The role of intrinsic muscle flaps of the foot for bone coverage in foot and ankle defects in diabetic and nondiabetic patients.

Local muscle flaps, pioneered by Ger in the late 1960s, were extensively used for foot and ankle reconstruction until the late 1970s when, with the evolution of microsurgery, microsurgical free flaps became the reconstructive method of choice. To assess whether the current underuse of local muscle flaps in foot and ankle surgery is justified, the authors identified from the Georgetown Limb Salvage Registry all patients who underwent foot and ankle reconstruction with local muscle flaps and microsurgical free flaps from 1990 through 1998. By protocol, flap coverage was the reconstructive choice

Three-dimensional kinematics at the ankle joint complex in rheumatoid arthritis patients with painful valgus deformity of the rearfoot.

OBJECTIVE: To evaluate the three-dimensional (3D) kinematics of the ankle joint complex (AJC) in rheumatoid arthritis (RA) patients with painful valgus deformity of the rearfoot. METHODS: Fifty patients with RA underwent gait analysis using electromagnetic tracking techniques to measure 3D kinematics at the AJC under barefoot and shod walking conditions. Three axial rotations (dorsiflexion/plantarflexion, inversion/eversion and internal/external rotation) were measured during the gait cycle, and the angular positions at key gait events, range of motion and the motion: time integral were measu

Incidence and clinical significance of bone bruises after supination injury of the ankle. A double-blind, prospective study.

We used MRI to study a prospective series of 95 patients with inversion injuries of the ankle and no fracture on plain radiographs. We found an incidence of bone bruises of 27%, but these made no difference to the time of return to work, limitation of walking or physical activity, or the clinical outcome scores at three months. We conclude that bone bruises have very little clinical significance after inversion injuries of the ankle.

Locomotion of the hindlimbs after neurectomy of ankle flexors in intact and spinal cats: model for the study of locomotor plasticity.

To study the potential plasticity of locomotor networks in the spinal cord, an important issue for locomotor rehabilitation after spinal injuries, we have investigated the locomotor performance of cats before and after a unilateral denervation of the ankle flexors tibialis anterior (TA) and extensor digitorum longus (EDL) both in cats with intact spinal cord and after spinalization. The effects of the inactivation of the ankle flexors were studied in three cats with intact spinal cord during periods of 4-7 wk. Cats adapted their locomotor performance very rapidly within a few days so that the

[Experiences and results of replacement of the fibulo-talar ligaments in relatively old ankle-joint traumas (author's transl)].

This is a report about the experiences and results of replacement of the fibulo-talar-ligaments by using the peronaeus brevis tendon. The typical anatomic and biomechanic findings are shown as well as the typical traumatisms. The importance of a good primary diagnosis and a good primary treatment is shown. In case of persisting of older instabilities we have the possibility of operative reconstruction. This is a report about 18 patients operated by the technique of Watson-Jones by using the peronaeus brevis tendon. Our results can demonstrate the big values of this technique for operative trea

Outcomes of Revision Modified Broström Procedure with Periosteal Flap Augmentation in Patients with Recurrent Chronic Ankle Instability.

BACKGROUND: Although the modified Broström procedure (MBP) is widely accepted as the standard surgical treatment for chronic ankle instability (CAI), recurrent instability remains a clinical challenge, particularly in patients with poor tissue quality or high functional demands. To overcome these limitations, periosteal flap augmentation has been proposed as a biological alternative for patients with recurrent CAI after failed MBP. This study evaluated the clinical and radiographic outcomes of revision MBP with periosteal flap augmentation. METHODS: Fourteen patients (mean age, 29.5 years; ra

Ankle-Foot Orthosis and Footwear Interactions in Adult Healthy and Patient Populations: Systematic Review of Biomechanical Outcomes.

