Long-term pharmacological therapy of Brugada syndrome: is J-wave attenuation a marker of drug efficacy?

We herein describe two patients with Brugada syndrome in whom J-waves were successfully modified by drugs. Case 1 was a 54-year-old man who presented with repeated ventricular fibrillations (VF) and J-point elevation in the right precordial and lateral leads. After administration of cilostazol (200 mg/d), J-waves disappeared and coved-type ST-segment elevation changed to a saddleback-type for 25 months. Case 2 was a 31-year-old man who presented with a VF storm and J-point elevation in the lateral leads. After administration of quinidine (300 mg/d), J-waves and coved-type ST-segment elevation

Pharmacogenetics of opiates in clinical practice: the visible tip of the iceberg.

Opioids are the cornerstone of analgesic therapy and are used as a substitution therapy for opiate addiction. Interindividual variability in response to opioids is a significant challenge in the management of pain and substitution. Therefore, treatment with opioids requires a careful individualization of dosage to achieve an appropriate balance of efficacy and adverse effects and, consequently, avoid toxicity, particularly respiratory depression, sedation and for some, cardiac ventricular fibrillations. Many studies have investigated the association between genetic factors and the variability

What have we learned about patients with heart failure and preserved ejection fraction from DIG-PEF, CHARM-preserved, and I-PRESERVE?

Examination of patients with reduced and preserved ejection fraction in the DIG (Digitalis Investigation Group) trials and the CHARM (Candesartan in Heart Failure: Assessment of Reduction in Mortality and Morbidity) trials provides comparisons of outcomes in each of these types of heart failure. Comparison of the patients in these trials, along with the I-PRESERVE (Irbesartan in Heart Failure with Preserved Systolic Function Trial), with patients of similar age, sex distribution, and comorbidity in trials of hypertension, diabetes mellitus, angina pectoris, and atrial fibrillation provides eve

Sudden cardiac arrest due to subtotal absence of left-sided pericardium--case report and review of the literature.

Congenital absence of the pericardium is a very rare cardiac malformation, usually diagnosed fortuitously on autopsy or surgery. Symptoms related to these abnormalities are usually benign, and fatalities reported in the literature are almost exclusively secondary to herniation of the heart through a partial defect. We present the unusual case of a 44-year-old woman admitted for sudden cardiac arrest. Initial evaluation suggested acute anterior myocardial infarction, but further investigation ruled out coronary heart disease. No arrhythmia could be initiated on electrophysiological study, and a

[Treating congestive heart failure with cardiac contractility modulation (CCM) : possibilities and study overview].

Cardiac contractility modulation (CCM) is a device therapy for patients with systolic heart failure. CCM therapy applies non-excitatory signals during the absolute refractory period of the heart cycle. It influences myocardial contractility by modulating the regulation of calcium cycling. CCM therapy has been proven to enhance peak VO(2), quality of life and exercise tolerance in patients with congestive heart failure. It can be used as an additional therapy to an implantable cardioverter defibrillator (ICD), if present. CCM therapy should be considered in symptomatic patients with congestive

Coronary blood flow and perfusion pressure during coronary angiography in patients with ongoing mechanical chest compression: a report on 6 cases.

Patients with pulseless electrical activity or refractory ventricular fibrillation have a very bad prognosis. Coronary angiography and angioplasty may be required to restore an effective circulation, but this must be performed whilst chest compressions are continued. The LUCAS chest compression device is suitable for this purpose. So far there are no reports on the effect of this device on coronary circulation in humans. We monitored the coronary perfusion pressure assessed invasively as the difference between the diastolic pressures at the coronary ostium and right atrium, and compared these

Should aldosterone blockade be used beyond current indications in heart failure?

