Clinical delineation of 18q11-q12 microdeletion: Intellectual disability, speech and behavioral disorders, and conotruncal heart defects.

BACKGROUND: Since the establishment of chromosomal microarrays in clinical practice, many new microdeletion/microduplication syndromes have been identified, including 18q11.2 microdeletion. Chromosome 18q deletion syndrome is commonly classified into distal deletion and a much rarer proximal interstitial deletion spanning the 18q11.2-q21.1 region. METHODS: We report two new patients and review 27 additional cases in DECIPHER/ClinGen databases and four cases from the literature, with more proximal 18q deletions involving 18q11-q12 (band 1 only; 17.2-43.5 Mb position) deletion. RESULTS: Common

De novo microdeletion of BCL11A is associated with severe speech sound disorder.

In 10 cases of 2p15p16.1 microdeletions reported worldwide to date, shared phenotypes included growth retardation, craniofacial and skeletal dysmorphic traits, internal organ defects, intellectual disability, nonverbal or low verbal status, abnormal muscle tone, and gross motor delays. The size of the deletions ranged from 0.3 to 5.7 Mb, where the smallest deletion involved the BCL11A, PAPOLG, and REL genes. Here we report on an 11-year-old male with a heterozygous de novo 0.2 Mb deletion containing a single gene, BCL11A, and a phenotype characterized by childhood apraxia of speech and dysarth

Elevation of guanidinoacetate in newborn dried blood spots and impact of early treatment in GAMT deficiency.

Guanidinoacetate methyltransferase (GAMT) deficiency is a good candidate disorder for newborn screening because early treatment appears to improve outcomes. We report elevation of guanidinoacetate in archived newborn dried blood spots for 3 cases (2 families) of GAMT deficiency compared with an unaffected carrier and controls. We also report a new case of a patient treated from birth with normal developmental outcome at the age of 42 months.

5-HT2 receptor distribution shown by [18F] setoperone PET in high-functioning autistic adults.

The serotonergic system is implicated in disordered emotional behavior. Autism is characterized by impaired processing of emotional information. The serotonergic (5-HT) system is also critically involved in brain development, and abnormal brain synthesis of serotonin is observed in autism. Furthermore, whole blood and platelet serotonin have been reported to be elevated in autism. The authors examined the CNS serotonin system in autism in vivo. 5-HT2 receptors were visualized by PET imaging of [18F]setoperone-binding in this pilot study of 6 high-functioning autistic adults and 10 matched-cont

Lexical knowledge and lexical use in autism.

One aspect of autistic language that has been infrequently researched is vocabulary and the conceptual knowledge underpinning individual words or word types. In this descriptive study we investigate anomalous vocabulary use in a 70,000-word corpus of conversational autistic language and examine evidence that concept formation, and hence vocabulary, is abnormal in autism. Particular attention is paid to the expression of artifact and temporal concepts which some believe may develop abnormally in autism. Little evidence is found of anomalous use of artifact terms, though errors with temporal (an

Transcranial micropolarization in the combined therapy of speech and general psychomotor retardation in children of late preschool age.

The efficacy of a method of combined treatment, developed by ourselves, using transcranial micropolarization in patients with delayed development of speech and general mental development, was studied in 30 children aged 4-6 years. High therapeutic effects were seen with this method in 60-80% of children, with accelerated development of communicative behavior and formation of speech-motor functions, along with increases in the stability of attention and improvements in the processes of perception and the productivity of cognitive activity.

A report of behavioral data on three groups of patients with craniofacial disorders.

Longitudinal follow-up data on development and school placement are presented for three groups of patients. Having had or not having had early surgical intervention and stimulation distinguishes the two groups of patients with craniosynostosis. The third group is patients with cleft lip and palate who received early intervention but did not achieve minimal criteria for communication by age 22 months. Educational placement (for patients 8 to 20 years of age) follows the pattern of distribution of students nationally for patients who had early intervention.

Development of the pediatric test of brain injury.

OBJECTIVE: The Pediatric Test of Traumatic Brain Injury (PTBI) (currently in its research edition) is a tool for assessing the cognitive-linguistic skills of school-aged children and adolescents in acute care and rehabilitation settings after traumatic brain injury. Development of the PTBI was motivated by the fact that, to date, no standardized test has been available to assess the full range of cognitive-linguistic impairments associated with pediatric brain injury. In this article we describe how the research edition of the PTBI was developed, provide rationale for the areas of assessment,

Paroxysmal tonic upgaze: physiopathological considerations in three additional cases.

