An overview of genetic influences in alcoholism.

This review summarizes recent findings from human research regarding genetic influences in alcohol abuse and dependence. Genes explain about 50% of the vulnerabilities leading to heavy drinking and associated problems. Most genetic influences appear to impact at least four prominent intermediate characteristics (phenotypes) that interact with environmental events to produce the alcoholism risk: a flushing response to alcohol; a low level of response to alcohol; personality characteristics that include impulsivity, sensation seeking, and neuronal and behavioral disinhibition; and through psychi

The health and health behaviors of young men who have sex with men.

PURPOSE: To describe the range of health and mental health problems for which young men who have sex with men (YMSM) may be at risk. METHODS: An audio-computer-assisted survey was administered to a large, ethnically diverse sample of 526 YMSM (aged 18-24 years) recruited from bars, clubs, and other social venues using a venue-based probability sampling method. RESULTS: Subjects reported a range of health and mental health problems, and involvement in health-compromising behaviors, such as overweight/obesity, depression, and suicidal thoughts/attempts, and many were found to have high rates o

Weekly problems scales: instruments for sexually abused youth and their nonoffending parents in treatment.

This study's purpose was to determine if efficient measures could be created to assess multiple problematic behaviors identified in youth who were sexually abused and in treatment. Because of the lack of easily administered brief instruments that assess multiple domains of interest in this population, complementary parent and child assessment measures were developed. The Weekly Problems Scale-Child Version (WPSC) and the Weekly Problems Scale-Parent Version (WPS-P) were created to monitor the weekly progress of the child and family in treatment and focus specifically on common areas of difficu

Predictors of use of services among dementia caregivers.

Caregivers of persons with dementia do not use community resources until late in the disease process, despite the fact that judicious use of community resources can delay nursing home admission. Data from the National Caregiver Training Project, based on Hall and Buckwalter's (1987) progressively lowered stress threshold (PLST) model, were used to examine variables related to use of community resources. Spouse and adult child caregivers were divided into two groups based on amount of community resources used per week. Within this geographically diverse sample of caregivers, 64% did not use pro

Do street youths' perceptions of their caregivers predict HIV-risk behavior?

This study examined street youths' perceptions of their caregivers and the association between these perceptions and HIV-risk behavior in a random probability sample of 715 12- to 23-year-old street youths from Los Angeles and San Diego, CA (mean age, 18.7 years). All participants had been homeless at some point during the past 12 months, with 70% recruited from nonshelter sites. Although youths reported high rates of hostility, unavailability, substance use, and legal problems among their caregivers, 86% reported that their caregivers had at least one attribute associated with support. Falls associated with dementia: how can you tell?

In any practice setting, professional nurses must be skilled to assess and manage medical problems rapidly. Post-fall assessment among those with dementia is a daunting task. Emergent medical problems after a fall are difficult to detect among those with dementia, because impaired communication portrays a false reality that the older adult is uninjured. Furthermore, early detection of potential post-fall injuries may not occur within a health care system that relies on verbal communication and eyewitness accounts as the main source of fall occurrence information. Professional nurses must turn

Abuse, addiction, and depression as pathways to sexual risk in women and men with a history of substance abuse.

PURPOSE: In this paper, we develop and test a model for predicting sexual risk for HIV and other STDs. METHODS: Researchers interviewed 528 women and men with a history of substance abuse about their past experiences of physical and sexual abuse, symptoms, and sexual risk behavior (number of partners, trading sex for drugs or money, unprotected intercourse, and sexual orientation). The model explores direct and indirect associations of physical abuse and sexual abuse with sexual risk. RESULTS: Depression and severity of problems with drugs and alcohol were expected to mediate associations be

Prediction of intravenous drug use.

Information about lifetime intravenous drug use (IVDU) was obtained from 1,062 relatives of hospitalized alcoholics, felons, and control subjects, of whom 92 had a history of IVDU and an additional 230 had a history of substantial, but non-IV drug use. The IVDUs were significantly more likely than all other subjects to have reported experiencing numerous behavioral and social difficulties relating to conduct difficulties, early initiation of substance use, and disrupted living situation before the age of 15. They were also more likely to have exhibited conduct problems than other drug users. I

Factors related to seroconversion among homo- and bisexual men after attending a risk-reduction educational session.

Thirteen homosexual men, volunteers in a study of the natural history of HIV, who seroconverted to HIV after participating in an educational program on HIV prevention, were interviewed about the circumstances leading to their seroconversion. Six men had participated in unprotected anal intercourse with at least one partner whom they believed was HIV-negative. Four men attributed their conversion to mental health problems or to drug and alcohol use. Two men's seroconversions could not be ascertained and one man attributed seroconversion to a condom break. Most men who had learned how to avoid i

Practical management of urinary incontinence.

