Accidental Pyrethroid Ingestion in Toddler: Near-Fatal Atypical Presentation and Successful Recovery.

We are reporting a case of pyrethroid poisoning with atypical presentation in a 21-month-old toddler who was transferred to us from a peripheral center. Signs and symptoms at presentation were predominantly of cardiopulmonary dysfunction contrary to more common presenting features of gastrointestinal and neurological impairment. The reason for this seems to be the aspiration pneumonitis as a consequence of vomiting induced by parents at home, rather than the toxin itself even though a rather rapid progression of lung injury does not rule out the possibility. He had developed decreased level of

Patient-Reported Financial Burden in Head and Neck Cancer Undergoing Radiotherapy.

Background/Objectives: Financial toxicity (FT) refers to the financial burden directly or indirectly caused by a patient's medical care. Patients with head and neck cancer (HNC) are particularly vulnerable to FT due to lower rates of return to work and higher out-of-pocket payments (OOPP). In this cross-sectional study, we assessed the amount and types of OOPP, as well as the prevalence of FT, in HNC patients who had completed curative radiotherapy. Methods: We included HNC patients who underwent curative-intent radiotherapy at four private clinics in Romania, within 12 months of completing tr

Innovative optimization studies to advance the current state of isotope production target design at the high flux isotope reactor.

A growing demand for various stable and radioactive isotopes is limiting the ability to incorporate new and innovative target designs for isotope production (IP) at the US Department of Energy's High Flux Isotope Reactor (HFIR) located at Oak Ridge National Laboratory. This demand has increased reliance on historical and conservative approaches, which has forced scientists to operate in a "just-in-time" manner, producing routine yield calculations with few advancements. The goal of this work is to demonstrate the effectiveness and applicability of a recently proposed HFIR-specific optimization

Prevalence and Potential Impact of Gastrointestinal Insufflation During Cardiopulmonary Resuscitation.

Background/Objectives: Insufflation of the gastrointestinal tract, as a side effect of improper ventilation, is a known complication in resuscitation patients. As animal studies have shown, this can be associated with an increase in intra-abdominal pressure with adverse effects on hemodynamics and respiratory mechanics. In this study, we investigated the prevalence and severity of insufflation and discussed the potential impact on the outcome of resuscitation. Methods: This study was based on computed tomography (CT) images from two university hospitals in Munich, Germany, which were taken as

Shoulder Rehabilitation Exercises With Kinematic Biofeedback After Arthroscopic Rotator Cuff Repair: Protocol for a New Integrated Rehabilitation Program.

BACKGROUND: The recovery of scapular and humeral physiological kinematic parameters, as well as the sensorimotor control of movement, plays a primary role in the rehabilitation process after arthroscopic rotator cuff repair. A highly customized rehabilitation approach is required to achieve this aim. Biofeedback can be a useful tool, but there is poor evidence of its application in the rehabilitation after arthroscopic rotator cuff tear repair. OBJECTIVE: The aim of this paper is to outline an innovative exercise-based rehabilitation program exploiting visual biofeedback for the recovery of p

Is lower limb salvage worthwhile after severe open tibial fractures in a developing country? An analysis of surgical outcomes, quality of life and cost implications.

INTRODUCTION: Open tibial fractures are rare and difficult-to-treat injuries because of the involvement of bony, skin and neuromuscular injury along with co-morbidities. Often, during the management of very severe cases these injuries, the question arises, should we amputate or salvage the limb? This question has been explored previously in civilian and military contexts in the US and UK but remains unstudied in the alternative sociocultural and economic context of the developing world. METHODS: We studied 78 adult patients with severe open tibial fracture that presented to our institution, a

Clinical Outcomes After Cervical Transcorporeal Microdecompression and Vertebral Body Access Channel Repair.

BACKGROUND: Although anterior cervical decompression and fusion (ACDF) can be performed using minimally invasive techniques, the extensive removal of anatomical keystones during decompression requires a segmental fusion to restore biomechanical stability. Treatment with arthrodesis techniques may result in a prolonged recovery time, loss of motion, and the need for further treatment if a pseudarthosis or adjacent segment disease occur. Transcorporeal micro decompression (TCMD) is a newly developed motion sparing, minimally invasive anterior cervical spine decompression procedure that utilizes

Treatment of acute lateral ankle ligament rupture in the athlete. Conservative versus surgical treatment.

Acute lateral ankle ligament sprains are common in young athletes (15 to 35 years of age). Diagnostic and treatment protocols vary. Therapies range from cast immobilisation or acute surgical repair to functional rehabilitation. The lateral ligament complex includes 3 capsular ligaments: the anterior tibiofibular (ATFL), calcaneofibular (CFL) and posterior talofibular (PTFL) ligaments. Injuries typically occur during plantar flexion and inversion; the ATFL is most commonly torn. The CFL and the PTFL can also be injured and, after severe inversion, subtalar joint ligaments are also affected. Com

The adult with congenital heart disease.