Paroxysmal tonic upgaze of childhood has been described as a benign distinctive syndrome of abnormal ocular movement, with or without concomitant ataxia. After the first observation of four children, a further 29 patients have been reported with a wide spectrum of neurologic abnormalities such as ataxia, unsteady of gait, learning disabilities and mental retardation at follow-up. Electroencephalograms were normal in all the subjects and magnetic resonance imaging showed deficient myelination in only one patient. Recently it has been suggested that paroxysmal tonic upgaze could be a heterogeneo

Sex ratios in early childhood autism and related conditions.

Children aged 0-14 on a specified census day, with impairments of verbal and nonverbal communication, social interaction and imaginative play, and with repetitive, stereotyped activities, were identified in an epidemiological study carried out in an area of southeast London. Sociable moderately, severely, and profoundly retarded children were included for comparison. An overall male:female ratio of 2.6:1 was found in those with language and social impairments, but, in the children of this group who were moderately, severely, or profoundly retarded, the ratio was 2.1:1, closely similar to that

The genetics of autism.

Autism is a complex, behaviorally defined, static disorder of the immature brain that is of great concern to the practicing pediatrician because of an astonishing 556% reported increase in pediatric prevalence between 1991 and 1997, to a prevalence higher than that of spina bifida, cancer, or Down syndrome. This jump is probably attributable to heightened awareness and changing diagnostic criteria rather than to new environmental influences. Autism is not a disease but a syndrome with multiple nongenetic and genetic causes. By autism (the autistic spectrum disorders [ASDs]), we mean the wide s

Benchmark evaluation of video large language models in quality assessment of science popularization videos for dry eye.

The rapid growth of short-video platforms has reshaped how individuals access health information, but it has also fueled the spread of misinformation and disinformation. Dry eye, a prevalent ocular surface disorder, provides a representative case for examining these challenges. Reliable and scalable methods are urgently needed to identify and mitigate misinformation risks in online health content. We proposed a framework employing Video Large Language Models (VideoLLMs) for automated evaluation of science popularization videos. Three representative VideoLLMs (VideoLLaMA3, QwenVL, and InternVL)

A vision-language model for enhanced MCI identification in Alzheimer's disease through neuropsychological and neuroimaging data integration.

Alzheimer's Disease (AD) is a prevalent neurodegenerative disorder with the progression typically spanning from mild cognitive impairment (MCI) to severe dementia. However, in clinical practice, diagnosing MCI is challenging due to conflicts between neuroimaging findings and clinical neuropsychological assessments. Structural and metabolic changes in imaging may not immediately correlate with cognitive symptoms, making early MCI detection difficult. This diagnostic complexity requires considerable clinical expertise and can lead to delays in detection. Moreover, multi-modal fusion using comput

Can AI replace clinician-rated depression scales? The psychometric properties of HAMLET - Hamilton Large-Language-Model Evaluation Tool.

BACKGROUND: The Hamilton Depression Rating Scale (HAMD) is the gold standard for assessing depression but requires clinician administration, limiting its accessibility. Large language models (LLMs) offer potential for automated, valid assessments. We developed HAMLET (Hamilton Large-language-model Evaluation Tool), an interactive LLM-based tool designed to replicate the HAMD-17, and compared its results with those obtained by a psychiatrist. METHODS: HAMLET utilizes Qwen-Max in the temperature of 0.7 and a top-p value of 0.6, guided by structured prompts engineered via clinician-supervised tu

Safety and feasibility of allogeneic sibling cord blood infusion in Japanese children with cerebral palsy: A single-center pilot study.

INTRODUCTION: Cerebral palsy (CP) is the most common motor disorder in childhood that causes lifelong disabilities. Studies suggest that administration of autologous cord blood (CB) or sibling cord blood (SCB) may improve gross motor function and brain connectivity in CP. In this pilot study, we evaluated the safety and efficacy of allogeneic SCB in Japanese children with CP. METHODS: A single-arm pilot study of a single intravenous dose of HLA-matched or partially matched (at least 4/6 HLA molecular matches) SCB was conducted in five Japanese patients with CP (5.0-6.3 years old). The primary

In-silico tool for predicting and scanning rheumatoid arthritis-inducing peptides in an antigen.

INTRODUCTION: Rheumatoid arthritis (RA) is an autoimmune disorder in which the immune system mounts an abnormal response to self-antigens, resulting in chronic inflammation and joint damage. Identifying antigenic regions in proteins that trigger RA is essential for the development of protein-based therapeutics. METHODS: We developed predictive models for HLA class II binding RA-inducing peptides using a dataset of 291 experimentally validated RA-inducing peptides and 165 RA non-inducing peptides. Positional and compositional analyses were performed to identify residue preferences. Alignment-b
