Remote Injuries and Outcomes After Distal Radius Fracture Management.

BACKGROUND: Distal radius fracture (DRF) is a common fracture of the upper extremity. The role of concurrent injuries in patients treated for DRFs is poorly elucidated. The authors sought to determine whether remote injuries were associated with worse outcomes after management of DRFs. METHODS: A retrospective cohort study including all consecutively seen patients by a university hospital hand service between 2010 and 2015. Preoperative radiographs were analyzed, and patients were managed by surgeon preference and evaluated postoperatively using pain scores. Remote injury was defined as any o

A High-Energy Military Femoral Fracture With Multidrug-Resistant Infection: A Case Report.

BACKGROUND The Masquelet induced membrane technique is a 2-stage surgical procedure used to reconstruct large bone defects and uses a temporary cement spacer to generate a biologically active, vascularized membrane that promotes bone healing when filled with an autograft. This report describes the case of a 37-year-old military man with a high-energy femoral fracture, multiple debridement procedures, and a bone defect managed with the Masquelet induced membrane technique. CASE REPORT In the patient's home country, life-saving surgery was performed, followed by repeated excision of damaged tiss

Acute compartment syndrome in tibial fractures: a meta-analysis.

PURPOSE: Acute compartment syndrome (ACS) is a severe complication associated with tibial fractures, which can result in irreversible muscle and nerve damage if not promptly identified and treated. METHOD: This study systematically searched PubMed, EMBASE, the Cochrane Library, and Web of Science. Data on demographics, fracture attributes, injury mechanisms, and biomarkers were extracted. Meta-analyses were performed using both fixed- and random-effects models, depending on the degree of heterogeneity. RESULT: A total of 17 studies were included. Younger adult age and older age in pediatric

Effects of Occult Hypoperfusion on Local Circulation and Inflammation - An Analysis in a Standardized Polytrauma Model.

INTRODUCTION: Occult hypoperfusion (OH) is defined as persistent lactic acidosis despite normalization of vital parameters following trauma. The aim of this study was to analyze the association of occult hypoperfusion with local circulation and inflammation of injured soft tissue in a porcine polytrauma model. METHODS: This experimental study was performed with male landrace pigs who suffered a standardized polytrauma, including a femoral fracture, blunt chest trauma, liver laceration and a mean arterial pressure (MAP) controlled hemorrhagic shock. One hour after induction of trauma, the anim

Antibiotic Prophylaxis in Open Fractures: Evidence, Evolving Issues, and Recommendations.

Open fractures are often associated with high-energy trauma and have an increased risk of infection because of surrounding soft-tissue damage and the introduction of environmental contaminants that may communicate with the fracture site. The Gustilo-Anderson classification of open fractures has been used to guide prophylactic antibiotic therapy because different types of open fracture have been shown to have varying rates of surgical site infections with different combinations of pathogens. Prophylactic treatment with various classes of antibiotics, including penicillins and cephalosporins, am

Detecting severe injuries of the upper body in multiple trauma patients.

BACKGROUND: The clavicle limits the upper thoracic cage and connects the body and upper extremities. The clavicle is easy to examine and is visible on standard emergency room radiographs. We hypothesized that clavicular fracture in polytrauma patients would indicate the presence of further injuries of the upper extremities, head, neck, and thorax. METHODS: A population-based trauma registry was used. All patients were documented between 2002 and 2013. Inclusion criteria were age ≥16 y and injury severity score (ISS) ≥16. Patients were divided into two groups according to the presence or absen

Multiple myeloma: from diagnosis to treatment.

BACKGROUND: Multiple myeloma is characterised by the proliferation of malignant plasma cells within the bone marrow, which produce an abnormal monoclonal paraprotein and evidence of end organ damage. OBJECTIVE: Multiple myeloma can pose a diagnostic challenge and this article highlights issues in diagnosis and provides an overview of management. DISCUSSION: Multiple myeloma can present with a wide constellation of symptoms including hypercalcaemia, anaemia, renal impairment and/or bony pain. A combination of these symptoms, particularly if unexpected or unexplained, should prompt diagnostic

Head trauma in the cat: 1. assessment and management of craniofacial injury.

PRACTICAL RELEVANCE: Feline trauma is commonly seen in general practice and frequently involves damage to the head. CLINICAL CHALLENGES: While craniofacial injuries following trauma vary widely in severity, affected cats can often be severely compromised in terms of their neurological, respiratory and cardiovascular status, and their management can be challenging in both the short and long term. They need prompt stabilisation and careful monitoring in the initial period to maximise prospects of a successful outcome. Many cats with severe facial trauma will require surgery to stabilise skull f

Pelvic fracture with complete urethrovaginal complex avulsion.