BACKGROUND: Both ankle-foot orthoses (AFOs) and footwear adaptations are used to improve gait across a broad range of neuromuscular disorders. AFOs and footwear can each independently influence gait through the manipulation of certain design parameters, such as AFO footplate stiffness or the use of a forefoot rocker. While the effects of design parameters from AFOs and footwear on gait have been investigated independently, how these parameters interact to influence walking biomechanics remains poorly understood. OBJECTIVE: This systematic literature review aims to provide an overview of the i

Push-Pull Ratio and Ankle Dorsiflexion Strength in Nurse Home Residents: Novel Insights Into Fall Prevention Strategies.

BACKGROUND AND PURPOSE: Falls are a leading cause of disability and mortality in older adults. Muscle strength, neuromuscular activation, and balance, particularly the push-pull ratio (PPR), play key roles in fall prevention. The purpose of this study is to analyze among muscle strength, time to peak force (TP), and PPR between fallers and non-fallers in nursing home residents. METHODS: A total of 51 older adults participated in the study. Participants were classified as fallers or non-fallers based on their fall history in the past year. Isometric muscle strength assessments were conducted u

Influence of posterior malleolar fragment size on clinical function and spatiotemporal gait parameters in trimalleolar ankle fractures.

BACKGROUND: Posterior malleolar fractures accompanying trimalleolar ankle injuries may compromise joint stability and lead to long-term functional impairment. However, the impact of posterior malleolar fragment (PMF) size on postoperative functional recovery remains unclear. This study aimed to evaluate the association between PMF size and morphology with postoperative clinical outcomes using both functional scoring systems and smartphone-based gait analysis. METHODS: Seventy-two patients who underwent surgical treatment for trimalleolar fractures between 2017 and 2022 and met the inclusion c

Impact of surgical timing on wound complications following ankle fracture fixation: A 22-year retrospective cohort study.

BACKGROUND: Ankle fractures are common injuries, with a significant proportion requiring surgical intervention. Despite established treatment principles, the optimal timing for open reduction and internal fixation (ORIF) remains debated. Concerns regarding wound infection risk have led some surgeons to delay fixation, while others advocate for early intervention. This study aimed to assess the impact of surgical timing on postoperative wound infections. METHODS: A retrospective review was conducted on 1130 patients with surgically treated closed ankle fractures at a single institution between

Early osteopathic treatment delivered to patients with an acute lateral ankle sprain improves recovery: an investigative study.

CONTEXT: Acute lateral ankle sprain (ALAS) is responsible for short-term disability, increased risk for future acute ankle sprain, and chronic ankle instability. Several studies highlighted that manipulative therapy may reduce pain and increase the range of motion (ROM) of the injured ankle, but not all of them targeted acute injuries. Some studies focused their interest on a specific manipulative technique or utilized either physiotherapy or chiropractic techniques. OBJECTIVES: The objectives of this study were to evaluate the effectiveness of osteopathic care in ALAS, to assess pain, edema,

Does Platelet-Rich Plasma Speed Recovery After Acute Lateral Ankle Sprain? A Prospective Comparative Study Among Athletes.

BACKGROUND: Acute lateral ankle sprains are among the most common musculoskeletal injuries, accounting for up to 20% of all sports-related injuries and a leading cause of time lost from athletic participation. Although most cases respond to conservative care, 20% to 40% of athletes develop persistent pain, recurrent sprains, or chronic instability. Biologic therapies such as platelet-rich plasma (PRP) have gained interest for their potential to accelerate ligament healing, but evidence in acute athletic ankle sprains remains limited. METHODS: This study was designed as a nonmasked randomized

Impact of Initial Immobilization Method on Outcomes in Unstable Weber B Ankle Fractures: A Retrospective Cohort Study.

BACKGROUND: Weber B ankle fractures with medial clear space (MCS) widening on stress radiographs are unstable injuries that may require surgical stabilization. Initial emergency department management varies, with patients receiving a cast or splint after reduction, although concerns for soft tissue complications often limit cast use. It remains unclear whether splint or cast immobilization affects clinical or radiographic outcomes. METHODS: We retrospectively reviewed 295 patients treated for closed Weber B ankle fractures between January 1, 2017, and February 15, 2025; among them, 96 were de
