Steady state VER measurement of visual system latency in normal and multiple sclerosis patients.

VERs were recorded from 30 normal and two multiple sclerosis (MS) subjects using a steady state checkerboard stimulus. The normal subjects produced a mean OU latency of 90.8 msec with a mean absolute interocular latency difference of 3.2 msec. Latencies from the MS patients were up to 13 standard deviations longer than the latencies from the normal subjects.

Autoimmune T cell repertoire in optic neuritis and multiple sclerosis: T cells recognising multiple myelin proteins are accumulated in cerebrospinal fluid.

Monosymptomatic unilateral optic neuritis is a common first manifestation of multiple sclerosis. Abnormal T cell responses to myelin components including myelin basic protein (MBP), proteolipid protein (PLP), and myelin-associated glycoprotein (MAG) have been implicated in the pathogenesis of multiple sclerosis. Antigen-reactive T helper type 1 (Th1)-like cells that responded by interferon gamma (IFN-gamma) secretion on antigen stimulation in vitro were counted. Untreated patients with optic neuritis and multiple sclerosis had similarly raised levels of T cells recognising MBP, PLP, and MAG in

Transposition of the great arteries, pulmonary atresia, and multiple ventricular septal defects associated with multiple cardiac rhabdomyomas in a case of tuberous sclerosis.

Multiple cardiac rhabdomyomas are frequently associated with tuberous sclerosis. However, very few cardiac malformations have been described with tuberous sclerosis. We report an unusual case of tuberous sclerosis with cerebral and renal lesions associated with dextrotransposition of the great arteries, pulmonary atresia, patent ductus arteriosus, multiple ventricular septal defects, and parachute mitral valve deformity. After reviewing other cases of congenital heart diseases associated with tuberous sclerosis, emphasis is made on the potential influence of multiple rhabdomyomas developing ve

Incident Comorbid Diabetes and Outcomes in Multiple Sclerosis: An English Population-Based Cohort Study.

OBJECTIVE: To compare the incidence of diabetes mellitus and its associations with hospitalisation and mortality in adults with multiple sclerosis (MS) and matched controls. METHODS: Using English primary care data from the Clinical Practice Research Datalink, linked to hospital and mortality records, we matched adults with MS with up to 10 controls without MS by age, sex and practice. We excluded individuals with pre-existing diabetes mellitus. Outcomes included all-cause hospitalisation (number and duration) and mortality. We used Poisson, negative binomial, linear and Cox proportional haza

Prevalence of Self-Reported Pelvic-Floor-Related Symptoms Among Female Patients with Multiple Sclerosis.

Introduction: Multiple sclerosis (MS) is more prevalent among females worldwide. The Expanded Disability Status Scale (EDSS) assesses MS progression, including disability related to pelvic floor dysfunction (PFD), such as bowel and bladder dysfunction. However, data on PFD symptoms in patients with MS remain limited. Objective: To evaluate the prevalence and impact of self- reported pelvic-floor-related symptoms among women with MS in Saudi Arabia and their effects on quality of life (QOL). Methods: A cross-sectional study was conducted among 112 female patients with MS (EDSS ≤ 7, age ≥ 18 yea

Mechanisms underlying exercise-induced pain modulation in people with multiple sclerosis - a scoping review.

BACKGROUND AND STUDY AIM: Multiple Sclerosis (MS) is a prevalent chronic neuroinflammatory disease, characterized by demyelination, inflammation and axonal degeneration. Up to 40% of people with MS (pwMS) experience central neuropathic pain (CNP). Despite numerous debilitating consequences, CNP is often underdiagnosed and undertreated, with limited pharmacological options available. However, exercise is a promising non-pharmacological intervention in the management of pain. This scoping review therefore aims to (a) outline the pathophysiological mechanisms of CNP in MS, (b) summarize the neuro

Impact of Oral Cladribine on TSPO-PET-Measurable Immune Cell Activation in Multiple Sclerosis.

INTRODUCTION: Oral cladribine reduces focal inflammatory activity in relapsing-remitting multiple sclerosis (RRMS). Preclinical studies suggest it may also modulate microglial activation. In this prospective, single-arm pilot study, we investigated the effect of oral cladribine on glial activation using 18 kDa translocator protein (TSPO) positron emission tomography (PET) in a real-world, longitudinal RRMS cohort. METHODS: People with RRMS initiating oral cladribine underwent TSPO-PET with [11C]PK11195, 3-T brain magnetic resonance imaging (MRI), serum neurofilament light chain (sNfL) and gli

Targeting the HPGDS-PGD2 axis to treat Th17 cell-mediated autoimmunity in multiple sclerosis.

Th17 cells are central drivers of autoimmune pathology, including multiple sclerosis (MS), yet the endogenous metabolic signals that shape their pathogenic differentiation remain incompletely defined. Prostaglandin D₂ (PGD2) is traditionally viewed as an extracellular mediator in allergic diseases. Here, we identify PGD2 as a cell-intrinsic antagonist of the nuclear receptor PPARγ, thereby promoting Th17 cell differentiation and exacerbating EAE pathology. PGD2 is selectively elevated in patients with MS compared with healthy individuals. Within the MS cohort, PGD2 correlates with Th17 cell fr

Allogeneic MHC-mismatched microglia-like cell replacement as a therapeutic approach for multiple sclerosis.

BACKGROUND: Dysfunctional microglia contribute to the pathology of numerous neurological diseases. Depletion of harmful microglia and repopulation with healthy progenitors represents a new therapeutic option for neurodegenerative diseases with an urgent need for effective treatments. However, repopulation with patient-derived progenitors could result in similar dysfunction over time. We therefore propose obtaining microglia-like cells (MLCs) derived from healthy donors for allogeneic transplantation. We hypothesize that the immunosuppressive phenotype of MLCs, combined with the brain´s high im

Brain states analysis of EEG predicts multiple sclerosis and mirrors disease duration and burden.

