Concurrent dengue and malaria in Cayenne Hospital, French Guiana.

Dengue-malaria co-infection reports are scarce. Of 1,723 consecutive febrile patients in Cayenne Hospital, 238 had dengue (174 early dengue fever cases) and 393 had malaria (371 acute malaria); 17 had both. Diagnosis of 1 of these 2 infections should not rule out testing for the other infection.

Initial risk stratification and presenting characteristics of patients with evolving myocardial infarctions.

OBJECTIVES: To describe the presenting characteristics and risk stratification of patients presenting to the emergency department with chest pain who have a normal initial troponin level followed by a raised troponin level within 12 h (evolving myocardial infarction (EMI)). METHODS: Data from the Internet Tracking Registry for Acute Coronary Syndromes (i*trACS), a registry of patients presenting with undifferentiated chest pain, were used. This analysis included patients without ST segment elevation with at least two troponin assay results < or = 12 h apart. Patients were stratified into thre

Predicting adverse outcomes in syncope.

Syncope is a common presentation to the Emergency Department (ED); however, appropriate management and indications for hospitalization remain an ongoing challenge. The objective of this study was to determine if a predefined decision rule could accurately identify patients with syncope likely to have an adverse outcome or critical intervention. A prospective, observational, cohort study was conducted of consecutive ED patients aged 18 years or older presenting with syncope. A clinical decision rule was developed a priori to identify patients at risk if they met any of the following 8 criteria:

Stroke among patients with dizziness, vertigo, and imbalance in the emergency department: a population-based study.

BACKGROUND AND PURPOSE: Dizziness, vertigo, and imbalance are common presenting symptoms in the emergency department. Stroke is a leading concern even when these symptoms occur in isolation. The objective of the present study was to determine the "real-world" proportion of stroke among patients presenting to the emergency department with these dizziness symptoms (DS). METHODS: From a population-based study, patients >44 years of age presenting with DS to the emergency department, or directly admitted to the hospital, were identified. Demographics, the frequency of new cerebrovascular events,

A comparison of dog bite injuries in younger and older children treated in a pediatric emergency department.

OBJECTIVE: Dog bites account for a significant number of traumatic injuries in the pediatric population that often require medical treatment. Although agent, host, and environmental characteristics of dog bites have been well documented, no attempt has been made to compare these characteristics by patient age group. The purpose of this study is to determine if differences exist in agent, host, and environmental characteristics among younger (< or =6 y) and older (>7 y) patients treated in a pediatric emergency department (ED) for dog bites. Findings from our study could be used to develop age-

[Boerhaave's syndrome: also in the emergency room].

Two men, aged 52 and 57 years, had vomited and then developed chest pain, dyspnoea and tachypnoea. After a myocardial infarction had been excluded in the cardiac emergency room, further examination revealed a rupture of the oesophagus. This was treated surgically with the ultimate creation of a tubular stomach. Both patients then recovered well. The Boerhaave's syndrome, a 'spontaneous' perforation of the oesophagus, is a rare and potentially lethal condition which should be diagnosed at an early stage. Pain in the chest, dyspnoea and vomiting are frequent symptoms. A cardiac cause is sometime

Pelvic organ prolapse.

The separate disease entities that are included in the general term of pelvic organ prolapse have been discussed in detail in this chapter. The focus of discussion centered on the pathophysiology and clinical presentation of these conditions. At this point, the emergency physician should be able to properly recognize, assess, initiate treatment, and obtain appropriate referral in cases of pelvic organ prolapse. Postoperative complications commonly seen after these entities are repaired have also been reviewed to help the emergency medicine physician recognize and mange common postoperative com

Acute monocular blindness associated with spontaneous common carotid artery dissection.

The evaluation of patients with loss of vision is common in the emergency department. Central retinal artery occlusion (CRAO) is a potentially reversible cause of acute monocular blindness. When evaluating a patient with CRAO, the potential underlying causes should be considered while simultaneously initiating treatment. We present a case of spontaneous common carotid artery dissection manifesting as CRAO and propose that spontaneous carotid artery dissection be included in the differential diagnosis of CRAO.

The management of hyperkalaemia in the emergency department.

Life threatening hyperkalaemia (> 7.0 mmol/l) is commonly associated with acute renal failure. Moderate hyperkalaemia (6.1-6.9 mmol/l) is also common and well tolerated in patients with chronic renal failure. Renal failure is the most common cause of hyperkalaemia although other causes to consider include drugs (potassium sparing diuretics, angiotensin converting enzyme inhibitors), hyperglycaemia, rhabdomyolysis and adrenal insufficiency. Hyperkalaemia affects the cardiac conducting tissue and can cause serious arrhythmias including ventricular fibrillation and asystolic arrest. Therefore it

The work of the American emergency physician.

The organisation of the American emergency health care system has changed rapidly during recent years, but it remains very different to the system in the United Kingdom. American emergency departments are organised around an attending physician based service, rather than a consultant led service. As a result, the work of the American emergency physician differs considerably from that of the United Kingdom A&E consultant. The problems associated with working in an attending physician based service include antisocial hours of work, sleep deprivation, decreased job satisfaction, and "burn out," a

Midazolam use in the emergency department.

