The Effects of Daytime Variation on Short-Term Outcomes of Cardiac Surgical Patients: A PRISMA-Compliant Systemic Review and Meta-Analysis.

OBJECTIVE: Circadian rhythms influence cardiovascular physiology and surgical stress responses, raising the possibility that the timing of cardiac surgery may affect outcomes. Previous studies have reported conflicting results regarding whether morning (AM) versus afternoon (AF) surgery influences perioperative morbidity and mortality. The present study aimed to systematically evaluate whether the timing of cardiac surgery, specifically AM versus AF procedures, influences short-term outcomes. DESIGN: A meta-analysis of observational cohort studies. SETTINGS: Cardiovascular centers. PARTICIP

Fluid balance and mortality in adult ICU patients with sepsis or septic shock: a systematic review and meta-analysis of observational studies.

BACKGROUND: Fluid therapy is a cornerstone of sepsis management; however, excessive fluid accumulation might be associated with adverse outcomes. The association between fluid balance (FB) and mortality in critically ill patients with sepsis or septic shock remains uncertain. METHODS: We conducted a systematic review and meta-analysis of observational studies assessing the association between FB and mortality in adult ICU patients with sepsis or septic shock (PubMed, Scopus, CINAHL; through May 2024). Random-effects models estimated pooled odds ratios (ORs) for all-cause mortality; meta-regre

Prehospital Delta Shock Index Predicts Mortality and Need for Life Saving Interventions in Trauma Patients.

OBJECTIVES: The delta shock index (ΔSI), defined as the change in shock index (SI) over time, is associated with hospital morbidity and mortality, but prehospital studies about ΔSI are limited. We investigate the association of prehospital ΔSI with mortality and resource utilization, hypothesizing that increases in SI among field trauma patients are associated with increased mortality and blood product transfusion. METHODS: We performed a multicenter, retrospective, observational study from the Linking Investigators in Trauma and Emergency Services (LITES) network. We obtained data from Janua

Mixed Shock Complicating Cardiogenic Shock: Frequency, Predictors, and Clinical Outcomes.

BACKGROUND: Patients presenting with cardiogenic shock (CS) are at risk of developing mixed shock (MS), characterized by distributive-inflammatory phenotype. However, no objective definition exists for this clinical entity. METHODS: We assessed the frequency, predictors, and prognostic relevance of MS complicating CS, based on a newly proposed objective definition. MS complicating CS was defined as an objective shock state secondary to both an ongoing cardiogenic cause and a distributive-inflammatory phenotype arising at least 12 hours after the initial CS diagnosis, as substantiated by prede

Predictors of in-hospital death in children with myocardial injury related to COVID-19.

INTRODUCTION: Myocardial injury due to COVID-19 infection is associated with higher mortality rates, higher intensive care unit admissions, and greater levels of inflammatory and cardiac markers. However, given the smaller number of papers regarding pediatric patients, there is still scarce data about mortality predictors in this population. This study aims to identify independent predictors of in-hospital death among pediatric patients hospitalized with myocardial injury related to COVID-19. METHODOLOGY: This is an observational, prospective, and longitudinal study of pediatric patients who
