Clinical effect and safety analysis of long-round needle usage in treating cervical spondylotic radiotelegraphy and its effect on pain and functional recovery.

BACKGROUND: Long-round needle usage can treat muscular pain, but there is little research on cervical spondylotic radiculopathy (CSR). OBJECTIVE: To explore the efficacy and safety of long-round needle usage in treating CSR. METHODS: Ninety-eight patients with CSR were randomly divided into control and observation groups. They were treated with filiform needles and long-round needles, respectively. The therapeutic effect, safety, inflammatory factors and neck dysfunction index (NDI), McGill pain questionnaire (MPQ) and Generic Quality of Life Inventory-74 (GQOL-74) scores were compared betwe

Clinical benefits of Tocilizumab in COVID-19-related cytokine release syndrome in a patient with end-stage kidney disease on haemodialysis in Australia.

COVID-19 remains a global pandemic with more than 10 million cases and half a million deaths worldwide. The disease manifestations in patients with chronic kidney disease and especially those on haemodialysis are still being understood, with only a few overseas case series, and small observational trials thus far. It appears that the disease is more severe in this patient cohort. Part of the pathophysiology of severe COVID-19 is related to accompanying cytokine release syndrome (CRS). Tocilizumab, an interleukin-6 inhibitor, has been trialled for treatment of CRS in COVID-19, but not yet appro

Rib soft fixation produces better analgesic effects and is associated with cytokine changes within the spinal cord in a rat rib fracture model.

Traumatic rib fracture can cause severe pain and is usually associated with the depression of respiratory drive followed by severe respiratory complications. It is critical for patients with rib fracture to receive adequate analgesia. However, strong opioids and other analgesics often produces side effects and may even cause respiratory suppression. Meanwhile, rib fixation now has become a popular method for treating rib fracture patients. However, the actual molecular mechanism leading to its effectiveness as an analgesia has not been fully investigated, and the best analgesic method for its

Cutaneous and systemic plasmacytosis vs. cutaneous plasmacytic castleman disease: review and speculations about pathogenesis.

Cutaneous and systemic plasmacytosis (C/SP), human herpes virus-8 (HHV8), negative multicentric plasmacytic Castleman disease (MPCD), and idiopathic plasmacytic lymphadenopathy are polyclonal plasma cell proliferations of unknown etiology that predominantly affect Asian individuals. Herein, we present our experience with a Vietnamese man with typical C/SP limited to the skin but, after 10 years, may have developed perirenal involvement, and with a white man with human immunodeficiency virus and HHV8 negative MPCD with involvement of skin, lymph nodes, and kidneys at presentation, and who later

Gastric inflammatory markers and interleukins in patients with functional dyspepsia treated with astaxanthin.

The chronic active inflammation caused by Helicobacter pylori is dominated by neutrophils, macrophages, lymphocytes and plasma cells. Several interleukins are involved in the inflammatory process. The aim of this study was to investigate the effect of astaxanthin on gastric inflammation in patients with functional dyspepsia. Forty-four consecutive patients were included, and biopsies were examined for IL-4, IL-6, IL-8, IL-10, interferon-gamma, CD4, CD8, CD14, CD19, CD25 and CD30. Patients were randomized: 21 patients were treated with 40 mg of astaxanthin daily, and 23 patients were treated wi

3D bioprinted porous PLLA implants with dual gene modified BMSCs for osteoarthritis cartilage repair.

This study evaluated whether three-dimensional (3D) bioprinted spherical, porous poly-L-lactic acid (PLLA) drug- loaded implants, combined with bone marrow mesenchymal stem cells (BMSCs) overexpressing transforming growth factor-β1 (TGF-β1) and interleukin-10 (IL-10), can enhance the repair of osteoarthritis (OA)-related cartilage defects. Rat BMSCs were transduced with lentiviral vectors to achieve dual overexpression of TGF-β1 and IL-10, induced toward chondrogenic differentiation, and assessed for chondrogenic marker expression (COL2A1, SOX-9, and aggrecan). Spherical porous PLLA drug-loade

Oxymatrine attenuates melanoma progression and metastasis through SORBS2-mediated suppression of M2 macrophage polarization.

BACKGROUND: Melanoma is an aggressive malignancy characterized by a highly immunosuppressive tumor microenvironment (TME), in which tumor-associated macrophages (TAMs) play a key role. Although oxymatrine (OMT) has been shown to exert anticancer effects, its mechanism of action in melanoma, particularly its interaction with Sorbin and SH3 Domain-Containing Protein 2 (SORBS2), remains poorly understood. PURPOSE: To elucidate the molecular mechanisms by which oxymatrine inhibits melanoma growth and metastasis, suppresses M2-like macrophage polarization, and remodels the TME. METHODS: Cell viab

Spatiotemporal analysis reveals distinct inflammatory programs underlying chronic colitis.

Inflammatory bowel disease (IBD) is a complex disorder that is often resistant to immunomodulatory treatments. Here, to understand how immune, epithelial, and stromal compartments are rewired during disease initiation and progression, we leveraged T cell transfer and Il10-/- spontaneous colitis models, including anti-IL-12p40 intervention, and integrated time-course transcriptomic analyses at bulk, single-cell, and spatial resolution. These well-established models exhibited conserved features of chronic inflammation, including neutrophil infiltration, and impaired tissue regeneration. Comparis

Molecular and Cellular Mechanisms of Fibrosis of Abdominal Adhesions under the Influence of Oxidized Dextran in the Experiment.

Formation of abdominal adhesions is accompanied by the synthesis of collagen monomers by fibroblasts and deposition of type I and III collagens. This process is regulated by various factors, including TIMP-1, MMP-1, TGFβ, IL-10, and IFNγ. Oxidized dextran prevents fibrosis, adhesion formation, affecting the functional state of macrophages and fibroblasts. Collagen formation and fibroblasts in abdominal adhesions of Wistar rats were studied against the background of intraperitoneal administration of oxidized dextran. Electron and light microscopy and immunohistochemical method (collagen I, TIMP

In Skin Biopsies of Localized Cutaneous Leishmaniasis Patients from Ethiopia, Expression of Anti-Inflammatory Cytokines was Greater than that of Proinflammatory Cytokines.

The immunopathology of cutaneous leishmaniasis (CL) caused by Leishmania aethopica is not well understood. This study aimed to characterize the host's localized immune response in formalin-fixed paraffin-embedded (FFPE) skin biopsies taken from skin lesions of patients who had developed localized CL due to infection with L. aethiopica. A retrospective cross-sectional study was conducted on 40 FFPE skin biopsy samples collected from 2018 to 2022 in the ALERT hospital, Addis Ababa, Ethiopia. Immunohistochemical techniques were used to examine localized immune T helper 1 (Th1) and T helper 2 (Th2

Autoantigen-loaded Polymeric Microparticles associate with B cells and promote tolerogenic antigen presentation in a mouse model of experimental autoimmune encephalomyelitis.

Almost 1 million people in the United States suffer from multiple sclerosis (MS). Current therapies suppress protective immunity and are non-curative. Antigen-specific therapies can selectively suppress autoreactive cells, preserving protective immunity. B cell dysregulation, a major driver of MS, remains largely untargeted for antigen-specific tolerance. We hypothesize that targeting B cells with antigen-loaded microparticles will enhance therapeutic efficacy. Here, we demonstrate that antigen-loaded acetalated dextran microparticles (AMP) surface-associate with B cells and enhance IL-10 secr

Altered B cell metabolic pathways characterize type 1 diabetes progression.

Type 1 diabetes (T1D) results from immune-mediated destruction of pancreatic beta cells. B cells serve as critical antigen-presenting cells whose autoreactive specificities drive disease progression. Conversely, IL-10 producing regulatory B cells (Bregs) exert immunosuppressive functions and have been shown to protect against autoimmunity in mouse models of rheumatoid arthritis and multiple sclerosis, where microenvironmental cues promote their differentiation. In particular, signaling through hypoxia-inducible factor 1α (HIF-1α) induces glycolytic flux that supports Breg expansion. Defects in

Exploring the Prognostic Significance of IL10 Variants and Their Mechanistic Regulation in Diabetic Nephropathy.

IL10 is a very effective anti-inflammatory cytokine. IL10 imbalance is linked to type 2 diabetes mellitus (T2DM) and also to renal hypertrophy, glomerular membrane thickening, and onset of diabetic nephropathy (DN). We aimed to investigate the association of IL10 gene polymorphism (rs1800871T/C, rs1800896A/G) with DN and determine the influence of variants on its expression level and interaction with transcription factors. We genotyped 301 study subjects, comprising 75 DN, 126 T2DM patients, and 100 controls. All were analysed for biochemical assays and genotypic analysis by PCR-RFLP and confi

6-Bromo-Indirubin-3'-Oxime (6BIO) mitigates oxidative stress and immune dysregulation to promote melanocyte regeneration in vitiligo.

