collaboratively map out the surgical intervention before procedures. Computed tomography (CT)-based surgical guides are strongly recommended for patients with delicate and complex anatomical conditions.

BACKGROUND: This study aimed to evaluate the relationship between maxillary sinus volume (MSV) and alveolar bone trabeculation using fractal dimension (FD) analysis across different vertical skeletal patterns, and to assess its potential implications for orthodontic mini-implant (OMI) stability.
METHODS: CBCT images of 84 skeletal Class I individuals (47 females, 37 males; mean age 20.08 ± 2.25 years) were analyzed and categorized into three vertical skeletal groups (28 normodivergent, 28 hyperdivergent, 28 hypodivergent). MSVs were measured bilaterally using 3D Slicer, and FD analyses were c

Orthodontic treatment in patients with periodontally compromised dentition often presents challenges, necessitating special considerations. This case report describes treatment of a 52-year-old female patient with advanced chronic periodontitis and pathologic tooth migration through an interdisciplinary orthodontic-periodontal approach. By integrating comprehensive periodontal treatment with strategic use of miniscrew-assisted rapid palatal expansion (MARPE) and careful consideration of the applied force systems, both functional and esthetic concerns were addressed without causing adverse peri

PURPOSE: Fracture of orthodontic mini-implants during insertion is a limiting factor for their clinical success. The purpose of this study was to determine the fracture potential of commonly used self-drilling orthodontic mini-implants when placed into simulated thick, dense mandibular bone.
MATERIALS AND METHODS: Six mini-implant systems were assessed for the potential for fracture (Aarhus, Medicon; Dual-Top, Jeil Medical; OrthoEasy, Forestadent; tomas-pin, Dentaurum; Unitek, 3M; and VectorTAS, Ormco). First, mini-implants were inserted manually, without predrilling, into bone substitutes (S

PURPOSE: There is a controversial body of evidence regarding optimal insertion angulation of an orthodontic miniscrew as a mean of skeletal anchorage.
MATERIALS AND METHODS: A bracket head-type 8-mm-long miniscrew (60-degree symmetrical trapezoid thread, 0.1-mm thread fillet, 0.2-mm thread height, with 0.5-mm thread pitch) was designed and monocortically inserted at 30, 45, 55, 70, 90, 110, 125, 135, and 150 degrees of inclination to the surface of bone. A bilayer cortical (1.6 mm) and cancellous (8.5 mm) bone model was constructed, adopted from a 22-year-old male patient's cone beam computed

Cone-beam computed tomography (CBCT) has become popular, and its many inherent advantages are indisputable. Nevertheless, CBCT is prescribed cautiously because the radiation dosage is higher than that of conventional radiography. When and to what extent should CBCT be prescribed for orthodontic patients? The purpose of this article is to present 4 curious cases in which a considerable discrepancy was found between the conventional panoramic radiograph and the CBCT view. Is it time to spare patients an unnecessary conventional panoramic radiograph and shift to CBCT for all patients?

Adequate intrusion and torque control of the retroclined maxillary incisors are critical for the treatment of Class II division 2 (div2) malocclusion. In addition, anterior retraction via lingual root movement can be challenging. This case report demonstrates a combined use of miniscrews and continuous arch with additional torque for intrusion, retraction, and torque control of maxillary incisors in the Class II div2 with gummy smile. A 20-year-old woman presented with multiple issues, including impacted canine, lip protrusion, prolonged retained mandibular primary molar, and two missing maxil

In this article, we report the successful use of miniscrews in a patient with an Angle Class III malocclusion, lateral open bite, midline deviation, and severe crowding. Simultaneously resolving such problems with conventional Class III treatment is difficult. In this case, the treatment procedure was even more challenging because the patient preferred to have lingual brackets on the maxillary teeth. As a result, miniscrews were used to facilitate significant asymmetric tooth movement in the posterior and downward directions; this contributed to the camouflage of the skeletal mandibular protru

An open bite is commonly one of the main symptoms of an overall dentofacial deformity. Most anterior open-bite cases are characterized by excessive vertical development of the posterior maxilla. Intrusion of the overerupted molar teeth by traditional orthodontic methods is hardly possible; there is therefore no real alternative to a combined orthodontic and surgical approach. Skeletal anchorage has recently been proposed for the orthodontic movement of teeth. Titanium miniplates implanted in the zygomatic buttress area can serve as the absolute anchorage for maxillary molar intrusion. The aim
