Chronic traumatic tricuspid insufficiency.

Isolated tricuspid insufficiency (TI) is relatively uncommon and mostly of traumatic origin. We report clinical noninvasive and invasive findings and surgical results in 5 cases. All patients had complete clinical, noninvasive and invasive studies including right and left catheterization, and coronary angiographies in 3 patients. All but 1 patient had nonpenetrating trauma. All had large jugular V waves, right precordial impulse, systolic liver pulse, positive Carvallo sign documented also by noninvasive techniques. Right heart failure was present in 3 patients. Chest x-ray showed prominent ri

Clinical predictors of cardiac events in patients with isolated syncope and negative electrophysiologic study.

BACKGROUND: Patients with syncope or near syncope of unknown etiology represent a great challenge to cardiologists. An initial symptomatic episode triggers a series of diagnostic analysis which remain unsatisfactory when negative. More invasive tools such as electrophysiologic testing yield only partial answers to risk stratification while the complementary implantable holter diagnostics are not usually considered until a recurrent episode is documented. OBJECTIVE: This study targets predictors of significant cardiac rhythmic events in patients with a reported episode of syncope or near synco

Cardiac autonomic dysfunction in Brugada syndrome.

BACKGROUND: Patients with Brugada syndrome present with characteristic ECG abnormalities (atypical right bundle-branch block and ST-segment elevation) and life-threatening ventricular tachyarrhythmias despite structurally normal hearts. Involvement of the autonomic nervous system is suggested by the occurrence of ventricular tachyarrhythmias and sudden death at rest or during sleep and by changes of typical ECG signs under pharmacological modulation of the myocardial autonomic tone. METHODS AND RESULTS: This study investigated the presynaptic cardiac neuronal reuptake of norepinephrine (uptak

Pacemaker Implantation for Low-Grade Conduction Abnormalities After Balloon-Expandable Transcatheter Aortic Valve Implantation.

INTRODUCTION: A frequent complication after TAVI are postinterventional conduction abnormalities requiring permanent pacemaker implantation. In this study, we analyzed the characteristics of borderline conduction abnormalities leading to pacemaker implantation and the resulting ventricular pacing amounts. METHODS AND RESULTS: All patients who underwent balloon-expandable TAVI between 2014 and 2019 in our tertiary center were analyzed in a retrospective manner. One hundred and sixty-five patients of 1083 TAVI-patients developed postinterventional conduction abnormalities leading to pacemaker i

Feasibility and safety of endomyocardial biopsy by lumenless pacing lead sheath-directed method during cardiac implantable electronic device implantation.

BACKGROUND: Patients with an indication for a cardiovascular implantable electronic device (CIED) are complicated with special cardiomyopathy or other unspecified cardiac abnormalities and may need endomyocardial biopsy (EMB). However, EMB by a bioptome is usually avoided to reduce the risk of lead displacement in the CIED periprocedural period. OBJECTIVE: We aimed to assess the safety and feasibility of a novel approach for transvenous right ventricular (RV) EMB using the lead sheath method (L-S-M) during CIED implantation and compared it with the traditional bioptome method (T-B-M). Efficacy and Safety of the LifeTech™ Multifunctional Occluder (Konar-MFO) in Transcatheter Closure of Ventricular Septal Defects: A Systematic Review and Meta-Analysis.

Ventricular septal defect (VSD) constitutes nearly 40% of congenital heart defects, and around 60% are perimembranous in origin. Transcatheter closure of VSDs has gained popularity in recent years due to its less invasiveness, shorter duration of hospital stays, and avoidance of complications associated with cardiopulmonary-bypass-related conditions. The main emerging issues with the transcatheter closure approach are the development of complete atrioventricular block and valvular injury. LifeTech™ Multifunctional Occluder (Konar-MFO) is a novel device, recently approved by CE in 2018, that af

Permanent pacemaker reduction using temporary-permanent pacemaker as a 1-month bridge after transcatheter aortic valve replacement: a prospective, multicentre, single-arm, observational study.

BACKGROUND: The permanent pacemaker (PPM) implantation and pacemaker dependency rates after transcatheter aortic valve replacement (TAVR) are highly variable as some of the conduction disturbances are reversible. It remains poorly investigated how to optimise temporary pacing in these patients. This study aimed to explore the potential reduction in the PPM implantation rate using temporary-permanent pacemaker (TPPM) as a 1-month bridge. METHODS: This is a prospective, multicentre, single-arm, observational study. Consecutive patients undergoing TAVR from March 1, 2022 to March 1, 2023 in 13 t

Three-dimensional ultrastructural analysis of human skin with the arrector pili muscle interacting with the hair follicle epithelium.

This study developed a three-dimensional ultrastructural analysis application using serial block-face scanning electron microscopy (SBF-SEM) to investigate surgically acquired human skin tissues containing the arrector pili muscle. We utilized the en bloc staining, including reduced osmium, thiocarbohydrazide, and lead aspartate, as well as the embedding using a carbon-based conductive resin. Next, we obtained serial images with SBF-SEM. The results revealed dense nerve fiber networks branching from nearby nerve fiber bundles outside the muscle and running among muscle fibers. Additionally, th

Bidirectional Ventricular Tachycardia in Acute Multivessel Myocardial Infarction.

