Cancer develops through the interactions between cancer stem cells and components of the tumor microenvironment (TME). To model in vivo cancer stem cell-TME interactions and elucidate their functional consequences, we focused on myelofibrosis (MF), a stem cell-driven myeloproliferative neoplasm. We cocultured MF hematopoietic stem and progenitor cells (HSPCs) with normal donor endothelial cells (ECs) and mesenchymal stromal cells (MSCs) to investigate the consequences of interactions between malignant MF HSPCs and nonmalignant microenvironmental cells. This tricultivation system proved to be a

Biliary atresia (BA) is a disease of the liver and bile ducts. Newborns with BA must undergo the Kasai procedure immediately after birth to survive. Postoperative nutrition, portal hypertension, and splenomegaly affect prognosis on an ongoing basis. With recent advancements in medicine and health care quality, children treated for BA can survive into adolescence and even early adulthood. This qualitative descriptive study explores how these youths have adapted, and their concerns as they continue to age. A total of 13 patients with BA aged 16-24 years were recruited using purposive sampling at

Fibrotic remodeling of the bone marrow (BM) niche is a characteristic feature of myelofibrosis (MF) that contributes to disease progression. In MF, mesenchymal stromal cells (MSCs) produce excessive amounts of inflammatory cytokines and extracellular matrix, leading to BM fibrosis, impaired blood production, extramedullary hematopoiesis, and progressive BM failure. While the genetic events that initiate MF in hematopoietic cells are well defined, our understanding of the mechanisms responsible for BM fibrosis remains incomplete. Here, we show that transcription factor EBF1 is a key regulator o

Cluster of differentiation 14+ monocytes produced spindle-shaped fibrocytes, similar to fibroblasts. Recent studies have reported that fibrocytes are crucial for the development and progression of primary myelofibrosis (MF); however, their functional role remains unclear. We compared monocytes and fibrocytes using RNA sequencing for gene expression profiles. We focused on chitinase 3-like-1 (CHI3L1), which causes inflammation, organ fibrosis, and extracellular tissue remodeling. We found higher CHI3L1 levels in patients with myeloproliferative neoplasm with MF than in those without MF. Further

Extramedullary hematopoiesis (EMH) in myelofibrosis manifests beyond classical splenomegaly, causing life-threatening complications, yet lacks systematic characterization in the JAK inhibitor era. We systematically reviewed 36 publications (2011-2025), available on PubMed database, reporting 47 unique myelofibrosis-associated EMH cases, analyzing demographics, anatomic distribution, presentations, diagnostics, treatments, and outcomes.Median age was 68 years (range 43-82; M: F 26:21); primary myelofibrosis predominated (64%). EMH favored atypical sites: abdominopelvic (28%), thoracic (26%; pul

Immune thrombocytopenia (IT) is an autoimmune disorder characterized by isolated thrombocytopenia resulting from increased platelet destruction and impaired production. Although most cases are idiopathic, secondary causes, including malignancies, should be considered, particularly in atypical or treatment-resistant presentations. Non-Hodgkin lymphomas (NHL) may rarely present with autoimmune cytopenias, leading to diagnostic challenges. We report the case of a 77-year-old male patient presenting with fatigue, weight loss, severe thrombocytopenia, anemia, and leukopenia who was initially manage

Although adult-onset Still's disease (AOSD) typically presents with a triad of fever, rash, and arthritis, atypical manifestations can significantly hinder the diagnostic process of fever of unknown origin (FUO). We describe the case of a 57-year-old woman who presented with prolonged fever, serositis, and extreme hyperferritinemia (ferritin levels 117,904 ng/mL), further complicated by aseptic encephalitis. Following a comprehensive diagnostic workup to exclude infectious, neoplastic, and other autoimmune etiologies, the patient was evaluated using the Yamaguchi criteria. Despite the absence

Psoriasis is a chronic inflammatory skin disease characterized by keratinocytes (KCs) hyperproliferation and persistent inflammation. Current biologics are limited by high relapse rates and long-term safety concerns, prompting the search for multi-target natural small molecules. A murine psoriasis model was induced by 6-day continuous topical imiquimod (IMQ) application; cellular models were established by stimulating keratinocyte lines with tumor necrosis factor-alpha (TNF-α)/interleukin-17A (IL-17A). Network pharmacology and molecular docking were employed to predict targets, and small inter

Inflammatory bowel disease (IBD)-including Crohn's disease, ulcerative colitis, and indeterminate colitis-is a chronic immune-mediated condition that primarily affects the intestinal mucosa but often presents with extraintestinal digestive manifestations, which are important yet frequently underrecognized sources of morbidity. These heterogeneous manifestations reflect diverse genetic, microbial, immunologic, and environmental influences, highlighting the value of a personalized medicine approach. Hepatobiliary involvement affects IBD adults patients and is even more common in children, rangin

Background: Hereditary Spherocytosis (HS), the most common congenital hemolytic anemia, is characterized by highly heterogeneous clinical manifestations with variable hemolytic anemia, jaundice, reticulocytosis, splenomegaly, and gallstones. The ektacytometric analysis is considered a gold standard tool for HS diagnosis. Two distinct profiles may be observed: the typical bell-shaped profile (HS1) and a right-shifted curve (HS2); nonetheless, the clinical and biochemical significance of these differences, and their correlation with the clinical phenotype of HS remain poorly understood. Methods:

Azathioprine (AZA) is widely used during pregnancy to manage autoimmune diseases; however, concerns regarding fetal immunological vulnerability persist. Although neonatal lymphocytopenia and chromosomal abnormalities have been reported following prenatal AZA exposure, little is known about immune recovery in infants after congenital infections in these infants. Here, we describe an infant with congenital cytomegalovirus (CMV) infection, who was born to a mother treated with azathioprine for systemic lupus erythematosus and developed hemophagocytic lymphohistiocytosis (HLH) during immune recons

A case of pathological splenic rupture associated with malaria complicated by Plasmodium vivax is presented in a 28-year-old male patient from El Retorno (Guaviare). The patient presented with fever, left side chest pain, vomiting, diarrhea, tachycardia, tachypnea, and signs of hemodynamic hypotension associated with hypovolemic shock. Upon admission, he was treated with intramuscular analgesics (dipyrone, 500 mg, and tramadol, 100 mg) and intravenous fluids. The hemogram revealed neutrophilia and thrombocytopenia, while the thick smear was positive for P. vivax with 24 000 parasites per μl. D

Essential thrombocythemia (ET) and polycythemia vera (PV) are rare in adolescent and young adult (AYA). These conditions, similar to those in older patients, are linked with thrombotic complications and the potential progression to secondary myelofibrosis (sMF). This retrospective study of ET and PV patients diagnosed before age 25 evaluated complication rates and impact of cytoreductive drugs on outcomes. Among 348 patients (278 ET, 70 PV) with a median age of 20 years, the of thrombotic events was 1.9 per 100 patient-years. Risk factors for thrombosis included elevated white blood cell count