Aldosterone receptor antagonists (ARAs) are becoming important supplementary options in the pharmacologic treatment of congestive heart failure (CHF), and the indication for ARAs continues to grow. Aldosterone is a stimulator of myocardial fibrosis, causing progression of CHF and serving as an important factor in the pathogenesis of diastolic heart failure (ie, heart failure with normal ejection fraction). The beneficial effects of ARAs have already been demonstrated in patients with severe CHF and patients with symptoms of CHF and left ventricular dysfunction early after myocardial infarction

Atrial septal defect in adults. Thirty-eight-year follow-up of a surgically and a conservatively managed group.

This report describes the long-term (up to 38 years) outcome, in terms of mortality and cardiovascular morbidity, in a non-randomized clinical observation study of a surgically and a conservatively managed group of adult patients with shunt lesions at atrial level. Haemodynamic status was assessed at baseline and at repeat examination. Twelve patients underwent surgical repair of the lesion between these two heart catheterizations, resulting in a marked reduction in heart size and right ventricular systolic pressure. These changes were associated with improvement in functional class and a tren

Indications for anticoagulation in atrial fibrillation.

Factors associated with an increased risk of thromboembolic events in patients with atrial fibrillation (AF) include increasing age, rheumatic heart disease, poor left ventricular function, previous myocardial infarction, hypertension and a past history of a thromboembolic event. Patients with AF should be considered for anticoagulation or antiplatelet therapy based on the patient's age, the presence of other risk factors for stroke and the risk of complications from anticoagulation. In general, patients with risk factors for stroke should receive warfarin anticoagulation, regardless of their

Atrial fibrillation: preventing thromboembolism and choosing nondrug therapies.

A major consequence of atrial fibrillation (AF) is stroke. For stroke prevention in AF, the American Heart Association recommends aspirin, 325 mg/d, for low-risk patients. For all others, anticoagulation with warfarin to a target INR of 2 to 3 is recommended if warfarin is not contraindicated. Approximately 0.3% of patients receiving warfarin suffer intracranial hemorrhage. For restoration of sinus rhythm in recent AF, direct current cardioversion is the treatment of choice if a trial of antiarrhythmic drug therapy has failed or is contraindicated. Potential complications include thromboemboli

Epidemiologic studies demonstrating increased morning incidence of sudden cardiac death.

To determine if sudden cardiac death shows a circadian variation, the mortality records of the Massachusetts population and the Framingham Heart Study were analyzed. The Massachusetts mortality data analysis revealed an increased morning incidence for out-of-hospital cardiac deaths similar to that previously described for nonfatal myocardial infarction. The records of the Framingham Heart Study allowed more comprehensive analysis of the time of sudden cardiac death. In this study, a prominent circadian variation was also identified in which frequency of sudden cardiac death increased sharply b

[A case of hypertrophic obstructive cardiomyopathy with left atrial giant thrombus during anti-platelet therapy].

A case of a 77-year-old female with hypertrophic obstructive cardiomyopathy having the left atrial "giant" thrombus was reported; the diagnosis of the localization and size of the thrombus was made by employing echocardiography and contrast enhancement computed tomography as well. The clinical record indicated that the thrombus formation, which happened in the course of the antiplatelet therapy, was to be considered due to the complicated condition of the patient, i.e., the combination of atrial fibrillation, low cardiac output, enlargement of left atrium and left ventricular diastolic disfunc

[Pacemaker implantation in 7 days or less old of early neonates].

We have implanted cardiac pacemaker (PM) in 3 early neonates ranging from 5 hours old to 7 days old of age with congenital complete or incomplete AV block. They were in advanced stage of rapidly developed cardiac failure with resistance to drug therapy and with ventricular tachycardia or fibrillation. Cardiac failure was improved immediately in the 3 cases after the operation and the effectiveness by the pacing was recognized for a long term observation (4 months to 2 years). There appeared PM implantation in-14 neonates, including our cases, in Japanese literatures. In review of the 14 neonat

Pathophysiology of re-entrant dysrhythmias.