Bidirectional ventricular tachycardia (BVT) is a rare and unusual ventricular dysrhythmia that is characterized by a beat-to-beat alternation of the QRS axis. This can sometimes manifest as alternating left and right bundle branch blocks. To the best of our knowledge, there are two previous cases of BVT in the setting of type I myocardial infarction. Our case would be the third and showed a subtle change in the anterior-posterior axis that can be seen in lead V2. The coronary angiography of our patient demonstrated severe multivessel coronary artery disease with complete total occlusion of the

Exploring the association of electrocardiographic markers and arrhythmic risk in hypertrophic cardiomyopathy: A systematic review and meta-analysis.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a primary genetic myocardial disorder associated with sudden cardiac death (SCD) due to ventricular arrhythmias, however risk stratification remains challenging. Electrocardiographic (ECG) markers may offer non-invasive tools for improved risk assessment, but their prognostic value remains unclear. The purpose of this study is to systematically evaluate the association between ECG markers and the risk of major arrhythmic events (MAEs) in HCM patients. METHODS: We conducted a systematic search of PubMed, Cochrane Central Register of Controlled T

Bidirectional ventricular tachycardia caused by occlusion myocardial infarction-a case report.

BACKGROUND: Bidirectional ventricular tachycardia (BdVT) is an uncommon form of ventricular arrhythmia typically associated with digoxin toxicity or catecholaminergic polymorphic ventricular tachycardia. BdVT in the setting of acute myocardial ischaemia is limited to case reports. We present an example of BdVT caused by occlusion myocardial infarction with original and modified Sgarbossa criteria being positive. CASE SUMMARY: An elderly male with known coronary artery disease presented with chest pain. The initial ECG demonstrated a regular wide QRS complex tachycardia with alternating fronta

Muscular VSD Device Complications: Literature Review and Possible Implications for Echocardiographic Follow-Up.

Long-term transthoracic echocardiogram (TTE) follow-up guidelines after muscular ventricular septal defect (mVSD) device closure are vague. The primary goal of this study was to perform a literature search to characterize the type and timing of complications that occur after mVSD device placement. The search was performed in Medline (PubMed) with English language and publication date (1983 to 2024) filters applied. Studies were included if they reported on patients who underwent mVSD device closure. Studies were excluded if they reported on other types of ventricular septal defect (VSD) device

Electrocardiographic P-wave amplitude as a discriminative marker for pediatric atrial septal defects.

BACKGROUND: Atrial septal defects (ASD) are often asymptomatic during childhood, resulting in a delayed diagnosis. Electrocardiography (ECG) screening is known to have a limited ability to detect ASD, relying mainly on QRS abnormalities, such as incomplete right bundle branch block (IRBBB). In contrast, P waves have been underused because they are small, often on a trended baseline, and thus difficult to quantify accurately. This observational study examined the utility of baseline-corrected P-wave amplitude in the identification of ASD. METHODS: We analyzed digital ECG data from 45 secundum

Electrophysiological Mechanisms of ST Segment and T-Wave Changes: Insights into Ischemia, Conduction Delay, Hypertrophy, and Electrolyte Disturbance.

ST-T abnormalities are commonly encountered in cardiology, cardiac surgical, critical care, and emergency settings. ST segment depression refers to displacement of the ST segment below the PR-segment (baseline), whereas ST segment elevation refers to displacement above this baseline. A downward displacement greater than 0.5 mm below the baseline or an upward displacement exceeding 0.1 mV in the limb leads or 0.2 mV in the precordial leads, measured 40 ms after the J point relative to the baseline, is considered significant. ST-T abnormalities can be primary or secondary. Primary ST-T changes a

Prognostic impact of prolonged QRS duration or LBBB morphology on cardiovascular outcomes in chronic heart failure without ventricular pacing or cardiac resynchronization therapy: A systematic review and meta-analysis.

BACKGROUND AND AIMS: Prolonged QRS duration and left bundle branch block (LBBB) morphology are markers of ventricular dyssynchrony associated with adverse outcomes in chronic heart failure (CHF). Whether these findings independently predict prognosis across heart failure phenotypes without ventricular pacing or cardiac resynchronization therapy (CRT) is uncertain. Therefore, we conducted a systematic review and meta-analysis to address this question. METHODS: We searched PubMed, Embase, and Cochrane Library for studies comparing prolonged versus narrow QRS (≥120 ms vs <120 ms) or LBBB versus

Towards new care strategies in a suburban healthcare center in La Matanza, Argentina: performance evaluation of rapid diagnostic tests for Chagas disease in a non-endemic area.

INTRODUCTION: Chagas disease, caused by the protozoan parasite Trypanosoma cruzi, remains a major public health problem, affecting approximately seven million people worldwide. Although historically restricted to endemic regions, migration has led to its global distribution, increasing the relevance of congenital transmission in non-endemic areas. If not timely diagnosed and treated, infection can progress to chronic Chagas cardiomyopathy in nearly 30 % of patients, generating substantial social and healthcare burdens. Diagnosis requires concordant results from two or three serological assays
