BACKGROUND: Vestibular nerve section is considered to be the most effective surgical procedure to control intractable symptoms secondary to Meniere's disease (MD). This study was developed to analyze the adequacy of retrosigmoid vestibular neurectomy in terms of vertigo control, hearing preservation and clinical complications of this procedure.
METHODS: A retrospective review was carried out on 14 patients affected by definite unilateral MD who underwent vestibular neurectomy via the retrosigmoid approach.
FINDINGS: One patient was lost from follow-up; another one had only a short postoperat

OBJECTIVE: Cartilage shield tympanoplasty is a procedure for repairing total tympanic membrane perforations. This procedure is indicated primarily for patients with total perforations, severely atelectatic tympanic membranes, and failures of previous tympanoplasty associated with chronic eustachian tube dysfunction. Although the graft take of this technique has been reported to be excellent, there have been concerns regarding hearing results because it replaces the entire tympanic membrane with cartilage. The purpose of this study was to report our experience with this technique.

The peripheral auditory sense organ, the cochlea, receives innervation from lateral and medial olivocochlear neurons in the brainstem. These neurons are able to modulate cochlear neural output. Anatomical studies have shown that one of the neurotransmitters which is present in varicosities surrounding the olivocochlear neurons in the brainstem is noradrenaline and previous work on brainstem slices has demonstrated a generally excitatory effect of noradrenaline on medial olivocochlear neurons. In order to assess in vivo the function of the noradrenergic inputs to olivocochlear neurons, we injec

HYPOTHESIS: Blockade of tumor necrosis factor-alpha with tumor necrosis factor-alpha antibody will reduce the extent of cochlear injury and hearing loss associated with Streptococcus pneumoniae meningitis.
BACKGROUND: Inflammatory mediators play a significant role in the morbidity associated with bacterial meningitis, including hearing loss and labyrinthitis ossificans. Previous studies have shown the attenuation of hearing loss by the nonspecific blockade of such pathways.
METHODS: Fifty Mongolian gerbils were divided into four groups. Auditory brainstem response testing was conducted to me

Sudden hearing loss is a reduction of sensoneurinal sound perception with a sudden onset. It is usually occurring only in one ear and without a recognizable cause. If a hearing loss is suspicioned due to otoscopic and tuning fork test findings, the patient should rapidly be sent to an ENT-specialist because of the possible consequences. After the first examinations by means of ear microscopy and audiogram further diagnostics may be necessary. If no cause is found, the idiopathic sensorineural hearing loss remains as the exclusion diagnosis. Therapy follows a polypragmatic approach based on the

AIM: To study the prevalence and pattern of sensorineural hearing loss (SNHL) in Nigerian children with sickle cell disease (SCD).
MATERIALS: Fifty-two children with SCD were assessed. Otologic examination and audiometric tests were performed. There were 36 males and 16 females. Their age ranged from 6 to 19 years.
RESULT: Seven children had hearing impairment. After hemoglobin categorization, all the SCD patients were found to belong to the homozygous (HbSS) group. There were no HbSC nor other variants detected. Prevalence of SNHL was 13.4% in these patients but the general population preva

PURPOSE: Although surgical excision is the traditional treatment modality for acoustic neuroma, radiotherapy (RT) is gaining momentum as an alternative. This is particularly evident in patients with useful hearing, in whom fractionated RT offers the potential for hearing preservation. Our objective was to determine the disease control, hearing preservation (via audiograms), and toxicity rates after linear accelerator-based stereotactic radiation for acoustic neuroma.
METHODS AND MATERIALS: A total of 72 acoustic neuroma patients underwent stereotactic irradiation and had at least 6 months of

In spite of strict safety regulations concerning firearm shooting, several hundred conscripts still suffer acute acoustical trauma (AAT) in the Finnish Defence Forces (FDF) every year, resulting in symptoms such as tinnitus and/or hearing impairment. The causes leading to AAT, causative weapons and mode and level of hearing protection were analysed to find out why so many AATs still occur in the FDF. The material consisted of 119 patients of the total 163 AAT patients treated at the Central Military Hospital during the year 2000. In 87.5% of cases, the AATs occurred in unprotected ears. The mo

