Association of osteopenia of the humeral head with full-thickness rotator cuff tears.

Rotator cuff tendon repair may fail for various reasons. Although the role of repair techniques and of the musculotendinous unit has been studied, there is little information on the quality of the bone to which the tendon is to be repaired. Therefore, 14 cadaveric humeral heads, 7 specimens without and 7 with a full-thickness rotator cuff tendon tear, were quantitatively assessed by use of high-resolution micro-computed tomography. Bone density is higher below the articular surface than in the greater tuberosity (40% vs 10%-20%), and tendon tears are associated with a reduction in cancellous b

Bone health in pediatric epilepsy.

Childhood and adolescence are critical periods of skeletal mineralization. Peak bone mineral density achieved by the end of adolescence determines the risk for later pathological fractures and osteoporosis. Chronic disease and medication often adversely affect bone health. Epilepsy is one of the most common neurological conditions occurring in persons under the age of 21. Epilepsy may affect bone in a number of ways. Restrictions of physical activity imposed by seizures; limitations on physical activity resulting from cerebral palsy, frequently present in patients with symptomatic epilepsy; an

Effects of ipriflavone administration on bone mass and metabolism in ovariectomized women.

The aim of the present study was to assess the effects of ipriflavone administration in the prevention of the rapid bone loss that follows ovariectomy in women. After 10-30 days from bilateral ovariectomy, patients received either the sole calcium supplementation (500 mg/day, n = 16) or ipriflavone (600 mg/day, n = 16) in addition to the same daily calcium supplement for 12 months. In calcium-treated subjects urinary hydroxyproline excretion, serum alkaline phosphatase and plasma bone Gla protein levels showed a substantial (p < 0.01) increase, while radial bone density significantly (p < 0.01

Genetics of osteoporosis.

There is clear evidence of genetic modulation of bone phenotype parameters including bone density, quantitative ultrasound, bone size, and bone turnover. At any particular age and phase of life, genetic factors explain about 70% of the variance in bone phenotype after adjustment for major medical and disease factors. Hormonal factors, diet, and lifestyle interact with those genetic factors over time. Common allelic variation in the VDR was the first of several genes and now chromosomal loci to be implicated in the genetic determination of bone phenotype. The VDR polymorphisms have an effect we

Quantification of bone structural parameters and mechanical competence at the distal radius.

For the clinician, predicting the fracture risk for individual patients is mainly restricted to the quantitative analysis of bone density. Several studies have shown that bone strength, an indicator for bone fracture risk, is only predicted moderately by bone density, indicating that there are other factors influencing bone competence. However, the relative importance of "bone quantity" and "bone quality" remains poorly understood. The objectives of this article are to describe some of the techniques used to measure the microarchitectural aspects of bone quality, how they can be quantified, an

The Management of Bone Defects in Rett Syndrome.

Rett syndrome (RS) is a rare neurodevelopmental disorder primarily caused by mutations in the X-linked methyl-CpG binding protein 2 (MECP2) gene, responsible for encoding MECP2 which plays a pivotal role in regulating gene expression. The neurological and non-neurological manifestations of RS vary widely in severity depending on the specific mutation type. Bone complications, mostly scoliosis but also osteoporosis, hip displacement, and a high rate of fractures, are among the most prevalent non-neurological comorbidities observed in girls with RS. Low bone mineral density (BMD) is primarily du

Insight into the potential of bone turnover biomarkers: integration in the management of osteoporosis and chronic kidney disease-associated osteoporosis.

PURPOSE OF REVIEW: Disturbances in mineral and bone metabolism occurring in osteoporosis and chronic kidney disease-associated osteoporosis place patients at high risk of fracture making these conditions a major public health concern. Due to the limited use of bone histomorphometry in clinical practice, the gold standard for assessing bone turnover, extensive efforts have been made to identify bone turnover markers (BTMs) as noninvasive surrogates. Since the identification of certain commonly used markers several decades ago, considerable experience has been acquired regarding their clinical u

Changes in bone turnover markers and bone modulators during abatacept treatment.

Rheumatoid arthritis (RA) causes bone loss, only partly related to inflammation. The impact of RA treatments on bone metabolism and their ability to mitigate bone loss remains uncertain. The primary goal of our study was to examine the influence of abatacept on serum levels of markers and regulators involved in bone turnover. Secondary objectives included evaluating changes in bone mineral density (BMD), bone health parameters, erosions, and exploring potential correlations among these parameters. We conducted a prospective observational study on patients with active seropositive RA failure to

Identifying Bone Matrix Impairments in a Mouse Model of Neurofibromatosis Type 1 (NF1) by Clinically Translatable Techniques.

Three-to-four percent of children with neurofibromatosis type 1 (NF1) present with unilateral tibia bowing, fracture, and recalcitrant healing. Alkaline phosphatase (ALP) enzyme therapy prevented poor bone mineralization and poor mechanical properties in mouse models of NF1 skeletal dysplasia; but transition to clinical trials is hampered by the lack of a technique that (i) identifies NF1 patients at risk of tibia bowing and fracture making them eligible for trial enrollment and (ii) monitors treatment effects on matrix characteristics related to bone strength. Therefore, we assessed the abili

Type 2 diabetes and the skeleton: new insights into sweet bones.