BACKGROUND: The anatomic support of the urethra is controversial. We present a case of an intact urethra and vagina avulsing from the symphysis pubis after pelvic fracture. CASE: An 18-year-old female pedestrian was struck by a motor vehicle. Operative reevaluation revealed an intact vagina and urethra; however, they were avulsed from the symphysis pubis. The avulsed urethra and vagina were reapproximated to the periosteum and surrounding soft tissue circumferentially with no urogenital sequelae. CONCLUSION: Complete anterior vaginal wall avulsion may occur with sparing of the lower urinary

The black eyebrow sign in orbital blowout fracture.

Orbital blowout fractures are frequently associated with intraorbital emphysema. This may be seen on a frontal skull radiograph as a lucent area at the superior side of the orbit, simulating a black eyebrow. Although computed tomography remains the primary imaging modality for assessing patients with clinically obvious blowout fracture, conventional radiography is often the first imaging study performed in cases of orbital trauma. By consequensce, knowledge of the black eyebrow sign is very useful especially to more unexperienced radiologists working at the emergency department.

Compartment syndrome associated with distal radial fracture and ipsilateral elbow injury.

BACKGROUND: Forearm compartment syndrome is an uncommon sequela of distal radial fractures. This investigation tested the hypothesis that the risk of forearm compartment syndrome associated with an unstable, operatively treated fracture of the distal end of the radius is higher with a concomitant injury of the ipsilateral elbow. METHODS: All patients who sustained an unstable fracture of the distal end of the radius and/or injury to the elbow (a fracture of the proximal end of the radius and/or ulna, simple elbow dislocation, elbow fracture-dislocation, or distal humeral fracture) and were op

Lower extremity injury criteria for evaluating military vehicle occupant injury in underbelly blast events.

Anti-vehicular (AV) landmines and improvised explosive devices (IED) have accounted for more than half of the United States military hostile casualties and wounded in Operation Iraqi Freedom (OIF) (Department of Defense Personnel & Procurement Statistics, 2009). The lower extremity is the predominantly injured body region following an AV mine or IED blast accounting for 26 percent of all combat injuries in OIF (Owens et al., 2007). Detonations occurring under the vehicle transmit high amplitude and short duration axial loads onto the foot-ankle-tibia region of the occupant causing injuries to

Imaging patients with myeloma.

Multiple myeloma (MM) is a neoplastic proliferation of plasma cells within the bone marrow. The disease is characterized by a plasma cell infiltrate of the bone marrow, osteolytic bone lesions, and the presence of monoclonal protein in the serum or urine with extraosseous involvement by disease less common. Although the skeletal survey has long been the standard investigation in these patients, there have been significant recent advances in computed tomography (CT), magnetic resonance imaging (MRI), and functional imaging. We present a comprehensive review of the evidence for the use of each o

[Possible nerve injuries in ankle dislocations. Trimalleolar fracture including the proximal fibula].

We present a patient with an exceptional fracture type--a Maisonneuve fracture associated with a lateral ankle fracture--and the therapy of this injury. The Maisonneuve fracture consists of a proximal fibular fracture which is associated with failure of the deltoid ligament or medial malleolus and diastasis of the distal tibiofibular syndesmosis. Diverse nerves of the lower leg can be affected by severe trauma as well as by fracture-associated complications or an operation. Fracture fragments, an excessive distension caused by the trauma, or a fracture-associated hematoma can cause injury to t

Ocular injuries in patients with major trauma.

AIM: To study the epidemiology of ocular injuries in patients with major trauma in the UK, determining the incidence and causes of ocular injuries, and their association with facial fractures. METHODS: A retrospective analysis of the Trauma Audit Research Network database from 1989 to 2004, looking at data from 39,073 patients with major trauma. RESULTS: Of the 39,073 patients with major trauma, 905 (2.3%) patients had associated ocular injuries and 4082 (10.4%) patients had a facial fracture (zygoma, orbit or maxilla). The risk of an eye injury for a patient with a facial fracture is 6.7 ti

Post-traumatic lumbar nerve root avulsion.

Lumbar nerve root avulsion is a rarely seen clinical entity that may complicate major trauma. The majority of previously reported cases have associated pelvic or lumbar vertebral fractures. Two cases of traumatic pseudomeningoceles at the lumbar level with associated avulsions of the lumbar nerve roots are presented. Both patients were involved in high velocity motor vehicle accidents. Case 1 had associated pelvic fractures but no spinal fractures and, interestingly, case 2 had no fractures of the spine or pelvis. The value of MRI in making the diagnosis is demonstrated.

[Compartment syndrome of the thigh with sciatic nerve paralysis].

UNLABELLED: Acute compartment syndrome of the thigh has been infrequently reported in the literature. Closed femoral fractures and blunt soft tissue trauma are the main causes of this injury. The multiple injured patient in this case report developed a compartment syndrome of the thigh after intramedullary nailing of a comminuted fracture of the femur. Fasciotomy was performed two days after surgery because of extense swelling of the thigh in the ventilated and sedated patient. Sciatic and femoral nerve palsy was recognized after extubation of the patient nine days after the injury. During the
