A 63-year-old woman presented with a four-month history of bilateral hearing loss and three months of progressive, painful peripheral neuropathy. Her clinical course was complicated by deep vein thrombosis with pulmonary emboli and an incomplete response to initial empiric glucocorticoid therapy. Following glucocorticoid taper, she developed temporal headaches, jaw claudication, and scalp tenderness, raising concern for giant cell arteritis (GCA). Laboratory evaluation revealed markedly elevated inflammatory markers (erythrocyte sedimentation rate and C-reactive protein), microcytic anemia, mi

Systemic lupus erythematosus (SLE) is a complex autoimmune disease characterized by a broad spectrum of clinical manifestations, including gastrointestinal (GI) involvement. Although the joints, skin, and kidneys are most commonly affected, GI manifestations are frequently underrecognized despite their potential to significantly influence patient outcomes. Eleven major GI manifestations have been identified and described: oral ulcers, lupus enteritis (LE), lupus peritonitis (LP), mesenteric vasculitis, mesenteric thrombosis, protein-losing enteropathy (PLE), intestinal pseudo-obstruction (IPO)

Diffuse alveolar hemorrhage (DAH) is a medical emergency that is known to be caused by systemic vasculitis, autoimmune conditions, coagulation disorders, and toxins. While it can be caused by chronic mitral regurgitation (MR), it has a rare association with acute MR. We report the case of a patient presenting with DAH as the first sign of acute MR. A 58-year-old man with type 2 diabetes mellitus and no cardiac history except for hypertension presented with dyspnea and cough with pink-stained white sputum for two days. On examination, bilateral crepitations and a pansystolic murmur were noticed

Lupus mesenteric vasculitis (LMV) is a rare but potentially life-threatening complication of systemic lupus erythematosus, with reported mortality rates of up to 13.4% despite aggressive immunosuppression. Standard management relies on high-dose glucocorticoids (GCs) combined with cyclophosphamide (CYC) or mycophenolate mofetil (MMF); however, prolonged glucocorticoid dependence and disease recurrence remain significant clinical challenges. Telitacicept, a recombinant transmembrane activator and calcium-modulating cyclophilin ligand interactor (TACI)-Fc fusion protein that simultaneously neutr

Background/objective: Kawasaki disease is an acute, systemic vasculitis most commonly observed in young Asian children. While the exact pathogenesis remains unknown, it is thought to be triggered by a pathogenic exposure that elicits an inflammatory response several days to 2 weeks before the onset of symptoms. Symptoms include edematous hands and feet, conjunctivitis, mucositis, a diffuse erythematous rash, lymphadenopathy, and a fever lasting for at least 5 days, which is required for diagnosis. In addition, coronary vessels may exhibit aneurysms, thrombosis, and stenosis. Without treatment,

Posterior reversible encephalopathy syndrome (PRES) is a neurologic condition characterized by distinctive radiologic findings and altered mental status, often associated with chemotherapy or systemic endothelial dysfunction. To date, there have been no documented cases of PRES occurring after chimeric antigen receptor T cell therapy (CAR-T) for B-cell acute lymphoblastic leukemia (B-ALL). We describe the case of a 30-year-old female patient with relapsed B-ALL who received brexucabtagene autoleucel and subsequently developed a progressively worsening cytokine release syndrome and immune effec

The precipitation of cryoglobulins, serum immunoglobulins, below 37 °C defines the clinical cryoglobulinemic syndrome, a systemic vasculitis usually characterized by purpura, weakness, and arthralgia. In most cases, this condition is associated with chronic infection by the hepatitis C virus (HCV) and can evolve into B-cell dysregulation and malignancies. The current literature on non-HCV-associated cryoglobulinemia is very limited, and little is known about the immunological and serological profile of affected patients. The cryoglobulinemic syndrome not associated with HCV infection is often

Kawasaki disease (KD) is an acute inflammatory condition of the blood vessels that primarily manifests in children under five years of age, often leading to coronary artery complications. And KD is the primary contributor to non-congenital cardiac conditions in the pediatric population. In a KD mouse model, Lactobacillus plantarum CCFM639 (CCFM639) was administered, and its effects on systemic and vascular inflammation were evaluated by measuring the spleen index, inflammatory factors, and histopathological changes. Additionally, the effect of CCFM639 on intestinal barrier function was assesse

BACKGROUND: Primary Sjögren's syndrome (pSS) is a systemic autoimmune disorder predominantly affecting exocrine glands, with symptoms including dry eyes and mouth. Involvement of the autonomic nervous system (ANS) has been reported, though data on its frequency and clinical significance remain inconsistent. This study aimed to evaluate autonomic dysfunction in patients with pSS using Sympathetic Skin Response (SSR) and the Composite Autonomic Symptom Score 31 (COMPASS 31).
METHODS: Fifty-nine female pSS patients and 38 age-matched healthy controls were enrolled. All participants underwent neu

Deficiency of adenosine deaminase 2 (DADA2) has been a challenging diagnosis to make since it was first described in 2014. The disease represents a wide range of phenotypes. Therefore, it may present with various clinical patterns. Throughout the years, several difficult-to-diagnose cases of DADA2 were reported in the literature. Although several studies and reviews were published regarding different phenotypes and manifestations of DADA2, a review of challenging cases with diverse combinations of DADA2 manifestations was needed to integrate the knowledge from the literature into the clinical

Leptospirosis is a re-emerging zoonosis fast becoming a global burden. Pulmonary haemorrhages are a deadly complication, with case fatality rates >50%. Systemic vasculitis is thought to be the pathological process responsible for pulmonary haemorrhages and therapeutic plasma exchange (TPE) is one of the treatments offered. This systematic review aims to evaluate the effectiveness of TPE in managing leptospirosis-associated severe pulmonary haemorrhagic syndrome (SPHS). A systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis protocol. The s

BACKGROUND: Even though anti-neutrophil cytoplasmic antibodies (ANCA) are frequently linked to ANCA associated vasculitis (AAV), it's important to understand that other illnesses, including lung diseases, can also manifest as ANCA positivity. Finding the incriminated pathology might be difficult.
AIM: To report four ANCA-associated cases with a diagnosis problem.
METHODS: Four patients were recruited from the allergo-pneumology department of the Fattouma Bourguiba University Hospital of Monastir, Tunisia, over two years (2020-2022). Indirect immunofluorescence technique on neutrophil cells (E

Granulomatosis with polyangiitis (GPA) is a rare systemic ANCA (Anti-neutrophil cytoplasmic antibodies) associated vasculitis (AAV). In the last couple of decades, GPA has emerged as a disease of concern due to rapid increase in the prevalence and incidence especially in developing countries. Unknown aetiology and rapid progression have made GPA a critical disease. Thus, establishing specific tools to facilitate early and faster disease diagnosis and efficient disease management has immense importance. GPA may develop in genetically predisposed individuals on receiving the external stimulus (i

Granulomatosis with polyangiitis (GPA) is a necrotizing systemic vasculitis of small and medium-sized vessels with renal and sinopulmonary involvement. Its symptoms include chronic sinusitis, recurrent pneumonia, glomerulonephritis, constitutional symptoms, and skin manifestations with a typical onset in the fourth to sixth decade of life. We present a rare case of GPA in a 16-year-old female who presented with facial numbness and nasal regurgitation via a palatal defect. The patient reported a several-month history of recurrent epistaxis and chronic nasal congestion accompanied by several wee