Substantial evidence shows that skeletal fragility should be considered among the complications associated with type 2 diabetes. Individuals with type 2 diabetes have increased fracture risk, despite normal bone mineral density (BMD) and high BMI-factors that are generally protective against fractures. The mechanisms underlying skeletal fragility in diabetes are not completely understood, but are multifactorial and likely include effects of obesity, hyperglycaemia, oxidative stress, and accumulation of advanced glycation end products, leading to altered bone metabolism, structure, and strength

Bone anabolic versus bone anticatabolic treatment of postmenopausal osteoporosis.

Increased bone fragility after menopause is commonly associated with accelerated bone loss and aggressive osteoclastic function. This is attributed to increased RANKL production and impaired osteoprotegerin synthesis. Fast bone loss leads to trabecular perforations, dramatic diminution of bone strength, and unexpected fractures. To avoid osteoporotic fractures, elimination of fast bone loss is recommended. Antiosteoclastic drugs, apart from estrogens, are the selective estrogen receptor modulators, calcitonins, and amino-bisphosphonates. These drugs increase bone mass by 1-5%, but reduce the r

Determination of the influence of walking with orthosis on bone osteoporosis in paraplegic subjects based on the loads transmitted through the body.

BACKGROUND: Spinal cord injury is a damage to spinal cord that results in loss of function and mobility below the level of injury. The patients use various orthoses to improve their general health, to decrease bone osteoporosis, and to improve bone mineral density. It was controversial if how much percentage of the loads applied on an orthosis and body complex is transmitted by orthosis. Therefore, it was aimed to determine the magnitude of the loads transmitted by orthosis to find the influence of walking with orthosis on bone mineral density. METHODS: Three spinal cord injured subjects were

Abdominal Body Composition Reference Ranges and Association With Chronic Conditions in an Age- and Sex-Stratified Representative Sample of a Geographically Defined American Population.

BACKGROUND: Body composition can be accurately quantified from abdominal computed tomography (CT) exams and is a predictor for the development of aging-related conditions and for mortality. However, reference ranges for CT-derived body composition measures of obesity, sarcopenia, and bone loss have yet to be defined in the general population. METHODS: We identified a population-representative sample of 4 900 persons aged 20 to 89 years who underwent an abdominal CT exam from 2010 to 2020. The sample was constructed using propensity score matching an age and sex stratified sample of persons re

Atypical Fracture From Bisphosphonate Use in Hypophosphatasia With Improved Bone Response to Teriparatide Therapy.

Hypophosphatasia is a rare metabolic bone disorder that is often misdiagnosed. We present the case of a middle-aged woman initially misdiagnosed with rickets and later as osteogenesis imperfecta and treated with zoledronate, after which she developed atypical femoral fractures. After switching to teriparatide therapy, her bone mineral density improved significantly. This case underscores that persistently low alkaline phosphatase with fragility or atypical fractures should prompt evaluation for hypophosphatasia and that antiresorptives (eg, bisphosphonates) may precipitate atypical fractures i

1alpha-hydroxycholecalciferol: a promising therapeutic approach for renal osteodystrophy.

The effectiveness of 1alpha-hydroxycholecalciferol [1alpha-OH)D3] in healing uremic bone disease was investigated in 10 uremic patients on maintenance dialysis. Seven patients received 1alpha-(OH)D3, 2 microgram/day, for six months, and three patients served as a control group. Bone density was determined from X-rays of the second phalanx of the second digit. The apparent intestinal calcium absorption improved on treatment with 1alpha-(OH)D3, serum calcium levels and bone density significantly increased, and a significant decrease in serum alkaline phosphatase activity was observed. In one pat

Clinical and radiographic evaluation of natural bovine bone with hyaluronic acid on osseointegration of immediate implants: a randomized clinical study.

Hyaluronic acid (HA) is a natural component of the extracellular matrix (ECM) and has gained wide application in bone regeneration, particularly in maxillofacial and dental research. HA derivatives and HA-incorporated composite scaffolds demonstrate strong potential in enhancing osteogenesis and mineralization. The aim of this work was to evaluate clinical and radiographic effects of natural bovine bone with HA on immediate implants. Sixteen immediate implants were placed in medically free patients with non-restorable single-rooted maxillary anterior or premolar teeth; Patients were randomly a

Can biochemical markers predict bone loss at the hip and spine?: a 4-year prospective study of 141 early postmenopausal women.

A number of recent studies have suggested that non-invasive measures of bone turnover are associated with bone loss at the forearm in postmenopausal women. Whether bone turnover markers are predictive of bone loss from the clinically important sites of lumbar spine and femoral neck remain unclear, and was the aim of this 4-year prospective study. One hundred and forty-one normal, postmenopausal women (mean age 52.0 +/- 3.3 years, mean menopause duration 20.4 +/- 5.7 months) were recruited for the study in 1988. Fasting early morning samples of blood and urine were collected at the baseline vis
