Nonoperative management of complicated diverticular disease.

The complications of diverticular disease of the colon can be divided into those related to inflammatory conditions (diverticular abscess, fistula, and perforation) and those related to noninflammatory conditions (lower gastrointestinal hemorrhage and noninflammatory stricture or obstruction). Nonoperative management of uncomplicated diverticulitis includes bowel rest and antibiotics. For abscesses, percutaneous drainage by radiologic guidance often turns complicated diverticulitis to an uncomplicated condition. In very select instances, fistulas or even perforation may be managed without oper

A case of mucinous adenocarcinoma arising in long-standing multiple perianal and presacral fistulas.

Chronic anal fistulas are not rare; however, the development of a carcinoma in long-standing, perianal fistulas is rare. We describe a case of an 85-year-old man with multiple, recurring, perianal fistulas, extending to the natal cleft. The patient underwent en bloc resection of the fistulas which were in direct continuity with the middle rectum. Histological examination revealed a mucinous colonic adenocarcinoma. Abdominal CT and colonoscopy revealed an extramural residual rectal mass. The patient refused a radical colorectal operation. Three years later, because of fistula recurrence, he und

Carcinosarcoma of the rectosigmoid colon: report of a case.

We report an unusual case of carcinosarcoma of the colon. An 80-year-old woman was admitted to our hospital with lower abdominal pain. Computed tomography showed a large pelvic mass, 18cm in maximal diameter, and barium enema and colonoscopy both showed a type-2 tumor in the sigmoid colon. We performed Hartmann's procedure with resection of the ileocolic segment. Immunohistochemical stains of the resected specimen revealed that most of the tumor consisted of spindle cell sarcoma with neural and muscle differentiation, while only the superficial area of an ulcerated lesion contained adenocarcin

The double contrast barium enema: a retrospective single centre audit of the detection of colorectal carcinomas.

AIM: To determine retrospectively the sensitivity and specificity of the double contrast barium enema (DCBE) as performed in one institution for the detection of colorectal carcinoma. SUBJECTS AND METHODS: Eight hundred and eighty barium enema reports were reviewed of consecutive adult patients who underwent DCBE and also had hospital case notes with a minimum follow up of two years, a later diagnostic colonoscopy, or operative and histological findings. RESULTS: Seventy-four true positive cases of colorectal carcinoma diagnosed at DCBE were confirmed at surgery and histological examination.

Successful resection of rectal carcinoma in an Evans' syndrome patient followed by predonisolone and high-dose immunoglobulin: report of a case.

A 69-year-old woman was admitted to our hospital because of anal bleeding and fatigue. The patient was previously diagnosed as having Evans' syndrome on the basis of hematological examination and had been treated with predonisolone for 8 years. On admission, severe anemia and thrombocytopenia were noted. Colonoscopy and Barium enema studies demonstrated an irregular tumor with hemorrhagic ulceration in the rectum, which was histopathologically confirmed as an adenocarcinoma. After red blood cells and platelets were transfused, and the patient was treated with high-dose gammaglobulin, predoniso

Chilaiditi's syndrome. Successful surgical correction by colopexy.

We describe the case of a patient affected by the Chilaiditi's syndrome, the interposition of the small or large bowel between the inferior face of the diaphragm and the liver, a rare anomaly, often asymptomatic. A 50-year-old man came to our outpatients clinic because of persistent pain in the right abdominal region. Two attempts to perform colonoscopy failed because of the impossibility of passing through the transverse colon. A double contrast enema indicated only sigmoid diverticulitis. Only after abdominal radiography was the abnormal position of the right colon noted. At surgery the righ

[Toxic megacolon presenting as Crohn's disease].

We introduce a young patient, without history of inflammatory bowel disease (I.B.D.) who started with an acute gastroenteritis, which in the following days progressed to a toxic megacolon. The patient had come to hospital with nausea, vomiting, fever and liquid, explosive diarrhoea without pathologic products. There was no clinical remission with astringent diet, hydroelectrolitic reposition and antidiarrheic opiates. The patient was admitted in hospital when he had blood in the diarrhoea. This progressed to a toxic megacolon in three days and the patient had to be operated on urgently. The su

Gastrointestinal bleeding after cold biopsy.

