The technique of obliquely drilling out the postero-lateral part of the cervical vertebral bodies is described. It uses the antero-lateral (retro carotico-jugular) approach to control and displace the vertebral artery postero-laterally and to expose the lateral aspect of the vertebral bodies. It provides, through a wide field and with minimal retraction of the carotid artery and the internal jugular vein, an extensive view of the anterior aspect of the spinal cord. It has already been used to treat 15 anterior lesions compressing the spinal cord including neurinomas and osteophytes.

Localized C1-C2 lateral mass osteoarthritis is a degenerative disorder of the upper cervical spine that has a natural history markedly different from that of degenerative afflictions of the lower cervical spine. Atlantoaxial lateral mass arthritis is a distinct cause of occasionally severe occipitocervical pain in elderly persons. In this series, the diagnosis was suggested by the medical history of nine elderly patients who presented with severe occipitocervical pain (frequently diagnosed as occipital neuralgia). Physical examination demonstrated marked restriction of rotation of the cervical

The case is described of a lady who developed dysphagia and dysphonia misdiagnosed first as hysteria, then as myasthenia gravis, one year after a Cloward's operation for cervical disc disease. After a bout of coughing the bony dowel was expelled and the correct diagnosis was made.

It is generally believed that significant head injury after a traffic accident or fall is associated with a high incidence of concurrent cervical spine injury. This study prospectively examined 260 victims of traffic accidents or falls with significant head injury. The incidence of associated cervical spinal injury was only 3.5 per cent. There was no association between the severity of head injury and the incidence of cervical spine injury. The risk of concurrent spinal injury in head injury patients is not higher than the reported risk in patients without head injury. Although care should be

A 4-year-old boy with hypochondroplasia presented with delay in gross motor development. Magnetic resonance imaging demonstrated basilar impression with compression at the craniovertebral junction and mild degree of hydrocephalus. Posterior fossa decompression resulted in improvement in neurologic function and relief of hydrocephalus.

Microsurgical decompression of the accessory nerve was conducted in 10 patients with spasmodic torticollis (tonic and tonic-clonic forms). Compression of the roots of the accessory nerve of the posterior inferior cerebellar and the vertebral arteries was discovered. Decompression was accomplished by division of the arachnoid adhesions, anastomosis with sensory roots of C1 and C2, and placing a layer of muscular tissue between the nerve and the vessel. The results of treatment of spasmodic torticollis are analysed and the efficacy of neurovascular decompression is emphasized.

Cervicogenic headache (CH) is a clearly defined headache syndrome and can usually be differentiated from other unilateral headaches like cluster headache, chronic paroxysmal hemicrania (CPH) and hemicrania continua by a thorough history and neurological examination. Combinations of CH with other headaches like migraine, tension headache, combination headache and drug induced headache, that occur quite frequently, can complicate the diagnosis. In our own material of 5520 headache patients the prevalence of CH was 13.8% (763). Four hundred and thirty of these patients suffered from CH in combina

After a fracture of the spinal column a 49-year-old male developed a mucormycosis due to Rhizopus oligosporus. This appears to be the first report of a disease caused by this thermotolerant Rhizopus species in a human being.

Between June 1984 and April 1988, 41 patients with severe posttraumatic lesions of the cervical spine between the C2-C3 and the C7-T1 level seen consecutively were treated by an anterior cervical fusion and osteosynthetic stabilization according to Caspar. These patients were prospectively studied. Seven patients had a bilateral facet dislocation, 5 a unilateral facet dislocation, 9 an anterior subluxation, 9 an anterior compression fracture, 5 a hangman's fracture, and 6 a hyperextension injury at a lower cervical level. From a neurological point of view, there were 12 patients with an initia

Myelopathy is a common consequence of cervical spondylosis. Typically, the syndrome develops gradually over a long period. Rapidly progressive deterioration in the absence of trauma or other precipitating factors is exceptional. We report a case of a rapidly progressive acute myelopathy secondary to cervical spondylosis without identifiable precipitating cause. The importance of recognizing this syndrome lies in the fact that rapid operative intervention may result in significant neurological recovery. The value of magnetic resonance imaging preoperatively and postoperatively is emphasized.

The authors reviewed 12 cases of anterior dislocation of the lower cervical spine due to flexion-rotation trauma, with late instability after closed treatment. The 12 patients were treated at the Instituto Ortopedico Rizzoli between 1977 and 1982. Epidemiology, causes, type of lesion, and associated complications were studied. After an average period of 3 years (range: 2-6), correction appeared stable in all of the patients. In 4 patients, posterior stabilization with plates was performed after presurgical correction in a halo-thoracic cast. In 4 patients, reduction with combined, treatment co

Anterior fixation of traumatic lesions of the cervical spine with a plate and bone grafts has been proved to be a safe procedure. Patients without neurologic lesions can be out of hospital 7 to 10 days after the accident. Even in cases of serious neurologic lesions, rehabilitation and nursing of the patient can be improved. Although the method can be used in any case of traumatic instability of the cervical spine, the main indications are teardrop fractures not reduced by skull traction, severe wedging fractures, severe extension injuries, and any lesion with compression in the anterior part o

One hundred cervical myelographies in patients without degenerative changes on plain radiographs were evaluated. Pathologic changes were seen in 75 patients, most of them with congenital spinal canal stenosis and dural sac stenosis. Normal values for sagittal diameter of the dural sac from C2 to C6 were established. It was found that a quotient less than 0.9 between the sagittal diameter of the spinal canal and the midsagittal diameter of the vertebral body indicated congenital stenosis. It is concluded that plain radiographs of the cervical spine are unreliable in predicting the diagnostic va

Calcifying discopathy in infancy involving in the cervical spine has already been observed and described by many authors, as a well-defined clinico-radiological syndrome with a benign course. The clinical picture is composed of: pain and functional limitation, sometimes with a stiff neck, more rarely slight fever, increase of the erythrocyte sedimentation rate or leukocytosis. The radiographic picture consists of the association of morphological and structural alterations of vertebral bodies adjacent to one or more disc calcifications usually centrally sited, sometimes associated with anterior

Calcification of cervical intervertebral discs in children is due to an uncommon, but distinct, disease of unknown etiology. Signs and symptoms of nerve root or spinal cord compression are unusual and acute symptoms are followed by a benign course and spontaneous recovery. We describe a 5-year-old patient with symptomatic cervical disc calcification and discuss the relevant clinical and radiographic features.

In order to determine the validity of cervical discography in the diagnosis and treatment of patients presenting with cervical discogenic syndrome, a retrospective analysis of 34 patients who underwent cervical arthrodesis on the basis of positive cervical discography was performed. The symptomatic cervical levels were selected by reproduction of the patient's symptoms at the time of injection. No patient had radicular symptoms, and other diagnostic modalities such as computed tomography (CT) scanning or myelography had been within normal limits. Seventy percent of patients who underwent surgi
