Lateral mass fusion for lower back pain.

Forty patients who underwent Wiltse's lateral mass fusion by two different surgeons for spondylolisthesis or degenerative disc disease have been independently reviewed. It was found at follow-up (mean 22 months +/- S.D. 14 months) that factors that affected the final result were the age of the patient, the presence of preoperative nerve root symptoms, previous disc surgery, and incomplete fusion. The duration of symptoms, length of follow-up, preoperative spondylolysis, and the extent of the fusion did not appear to affect the result. This series confirms the previous reports that Wiltse's mas

The lumbar facet arthrosis syndrome. Clinical presentation and articular surface changes.

We describe a lumbar facet syndrome in which disabling symptoms are associated with normal or near-normal plain radiographs. Local spinal fusion relieved symptoms in 12 patients; the excised facet joint surfaces showed some of the histological changes seen in chondromalacia patellae and in osteoarthritis of other large joints. The most frequent change was focal full-thickness cartilage necrosis or loss of cartilage with exposure of subchondral bone, but osteophyte formation was remarkably absent in all specimens. We suggest that there are both clinical and histological similarities between the

[Kyphosis caused by severe spondylodiscitis in newborn infants and young children].

The authors report their experience of 12 discitis occurring in the new born and in young children. Pyogenic infections were found with general septicemia in the new born children. The onset of cases of Pott's disease occurred later. Anterior destruction of several vertebrae accounted for severe kyphosis which were found on clinical examination. Conservative immobilisation of the kyphosis during growth does not prevent any increase in deformity before puberty with the possible onset of neurological symptoms. The only possible treatment is surgical with anterior arthrodesis of the damaged regio

The surgical treatment of instability of the upper part of the cervical spine in children and adolescents.

In a retrospective review of the cases of thirteen skeletally immature children and adolescents (four to eighteen years old) with instability of the upper part of the cervical spine (occiput to fifth cervical vertebra), we determined the efficacy of posterior arthrodesis and halo-cast immobilization in the management of this condition. The patients were divided into two groups: those with congenital vertebral anomalies alone (fusion or structural defects, or both) and those with cervical anomalies and systemic disorders (dwarfism, juvenile rheumatoid arthritis, Down syndrome, and cerebral pals

Nocardial osteomyelitis of the spine with epidural spinal cord compression--a case report.

A 53-year-old man presented with complaints of back pain and weakness in his lower extremities. Physical examination demonstrated a thoracic myelopathy with a sensory level at T-4. The diagnostic work-up revealed vertebral osteomyelitis of the thoracic spine with epidural compression at T-2-T-4 causing a nearly complete block on myelography. A decompressive laminectomy and debridement were performed, followed by anterior spinal fusion. Nocardia asteroides was cultured from the epidural space. The patient was concurrently treated with sulfonamides. No underlying malignancy or immunosuppression

Surgery and postsurgical management of the patient with low back pain.

The role of surgery in the care of back pain and sciatica is discussed. Surgery's role in this case is relatively minor considering the overall problems of back disorders. The history, signs, and symptoms of various diagnostic categories are reviewed. Factors in deciding on the need for surgical care are analyzed. An algorithm that displays the diagnostic approach as well as various treatment plans helps in the decision-making process. Various exercise programs for postsurgical care are outlined. Details of the surgical procedures are also presented to explain the degree of tissue damage creat

Open versus minimally invasive fixation of thoracic and lumbar spine fractures in patients with ankylosing spinal diseases.

PURPOSE: Posterior multilevel fixation of traumatic instability in ankylosing spinal disease (ASD) can be performed by open surgery (OS) or minimally invasive surgery (MIS). We investigated whether both methods differ based on the reduction results and perioperative parameters. METHODS: In this retrospective cohort study, OS and MIS groups were investigated. The bisegmental Cobb angles and dislocation angles were measured using pre- and postoperative CT images, and the initial malalignment and achieved reduction were calculated. Cut-seam time, calculated blood loss, transfusion number, fluoro

The C2 Cortical Screw, an Alternative Fixation Technique for the C2 Segment During High Cervical Spine Surgery: Technical Note.

BACKGROUND: For posterior fixation of C2 vertebra (the axis), several fixation techniques such as pedicle screw, laminar screw, and pars screw have been reported. A pedicle screw (PS) is considered the strongest among the techniques, but certain situations make PS fixation impossible or difficult. These include patients with a narrow C2 pedicle or high-riding vertebral artery. We introduced an alternative screw technique for C2 that addressed the aforementioned problems with safely placing it, which we termed a cortical screw (CS) for C2. METHODS: Among a total of 28 cases using C2 CS for hig

Chronic low back pain after lumbosacral fracture due to sagittal and frontal vertebral imbalance.