Cold biopsy of the gastric mucosa is useful in many gastroduodenal disorders. Antral biopsies are done with increasing frequency to confirm Helicobacter pylori infection and to determine the type and content of gastritis. Gastrointestinal bleeding after gastric cold biopsy is rare. We report two patients who developed melena after cold biopsy of the gastric antrum. Repeat gastroscopies excluded lesions other than the biopsied sites as the source of bleeding. Colonoscopies in both cases did not reveal any evidence of lower GI bleed. Relevant medications include amlodipine, in case 1, and brufen

Colonic adenoma detected by positron emission tomography (PET): a case report.

A 61-year-old asymptomatic woman underwent whole-body positron emission tomography (PET) with 18F-fluorodeoxyglucose and was found to have a lesion in the ascending colon. Further colonic examination was not performed due to her medical condition. One year later, the lesion was demonstrated again by PET. After the second PET study, she underwent colonoscopy, which revealed a pedunculated polyp in the ascending colon. A polypectomy was performed. Histopathological study showed a 1.8-cm adenoma with mild to moderate atypia. The findings in our case suggest that increased glucose metabolism can b

Non-evidence of estrogen receptors in the rectal mucosa.

The aim of the study was to investigate whether estrogen receptors are present in the rectal mucosa of premenopausal women compared to postmenopausal women and men. Thirty biopsies obtained from the rectal mucosa at colonoscopy, performed to investigate inflammatory bowel disease in 23 patients and neoplasia in 7, were examined by the avidin-biotin-peroxidase immunohistochemical technique for the presence of estrogen receptors. The study group (n = 10) were non-pregnant premenopausal women and the control group (n = 20) were postmenopausal women (n = 10) and men (n = 10). None of the subjects

Feminizing Sertoli cell tumors associated with Peutz-Jeghers syndrome: an increasingly recognized cause of prepubertal gynecomastia.

Testicular sex cord tumors with annular tubules are an increasingly recognized cause of prepubertal gynecomastia typically accompanied by accelerated linear growth and advanced bone maturation. Serum estrogen levels may be elevated. Testicular ultrasound and biopsy are diagnostic, and mastectomy is indicated. Although these tumors can occur independently, causing gynecomastia in 10 percent of cases, they usually occur in patients with Peutz-Jeghers syndrome. In any Peutz-Jeghers syndrome patient developing gynecomastia, a testicular tumor should be sought. Conversely, because a significant pro

Successful treatment of a rectal anastomotic stenosis by transanal endoscopic microsurgery (TEM) using the contact Nd:YAG laser.

We report the advantage of employing transanal endoscopic microsurgery (TEM) using the contact Nd:YAG laser for the treatment of a rectal anastomotic stenosis. A 72-year-old woman was admitted to our hospital with a postoperative rectal anastomotic stenosis. Twenty months prior to admission, the patient underwent a low anterior resection for the treatment of the rectal cancer using an EEA stapling device. A barium enema and colonoscopy revealed a rectal stenosis, 0.8-cm diameter. This stenosis was at the anastomotic site, approximately 4.0 from the dental line. An endoscopic treatment was perf

Colonic adenomas in asymptomatic women with a history of breast cancer.

One hundred ninety-three asymptomatic women with a personal history of breast cancer underwent screening colonoscopy. One hundred sixty-eight women had breast cancer as their only potential risk factor for colonic neoplasia, and 25 had a family history of colorectal neoplasia in addition to their personal history of breast cancer. Among women with breast cancer, increasing age and body weight were each predictive of an increasing prevalence of colonic adenomas. The prevalence of colonic adenomas in women aged 50-75 yr whose only potential risk factor was breast cancer was 18%, and was identica

Upper and lower gastrointestinal endoscopy in children and adolescents with Crohn's disease: a prospective study.

