Early initiation of low-dose aspirin before 12 weeks improves pregnancy outcomes in women with moderate-risk factors for preeclampsia.

BACKGROUND: Preeclampsia is thought to originate from abnormal placentation during early pregnancy. Low-dose aspirin (LDA) reduces the risk of preeclampsia, but most clinical guidelines recommend initiation at 12 weeks of gestation because evidence supporting earlier use remains limited. OBJECTIVE: To evaluate whether initiation of LDA before 12 weeks of gestation improves maternal and neonatal outcomes compared with initiation between 12 and 20 weeks among women with multiple moderate-risk factors for preeclampsia. STUDY DESIGN: This retrospective cohort study included singleton pregnancies

Protocol for an observational study to assess the impact of pharmacogenetics on outcomes in vascular surgery (PROSPER).

INTRODUCTION: Patients with chronic limb-threatening ischaemia (CLTI) are often prescribed clopidogrel in order to reduce their risk of major adverse limb and cardiovascular events. Clopidogrel is metabolised by the CYP2C19 enzyme and genetic variations in CYP2C19 are common. These variants can influence an individual's ability to metabolise clopidogrel to its active metabolite. Few studies have investigated the relationship between patient genotype and outcomes in vascular surgery. This work aims to establish the relationship between patient genotype and outcomes after revascularisation in pa

Investigation of Genomic and Transcriptomic Risk Factors of Clopidogrel Response in African Americans.

Clopidogrel, an anti-platelet drug, is used to prevent thrombosis after percutaneous coronary intervention. Clopidogrel resistance results in recurring ischemic events, with African Americans (AA) suffering disproportionately. The aim of this study was to discover novel biomarkers of clopidogrel resistance in African Americans using genome and transcriptome data. We conducted a genome-wide association study (GWAS), including local ancestry adjustment, in 141 AA on clopidogrel to identify genetic associations with high on-treatment platelet reactivity (HTPR), with validation of genome-wide sign

Direct oral anticoagulants compared to aspirin for embolic stroke of undetermined source: A comprehensive meta-analysis.

BACKGROUND: The use of anticoagulation for stroke prevention in embolic stroke of undetermined source (ESUS) is hypothesized to be beneficial over conventional antiplatelet use. However, randomized controlled trials (RCTs) have not found clear benefits, even when assessing cardioembolic enriching features. This study aimed to perform a meta-analysis exploring the efficacy and safety of direct oral anticoagulants (DOACs) following ESUS. METHODS: PubMed, Scopus, and Cochrane Central were systematically searched for studies comparing DOACs versus aspirin after ESUS. The primary outcome was strok

Prevalence of bleeding in patients on single or dual antiplatelet therapy combined with vitamin K antagonists or direct oral anticoagulants.

BackgroundPatients with atrial fibrillation often require anticoagulation therapy to prevent stroke and thromboembolism. However, anticoagulants can have serious side effects, such as bleeding, particularly when combined with antiplatelet therapy.ObjectiveThe aim of this study is to estimate the prevalence of major bleeding in patients receiving either dual or triple antithrombotic therapy.MethodThis study is a single-center retrospective chart review utilizing the hospital electronic health record. The prevalence and percentage of bleeding events were reported for each antithrombotic regimen.

Tibial vein thrombosis after foam sclerotherapy: A single-institution case series.

OBJECTIVE: Ultrasound-guided foam sclerotherapy is commonly used for superficial venous disease but carries a small risk of deep vein thrombosis (DVT). The management of tibial vein thrombosis after sclerotherapy remains controversial, with practices ranging from anticoagulation and antiplatelet therapy to observation alone. We sought to characterize the natural history of tibial DVTs identified after foam sclerotherapy and evaluate outcomes based on management strategy. METHODS: We retrospectively analyzed patients who underwent physician-compounded foam sclerotherapy (Asclera) at a multi-in

Strategies to minimize postoperative complications in microsurgical free tissue transfer: The role of anticoagulation, antiplatelets, and ambulation.

OBJECTIVE: Microsurgical free tissue transfer (FTT) is essential in reconstructive surgery, but thromboembolic events are a leading cause of flap failure. This study evaluates how perioperative anticoagulation, antiplatelet therapy, and ambulation affect postoperative complications in FTT. STUDY DESIGN: A retrospective cohort study of 497 free flap procedures assessed associations between antiplatelet and anticoagulation therapy, ambulation timing, and postoperative outcomes. The cohort included pediatric and adult patients who underwent a free tissue transfer from 2012 to 2021. Multivariable

Drug-coated balloons for coronary artery disease: An updated review with future perspectives.

Since the advent of coronary stents, two of the most common long-term complications after percutaneous coronary intervention (PCI) are in-stent restenosis (ISR) and stent thrombosis (ST). Although the rates of ST have been nearly abolished and ISR rates have declined with the current gold-standard second-generation drug-eluting stents (DES), late ISR of DES remains a valid concern in the field of interventional cardiology. The drug-coated balloon (DCB) is a non-stent technology that relies on the concept of targeted homogeneous drug delivery from an inflated balloon to restore luminal vascular

First-in-Human Evaluation of a Polymer-Free Everolimus-Eluting Stent Using a Titanium Dioxide Film.

BACKGROUND: In patients with coronary artery disease treated with permanent polymer-coated drug-eluting stents (DES), the persistent presence of a less biocompatible polymer might delay arterial healing. Thin strut polymer-free DES have the potential to improve clinical outcomes and reduce the duration of dual antiplatelet therapy (DAPT). The purpose of this first-in-human study was to assess the safety and effectiveness of a novel polymer-free DES in patients with de novo coronary lesions. The TIGERevolutioN® stent (CG Bio Co., Ltd., Seoul, Korea) consists of a cobalt chromium platform with a
