BACKGROUND: Although conservative treatment is typically recommended for pregnant patients with pituitary adenoma (PA), surgical treatment is occasionally necessary for those with acute symptoms. Currently, surgical interventions utilized among these patients is poorly studied.
AIM: To evaluate the surgical indications, timing, perioperative precautions and postoperative complications of PAs during pregnancy and to provide comprehensive guidance.
METHODS: Six patients with PAs who underwent surgical treatment during pregnancy at Peking Union Medical College Hospital between January 1990 and

The initial presentation of macroprolactinoma with visual field impairment, especially in males, is well recognized. Successful treatment with dopamine agonist therapy is characterized by a reduction in hyperprolactinaemia and often rapid and progressive resolution of the visual impairment. A small proportion of patients may subsequently develop a secondary deterioration in both their visual fields and visual acuities despite normalization of prolactin levels and tumour shrinkage. When pituitary apoplexy can be excluded this may result from traction on the optic chiasm which is pulled down int

Transsphenoidal reoperation of pituitary adenomas remain a difficult surgical procedure even with skilful microsurgical techniques. This paper retrospectively analysed how to overcome such difficult factors as absent normal surgical landmarks and an adhesive field. For residual or recurrent adenomas, transsphenoidal reoperations were carried out 14 times in 12 patients. Reasons for reoperations were intentionally multistaged operations in 10 cases, regrowth in three and apoplexy in one. In the first operation, we tried to preserve or restore as many anatomic landmarks as possible. Overly aggre

Recognising optic chiasmal disease early is important in order to avoid irreversible visual loss and the potential risk of mortality for patients. Yet, there is frequently a delay in the initial diagnosis. Whilst the signs of optic chiasmal disease, particularly the perimetric findings, are well documented in the recent literature, the symptoms have been less well reported. Whilst some patients with optic chiasmal disease will be asymptomatic, many will complain of visual symptoms including symptomatic field defects, problems with central vision, difficulty with near tasks, binocular visual di

The global deployment of SARS-CoV-2 vaccines has been pivotal in curbing the COVID-19 pandemic, reducing morbidity and mortality associated with the virus. While most of these vaccines have demonstrated high efficacy and overall safety, emerging reports have highlighted potential neurovascular adverse effects, albeit uncommon, associated with these vaccinations. This review aims to assess and summarize the current knowledge on the neurovascular complications arising post-SARS-CoV-2 vaccination. We conducted an extensive literature review, focusing on clinical studies and case reports to identi

BACKGROUND: Immunotherapy treatment with immune-checkpoint blockade has become a new paradigm in cancer treatment. Despite its efficacy, it has also given rise to a new class of adverse events, immune-related adverse events, which may affect any organ, including the thyroid and the pituitary.
CASE PRESENTATION: We present a case of a 77-year-old Caucasian man with metastatic renal cell carcinoma on immunotherapy treatment who was admitted to our hospital with a severe persistent headache of sudden onset. He had been on corticosteroid therapy for 10 days for suspected immune-related thyroiditi

Craniopharyngiomas are rare, slow growing tumors arising along the craniopharyngeal duct. The incidence of craniopharyngioma was 0.13 per 100,000 persons per year. Intratumoral hemorrhage is a serious complication that can mimic pituitary tumor apoplexy, a life-threatening condition. In this report, We presented a case of a 20-year-old male with 3 days of decreased consciousness, with a history of worsening headaches, vomiting, blurry vision, bitemporal hemianopia, and right-sided limb weakness. MRI findings revealed a mixed cystic and solid suprasellar mass with blooming artifacts and fluid-f

BACKGROUND: Giant pituitary adenomas (GPA) are considered difficult to treat and the operative procedures are associated with more complications. This study aimed to assess treatment strategies of GPAs in a large consecutive and uniformly documented series in a single specialized center.
METHODS: A total of 289 patients with GPA who underwent primary surgery in our department between December 1982 and December 2022 were analyzed in this retrospective study. GPAs were defined by a maximum diameter of ≥ 4 cm in at least one plane. Patients were reviewed for endocrine, radiological and ophthalmo