A comprehensive evaluation of incontinence using office-based techniques may discover reversible problems such as delirium, urinary infections, vaginitis, depression, drug effects, endocrinologic causes, immobility, or impaction. Treatment of these causes may alleviate incontinence. Bedside urodynamics are a sensitive, specific, and useful office-based technique that can identify detrusor instability, overflow incontinence, or stress incontinence. Low urinary flow rate, difficulty with catheterization, slow bladder filling or severe urgency, bladder capacity over 600 ml, and post-void residual

Disclosure of traumas and psychosomatic processes.

Results from a series of studies are summarized in support of a general theory of inhibition and psychosomatics. According to this view, to inhibit thoughts, feelings, or behaviors is associated with physiological work. In the short term, inhibition results in increased autonomic nervous system activity. Over time, inhibition serves as a cumulative stressor that increases the probability of psychosomatic disease. Actively avoiding thoughts and feelings surrounding a trauma and/or not discussing a trauma is a particularly insidious form of inhibition. The results from recent surveys and experim

Divorce and children.

The divorce rate in the United States is currently half the rate of marriages. Whether this rate will continue is not known. Millions of American children have already experienced their parents' divorce and millions more are likely to share the experience in the future. This makes divorce a problem that frequently appears in a pediatrician's patient population. Most children of divorce will experience it at the least as a potent transient stress that disrupts virtually all aspects of their lives. Many will accommodate to their new life circumstances successfully, but a substantial percentage w

Estimating the prevalence of school refusal and school-related fears. A Venezuelan sample.

This study investigated the prevalence of school refusal and school-related fears in a sample of 1034 Venezuelan children from 3 to 14 years of age. Fear surveys were administered to children older than 9 years and to the younger children's parents. In addition, parents and teachers completed questionnaires regarding the children's other behavior problems, and teachers reported on the children's school achievement and attendance. Although a significant proportion (17.7 per cent) of the children expressed intense fears of school, only rarely were such fears combined with high absenteeism or wit

Sleep disorders in children with Smith-Magenis Syndrome - a neurodevelopmental disorder with sleep-wake-rhythm-dysfunction - recommendations for pediatricians.

BACKGROUND: Sleep disorders in children with Smith-Magenis Syndrome (SMS) are common. They affect not only the child but also the entire family. Disturbed sleep in SMS is caused by a sleep-wake rhythm dysfunction. SMS patients mostly have an inverted melatonin secretion rhythm with a peak in melatonin levels during the day. Therefore, children with SMS are often awake at night and sleepy during the day, followed by extremely severe behavioral problems. There are many recent scientific studies with treatment recommendations. PURPOSE: Since SMS is a rare disorder, many doctors are not familiar w

Symptomatic Vitamin and Nutrient Deficiencies of Autism Spectrum Disorder and the Potential for Treatment.

Autism spectrum disorder (ASD) is a heterogeneous neurobehavioral disorder. Children with ASD often have restrictive diets that can be due to food aversion, sensory sensitivities, ritualistic behavior, or comorbid gastrointestinal issues. Diet and nutritional status play a critical role in the health of neurodevelopment, and the microbiome, and can affect cognition, motor and sensory status, behavior, and sleep. Children with ASD are 5 times more likely to develop eating problems and secondary vitamin and nutritional deficiencies. Such dietary restriction has been causative of vitamin and nutr

A case study of acceptance and commitment therapy for central disorders of hypersomnolence: opportunities to improve patient-centered and comprehensive treatment approaches.

UNLABELLED: Beyond daytime sleepiness, central disorders of hypersomnolence (including narcolepsy types 1 and 2 and idiopathic hypersomnia) affect the fundamental aspects of everyday life, including mental health, negative self-esteem, relationships, social stigma, and occupational/school problems, all of which can lead to disability. Limited scientifically tested behavioral treatments designed to address the psychological, social, and economic devastation associated with hypersomnia exist. This case report reviews 1 patient's experience with 2 adjuvant interventions for central disorders of h

Latent-state and model-based learning in PTSD.

Post-traumatic stress disorder (PTSD) is characterized by altered emotional and behavioral responding following a traumatic event. In this article, we review the concepts of latent-state and model-based learning (i.e., learning and inferring abstract task representations) and discuss their relevance for clinical and neuroscience models of PTSD. Recent data demonstrate evidence for brain and behavioral biases in these learning processes in PTSD. These new data potentially recast excessive fear towards trauma cues as a problem in learning and updating abstract task representations, as opposed to

Case Report: Vitamin C combined with multiple micronutrient deficiencies is associated with pulmonary arterial hypertension in children with autistic spectrum disorder.

Children with developmental and behavioral problems including autistic spectrum disorders (ASDs) may have inappropriate feeding behaviors, which leads to an increased risk of multiple nutrient deficiencies. Vitamin C deficiency is one of the common nutrient deficiencies reported in children with inappropriate feeding. This case report illustrates two cases of ASD children with a clinical presentation of pulmonary arterial hypertension, a rare presentation of vitamin C deficiency. Vitamin C supplementation, pulmonary vasodilator, and supportive treatment were provided. Patients could recover fr