PROBLEM AND HYPOTHESIS: Over time, some patients with unilateral or bilateral lumbosacral injuries experience chronic low back pain. We studied the sagittal and frontal balance in a population with these injuries to determine whether mismatch in the pelvic and lumbar angles are associated with chronic low back pain. PATIENTS AND METHODS: Patients with posterior pelvic ring fractures (Tile C1, C2, C3 and A3.3) that had healed were included. Foreign patients and those with an associated spinal or acetabular fracture or nonunion were excluded. The review consisted of subjective questionnaires, a

Increased intraoperative epidural pressure in lumbar spinal stenosis patients with a positive nerve root sedimentation sign.

PURPOSE: The sedimentation sign (SedSign) has been shown to discriminate well between selected patients with and without lumbar spinal stenosis (LSS). The purpose of this study was to compare the pressure values associated with LSS versus non-LSS and discuss whether a positive SedSign may be related to increased epidural pressure at the level of the stenosis. METHODS: We measured the intraoperative epidural pressure in five patients without LSS and a negative SedSign, and in five patients with LSS and a positive SedSign using a Codman(™) catheter in prone position under radioscopy. RESULTS:

Computed tomography study of implantation corridors in canine vertebrae.

OBJECTIVES: To define the characteristics of optimum implantation corridors in vertebral bodies C2 to C7 and T10 to S1, using computed tomography examination. METHODS: Measurements were taken from 207 vertebrae from 35 different adult dogs. RESULTS: Implantation corridors of the cervical vertebrae are narrow. The width preserving the transverse hole is less than 2.5 mm in 68.6 per cent of the 86 vertebrae C2 to C6. Dorsal implantation corridors of the last four thoracic vertebrae are narrow, and major anatomical structures are very close to their emergence point. In 63 per cent of the 40 tho

Percutaneous vertebroplasty of a myelomatous compression fracture in the presence of previous posterior instrumentation. Report of two cases.

The authors report the use of percutaneous transpedicular vertebroplasty performed using polymethylmethacrylate (PMMA) in two patients. These men (53 and 57 years old) had previously undergone open surgery and posterior instrumentation to treat myelomatous compression fractures. Both patients presented with acute back pain that manifested after minor activities. Kyphotic wedge fractures were diagnosed at T-1 in one case and at L-1 in the other. Both patients were treated at other hospitals with laminectomy and instrumented fusion; multiple myeloma was diagnosed after surgery. The patients expe

[Transpedicular implantation of screws in the thoracolumbar spine. Results of a survey of methods, frequency and complications].

Pedicular fixation has found great acceptance as a modality for spinal fusions. Being a "blind technique" it is associated with a potential high risk of neural and vascular morbidity. In an attempt to evaluate and/or establish a uniformly accepted concept of surgical management we designed a questionnaire composed of seventeen questions dealing with different pre-, intra- and postoperative procedures in transpedicular fixation. This was sent to 31 experienced surgeons organized in the working group "spine" of the german trauma association. Half of the answers to each question were similar. The

Reversible cortical blindness as a complication of rheumatoid arthritis of the cervical spine.

A case is presented in which a patient with rheumatoid damage to the cervical spine causing cervical cord and vertebral artery compression sustained transient cortical blindness with a partial left hemiparesis. Magnetic resonance imaging revealed anterior subluxation of middle cervical vertebrae, separation of the odontoid peg with resultant atlantoaxial subluxation, and proliferative pannus formation. The patient was almost symptom-free after transoral decompression and posterior cervical fusion.

Anterior spinal tuberculosis: paraplegia following laminectomy.

Of 16 patients with anterior spinal tuberculosis treated in this unit during the period 1979-1984, five presented with neurological complications following laminectomy at another hospital. After the decompressive laminectomy all five patients developed an increasing kyphosis, and four of the five patients had deteriorated neurologically. They were transferred to our unit where the four with neurological deterioration underwent surgical stabilisation. This paper illustrates the dangers of posterior spinal surgery in the presence of anterior disease.

Radicular derivation of sensory action potentials in lower extremity.

A patient with grade II spondylolisthesis and back pain underwent Harrington rod instrumentation. Postoperatively, he developed pain and numbness of the lateral aspect of the left foot and loss of the left AChilles reflex. Preoperative electrodiagnostic findings of the left lower extremity were normal. Two postoperative studies revealed that, concomitant with evidence of denervation if the left S1-supplied muscles, his sural sensory nerve action potential (SNAP) had been abolished but the superficial peroneal SNAP, recorded from the intermediate branch, was normal and equal to that of the cont

Comparing fusion and complication rates after instrumented versus uninstrumented fusion for lumbar spondylolisthesis: A systematic review and meta analysis of randomized controlled trials with trial sequential analysis.

INTRODUCTION: Spinal fusion is a common treatment for degenerative or isthmic lumbar spondylolisthesis (LS) in adult patients, where vertebral slippage can lead to significant neurological impairment. However, debate exists regarding the exact fixation method, as fusion and complication rates may differ via instrumented fusion (IF) or uninstrumented fusion (UIF). Therefore, the purpose of this study is to investigate the high-level literature for the fusion and complication rates associated with IF versus UIF to guide decision-making. METHODS: This systematic review and meta-analysis utilized