Fifty-six children and adolescents with Crohn's disease were prospectively investigated with gastroscopy and colonoscopy irrespective of localizing symptoms or signs. Routine biopsies were taken from endoscopically normal and abnormal areas. A high incidence (71%) of upper gastrointestinal (GI) involvement was found. In 41%, these findings were instrumental in making the diagnosis. The ileum was viewed in 49 of the 56 cases. Overall, the upper GI tract was involved in 71%, the terminal ileum in 53%, and the colon in 86% (oesophagus 16%, body of stomach 46%, antrum 36%, duodenum 21%, terminal i

[Sonography in the diagnosis and follow-up of colonic diverticulitis].

17 patients with colonic diverticulitis of various degree were examined by conventional abdominal ultrasound in comparison to either colonoscopy or operation. The sonography of the inflammated region showed an echo-poor enlargement of the colonic wall (normal less than 3 mm) ranging from 5 to 15 mm (mean 7.8 mm) over a short distance. In cases with bowel wall enlargement of more than 11 mm a spontaneous evacuation of pus from the diverticles was endoscopically seen. A sonographic enlargement of the colonic wall of more than 15 mm is very suspicious to be a peridiverticulitis or a beginning abs

Contribution of modern imaging in rectosigmoid neoplastic disease (comparison with fiberoptic endoscopy).

Double-contrast enema (DCE) remains the primary diagnostic exploration. Its specific method and interpretation require skillful handling. Unlike colonoscopy, DCE results in false-positives, the frequency of which varies widely in function of the skill of the interpretation. False negatives reach a significant level from a threshold of size which can be fixed at 5 mm. Despite these drawbacks, double-contrast enema, which is an exploration involving a two level of invasiveness and which provides a good panoramic image of the colon should remain the primary exploration in routine check-ups. [Systematic monitoring of the postoperative rectum].

This study concerns the detection and treatment of recurrence after operation for colo-rectal cancer. The recurrences were classified as being in the suture line, peri-colonic, pelvic or distant metastasis. The factors which determine the appearance of recurrence depend on the tumour (development of the tumour and degree of anaplasia), the surgical technique (removal of the metastatic pathway and techniques intended to prevent the implantation of migrating tumour cells) and the patient (age, immunological factors, etc.). Our survey for the detection of recurrence consists of history and clinic

Microsimulation model to identify suboptimal recurrence detection in patients with colorectal cancer following the current standard of care.

OBJECTIVES: Colorectal cancer (CRC) is one of the most common cancers worldwide. Stage II/III patients undergo curative-intent surgery yet still face the recurrence risk. Detecting recurrences early provides the best opportunity for optimal treatment. We aimed to develop a microsimulation model to evaluate CRC management-associated outcomes based on current guidelines, including the performance of guideline-recommended surveillance in detecting recurrences. DESIGN: Two separate individual-level state transition (microsimulation) models for colon and rectal cancer were built with a lifetime ho

Artificial intelligence detection of endoscopic moderate-to-severe ulcerative colitis: a novel tool to enhance clinical trial recruitment.

BACKGROUND AND AIMS: Artificial intelligence (AI) may detect endoscopically active ulcerative colitis (UC) and streamline clinical trial enrollment. Here, we investigate the performance of a commercially available AI model, autoinflammatory bowel disease (AutoIBD)-UC (Virgo Surgical Video Solutions, Inc, San Francisco, Calif, USA), to detect moderate-to-severe UC in colonoscopy videos collected as part of routine clinical practice. METHODS: AutoIBD-UC was applied to consecutive endoscopy videos between August 30, 2024, and October 17, 2024. Each video received an AutoIBD score (0-1); videos w

Non-invasive breath testing to detect colorectal cancer: protocol for a multicentre, case-control development and validation study (COBRA2 study).

BACKGROUND: Colorectal cancer (CRC) is the fourth most common cancer in the United Kingdom. The five-year survival rate from CRC is only 10% when discovered at a late stage, but can exceed 90% if diagnosed early. Symptoms related to CRC can be non-specific, and therefore the decision to refer for a colonoscopy can be challenging. Breath analysis potentially offers a simple and quick method to detect CRC specific volatile organic compounds (VOCs) in breath. This protocol describes the COBRA2 study which aims to develop and validate the clinical prediction model (CPM) in the detection of CRC bas