CONTEXT: A number of therapies have been proposed for the long-term management of corticosteroid-responsive, rapidly progressive, bilateral sensorineural hearing loss (autoimmune inner ear disease [AIED]). Methotrexate has emerged as the benchmark agent but has not been rigorously evaluated for hearing improvement in patients with AIED.
OBJECTIVE: To assess the efficacy of long-term methotrexate in maintaining hearing improvements achieved with glucocorticoid (prednisone) therapy in patients with AIED.
DESIGN, SETTING, AND PARTICIPANTS: A randomized, double-blind, placebo-controlled trial co

Tympanometry is well established as a means of assessing the presence of fluid in the middle ear. The type B tympanogram is usually considered a unique entity. However, its shape may vary from a rounded type B with a "pseudopeak" to a completely flat response. The aim of this study was to compare the characteristics of the B curve (maximum admittance, tympanometric peak pressure, and area under the curve) to the viscosity of the middle ear fluid and to the air-bone gap (ABG). In 67 children (93 ears) who underwent ventilation tube insertion, no correlation was found between the viscosity of th

OBJECTIVE: Vestibular evoked myogenic potential (VEMP) has recently been broadly studied in cochleo-vestibular disorders to elucidate its mechanism. Because it is evoked by loud sound stimulation, impairment of the sound transmission through the middle ear may affect VEMP results. This study aims to compare the response rate of VEMPs using the tone burst method and the tapping method in patients with chronic otitis media (COM).
DESIGN: Fourteen patients (22 ears) with conductive hearing loss due to COM were subjected to VEMP tests using both the tone burst method and the tapping method. Each

AIMS: There is increasing evidence that pseudoexfoliation (PXF) not only affects ocular anterior segment structures, but may also be a systemic disease. This study was undertaken to assess the relationship between PXF and sensorineural hearing loss.
METHODS: Patients with PXF were identified from hospital records and underwent complete ocular examination. The sum of pure-tone hearing thresholds measured at 1, 2 and 3 kHz (HTL1,2,3) in each ear was compared with the ISO 7029 standard sex-matched, median age-associated hearing loss summed over the same frequencies (AAHL1,2,3). Partial cochlear obstruction is a relatively common finding in candidates for cochlear implants and frequently involves the inferior segment of the scala tympani in the basal turn of the cochlea. In such patients, the scala vestibuli is often patent and offers an alternative site for implantation. The current report describes two patients with such partial obstruction of the inferior segment of the basal cochlear turn, caused in one case by systemic vasculitis (Takayasu's disease) and in the other by obliterative otosclerosis. A scala vestibuli implantation allowed for complete insertion of th

This is a report of three cases of mixed hearing loss that resulted from inner ear disorders. Two cases were unilateral and the third was bilateral. The diagnosis was based on the findings of normal middle and external ears in association with the absence of round window reflexes. The contralateral stapedial reflex was present in the two unilateral cases. This is the first documentation of conductive deafness due to inner ear abnormality. This diagnosis should be considered in cases of conductive hearing loss if the middle and external ears are normal. More studies are needed to establish the

The effects on hearing of simultaneous exposure to the ototoxic organic solvent ethyl benzene and broad-band noise were evaluated in rats. The effects of three ethyl benzene concentrations (0, 300 or 400 ppm) and three noise levels (95 or 105 dB(lin) SPL or background noise at 65 dB(lin) SPL) and all their combinations were investigated for a 5 day exposure at 8 h/day. Distortion product otoacoustic emissions and compound action potentials were affected after 105 dB noise alone, and after 105 dB noise in combination with ethyl benzene (300 and 400 ppm). However, the amount of loss for these co

OBJECTIVES: Because endolymphatic hydrops causes cochlear malfunction, and both otoacoustic emissions and cochlear microphonics measure specific cochlear activities, some insight into the pathology of Meniere's disease might be gained by using these two test modalities. Specifically, the involvement of cochlear outer hair cells in patients with endolymphatic hydrops may be detected. Furthermore, it is hoped that these two tests might help determine which regions of the cochlea are affected by endolymphatic hydrops, as well as where along the auditory pathway abnormalities are present.

Acoustic measurements were made of the external and middle ear resonance in health and disease in patients after radical surgery on the ear and in chronic perforative otitis media. It is shown that the absence of the posterior wall of the acoustic meatus deteriorates hearing acuity in the frequency 4000 Hz. Reconstructive surgery of this wall normalizes acoustic resonance of the ear. The value of bone-air interval on pure tone audiogram is different after cavity operation and perforation of the tympanic membrane. Characteristics of acoustic resonance of the ear are decisive for choice of recon
