Second-Opinion Interpretations of Gynecologic Oncologic MRI Examinations by Sub-Specialized Radiologists Influence Patient Care.

PURPOSE: To determine if second-opinion review of gynaecologic oncologic (GynOnc) magnetic resonance imaging (MRI) by sub-specialized radiologists impacts patient care. METHODS: 469 second-opinion MRI interpretations rendered by GynOnc radiologists were retrospectively compared to the initial outside reports. Two gynaecologic surgeons, blinded to the reports' origins, reviewed all cases with discrepancies between initial and second-opinion MRI reports and recorded whether these discrepancies would have led to a change in patient management defined as a change in treatment approach, counsellin

Use of closed suction devices and other drains in spinal surgery: results of an online, Germany-wide questionnaire.

PURPOSE: The risks of drains in spine surgery (e.g., increasing venous plexus bleeding, maintaining CSF leakage, and infections) must be balanced with their benefits (e.g., reduced rate of postoperative hematoma and seroma formation). Little is known about factors that influence surgeons' decision to employ a drain. METHODS: We conducted a survey among German spine surgeons regarding their use of drains. Neurosurgical and orthopedic departments along with privately practicing neurosurgeons were invited to complete an online questionnaire featuring general and case-specific questions with rega

Is open tracheotomy performed by residents in otorhinolaryngology a safe procedure? A retrospective cohort study.

Surgical or percutaneous tracheotomy is one of the commonest operations in the ENT practice and one of the first procedures to be taught to residents. No study exists that demonstrates the safety of this surgical procedure performed by unexperienced surgeons. The purpose was to compare outcomes of tracheotomies performed by supervised residents and surgeons in terms of postoperative complications and mortality, and identify risk factors for the onset of complications. Retrospective cohort study. Otolaryngology-Head and Neck Surgery Department, University of Florence, Italy. We included all pat

SESC Practice Committee survey: surgical practice in the duty-hour restriction era.

Debate continues as to the relevance of Accreditation Council for Graduate Medical Education (ACGME) duty-hour restrictions in actual practice and the adequacy of resident training in surgery. A survey of the membership of the Southeastern Surgical Congress using an Internet-based questionnaire was conducted: adherence to duty-hour restrictions, evidence of sleepiness and fatigue, opinions regarding the training, and clinical performance of surgeons who had trained after the institution of duty-hour restrictions in 2003 (termed "recently trained surgeons"). One hundred seventy-seven members re

A prospective randomized trial of ultrasound- vs landmark-guided central venous access in the pediatric population.

BACKGROUND: The purpose of this prospective randomized study was to compare landmark- to ultrasound-guided central venous access when performed by pediatric surgeons. The American College of Surgeons advocates for use of ultrasound in central venous catheter placement; however, this is not universally embraced by pediatric surgeons. Complication risk correlates positively with number of venous cannulation attempts. STUDY DESIGN: With IRB approval, a randomized prospective study of children under 18 years of age undergoing tunneled central venous catheter placement was performed. Patient accru

Critical analysis of consecutive unilateral cleft lip repairs: determining ideal sample size.

Objective : Cleft surgeons often show 10 consecutive lip repairs to reduce presentation bias, however the validity remains unknown. The purpose of this study is to determine the number of consecutive cases that represent average outcomes. Secondary objectives are to determine if outcomes correlate with cleft severity and to calculate interrater reliability. Design : Consecutive preoperative and 2-year postoperative photographs of the unilateral cleft lip-nose complex were randomized and evaluated by cleft surgeons. Parametric analysis was performed according to chronologic, consecutive order.

Tracking intraoperative fluoroscopy utilization reduces radiation exposure during ureteroscopy.

PURPOSE: Recent studies have demonstrated deleterious effects of ionizing radiation from diagnostic and therapeutic imaging procedures. One of the barriers to minimizing patient exposure is physician awareness. We prospectively studied whether providing surgeons with feedback on their fluoroscopy utilization would affect intraoperative fluoroscopy times. MATERIALS AND METHODS: In 2007, we prospectively began to track fluoroscopy usage for all urology cases. Nine months later, surgeons started to receive periodic reports with their mean fluoroscopy time compared with their peers. We reviewed a

Factors prolonging the operative time for transumbilical laparoscopic-assisted appendectomy in pediatric patients: a retrospective single-center study.