Midazolam is a new imidazobenzodiazepine derivative that is two to three times as potent as diazepam, is water-soluble, has a rapid onset and short duration of action, and produces a profound amnestic effect. These properties make it an extremely useful drug for outpatient sedation. We performed retrospective review of midazolam use in a general emergency department over a one-year period. Midazolam was used in 120 patients, 71 men and 49 women, with an average age of 46 years. The average dose given was 3.4 mg, with a range of 1 to 10 mg. In 69 of the cases (57%) other drugs were administered

Injury Epidemiology in Emergency Departments in Tbilisi, Republic of Georgia: A Multicenter Study.

INTRODUCTION: Injuries are a major cause of mortality, disability, and economic loss worldwide, disproportionately affecting low- and middle-income countries. In the Republic of Georgia, limited emergency department-level data constrain evidence-based injury prevention and trauma care planning. This study aimed to describe the epidemiological profile of injuries presenting to hospital emergency departments. METHODS: A retrospective observational study was conducted using data extracted from hospital electronic medical records and organized into a predefined injury registry dataset from select

Assessing Emergency Clinicians' Needs From Trauma Clinical Guidance: A Mixed-Methods Study.

BACKGROUND: In the United States, emergency clinicians are often the first to care for injured patients in the hospital setting. Current trauma clinical guidance does not consistently meet the needs of emergency clinicians caring for this population. OBJECTIVES: To better understand end-user needs, we evaluated emergency clinician priorities and preferences in accessing, interpreting, and applying trauma clinical guidance. METHODS: Emergency clinicians were recruited via email for semi-structured video conference interviews. Rapid directed qualitative analysis of interview notes and audio re

Social risks of geriatric patients presenting to the emergency department.

BACKGROUND: Emergency departments (ED) across the country are seeing an increasingly larger portion of visits from patients aged 65 and older due to the overall aging of the United States population. The ED is recognized as an important institution for screening and addressing social risks. Therefore, understanding the social risks of the geriatric ED population is of crucial public health importance. METHODS: This was a subgroup analysis of a prospective cohort study conducted between July 2022 and April 2023 in an ED at a single academic urban quaternary medical center in Los Angeles, Califo

When Caring Hurts: The Buffering Role of Social Support in Emergency Department Nurses' Coping With Secondary Traumatic Stress.

BACKGROUND: Emergency department nurses routinely operate in fast-paced and emotionally charged environments, where constant exposure to trauma places them at significant risk for developing secondary traumatic stress. Robust coping mechanisms and supportive social networks serve as critical buffers against these psychological effects. AIM: This study explored how coping strategies and social support interact to influence secondary traumatic stress among emergency department nurses in Taiwan, emphasizing the mediating function of social support. METHODS: A cross-sectional design was applied

Influence of primary language on follow-up after emergency department visits for acute renal colic: evidence from an ethnically diverse urban cohort.

PURPOSE: We aimed to determine whether preferred language influences follow-up for patients diagnosed with nephrolithiasis in the emergency department (ED). METHODS: We retrospectively reviewed patients referred from an ED to our clinic for symptomatic nephrolithiasis from 12/2021 to 6/2024. Patients were stratified by preferred language (English speakers [ES] vs. Non-English speakers [NES]) and compared across time to office visit, time to surgery, 24-hour urine collection, ED revisits, and failure to undergo surgery. Multivariate linear regression of log-transformed time to follow-up determ

Navigating Together for Equitable Asthma Management for children in families who communicate in languages other than English (Nav-TEAM): A type 2 effectiveness-implementation trial protocol.

Asthma is the most common chronic disease of childhood in the United States and disproportionately affects children in families who communicate in languages other than English (LOE), who experience worse asthma care, lower use of recommended management practices, and higher hospitalization and emergency department (ED) utilization. Navigating Together for Equitable Asthma Management (Nav-TEAM) was developed to address these asthma disparities through a culturally and linguistically responsive asthma navigation program for LOE families. This article describes the protocol for a hybrid type 2 ef

Two Brief Steps, Better Foresight: Cognitive Screening and Adverse Outcomes in Older Adults Admitted From the Emergency Department.

BACKGROUND: Delirium predicts adverse outcomes in older emergency department (ED) patients, but many acutely ill patients without delirium have underlying cognitive impairment that goes unrecognized. Whether cognitive impairment screening improves risk prediction beyond delirium remains uncertain. We compared practical bedside screening strategies for delirium, cognitive impairment, or both for predicting 90-day functional decline and mortality in older adults admitted from the ED. METHODS: A prospective cohort comprising patients aged ≥ 65 years admitted from the ED of a large hospital in Sã

Neonates admitted to the neonatal intensive care unit following pediatric emergency department visits in Türkiye: a retrospective single-center study.

BACKGROUND: The neonatal period is characterized by increased vulnerability to disease and often non-specific clinical presentations, leading to frequent visits to pediatric emergency departments (EDs). This study aimed to evaluate the characteristics of neonates presenting to the ED, determine the main causes of neonatal intensive care unit (NICU) admissions, and identify factors associated with prolonged hospitalization. METHODS: This retrospective single-center study included neonates aged 0-28 days who presented to the pediatric ED and were subsequently admitted to the NICU over a one-yea