Internal medicine cardiovascular specialists and internists are going to be seeing increasing numbers of adult patients with congenital heart malformations, and most of them will have had one or more surgical or therapeutic catheter procedures. The nonsurgical cases will have either benign defects or important but inoperable anomalies, or the physician may encounter the rare case which, though amenable to surgery, has escaped operation until adult life. Among patients with postoperative congenital heart disease, a few are totally cured and require no special follow-up or any special precaution

Lived experience of cognitive-communication changes for people with acquired brain injury and familiar communication partners: A qualitative evidence synthesis.

BACKGROUND AND OBJECTIVES: Cognitive-communication disorder (CCD) is common after acquired brain injury (ABI), reported in about two-thirds of people who sustain an injury. Quantitative studies have found that the disorder can negatively impact a person's ability to socially re-integrate into the community, return to work or education and achieve a good quality of life. However, little is known about how the disorder impacts people with ABI and the family. Therefore, the aim of this qualitative evidence synthesis was to provide a detailed exploration of the lived experience of CCD for people w

Long-Term Outcomes of Patients with Stroke Predicted by Clinicians to have no Chance of Meaningful Recovery: A Japanese Cohort Study.

BACKGROUND: Little is known about the natural history of comatose patients with brain injury, as in many countries most of these patients die in the context of withdrawal of life-sustaining therapies (WLSTs). The accuracy of predicting recovery that is used to guide goals-of-care decisions is uncertain. We examined long-term outcomes of patients with ischemic or hemorrhagic stroke predicted by experienced clinicians to have no chance of meaningful recovery in Japan, where WLST in patients with isolated neurological disease is uncommon. METHODS: We retrospectively reviewed the medical records

Assessment of Lead Exposure and Urinary-δ-aminolevulinic Acid Levels in Male Lead Acid Battery Workers in Tamil Nadu, India.

BACKGROUND: Exposure to lead (Pb) affects multiple health outcomes and physiological systems. In adults, even small increases in blood Pb levels have been associated with decreased glomerular filtration rate, increased risk of hypertension and increased incidence of essential tremor. To date, there have been few Pb-exposure assessments using the United States Occupational Health and Safety Administration (OSHA) regulations. OBJECTIVE: The aim of the present study was to assess Pb-exposure in terms of elevated blood lead levels (BLL) and urinary-δ-aminolevulinic acid (U-δ-ALA) levels of worker

[Open fractures of the leg - Series from the University Hospital of Nancy].

Thirteen cases from two teams were studied retrospectively between 1983 and 1992. There were 11 men and 2 women with an average of 30 years (16-52 years). Nine were due to road traffic accidents and in 3 cases the fracture involved the upper third, 5 cases the middle third and 5 cases the lower third of the tibia. Six patients suffered from multiple injuries. Arteriography was used in 6 cases. Initial debridement was adequate in 9 cases but required to be redone in 3. In one case it required to be repeated several times. Antibiotics were given for an average of 28 days but if 2 infected cases

[The lived meaning of becoming homeless and subsequently obtaining housing: A qualitative study of individuals with long-term experiences of homelessness].

Objective Homelessness has a profound impact on health and is often framed as an extreme form of social exclusion. In Japan, policies and support programs have promoted stable housing as a pathway to independence, contributing to a marked decline in the number of individuals experiencing homelessness. Nevertheless, the homeless population has aged, and homelessness has become prolonged; among this population, the avoidance of support services and return to street homelessness remain notable challenges. While previous studies have emphasized structural factors and formal outcomes, such as facil

Life-threatening Listeria meningitis: Need for revision of South African acute bacterial meningitis treatment guidelines.

BACKGROUND: The recent listeriosis outbreak in South Africa (SA) received widespread attention in the media. More than 1 000 laboratory-confirmed cases of listeriosis occurred during an 18-month period, with a case fatality rate of 28%. Acute bacterial meningitis due to listeriosis was extremely rare at Steve Biko Academic Hospital in Pretoria until 2017/18, when we saw two very sick adults with this condition during the listeriosis outbreak. OBJECTIVES: To describe the presentation, treatment and outcome of these patients to raise awareness of this potentially fatal but treatable infection t

Microendoscopic discectomy for lumbar disc herniations.

IntroductionLumbar disc herniation is one of the main causes of discogenic low back pain and reported to affect 60%-80% of people during their lifetime. The two main surgical modalities for intervertebral disc surgery are standard open discectomy and minimally invasive discectomy which include percutaneous endoscopic lumbar discectomy and microendoscopic discectomy (MED). We report our experience with the same technique of MED to evaluate the efficacy of MED for lumbar disc pathology.Aims and objectivesThe aims and objectives were to study the efficacy, advantages, and associated limitations a

Predictive modeling of the virtual Hemi-Fontan operation for second stage single ventricle palliation: two patient-specific cases.

Single ventricle hearts are congenital cardiovascular defects in which the heart has only one functional pumping chamber. The treatment for these conditions typically requires a three-staged operative process where Stage 1 is typically achieved by a shunt between the systemic and pulmonary arteries, and Stage 2 by connecting the superior venous return to the pulmonary circulation. Surgically, the Stage 2 circulation can be achieved through a procedure called the Hemi-Fontan, which reconstructs the right atrium and pulmonary artery to allow for an enlarged confluence with the superior vena cava