OBJECTIVE: Resection of nonfunctional pituitary adenomas (NFPAs) can precipitate transient, or in some cases, permanent hormone deficits requiring replacement. Predicting the risk of permanent hormone dysfunction and the possibility of improvement postsurgery is crucial for patient counseling. This study analyzed a large cohort of patients with NFPA to assess predictors of postoperative hormone function and to help both surgeons and patients better predict outcomes.
METHODS: The authors conducted a retrospective single-institution study on a series of patients treated for NFPAs at Weill Corne

OBJECTIVE: Silent corticotrophic adenomas (SCAs) of the pituitary gland present as clinically nonfunctioning sellar lesions, with normal serum and urine hormone testing results, but stain positively for adrenocorticotropic hormone in immunohistochemical analyses. These tumors are now more readily recognized, but determination of their natural history and responses to treatment is difficult because of their rarity. We report the diagnoses and outcomes for a series of patients with SCAs, and we describe the creation of an Internet-accessible database (www.hsc.virginia.edu/neuro/neurosurgery/pitu

Thunderclap headache (TCH) is an excruciating headache that reaches maximal intensity within a minute. It has numerous potential etiologies, the most concerning of which is subarachnoid hemorrhage (SAH) due to high morbidity and mortality. Thus, patients with TCH must be evaluated urgently to identify the underlying cause and initiate prompt therapy. Areas covered: This paper reviews PubMed-listed research articles and presents an update of the clinical features, diagnostic evaluation, and possible causes of TCH. Expert commentary: In addition to SAH, TCH has been associated with reversible ce

BACKGROUND: Retroclival hematomas are a rare entity. The pathology can be categorized into epidural hematoma or subdural hematoma based on the anatomy of the tectorial membrane. Frequently, the etiology is related to accidental trauma, though other mechanisms have been observed, including coagulopathy, non-accidental trauma, and pituitary apoplexy. There have been only 2 prior cases where both epidural and subdural hematoma co-present.
CASE PRESENTATION: An 8-year-old male was involved in a high-speed motor vehicle accident. He presented with a Glasgow Coma Score (GCS) of 14 with bilateral ab

Thyroid and pituitary disorders linked to the coronavirus SARS-CoV-2, responsible for the COVID-19 epidemic, are mainly due to direct infection of the endocrine glands by the virus and to cell damage induced by the immune response. The two most frequent thyroid complications of COVID-19 are low T3 syndrome, or "non-thyroidal illness syndrome" (NTIS), and thyroiditis. Studies among in-patients with COVID-19 have shown that between one out of six and half of them have a low TSH level, related to NTIS and thyroiditis, respectively, sometimes found in the same patient. In NTIS, the decrease in fre

BACKGROUND: Prolactin (PRL)-secreting pituitary adenomas (prolactinomas) are rare in childhood and adolescence; there are only a few published series of patients who were surgically treated. We discuss the gender-dependent differences, and the surgical indications and results for 14 patients with prolactinomas under 18 years of age at surgery.
METHODS: We reviewed the data for 14 patients (10 girls and 4 boys) treated surgically for prolactinomas between 1980 and 1996. Surgery was chosen because of intolerance and/or resistance to dopamine agonist (DA) in 9 patients, the patient's decision in

Wernicke's encephalopathy (WE) is an acute neurological disorder resulting from thiamine deficiency and is classically associated with chronic alcohol misuse. However, it may also occur in non-alcoholic patients with prolonged malnutrition, persistent vomiting, or impaired nutritional intake, making diagnosis particularly challenging when the classical triad of ophthalmoplegia, ataxia, and altered mental status is incomplete or develops gradually. Delayed recognition can result in irreversible neurological injury, whereas early treatment with thiamine is associated with promising outcomes. We

Acute visual symptom emergencies occur commonly and present a challenge to both clinical and radiologic facets. Although most patients with visual complaints routinely require clinical evaluation with direct ophthalmologic evaluation, imaging is rarely necessary. However, there are highly morbid conditions where the prompt recognition and management of an acute visual syndrome (AVS) requires an astute physician to probe further. Suspicious symptomatology including abrupt visual loss, diplopia, ophthalmoplegia, and proptosis/exophthalmos require further investigation with advanced imaging modal