Re-entry was demonstrated as a self-perpetuating mechanism as early as 1906. It may lead to single-cycle electrocardiographic phenomena as well as reciprocating tachycardias. Factors favouring re-entry are as follows. Unidirectional or temporary local block of conduction, a mechanism always involved in initiation of reciprocating rhythms but also of importance for the maintenance of the arrhythmia. Increased myocardial mass. Decreased conduction velocity. Accelerated repolarization, e.g. short refractoriness. Increased dispersion of refractoriness. Regular reciprocating tachycardias utilize re

Mitral valve disease.

Mitral stenosis is a progressive lesion carrying a relatively high risk of sudden incapacitation from systemic embolus or the onset of atrial fibrillation. Since the condition is likely to be significant when diagnosed, it is not compatible with single-crew professional operations and requires careful supervision. The presence of mild mitral regurgitation where the aetiology has been shown not to be due to chordal rupture, papillary muscle dysfunction secondary to coronary artery disease, rheumatic mitral valve disease, or Marfan's disease, where left atrial and left ventricular dimensions are

Pharmacological approaches in the treatment of atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and is associated with substantial cardiovascular morbidity and mortality. The arrhythmia can be initiated and/or maintained by rapidly firing foci, single- and multiple-circuit reentry. Once initiated, AF alters atrial electrical and structural properties (atrial remodeling) in a way that promotes its own maintenance and recurrence and may alter the response to antiarrhythmic drugs. Thus, initial episodes of paroxysmal (self-terminating) AF lengthens to the point where the arrhythmia becomes persistent (requires cardiove

Very early cardiac hemodynamic changes after atrial fibrillation ablation.

All chambers' systolic functions were improved with long-term follow-up after catheter ablation (CA). However, changes on cardiac function on very early phase (within 48 h) has not been thoroughly investigated. This study aimed to evaluate the acute effects of CA on cardiac function and morphology immediately post-procedural (≤ 48 h). We prospectively enrolled 117 consecutive patients. Hemodynamic parameters including biatrial pressure and echocardiographic data were acquired pre- and post-ablation. (i) All bi-atrial pressures significantly elevated post-ablation (p < 0.002). (ii) Bi-atrial fu

Utility of CT and MRI in Cardiac Electrophysiology.

Cardiac electrophysiology involves the diagnosis and management of arrhythmias. CT and MRI play an increasingly important role in cardiac electrophysiology, primarily in preprocedural planning of ablation procedures but also in procedural guidance and postprocedural follow-up. The most common applications include ablation for atrial fibrillation (AF), ablation for ventricular tachycardia (VT), and for planning cardiac resynchronization therapy (CRT). For AF ablation, preprocedural evaluation includes anatomic evaluation and planning using CT or MRI as well as evaluation for left atrial fibrosi

Worsening tricuspid regurgitation associated with permanent pacemaker and implantable cardioverter-defibrillator implantation: A systematic review and meta-analysis of more than 66,000 subjects.

BACKGROUND: Worsening tricuspid regurgitation (TR) after either permanent pacemaker (PPM) or implantable cardioverter-defibrillator (ICD) implantation is an emerging clinical challenge. Early recognition of this entity is essential in guiding treatment. OBJECTIVE: This meta-analysis was designed to identify the overall incidence and patient-specific predictors of TR post-device implantation. METHODS: We searched electronic databases from inception to January 2023 for published studies that reported the incidence of TR worsening post-device implantation. The log odds ratio (OR) was used to su

Early Remodeling of the Left Atrium Following Catheter Ablation of Atrial Fibrillation: Insights From DECAAFII.

BACKGROUND: Left atrial (LA) enlargement is prevalent among atrial fibrillation (AF) patients and constitutes an important marker of atrial myopathy. Several studies have described reduction in LA volume post-catheter ablation (CA) of AF, however, none have investigated differences related to additional ablation outside the pulmonary veins (PVs). OBJECTIVES: The authors sought to study early LA remodeling following CA of persistent AF and the impact of additional, fibrosis-guided extra-PV ablation. METHODS: In this DECAAF II (Effect of MRI-Guided Fibrosis Ablation vs Conventional Catheter Ab
