Posterior epidural migration of sequestrated lumbar disc fragments into the bilateral facet joints: case report.

BACKGROUND AND IMPORTANCE: Symptomatic lumbar disc herniation is common. Migration of a free disc fragment is usually found in rostral, caudal, or lateral directions. Posterior epidural migration is very rare. We report the first case with posterior epidural migration and sequestration into bilateral facet joints of a free disc fragment. CLINICAL PRESENTATION: A 78-year-old female presented with low back pain and right leg pain. Plain radiographs showed lumbar spondylolisthesis. Magnetic resonance imaging revealed a posterior epidural mass and intrafacet mass, which was hypointense on T1-weig

Upper thoracic spine arthroplasty via the anterior approach.

Significant progress has been made in lumbar and cervical disc replacement therapy. Several cervical disc prostheses have recently gained FDA approval. Although arthroplasty has not been previously described in the thoracic spine, selected patients with long-segment fusion to the level of C-7 have altered cervicothoracic and upper thoracic biomechanics and may benefit from motion-preservation therapy for T1-2 disc herniation. Currently, FDA-approved prostheses are indicated only for patients with single-level degenerative disc disease between C-3 and C-7 and no history of cervical arthrodesis.

Finite element investigation of the loading rate effect on the spinal load-sharing changes under impact conditions.

Sudden deceleration and frontal/rear impact configurations involve rapid movements that can cause spinal injuries. This study aimed to investigate the rotation rate effect on the L2-L3 motion segment load-sharing and to identify which spinal structure is at risk of failure and at what rotation velocity the failure may initiate? Five degrees of sagittal rotations at different rates were applied in a detailed finite-element model to analyze the responses of the soft tissues and the bony structures until possible fractures. The structural response was markedly different under the highest velocity

The myth of lumbar instability: the importance of abnormal loading as a cause of low back pain.

Spinal fusion became what has been termed the "gold standard" for the treatment of mechanical low back pain, yet there was no scientific basis for this. Operations of fusion for low back pain were initially done at the beginning of the last century for back pain thought to be related to congenital abnormalities or for past spinal infection. The recognition of the disc as a cause of sciatica, commonly associated with back pain, and the recognition that a degenerate disc led to abnormal movement suggested the concept that this abnormal movement was the cause of pain, and this abnormal movement c

Cauda equina syndrome: a comprehensive review.

Cauda equina syndrome (CES) is a rare syndrome that has been described as a complex of symptoms and signs--low back pain, unilateral or bilateral sciatica, motor weakness of lower extremities, sensory disturbance in saddle area, and loss of visceral function--resulting from compression of the cauda equina. CES occurs in approximately 2% of cases of herniated lumbar discs and is one of the few spinal surgical emergencies. In this article, we review information that is critical in understanding, diagnosing, and treating CES.

Disc displacement patterns in lumbar anterior spondylolisthesis: contribution to foraminal stenosis.

PURPOSE: To describe the particular disc displacement pattern seen at MRI in patients with spondylolisthesis, and its potential contribution to foraminal stenosis. METHODS: 38 patients with symptomatic lumbar anterior spondylolisthesis and 38 sex and aged matched control patients with herniated disc disease, at corresponding disc space levels, were included for study. In each case note was made of the presence, absence and direction of disc displacement and also the presence and location of neural contact with the displaced disc. RESULTS: In 33 of 38 (86.8%) patients in the spondylolisthesis

Postoperative spinal adhesive arachnoiditis presenting with hydrocephalus and cauda equina syndrome.

To our knowledge, the association between hydrocephalus and postoperative spinal adhesive arachnoiditis (SAA) has never been reported. Herein we describe an unusual case of a 45-year-old man with spinal adhesive arachnoiditis (SAA) who developed delayed-onset hypertensive hydrocephalus and cauda equina syndrome (CES) after multiple low-back surgeries. The patient's clinical presentation, imaging findings, surgical management, and the possible mechanisms are discussed in the light of the present literature.

Anterior approaches to fusion of the cervical spine: a metaanalysis of fusion rates.

OBJECT: Anterior cervical discectomy (ACD), ACD with interbody fusion (ACDF), ACDF with placement of an anterior plate system (ACDFP), corpectomy, and corpectomy with plate placement are used to fuse the cervical spine. The authors conducted a metaanalysis of studies published after 1990 in which fusion rates achieved with each procedure were reported for patients with degenerative disease at one, two, and three disc levels. METHODS: Twenty-one papers each included data on at least 25 patients. In each of the 21 studies the average clinical follow up was more than 12 months, and the results w

Influence of the type of load on the cervical spine: a study on Congolese bearers.

BACKGROUND CONTEXT: Cervical spine suffers degenerative changes with age and activity. PURPOSE: The aim is to study the cervical spine of African head bearers by analyzing clinical changes and radiographic degenerative signs in comparison with a control group. STUDY DESIGN: Two groups of bearers, 28 bearers of heavy loads (50 to 60 kg) over short stretches and 33 bearers who carried bundles or lighter loads (30 to 35 kg) over long distances were clinically and radiographically examined and compared with a control group of 36 building industry workers who did not bear weights on their heads.

[Effect of vertebral body stiffness before and after vertebroplasty on intradiscal pressure].

Fractures of osteoporotic vertebral bodies are increasingly stabilized with bone cement. The effects of vertebral-body stiffness before and after augmentation with bone cement and of wedge-shaped vertebral body fractures on intradiscal pressure are insufficiently known. In a finite element model of the lumbar spine the elastic modulus of cancellous bone as well as the amount and the elastic modulus of bone cement were varied and the dependency of intradiscal pressure on these parameters was calculated. In addition, a wedge-shaped vertebral-body fracture was simulated. The bulge of the vertebra

[Anesthetic management of a patient with paroxysmal nocturnal hemoglobinuria and suspected abdominal venous thrombosis].

A 59-year-old man with paroxysmal nocturnal hemoglobinuria (PNH) underwent an operation for lumbar disk herniation. He was given warfarin, aspirin, and steroid for PNH and suspected venous thrombosis. PNH is a hemolytic anemia due to an acquired red blood cell membrane defect at the stem cell level. The patient with PNH has a risk of massive hemolysis induced by stresses such as acidosis, fever, operation, trauma and overfatigue. Other main symptoms of PNH are venous thrombosis and deficient hematopoiesis. We prevented massive hemolysis by increasing the dose of steroid and avoiding acidosis,

Aplastic articular facets in a dog with intervertebral disk rupture of the 12th to 13th thoracic vertebral space.

A 6-year-old, female spayed Pomeranian was presented with acute hind-limb paraplegia with the presence of deep pain perception and urinary incontinence. Myelography showed a Hansen type I herniation of the12th to 13th thoracic intervertebral space (T(12-13)). Articular facets of the T(12-13) and T(13) to first lumbar vertebra (L(1)) were absent. The spinal cord was decompressed using a bilateral T(12-13) modified lateral hemilaminectomy (pediculectomy). The aplastic sites were associated with minimal instability of the vertebral column, and stabilization of the vertebral column was not require

Herniated nucleus pulposus as a result of emesis in a 20-yr-old man.

The causes of herniated disc in vivo vary, and recent studies stress the role of intradiscal pressure as an important factor. Identifying causes of increased intradiscal pressure is difficult. This case report describes the diagnosis of a herniated nucleus pulposus in a 20-yr-old healthy man, with sudden onset of emesis. The diagnosis was made by a complete history, physical examination, electrodiagnostic studies, and magnetic resonance imaging. This etiology of herniated nucleus pulposus has not been described in the medical literature, and the severity of the symptoms, clinical examination,

[MRT diagnosis for degenerative changes in the spine].

MRT is very well suited to the diagnosis of degenerative alterations in the spine. The option of imaging in multiple planes, the excellent soft-tissue contrast offering tissue differentiation, the absence of hardening artefacts and the avoidance of exposure to radiation have led to a shift in favour of MRT for diagnosis. In the present paper the MRT characteristics of the most important degenerative alterations that affect the spine are discussed.

Reducibility of cervical disk herniation: evaluation at MR imaging during cervical traction with a nonmagnetic traction device.

The authors evaluated the reducibility of cervical disk herniation at magnetic resonance (MR) imaging performed with the patient in cervical traction. After the acquisition of neutral-state images, cervical traction images were obtained in 29 patients and seven healthy volunteers while they wore a portable intermittent traction device. During traction, all volunteers and 21 patients had a substantial increase in the length of the cervical vertebral column. The disk herniation was completely resolved in three patients and partially reduced in 18. The reducibility of cervical disk herniation can
