External ophthalmoplegia with orbital myositis in an adult patient after chickenpox infection.

Herpes zoster and chickenpox are caused by a single virus, varicella-zoster virus. Herpes zoster ophthalmicus-associated ophthalmoplegia is well documented. Very rarely, herpes zoster and chickenpox cause external ophthalmoplegia. A 48-year-old man was diagnosed with chickenpox and treated with intravenous acyclovir. He suddenly reported diplopia and restricted left eye movement. MRI of the orbit revealed thickening and abnormal contrast enhancement of the preseptal space and lateral rectus muscle of the left eye. In this case, external ophthalmoplegia occurred following chickenpox with radiol

Fungus ball in concha bullosa: an unusual cause of retro-orbital pain.

Fungus ball in the concha bullosa is an extremely rare disease. We described a case of the fungus ball in the concha bullosa in a 22-year-old woman. Preoperative diagnosis was based on nasal endoscopy and computed tomography scanning. The patient was endoscopically operated on. The examination of the removed material was reported as fungal infection. This case was found worth writing because of the location of the concha bullosa and its rare occurrence in this location.

Management of sinonasal complications after endoscopic orbital decompression for Graves' orbitopathy.

OBJECTIVES/HYPOTHESIS: Endoscopic orbital decompression (EnOD) has proven to be safe and effective for the treatment of Graves' orbitopathy; however, complications do occur. Although the literature focuses on orbital complications, sinonasal complications including postobstructive sinusitis, hemorrhage, and cerebrospinal fluid (CSF) leak can also be challenging to manage. This study examines the incidence and management of sinonasal complications in these patients. STUDY DESIGN: Retrospective review. METHODS: Clinical data, surgical findings, and postoperative outcomes were reviewed of patie

Management of the optic canal invasion and visual outcome in spheno-orbital meningiomas.

OBJECTIVE: Spheno-orbital meningiomas often present with visual deficit due to invasion of the optic canal by the tumor. This study discusses the reasons of visual impairment, the choice of the surgical approach according to the type of optic canal involvement, and the factors correlated to the visual outcome in patients harboring a spheno-orbital meningioma. MATERIALS AND METHODS: A surgical series of 60 spheno-orbital meningiomas is reviewed. The preoperative visual symptoms, the involvement of the optic canal in both neuroradiological studies and surgical descriptions, the different surgic

Light weight prosthesis for a patient with bilateral orbital exenteration--a clinical report.

PATIENT: This article describes a case report of a patient with history of bilateral orbital exenteration due to squamous cell carcinoma. Spectacle retained acrylic prosthesis was considered to be the best possible option in this case due to total lack of vision and related manual dexterity. DISCUSSION: Main challenge in fabrication of spectacle retained prosthesis is to maintain its adaptation with the tissues during attachment of the screws. So, method of attachment of prostheses with the frame has been discussed in detail. The other concern was the increased weight of the prosthesis and as

Primary orbital teratoma with tooth in an adult: a rare association with cataract and corectopia.

INTRODUCTION: Orbital teratoma is a known cause of proptosis in neonates but it is rare in adults. CASE REPORT: An adult female presented with primary orbital teratoma with a well formed tooth inside the multicystic mass. Anterior orbitotomy with cyst excision was done with retrieval of a canine tooth. COMMENT: This report documents a unique case of orbital teratoma presenting in an adult with well formed canine tooth inside. Microcornea, corectopia and cataract were other distinctive coexisting features of our case.

Isolated inferior rectus muscle rupture after blunt orbital trauma.

A 44-year-old man was referred to our department with diplopia, periorbital swelling and haematoma of the left eye after orbital trauma due to a punch. During the examination, mild enophthalmos, hypertropia and a total absence of infraduction were observed. An orbital computed tomography (CT) scan demonstrated a left orbital floor blow-out fracture, with caudal herniation of periorbital fat and rectus inferior muscle. Repair was performed under total anaesthesia with placement of a Titan mesh. The following days were marked by the persistence of diplopia without improvement of infraduction. A

Endoscopic transmaxillary repair of orbital floor fractures: a minimally invasive treatment.

Although endoscopic transmaxillary repair of orbital floor fractures is a minimally invasive treatment, controversy remains regarding the method for supporting the orbital floor after elevation of the orbital contents. To date, a urethral balloon catheter has been widely used. However, it can be difficult to leave the catheter in place for a long time period due to the inconvenience, and prolapse of the orbital contents may recur in the case of its premature removal. This study described the techniques for endoscopic reduction and use of a balloon for orbital floor fractures. From June 2006 th

Orbital compartment syndrome following transvenous embolization of carotid-cavernous fistula.

Superior ophthalmic vein (SOV) thrombosis is a rare complication of carotid-cavernous fistula (CCF) embolization and is usually associated with a paradoxical worsening of signs followed by subsequent spontaneous resolution. We report a case in a 69-year-old female who developed orbital compartment syndrome due to SOV thrombosis following transvenous embolization of an indirect CCF. The patient was treated with an urgent lateral canthotomy and cantholysis and had good recovery. This report demonstrates that the paradoxical worsening due to SOV thrombosis in CCF may result in orbital compartment

Sino-orbital spelunking: stalagmite formation in fibrous dysplasia with dystrophic calcification.

Fibrous dysplasia (FD) involving the mid-face may be associated with cystic and other secondary changes that make diagnosis more difficult. We present a case of FD associated with a remote history of blunt facial trauma and extensive cystic changes involving the medial orbit and sinuses. An endoscopic exploration revealed "stalagmites" along the floor of the cystic cavity that were consistent with dystrophic calcification. This case was unusual given the degree of dystrophic calcification and the presence of sino-orbital stalagmites within the cystic cavity. Radiologic and pathologic features

Orbital complications of acute sinusitis: changes in the post-pneumococcal vaccine era.

IMPORTANCE: The widespread use of the 7-valent pneumococcal conjugate vaccine (PVC7), developed to combat invasive Streptococcus pneumoniae infections, has the potential to influence the prevalence and antibiotic resistance patterns of pathogens associated with orbital complications from acute sinusitis. Given the significant morbidity that may result from inadequate treatment of orbital infections related to acute sinusitis, determining the impact of PCV7 on the bacteriology and drug resistance of the pathogens associated with these infections may provide critical information needed to accura

Orbital inflammation secondary to a delayed hypersensitivity reaction to sub-Tenon's hyaluronidase.

We report a rare case of a delayed orbital inflammation with raised intraocular pressure as a result of hyluronidase allergy following sub-Tenon's anaesthesia. Here, we have shown evidence to prove the orbital inflammation to be an allergic response to hyluronidase with a skin patch test. This is the first case to our knowledge of a delayed hypersensitivity reaction to sub-Tenon's hyluronidase comprising of an initial exposure to hyluronidase in the fellow eye with no subsequent allergic response, but with a subsequent delayed reaction to hyluronidase during a second eye cataract surgery. Comparison of computed tomographic and cytopathological findings in the evaluation of adult orbital mass.

An observational study was conducted to demonstrate the role of Computed Tomographic (CT) scan to detect clinically suspected adult orbital mass in 47 patients which could not be differentiated clinically. The CT findings were compared and correlated with the findings of fine needle aspiration cytology (FNAC) or histopathology. CT diagnosis of optic nerve sheath meningioma were 12 cases, among them only nine cases confirmed cytopathologically as meningioma and rest three as lymphoma. Among ten cases of hemangioma, eight cases were confirmed cytopathologically as cavernous hemangioma and rest t
