Transection of Nasolacrimal Duct in Endoscopic Medial Maxillectomy: Implication on Epiphora.

OBJECTIVE: Management of the nasolacrimal system is usually recommended during medial maxillectomy via external approach because of reported higher rates of postoperative epiphora. Association of the endoscopic medial maxillectomy (EMM) with epiphora, however, is not clearly stated. In this study, we attempted to evaluate whether patients develop epiphora after simple transection of the nasolacrimal duct during EMM. PATIENTS AND METHODS: Medical records of 26 patients who underwent endoscopic tumor resection for inverted papilloma (IP) were retrospectively reviewed. Patients who underwent EMM

Isolated sphenoid sinus disease: an analysis of 122 cases.

Isolated sphenoid sinus disease (ISSD) is a relatively uncommon disease. The present study is a retrospective review of 122 patients with ISSD who were treated at the Department of Otolaryngology, Eye, Ear, Nose and Throat Hospital at Shanghai Medical University over a 25-year period. The diagnosis of ISSD was made on the basis of history and physical examination, signs and symptoms, nasal endoscopy, and computed tomography (CT) and magnetic resonance imaging (MRI). The final diagnosis of ISSD was confirmed by histopathologic and microbiological examinations of the surgical specimens. The path

A case report of epithelial adenomatoid hamartoma misdiagnosed as a nasal poly.

RATIONALE: Respiratory epithelial adenomatoid hamartomas (REAH) are benign tumors. The clinical presentation of REAH often resembles that of nasal polyps or inverted papillomas. Owing to the lack of specific clinical symptoms or imaging findings, the diagnosis is usually confirmed only by pathological examination after surgery and misdiagnosis is common. Therefore, we report this case of nasal REAH that was misdiagnosed as a nasal polyp. Clinical cases of nasal polyps with hamartoma are very rare. Pathological biopsy, complete removal of the lesion by surgery and postoperative follow-up are es

Nose and paranasal sinus tumours: United Kingdom National Multidisciplinary Guidelines.

This is the official guideline endorsed by the specialty associations involved in the care of head and neck cancer patients in the UK. With only limited high-level evidence for management of nasal and paranasal sinus cancers owing to low incidence and diverse histology, this paper provides recommendations on the work up and management based on the existing evidence base. Recommendations • Sinonasal tumours are best treated de novo and unusual polyps should be imaged and biopsied prior to definitive surgery. (G) • Treatment of sinonasal malignancy should be carefully planned and discussed at a

Is routine histopathological analysis of nasal polyposis specimens necessary?

Our objective is to evaluate the incidence of unexpected pathologies in routine nasal polyposis specimens and necessity for histopathological evaluation of nasal polyps. A retrospective chart review of all patients who underwent nasal polyposis surgery between January 2004 and June 2010 were reviewed. The patients were divided into two groups. Group 1 consisted of patients with bilateral nasal polyposis. Group 2 consisted of patients with unilateral nasal polyposis. One hundred and seventeen patients (81 male, 36 female) were involved in this study. The mean age was 44.9 ± 17.7 years, ranging

[Midfacial degloving: an alternative approach to the frontobasal area, the nasal cavity and the paranasal sinuses].

Midfacial degloving can be characterized as an alternative surgical approach for exposing the bony structures of the midface. In combination with transient partial osteotomies the nasal cavities, the paranasal sinuses, the pterygopalatine fossa and the posterior parts of the anterior skull base are easily accessible. Using an intercartilaginous, a transseptal and a circumvestibular incision in the nose and a vestibular incision in the oral cavity the soft tissues of the upper face are mobilized and transposed cranially up to the infraorbital rim, the nasion and the lacrimal sac. Thus one can a

Role of SCCA and CYFRA 21-1 in the differential diagnosis of sinonasal benign and malignant diseases.

OBJECTIVES: In previous study, we have found that preoperative serum squamous cell carcinoma antigen (SCCA) and cytokeratin 19 fragment antigen 21-1(CYFRA 21-1) could be used as serum tumor markers for the diagnosis of sinonasal inverted papilloma (SNIP). Thus, we detected the expression of SCCA and CYFRA 21-1 in tumor tissue and serum to further demonstrate the role of SCCA and CYFRA21-1 in SNIP and other differential diseases. METHODS: Clinical data including gender, age, and preoperative serum SCCA and CYFRA 21-1 levels were obtained respectively from 34,91,59 patients with chronic rhinosi

Endoscopic endonasal skull base surgery: past, present and future.

Endoscopic techniques have undergone tremendous advancement in the past years. From the management of phlogistic pathologies, we have learned to manage skull base lesions and even selected intracranial diseases. Current anatomical knowledge plus computer-aided surgery has enabled surgeons to remove large lesions in the paranasal sinuses extending beyond the boundaries of the sinuses themselves. In this sense, management of benign diseases via endoscopic routes is nowadays well accepted whilst the role of endoscopic techniques in sinonasal malignancies is still under investigation. Nowadays, it

Vascularized anterolateral thigh free flap for salvage reconstruction of complex anterior skull base and nasion defects after failed conventional reconstruction- how I do it.