BACKGROUND: 6-bromo-indirubin-3-oxime (6BIO) is a small compound with known antioxidative and immunomodulatory properties, yet its efficacy in vitiligo remains unexplored. OBJECTIVES: To investigate therapeutic potential of 6BIO in modulating oxidative stress, immune response, and melanogenesis through in vitro and in vivo models. METHODOLOGY: RESULTS: 6BIO significantly decreased intracellular ROS levels, promoted melanin synthesis and nuclear localization of Nrf2 along with upregulation of antioxidant target genes in melanocytes. In vivo, 6BIO reduced epidermal thickness, decreased CD8+ T-

Identification and Characterization of an HtrA Sheddase Produced by Coxiella burnetii.

Having previously shown that soluble E-cadherin (sE-cad) is found in sera of Q fever patients and that infection of BeWo cells by C. burnetii leads to modulation of the E-cad/β-cat pathway, our purpose was to identify which sheddase(s) might catalyze the cleavage of E-cad. Here, we searched for a direct mechanism of cleavage initiated by the bacterium itself, assuming the possible synthesis of a sheddase encoded in the genome of C. burnetii or an indirect mechanism based on the activation of a human sheddase. Using a straightforward bioinformatics approach to scan the complete genomes of four

Siderophore Immunization Restricted Colonization of Adherent-Invasive Escherichia coli and Ameliorated Experimental Colitis.

Inflammatory bowel diseases (IBD) are characterized by chronic inflammation of the gastrointestinal tract and profound alterations to the gut microbiome. Adherent-invasive Escherichia coli (AIEC) is a mucosa-associated pathobiont that colonizes the gut of patients with Crohn's disease, a form of IBD. Because AIEC exacerbates gut inflammation, strategies to reduce the AIEC bloom during colitis are highly desirable. To thrive in the inflamed gut, Enterobacteriaceae acquire the essential metal nutrient iron by producing and releasing siderophores. Here, we implemented an immunization-based strate

Substance-P prevents the cholestatic liver injury by regulating inflammatory responses.

Substance-P (SP) is a neuropeptide that modulates immune responses and accelerates tissue repair in critical inflammatory disease. Liver fibrosis and cirrhosis are the ultimate outcomes of almost all chronic liver diseases caused by viral infection, steatohepatitis, autoimmune, and cholestatic injury. Despite the development of new drugs, liver transplantation is still the only fundamental treatment; thus, new therapeutic approaches to mitigate liver fibrosis and chronic inflammation are constantly being needed. The aim of this study was to examine the effect of SP on liver damage due to chole

Alterations in CD8+ Tregs, CD56+ Natural Killer Cells and IL-10 Are Associated With Invasiveness of Nonfunctioning Pituitary Adenomas (NFPAs).

Invasive nonfunctioning pituitary adenomas (NFPAs) grow rapidly and the mechanisms are unclear. Among many complex mechanisms, the role of immunity in the development of NFPAs has not been fully explored. Here, we analyzed the clinical features 146 NFPA patients who underwent trans-sphenoidal surgery or craniotomy and examined the effects of immune tolerance in invasiveness of NFPA patients using fluorescence-activated cell sorting and immunohistochemical methods. We found patients with invasive NFPAs had more visual deficits and defective fields, higher tumor size, and lower white blood cell

Immunological factors, but not clinical features, predict visceral leishmaniasis relapse in patients co-infected with HIV.

Visceral leishmaniasis (VL) has emerged as a clinically important opportunistic infection in HIV patients, as VL/HIV co-infected patients suffer from frequent VL relapse. Here, we follow cohorts of VL patients with or without HIV in Ethiopia. By the end of the study, 78.1% of VL/HIV-but none of the VL patients-experience VL relapse. Despite a clinically defined cure, VL/HIV patients maintain higher parasite loads, lower BMI, hepatosplenomegaly, and pancytopenia. We identify three immunological markers associated with VL relapse in VL/HIV patients: (1) failure to restore antigen-specific produc

Exosome-mediated delivery of inflammation-responsive Il-10 mRNA for controlled atherosclerosis treatment.

Rationale: Tailored inflammation control is badly needed for the treatment of kinds of inflammatory diseases, such as atherosclerosis. IL-10 is a potent anti-inflammatory cytokine, while systemic and repeated delivery could cause detrimental side-effects due to immune repression. In this study, we have developed a nano-system to deliver inflammation-responsive Il-10 mRNA preferentially into macrophages for tailored inflammation control. Methods:Il-10 was engineered to harbor a modified HCV-IRES (hepatitis C virus internal ribosome entry site), in which the two miR-122 recognition sites were re