BACKGROUND: Any treatment of multiple sclerosis should preserve mental function, considering how cognitive deterioration interferes with quality of life. However, mental assessment is still realized with neuro-psychological tests without monitoring cognition on neurobiological grounds whereas the ongoing neural activity is readily observable and readable. OBJECTIVE: The proposed method deciphers electrical brain states which as multi-dimensional cognetoms quantitatively discriminate normal from pathological patterns in an EEG. METHOD: Baseline recordings from a prior EEG study of 88 subjects

Directions for advancing prognostic assessments in multiple sclerosis: Qualitative Insights from MS specialist Interviews.

BACKGROUND: Reliable prognostication in multiple sclerosis (MS) is essential for personalized care, yet remains challenging. In the absence of widely implemented prognostic tools, how MS specialists today formulate prognostic judgments and manage related challenges in routine practice is underexplored. OBJECTIVE: To investigate how MS specialists approach prognostication in practice and identify their priorities for improving prognosis-informed care. METHODS: Twelve MS specialists from European and Middle Eastern regions participated in semi-structured interviews. Data were qualitatively ana

Technology-Assisted Solutions to Enhance Auditory-Motor Synchronization During Walking in Progressive Multiple Sclerosis.

BackgroundStudies show that beat-based cues from music and metronomes benefit gait in persons with progressive multiple sclerosis (PwPMS) during short walking. While useful for controlled assessment, prolonged walking better reflects real-world function. Adaptive beat-alignment algorithms using biofeedback may enhance synchronization and benefit gait dynamics but their effects in PwPMS remain unknown.AimsTo investigate the effect of walking with and without an adaptive beat-alignment algorithm on synchronization consistency during 8 minutes of walking compared to walking in silence, and its ef

Impaired force control and muscle activation during isometric contractions in people with multiple sclerosis.

Motor impairments affect up to 80% of people living with multiple sclerosis (MS) and are often asymmetrical, resulting in self-reports of more- and less-affected legs. The purpose of our study was to examine asymmetries in the neural control of force applied by lower leg muscles in people with MS. Eighteen people with MS and 17 healthy controls performed two types of submaximal isometric contractions with dorsiflexors and plantar flexors. The outcomes included measures of force control and electromyographic (EMG) activity of lower-leg muscles. Force control was worse in people with MS compared

Large-scale proteomics across neurological disorders uncovers biomarker panel and targets in multiple sclerosis.

Cerebrospinal fluid (CSF) is central to neurological diagnostics, yet biomarkers are lacking for many clinical needs. To enable its large-scale proteomic characterization, we developed a high-throughput mass spectrometry workflow quantifying approximately 1,500 proteins per CSF sample across 5,000 individuals, covering a spectrum of neurological disorders. This revealed proteomic alterations associated with blood-CSF barrier impairment, age, and sex, enabling deconvolution of shared and disease-specific signatures. We then focused on multiple sclerosis (MS), using an improved analytical techno

Bile acid-microbiota interactions in multiple sclerosis: From experimental models to early clinical evidence.

This article examines the bidirectional relationship between bile acid metabolism and the intestinal microbiota and explores how disruptions in this interaction may contribute to the pathophysiology of multiple sclerosis. Bile acids are presented not merely as digestive end-products but as bioactive signaling molecules capable of regulating immune responses, maintaining epithelial and neural homeostasis, and influencing neuroinflammatory processes. Experimental work demonstrates that alterations in microbial composition affect bile acid diversity and circulation, while bile acids themselves sh

Intrathecal synthesis of Epstein-Barr virus antibodies 15 years before onset of multiple sclerosis.

A 48-year-old woman was diagnosed with multiple sclerosis (MS) in 2019. She participated with blood and cerebrospinal fluid samples as a spinal anaesthesia surgery patient for a case-control study on viruses and MS in 2003. The study had already demonstrated intrathecal antibody synthesis (ITS) to Epstein-Barr virus (EBV) but not against measles, rubella and varicella-zoster (MRZ) reaction viruses in this individual at the time of recruitment. A 2025 study review established that she was the only patient in her control subgroup who also developed MS. This case shows that a specific EBV-ITS may

Increased VH4+JH6+ antibody heavy chain use in plasmablasts from asymptomatic multiple sclerosis patients.

Radiologically isolated syndrome (RIS) is a neurological condition in people with demyelinating lesions on brain and/or spinal cord magnetic resonance imaging (MRI) studies, but without clinical symptoms of disease. Elucidating the immune profile of people with RIS (pwRIS) who will display MRI or clinical features of advancing disease is critical to our understanding of disease pathogenesis. Our lab previously identified features of B cell dysregulation in people with clinically isolated syndrome (pwCIS), who have both demyelinating lesions and a first clinical event of disease. Disease Activity Following Cessation of Dimethyl Fumarate in Patients with Relapsing-Remitting Multiple Sclerosis.

BACKGROUND: The optimal time between the cessation of one disease-modifying therapy (DMT) and initiation of another in relapsing-remitting multiple sclerosis (RRMS) is incompletely understood. Lymphopenia, an adverse effect of dimethyl fumarate (DMF), may prompt a washout period prior to initiating an alternate DMT to allow absolute lymphocyte count (ALC) recovery. We hypothesized that during the DMF-DMT treatment interval, there would be disease activity, either by MRI or clinical relapses, and that lymphopenia might be a risk factor. METHODS: A retrospective chart review was conducted, coll
