The risk of an internal mammary lymph node (IMN) metastasis and its prognostic value for patients with invasive breast cancer were assessed by evaluating 142 patients who had either a mastectomy with lymph node dissection or a biopsy of the IMN. By univariate analysis, overall survival significantly correlated with the patient's age, clinical axillary node status, tumor size, and DNA ploidy, as well as histologically confirmed axillary and IMN metastases. BACKGROUND: Cell synthesis of heat shock (stress-response) proteins is increased by a variety of environmental and pathophysiological stressful conditions. The 70-kd heat shock protein (hsp70) is thought to be involved in protein-protein interactions including those of the protein products of the human c-myc oncogene and the p53 (also known as TP53) tumor suppressor gene.

Cervical localizations of extra-adrenal paragangliomas are infrequently malignant, especially in their familial forms. An immunohistochemical study was performed on 6 cervical paragangliomas which were detected in 3 sisters at the age of 20 20. One had a lymph node metastasis. This study confirmed the diagnosis of paraganglioma with endocrine chief cells and supratentacular cells. It also enabled the hormonal contents of these cervical paraganglioma to be determined. In addition to catecholamine, these tumors may (like pheochromocytomas) produce serotonin and one or more other peptides.

Legionnaires' disease in a 37-year-old male who had had silicosis was reported. He was admitted because of dyspnea. The chest X-ray film and CT scan showed infiltrative shadow and swelling of mediastinal lymph nodes. Open lung biopsy was done and Legionella pneumohila was detected. REP and EM were started and infiltrative shadow of X-ray was disappeared. Pleuro-pneumonectomy and thoracoplasty were performed because of hemoptysis and postoperative empyema. The results of a largely prospective but non-randomized study of 2,347 patients with clinical Stage I malignant melanoma have demonstrated that there are certain subgroups of patients who benefit substantially from ELND. An operative method of extended radical mastectomy involving intrapleural en bloc resection of the internal mammary lymph nodes by sternal split was proposed. Cytology of peripheral blood and lymph node lymphocytes from a group of unselected patients with cutaneous T cell lymphoma (CTCL) was studied by light microscopy. Twenty of 45 patients had circulating lymphocytes with convoluted nuclei recognized in routine Wright-Giemsa-stained peripheral blood smears. Histopathological examination of biopsies of mesenteric lymph node obtained from the region of the terminal ileum proved to be a very accurate method of diagnosing Johne's disease in the living animal. Identical conclusions were reached in 100 clinical suspects following examination of either mesenteric lymph nodes alone, or a wider range of routine specimens including ileum, caecum, colon and ileocaecal lymph nodes. A method of obtaining biopsy specimens from mesenteric lymph nodes is described and is recommended when an early definitive diagnosis is required.

A 38-year-old man developed symptoms of arthalgias and arthritis, lymphadenopathy, and weight loss. Eleven patients suspected of having tumor involving para-aortic lymph nodes were studied with the Varian 6 second whole body CT scanner. All patients had a pedal lymphangiogram; six were abnormal. An ultrastructural study of sinus histiocytosis (SH) in axillary lymph nodes was undertaken in 20 patients with primary cancer of the breast. The lymph node responses were first classified by means of light microscopy, according to previously established criteria. A carcinostatic substance (5-fluorouracil) was instilled into the mediastinal lymph nodes, the pleural cavity or the internal thoracic artery by continuous perfusion before operation on patients with bronchial carcinoma. Degenerative changes could be demonstrated histologically in the cancer cells in the lymph node metastases. This method was also effective for the controlateral lymph node metastases. As a treatment additional to operation, it may improve the chances of survival.
