Individualized Treatment Approaches for Obstructive Sleep Apnea in Older Adults: Beyond Continuous Positive Airway Pressure.

With an aging population and greater prevalence of metabolic disorders, obstructive sleep apnea (OSA) is increasingly observed in older patients. OSA in older adults often occurs in the setting of various comorbidities, such as cognitive, cardiac, and metabolic dysfunction, placing these patients at greater risk. Continuous positive airway pressure (CPAP) is the first-line treatment for OSA in most patients, but adherence with CPAP decreases with increasing age. Current literature on considerations and efficacy of treatment modalities for OSA in older patients is limited. This narrative review

The Impact of Intravesical Instillations on Quality of Life in Patients with Non-Muscle-Invasive Bladder Cancer: A Systematic Review.

BACKGROUND: Non-muscle invasive bladder cancer (NMIBC) is commonly treated with transurethral resection of bladder tumor (TURBT) followed by intravesical chemotherapy and/or Bacillus Calmette-Guérin (BCG) instillations. While these treatments aim to prevent recurrence and progression, the frequency of their administration and associated side effects can substantially impact patients' quality of life (QoL). OBJECTIVES: We aimed to systematically review the impact of intravesical BCG and chemotherapy instillations on QoL in patients with NMIBC, with a specific focus on how instillation frequenc

Real-life effectiveness of anti-malarial treatment regimens: what are we aiming for?

Three-day artemisinin-based combination therapy (ACT) is the current standard of care for the treatment of malaria. However, specific drug resistance associated with reduced efficacy of ACT has been observed, therefore necessitating the clinical development of new anti-malarial drugs and drug combinations. Previously, Single Encounter Radical Cure and Prophylaxis (SERCAP) has been proposed as ideal target-product-profile for any new anti-malarial drug regimen as this would improve treatment adherence besides ensuring complete cure and prevention of early reinfection. Arguably, this concept may

Re-examining Hepatitis B Postexposure Prophylaxis Following Pediatric Community-acquired Needle-stick Injury in an Era of a National Immunization Registry.

BACKGROUND: Long-term hepatitis B immunity has been demonstrated following the completion of the primary vaccination series in childhood. Some guidelines recommend a hepatitis B surface antibody (anti-HBs) directed approach following community-acquired needle-stick injury (CANSI) to inform hepatitis B postexposure prophylaxis (PEP) management. We assessed the utility of anti-HBs testing post-CANSI, as well as the costing of, and adherence to PEP at a pediatric hospital. METHODS: Children presenting to an Australian tertiary pediatric hospital post-CANSI (2014-2019) were identified retrospecti

Risk of refracture associated with compliance and persistence with bisphosphonate therapy in Taiwan.

SUMMARY: Bisphosphonates have been used for the treatment of postmenopausal osteoporosis since the early 1990s and studies show that compliant patients experience a lower fracture rate. This cohort study showed that the compliance of Taiwanese patients was poor and the refracture risk was related to compliance with bisphosphonate therapy. INTRODUCTION: Bisphosphonates are potent inhibitors of osteoclast activity, and reduce bone turnover by inhibiting bone resorption. According to Taiwanese reimbursement guidelines, patients with osteoporosis-related fractures are eligible for bisphosphonate