PURPOSE: Transumbilical laparoscopic-assisted appendectomy (TULAA) is one of the first endoscopic surgeries performed by trainee pediatric surgeons. While the operative time is generally shorter than for conventional laparoscopic appendectomy, the indications for this procedure are unclear and many unknown factors can prolong the operative time. We conducted this study to identify the factors that may prolong the operative time for TULAA. METHODS: This retrospective, single-center study was conducted between 2015 and 2023. We performed multivariate analysis to identify the factors associated

[Perianal surgery in a non-sterile setting: risks, cost-saving ad climate benefits].

INTRODUCTION: Sustainable healthcare is essential to reduce environmental impact without compromising patient safety. This study investigates whether minor perianal surgery can be safely performed under a non-sterile condition, aiming to reduce material use, costs, and CO2-emissions, and whether these findings can be incorporated into a widely supported surgical protocol. METHODS: A retrospective cohort study was conducted among patients who underwent minor perianal surgery under sterile or non-sterile conditions. Primary outcomes were postoperative wound infections (POWIs) within 30 days, co

Being Honest with Yourself-Setbacks, Pivots, and Changes in Career.

Approximately 20 to 40% of physicians change jobs within the first 2 to 5 years of practice, and up to 80% eventually leave their first position. In colorectal surgery-a field that has become increasingly competitive-early career transitions are common, whether across institutions or within the same department. In this chapter, we explore the dynamic and non-linear nature of career development in colorectal surgery through firsthand narratives of surgeons who have navigated shifting roles, institutional limitations, and evolving goals. When selecting a first job or evaluating new opportunities

Bone graft in bilateral mandibular sagittal split osteotomy: the current state of practice in France.

INTRODUCTION: Bilateral sagittal split osteotomy of the mandible (BSSO) is the most common orthognathic surgery procedure. The use of bone grafting seems widely used whereas its interest has not been shown to date. The aim of this study was to describe the current state of practice of bone graft in BSSO in France. MATERIALS AND METHODS: An online survey was used to collect data about the use of bone grafting in BSSO procedures among French maxillofacial surgeons. RESULTS: One hundred and forty-eight surgeons responded to the survey, mostly maxillo-facial surgeons (96 %). Only 31.8 % of parti

Gabapentin: Perspective on Its Use as a Postoperative Analgesic by Colorectal Providers.

Gabapentin (Gb) is increasingly used in postoperative pain management largely to reduce opioid use but without a strong evidence base. There is no literature available to understand why and how surgeons use gabapentin. We surveyed 85 NESCRS (New England Society of Colon and Rectal Surgeons) members by email (August-October 2024). Of the 32 responses (38% response rate), 28 were included: 68% were colorectal surgeons, 18% were advanced practice providers, 39% were female, 46% were aged 31-50 years, and 68% were White. Commonly prescribed analgesics were opioids (96%), acetaminophen (93%), nonst

[Thoracic Surgery during Pregnancy and Breastfeeding - an Interdisciplinary Consensus Paper].

If thoracic surgery is to remain an attractive career path for young physicians, it is essential to provide opportunities to balance work, residency, and family time. With an increasing proportion of women in thoracic surgery, it has become increasingly important to create a work environment that allows safe employment during pregnancy and breast feeding becomes an important concern.Based on the legal requirements of the German Maternity Protection Act (Mutterschutzgesetz), this interdisciplinary consensus paper was developed by representatives of thoracic surgery, anaesthesiology, and occupat

Facing the unexpected: unusual causes of mechanical small bowel obstruction in adults.

Mechanical small bowel obstruction in adults is a common emergency condition that typically requires hospitalisation and usually acute surgical intervention. The majority of the cases are due to adhesive obstruction or common abdominal wall hernias or relevant related past history. However, the surgeons might face unexpected challenges in this concept due to rare causes of this condition. This paper explores in depth the challenges encountered by the emergency general surgeons in the concept of management of rare causes of mechanical small bowel obstruction in adults through a systematic revie

Interobserver Reliability of Pirani and Dimeglio Scores in the Clinical Evaluation of Idiopathic Congenital Clubfoot.

BACKGROUND: Dimeglio (DimS) and Pirani (PirS) scores are the most common scores used in congenital talipes equinovarus (CTEV) clinical practice. The aim of this study was to evaluate the interobserver reliability of these scores and how clinical practice can influence the clinical outcome of clubfoot through the DimS and Pirs. METHODS: Fifty-four feet were assessed by six trained independent observers through the DimS and PirS: three consultants (OS), and three residents (RS) divided into three pediatric orthopaedic surgeons (PeO) and three non-pediatric orthopaedic surgeons (NPeO). RESULTS:
