Treatment effects of the MARA appliance and Activator-Headgear combined with fixed appliances in Class II division 1 malocclusion patients: A retrospective longitudinal study.

INTRODUCTION: Class II division 1 malocclusion treatment with functional devices offers acceptable results. These devices can be removable or fixed, and the essential difference between them is the need for compliance. It is clinically important to investigate if there are differences in the treatment effects of these devices that present different characteristics. OBJECTIVE: This retrospective longitudinal study compared the treatment effects of Class II correction with the MARA appliance, Activator-Headgear (AcHg) combination, both followed by multibracket fixed appliances, and an untreated

Non-surgical adult class II high-angle treatment with an invisible appliance: A case report.

This case report describes the complex situation of a young adult Class II hyperdivergent patient treated by premolar extraction with a straight wire lingual appliance. Despite the patient's refusal to undergo surgical treatment, the dual goals of ideal occlusal relationship and profile improvement were achieved through a well thought-out biomechanical strategy with appropriate extraction choice and anchorage control during space closure. This case report demonstrates the possibility of successfully resolving severe sagittal and vertical discrepancies in an adult patient without surgical treat

Cephalometric evaluation of changes in vertical dimension and molar position in adult non-extraction treatment with clear aligners and traditional fixed appliances.

INTRODUCTION: Orthodontists have been using clear aligners to treat malocclusions, and one potential effect of treatment with orthodontic aligners is the intrusion and/or resists extrusion of the posterior teeth. This "bite-block effect" is primarily anecdotal due to the frequent occurrence of posterior open bites in patients after clear aligner therapy. OBJECTIVE: The purpose of this study was to compare changes promoted by clear aligners and traditional fixed appliances in cephalometric measurements of the vertical dimension and molar position in adult patients with Class I malocclusion tre

Smart distalization of the upper arch with an easy, efficient and no-compliance procedure.

In conventional Class II treatment, distalisation of the maxillary arch usually requires multiple phases of tooth movement during which anchorage loss can occur. In order to solve this issue, a rational and simple technique has been developed. Instead of using intraoral distalisers along with palatal mini-implants, Ni-Ti superelastic loops are used to obtain molar distalisation while buccal interradicular miniscrews (BIM), preferably placed between the roots of upper premolars, supply the necessary anchorage. Once the distalisation of molars and second premolars is performed, miniscrews are pl

Effectiveness of interceptive treatment of class III malocclusions with skeletal anchorage: A systematic review and meta-analysis.

Recently, new strategies for treating class III malocclusions have appeared. Skeletal anchorage appears to reduce the dentoalveolar effects while maximising the orthopaedic effect in growing patients. The purpose of this systematic review and meta-analysis is to examine the effectiveness of bone anchorage devices for interceptive treatment of skeletal class III malocclusions. Searches were made in the Pubmed, Embase, Scopus and Cochrane databases, as well as in a grey literature database, and were complemented by hand-searching. The criteria for eligibility were: patients who had undergone ort

Non-surgical treatment of transverse deficiency in adults using Microimplant-assisted Rapid Palatal Expansion (MARPE).

INTRODUCTION: Maxillary transverse deficiency is a highly prevalent malocclusion present in all age groups, from primary to permanent dentition. If not treated on time, it can aggravate and evolve to a more complex malocclusion, hindering facial growth and development. Aside from the occlusal consequences, the deficiency can bring about serious respiratory problems as well, due to the consequent nasal constriction usually associated. In growing patients, this condition can be easily handled with a conventional rapid palatal expansion. However, mature patients are frequently subjected to a more

Simultaneous alveolar cleft closure and dental midline correction with curvilinear intraoral distraction.

This article describes a new method that enables vector control during alveolar distraction osteogenesis in the treatment of a cleft palate patient. The patient presented with unilateral complete cleft lip and palate, and the alveolar part of the defect was covered by a mobile buccal flap. The distraction was performed by sliding the surgically released tooth segment with the help of an intraoral distractor over 1.5-mm stainless steel archwires held by metal crowns. This vector-controlled method enabled new bone and attached gingiva formation in harmony with the proper alveolar shape.

Influence of thermoplastic retainers on Streptococcus mutans and Lactobacillus adhesion.

INTRODUCTION: This study was designed to test the hypothesis that thermoplastic retainers influence oral microbial flora during the retention period because they prevent the flushing effect of saliva on dental and mucous tissues. METHODS: Twenty-four orthodontic patients finished the study. After debonding, the patients were given thermoplastic retainers (Essix ACE 0.040-in plastic, Dentsply International, York, Pa) for both jaws and instructed to wear them all day. Plaque samples from tooth surfaces and saliva samples were collected from each patient just after debonding (T0), and on day 15

[Everyday management in mixed dentition and Tweed's concept].

The topic of this paper is to emphasize the strategy of occlusion management during childhood. The orthodontist intervenes during the mixed dentition period, for transversal and antero-posterior cross-bites, for vertical problems such as open-bites and deep-bites, but he also deals with habits and functional diseases. Treatment has to be simple and short. Most of the appliances are fixed. The author presents some clinical cases, and then defines "short-cuts" as steps of therapy that shorten the active treatment. To conclude, a case report emphasizes the fact that before any diagnosis based on

Controlled, rapid uprighting of molars: a surprisingly simple solution the pivot arm appliance.

In orthodontic cases where the regional anatomy provides limited room for eruption, there is etiologically a higher occurrence of tipped/impacted second molars. Although second molar extraction with third molar replacement is a useful option, the "Pivot Arm Appliance" encourages the uprighting of the second molar as a preferred treatment. The most unique and important attribute of the "Pivot Arm Appliance" is the rotating tube. In cases of access limitation, the disto-occlusal surface of the molar presents as one area that is accessible. Other features of the "Pivot Arm Appliance" include: The

Use of removable appliances as an adjunct to fixed appliance therapy.

Although a fixed appliance may ultimately be required to correct a malocclusion, the author gives examples of how the use of a removable appliance initially could simplify the treatment. A removable appliance enables the benefits of growth and development that are occurring in young patients to be enlisted in the treatment, and thereby reduces the overall treatment time.