BACKGROUND: Sinonasal inverted papilloma (SNIP) is histologically benign but locally aggressive, with high recurrence rates and a relevant risk of malignant transformation. Tumor involvement of the nasal root and overlying skin may result in a full-thickness craniofacial defect with direct intracranial-external communication. Reconstruction is particularly challenging after failure of conventional skull base techniques (e.g., titanium mesh and pericranial flaps), which may lead to pneumocephalus, cerebrospinal fluid (CSF) leakage, and infection. OBJECTIVE: To describe a multidisciplinary stag

Immunostaining for polycomb group protein EZH2 as a diagnostic tool to differentiate urothelial carcinoma in situ from benign lesions.

Urothelial carcinoma in situ (CIS) is a flat-type noninvasive urothelial carcinoma. Appropriate diagnosis of CIS is important because treatment options depend on the diagnosis. However, it is often difficult to differentiate CIS from benign lesions, especially reactive atypia. Enhancer of zeste homolog 2 (EZH2) is a component of the polycomb repressor complex 2 that is involved in carcinogenesis by epigenetically regulating gene expression levels. The protein is highly expressed in various malignancies, including urothelial carcinoma. We hypothesized that immunostaining for EZH2 is useful to d

Immunohistochemical p16 expression in the prognosis of patients with sinonasal squamous cell carcinoma.

In sinonasal squamous cell carcinoma (SNSCC), the prognostic relevance of p16INK4a (p16) expression has been reported rarely. This study aims to examine the immunohistochemical expression of p16 and investigate the possibility of p16 as a prognostic factor for SNSCC. The medical records of 173 individuals with SNSCC between 2010 and 2022 were retrospectively reviewed. The researchers examined patients' demographics, p16 status, staging, tumor histological subtypes, treatment details, recurrence, metastasis, and survival outcomes. p16 was found in 22.0% (38/173) of SNSCC patients, and there was

A 10-Year Retrospective Review of the Histology of Symptomatic and Asymptomatic Unilateral Nasal Polyps.

Background The aim of this study was to determine the histological diagnosis of unilateral nasal polyps and to determine the prevalence of neoplastic pathologies. This study also assessed difference in pathologies whether patients presented symptomatically or were asymptomatic (if they had a mass found incidentally for unrelated throat symptoms).  Method This was a 10-year retrospective study of patients undergoing unilateral nasal mass surgery between 2004 and 2014 at a UK district general hospital. We recorded patient demographics, laterality, histology, symptoms, clinical suspicion, and ima

Value of a lateral inferior pedicle flap in Draf IIb for recurrent frontal sinus diseases: a prospective study.

PURPOSE: The Draf IIb procedure allows the widest unilateral access to the frontal sinus in a minimally invasive fashion, with efficiency and safety comparable to the Draf III. However, this technique is still associated with a high postoperative stenosis rate. The exposure of drilled bone induces osteitis predisposing to scarring and neo-osteogenesis causing ostium restenosis. We developed a novel lateral inferior pedicle flap (LIPF) to cover the exposed bone and prevent restenosis during Draf IIb. We aimed to describe our technique. METHODS: Adult patients requiring a Draf IIb for unilatera

Polypoidal lesions in the nasal cavity.

INTRODUCTION: Nasal polyps are polypoidal masses arising from mucous membranes of nose and paranasal sinuses. They are overgrowths of the mucosa that frequently accompany allergic rhinitis. They are freely movable and nontender. AIMS AND OBJECTIVES: The purpose of this study was to study the histopathologic spectrum of polypoidal lesions of the nasal cavity. MATERIALS AND METHODS: The study comprised of 100 consecutive cases of polypoidal lesions in the nasal cavity, received in the department of pathology. The age and sex of the patients were recorded. The tissues were routinely processed f

The presentation and clinical significance of sinonasal respiratory epithelial adenomatoid hamartoma (REAH).

BACKGROUND: Sinonasal respiratory epithelial adenomatoid hamartoma (REAH) is a benign glandular proliferation with ciliated epithelium. Little is known about REAH, with only a few published case reports appearing since its original description in 1995. Classically described as an isolated polypoid lesion arising from the nasal septum, more recent descriptions also suggest that REAH can occur among nasal polyps. We report the largest experience with REAH to date, and aim to better understand and characterize this unique entity. METHODS: In this case series, all cases of REAH diagnosed between

Early CPAP use after anterior skull base surgery: A cautionary tale of postoperative CSF leak case report.

INTRODUCTION: Continuous positive airway pressure (CPAP) therapy is standard of care for obstructive sleep apnea (OSA) but is often discouraged after skull base surgery due to serious potential risks. Halting CPAP can negatively impact patients' quality of life and worsen postoperative cardiopulmonary complications. Identifying appropriate timing to resume CPAP following extended skull base approaches is essential, as recent literature mainly explores early CPAP safety after transsphenoidal surgery. CASE: We report a 51-year-old male with OSA who developed a post-operative cerebrospinal fluid

Application of the endoscopic prelacrimal recess approach to the maxillary sinus in unilateral maxillary diseases.

BACKGROUND: The endoscopic endonasal prelacrimal recess approach to the maxillary sinus provides wide access to the walls and recesses of the maxillary sinus, and its use has been reported in many maxillary sinus and skull base diseases. The objective of this study was to determine the indication and feasibility of endoscopic sinus surgery using the prelacrimal recess approach in unilateral maxillary sinus diseases. METHODS: We reviewed 192 cases of unilateral maxillary sinus diseases; over 2 years, 15 cases underwent endoscopic surgery via the prelacrimal recess approach. Data regarding preo
